Background: Today, laparoscopy is frequently used in abdominal emergencies such as acute appendicitis. There are several techniques used to close the appendiceal stump during laparoscopic appendectomy. We aimed to compare the use of handmade loop and Hem-o-lok used to close the appendiceal stump in public hospitals where resources are limited, in terms of surgical outcomes and cost. Methods: Between January 2020 and December 2022, patients for whom handmade loops and Hem-o-loks were used to close the appendiceal stump during laparoscopic appendectomy in our clinic were included in the study. There were a total of 638 patients (mean age: 33 ± 13.5 years, 325 females and 313 males) in the patient cohort. Demographic and clinical data, duration of surgery, complications, hospital stay, pathology reports, mortality, and cost of supplies were compared between the two groups. Results: There were 308 patients in the handmade loop group (160 females, 148 males, mean age: 33.7 years, range: 18–85 years) and 330 patients in the Hem-o-lok group (166 females, 164 males, mean age: 32.5 years, range: 18–89 years). There was no significant difference between the two groups for American Society of Anesthesiologists score, duration of symptom, hospital stay, intensive care unit stay, preoperative laboratory values, histopathological results, mortality, and morbidity (P >.05). The mean operation time was 48.76 ± 16.16 minutes in the handmade loop group and 40.53 ± 11.63 minutes in the Hem-o-lok group (p = 0.001). In terms of cost, the cost per case of Hem-o-lok group was about 25.8 times as much as the group that used sutures ($31 versus $1.2). Conclusions: Both methods can be used safely in laparoscopic appendectomy. The use of Hem-o-lok has no advantage other than shortening the operation time. However, it is costlier. Especially in peripheral hospitals where resources are limited, closing the appendiceal stump using a handmade loop is an easy, safe, and cost-effective method. [ABSTRACT FROM AUTHOR]

Aim: Inflammatory markers are effective in determining the prognosis of malignant diseases. The aim of this study is to investigate the relationship of HALP and LCR with tumor response after neoadjuvant chemoradiotherapy and their effects on prognosis in patients with locally advanced rectal cancer. Methods: Eighty-eight patients who received nCRT with the diagnosis of LARC were included in the study. First, all patients were divided into 2 groups: patients with pathological and clinical complete response (pCR+cCR), group 1, and patients with non-complete response, group 2. The 82 patients who underwent surgery were divided into two groups according to the TRG Dworak: good response and poor response groups. Inflammation markers such as HALP and LCR were obtained using biochemical parameters. Results: HALP and LCR were higher in the complete response group than in the none-complete response group (P<0.05). When TRG 3-4 (good response group) and TRG 0-1-2 (poor response group) were compared, HALP and LCR were higher in the good response group (P<0.05). The cut-off point for the HALP value was 30.17, the sensitivity was 88.2%, and the specificity was 43.7%. The cutoff point for the LCR value was 0.402, the sensitivity was 88.2%, and the specificity was 63.4%. It was found that HALP and LCR calculated prior to neoadjuvant CRT could not predict overall survival. Conclusions: We believe that inflammatory markers such as HALP and LCR can effectively identify rectal cancer patients who respond best to nCRT. [ABSTRACT FROM AUTHOR]

Objective: This study investigated and compared the effects of antibiotic and steroid lavage on survival and cytokine levels in an experimental abdominal sepsis model., Background: In abdominal sepsis, abdominal lavage with saline or antibiotic solutions is a well-documented intervention known to have positive impact on survival; however, the effects of steroid lavage in abdominal sepsis have not yet been investigated., Materials and Methods: Ninety-six Wistar rats were divided into 4 groups (n = 24). Abdominal sepsis was induced by cecal ligation and puncture. Six hours after laparotomy, the authors performed a relaparatomy followed by cecal resection and an abdominal lavage. Abdominal lavage was performed using saline in group 1, equal volumes of cefazolin sodium in group 2, low-dose methylprednisolone (1 mg/kg) in group 3, and high-dose methylprednisolone (2 mg/kg) in group 4. After division of 2 subgroups from each of the 4 groups, the first of the rats (n = 12) were euthanized 6 hours later for evaluation of cytokines (ie, interleukin [IL] 1β, 2, 4, 10, and tumor necrosis factor alpha [TNF-α]), and the others were followed for 30 days for analysis of mortality rates., Results: The mortality rate of the rats in group 2 was significantly higher than group 4, which had no mortality (P = 0.032). Although insignificant, the lowest mean value of IL-1β, IL-2, and TNF-α were in group 1, and the highest was in group 2. The lowest IL-4 level was in group 3, and the highest level was in group 2 (P = 0.41). Interleukin-10 levels were significantly lower in group 4 and higher in group 2 (P = 0.014)., Conclusion: The authors state that peritoneal lavage with prednisolone improved survival rates with increasing doses in abdominal sepsis.

Introduction: Lindgren-Turan osteotomy used in hallux valgus deformity is a subcapital, transverse displacement osteotomy of the first metatarsal without any additional capsular repair. The aims of this study are to describe a transosseous capsuloplasty technique in this procedure and evaluate whether capsuloplasty would improve the clinical and radiological outcomes in patients with moderate to severe hallux valgus deformity., Methods: Twenty-three feet operated by Lindgren-Turan osteotomy (Group B) and 25 feet operated by the same osteotomy combined with transosseous capsuloplasty (Group A) were evaluated retrospectively for the correction of the hallux valgus, intermetatarsal and distal metatarsal articular angles, sesamoid reduction, American Orthopaedic Foot and Ankle Society (AOFAS) Clinical Rating Scale as well as patient satisfaction. The mean postoperative follow-up was 14 (range 12-28) months., Results: All radiological parameters improved considerably as a result of both groups. However, postoperative improvements in intermetatarsal and distal metatarsal articular angles were greater in Group A. Complete reduction of medial sesamoid was achieved in 52% of patients in Group A, whereas 17.4% of patients in Group B had complete reduction. AOFAS scores and number of patients with complete satisfaction in Group A were significantly greater than that in Group B., Conclusion: Better clinical and radiological outcomes can be achieved in patients with moderate to severe hallux valgus deformity operated by Lindgren-Turan distal metatarsal osteotomy, when it combines with transosseous capsuloplasty.

Background: We aimed to compare conventional suture closure of arteriotomy with N-butyl-cyanoacrylate-assisted suture closure., Methods: Forty Wistar rats were randomly divided into two groups. Standard arteriotomy was performed to the abdominal aorta through a midline incision. In the first group, arteriotomy was closed by 3 stitches with 45 degrees between each and in the second by two stitches with 0.1 ml (12-12.5 mg) cyanoacrylate. Amount of blood loss, operation time and severity of myointimal hyperplasia by immunohistochemistry on aorta segments were measured on postoperative days 7 and 30., Results: Mean anastomotic time was 13.5 +/- 1.64 in the first and 13.0 +/- 1.75 min in the second group (p = 0.356). Operation time was 23.45 +/- 3.63 in the control and 21.0 +/- 3.09 min in the second group (p = 0.027). Mean amount of bleeding was 473.75 +/- 260.5 in the first and 327.5 +/- 155.36 microl in the second group (p = 0.037). Intimal thickness on the 7th day was 80.62 +/- 7.92 in the first and 83.24 +/- 3.42 microm in the second group, and on the 30th day was 81.64 +/- 5.11 in the first and 88.77 +/- 11.03 microm in the second group. The early and late intimal thicknesses were similar (p = 0.35 and 0.87, respectively)., Conclusion: Reconstruction of arteriotomies with fewer sutures in combination with cyanoacrylate is a safe method associated with less blood loss and shorter operation time. It also does not lead to increased myointimal hyperplasia.

Intranodal palisaded myofibroblastoma (IPM) also called as intranodal hemorrhagic spindle cell tumor with amianthoid fibers is a distinctive and rare mesenchymal neoplasm of lymph nodes. This entity generally misdiagnosed as intranodal Kaposi's sarcoma or schwannoma in past. In contrast to Kaposi's sarcoma, it behaves in a benign fashion and does not need any further therapy except total surgical resection of the mass. This neoplasm has a great predilection for the inguinal region. The lesion presents typically as a unilateral, painless, solitary mass. To our knowledge, approximately 53 cases of IPM have been reported in the English-language literature. We present a 43-year-old-male patient with IPM and discuss histological, immunohistochemical features and pathogenesis of this rare benign neoplasm.

Minimally invasive percutaneous plate osteosynthesis (MIPPO) has well-documented biological advantages and appears to be a reasonable treatment option for complex femoral and tibial fractures. However additional radiation exposure during reduction of the fracture, application of the plate to the bone and screw fixation is one of the disadvantages of this technique. We describe a technical trick for screw fixation in MIPPO with locking compression plates which decreases the duration of fluoroscopy use during the operation.

Objectives: We evaluated the results of the Grice subtalar extra-articular arthrodesis for pes planovalgus deformity in patients with cerebral palsy and its effectiveness in providing hindfoot alignment., Methods: Grice subtalar extra-articular arthrodesis was performed for pes planovalgus deformity in 14 feet of nine patients (5 males, 4 females; mean age 10.3 years; range 6 to 12 years) with cerebral palsy. Six patients were diplegic, two patients were paraplegic, and one patient was hemiplegic. Four patients had unilateral, five patients had bilateral involvement. A fibular graft taken from the distal third of the fibula above the syndesmosis was used in all the patients without any fixation material. The patients were assessed with the AOFAS (American Orthopaedic Foot and Ankle Society) hindfoot scoring system pre- and postoperatively. The mean follow-up was 30 months (range 6 to 81 months)., Results: Compared with the preoperative values, the mean AOFAS hindfoot score increased from 53 (range 41 to 81) to 68.4 (range 51 to 96), the mean talocalcaneal angle decreased from 52.6 degrees to 30.4 degrees , and from 55.8 degrees to 35.9 degrees in frontal and sagittal planes, respectively (p<0.05). The mean ankle valgus angle measured on anteroposterior radiographs remained unchanged (mean 5 degrees ). Of five patients who underwent bilateral surgery, three (21.4%) developed donor-site fibular nonunion on one side. One (7.1%) of these patients also had graft nonunion and displacement. Treatment was rated as satisfactory by the parents of seven patients (77.8%; p<0.05)., Conclusion: Despite donor-site morbidity, the Grice subtalar extra-articular arthrodesis is an effective procedure for the correction of hindfoot alignment in patients with pes planovalgus deformity secondary to cerebral palsy.

The authors present a case of inguinal abscess occurring after inguinal hernia repair that was treated with a silver-containing wound care product without removing the prosthesis. A 75-year-old woman presented with inguinal abscess and acute renal failure initially underwent hemodialysis and then surgical drainage. The mesh was not removed, the wound was left open for secondary healing, and daily wound care was conducted using Aquacel Ag Hydrofiber® (ConvaTec, Skillman, NJ). She was discharged 15 days after admission and her wound healed 2 months after admission.

Objectives: We investigated the effect of extracorporeal shock wave therapy in the treatment of lateral epicondylitis., Methods: The study included 13 elbows of 12 patients (8 women, 4 men; mean age 40 years) who did not benefit from conservative therapy for lateral epicondylitis and refused surgical therapy. Under fluoroscopic guidance, extracorporeal shock wave therapy of 1200 pulses at 15-20 kV was administered at a mean of three sessions (range 3 to 7 sessions) with two-day intervals. No immobilization was applied after sessions. The patients were questioned about pain during rest and during dorsiflexion of the wrist against resistance before and after three and 12 months of treatment. The results were evaluated using the Roles and Maudsley pain scores. At the end of 12 months, the patients were asked to lift weight with the shoulder in flexion and the wrist in extension to measure grip strengths in comparison with the unaffected side., Results: The mean pain score which was 3.4 before treatment decreased to 2 after treatment. The mean grip strength of the affected extremities was 82.1% of the normal side. At final follow-ups, nine patients (10 elbows) (75%) stated that the treatment was beneficial; all these patients achieved at least 85% of grip strength of the normal side. Two patients (16.7%) reported no benefit from therapy and had a poor pain score. The result was fair in one patient (8.3%) who expressed dissatisfaction and had partial relief after the procedure. This patient achieved 60% of grip strength of the unaffected side., Conclusion: Extracorporeal shock wave therapy is an alternative method for patients who do not benefit from conservative treatment and refuse surgical treatment.

Background/aims: Recent epidemiologic studies mentioned a shift from left to right in colorectal cancer. We searched our hospital database to try to give an insight into the patient characteristics and also optimize the screening programs in our country., Methodology: A total of 1771 colorectal cancer patients were identified and analyzed. Patients' gender, age, operation dates, tumor localization, resectability and T stage of the lesions, were recorded and comparisons were made. Second step of the study consisted of a questionnaire including 6 questions that were asked to 278 physicians to document current physician attitudes and practices regarding colorectal cancer screening, and to formulate appropriate interventions., Results: Most of our patients were older than 51 in our study and a higher percentage of cases had sigmoid and rectal cancer (67%). The chronological trends in anatomic subsite distribution of colorectal cancer showed that by changing years when left colon cancer percentage was increasing, rectum cancer percentage was decreasing. There was no age-dependent variation in the topographical distribution of colorectal cancer by changing years. Also there was no relation between sex differences and subsite distribution when the whole group was taken into consideration. Specialist physicians had a poor understanding of colorectal cancer screening and lacked the responsibility of the screening programs., Conclusions: Data presented in this article do not support a progressive left to right shift in the distribution of colorectal carcinoma. Screening with fecal occult blood testing and sigmoidoscopy seems appropriate. Physician education may be an important step in screening before public awareness.

The aim of this prospective study was to compare the efficacy of oral versus parenteral prophylactic amoxicillin-clavulanic acid for preventing surgical site infection after open prosthetic mesh repair of inguinal hernia. A total of 480 inguinal-hernia patients were randomly assigned to two groups. Group I (n = 240) received 1.313 g oral amoxicillin-clavulanic acid 2 hours before operation, and group II (n = 240) received 1.2 g of the same drug combination intravenously approximately 30 minutes before surgery. Patients were examined four times during 1 year of follow-up (at 7-10 days, 4-6 weeks, 6 months, and 12 months postoperation), and data related to surgical site infections were collected. Seventy-two patients were excluded due to confounding factors during and after the operation. There were no statistically significant differences between group I (final n = 208) and group II (final n = 200) with respect to age, sex distribution, body mass index, American Anesthesiology Association grade, frequencies of different hernia types, duration of surgery, and the experience levels of the principal surgeon in the operations. One of the 208 (0.5%) patients in group I and 3 of the 200 (1.5%) patients in group II developed superficial surgical site infections (p > 0.05). None of the infections required mesh removal. There were no deep surgical site infections in either group, and there was one case of hernia recurrence in each group. For patients undergoing open prosthetic repair of inguinal hernia, oral amoxicillin-clavulanic acid is safe, significantly less costly, and equally effective in preventing surgical site infection as the same dose given parenterally.
