3. Evaluation of patients undergoing 24-hour rhythm Holter monitoring in the pediatric cardiology unit.

Abstract

Aim: This study aims to determine the value of 24-hour Rhythm Holter monitoring (HM) in diagnosing and treating pediatric patients. Somebody reviewed our clinic's indications and results of 24-hour rhythm Holter monitoring. Materials and Methods: The files of 1670 patients who applied to our clinic between January 1, 2015 and December 31, 2022, were scanned retrospectively. Somebody retrospectively evaluated and recorded age, gender, reason for admission, presence of cardiac disease, presence of cardiac disease in the family, treatment histories, family consanguineous marriages, Electrocardiography (ECG) findings, Echocardiography (ECHO) findings, and HM findings were from the hospital computer archive files. Results: Found to be expected in 1,005 (60.2%) of the patients who underwent Holter monitoring (HM). Detected tachyarrhythmia in 408 (24.4%) patients, bradyarrhythmia in 85 (5.1%) patients, preexcitation syndrome in 73 (4.4%) patients, right bundle branch block in 27 (1.6%) patients, atrial-derived arrhythmia in 27 (1.6%) patients, genetic-related arrhythmia in 12 (0.7%) patients, pace rhythm in 12 (0.7%) patients, and other different Holter findings in 21 (1.3%) patients. Conclusion: Since arrhythmic symptoms often occur intermittently in childhood, detecting them at presentation may not be possible. A missed ECG taken at rest provides information for a short period of cardiac rhythm and many arrhythmias, so the 24-hour Holter method (HM) is helpful for diagnosis. Our study has supported that HM is used increasingly frequently and is beneficial as an easy-to-apply, noninvasive method in diagnosing arrhythmia and in monitoring and treatment planning of cases with arrhythmia. 5. Essays on non-market returns to education : evidence from a natural experiment in Turkey

Abstract

As a result of the political events in the mid-1990s, the compulsory schooling in Turkey was extended to 8 from 5 years. Cohorts who were at the fourth grade or lower had to continue 8 years of compulsory education. This reform generated a natural experiment, and therefore, enables researchers to obtain endogeneity-robust estimates of market and non-market returns of education. In this thesis, I used this reform to investigate some nonmarket returns of education. Chapter 2 is the first study to investigate the causal impact of maternal schooling on childhood immunisation rates. I used the exposure to the Compulsory Education Law (CEL), enacted in Turkey in 1997, by date of birth and the differentiation in its implementation across regions as an instrument for the schooling of young mothers. CEL generated an exogenous change in compulsory education from 5 to 8 years for women born after 1986. Using the CEL as an instrument, this chapter finds that an increase in the education of mothers significantly increases the coverage of the third (full) dose of DPT and Hepatitis B for their children. Furthermore, the chapter investigates the causality between maternal education and a range of channels affecting immunisation rates suggested in the literature. I find that education increases the age of first marriage and birth, changes women`s and their spouse`s labour market status and significantly effects women`s attitude towards spousal violence against women and gender discrimination in a manner that empowers women. Chapter 3 provides empirical evidence regarding the causality between education and mental health in the context of a large emerging economy, Turkey. I exploit differentiation in the exposure to the Compulsory Education Law (CEL) change by the date of birth as an instrument for schooling of young women. When the CEL is used as an instrument for schooling of women, I find that schooling increases the incidence of some mental health disorders. These findings seem quite robust to alterations in the regression definitions and to the inclusion of additional individual-level control variables such as having one`s own income, employment status, marital status, occupation, domestic abuse history in the family, and rural/urban status. I also find a heterogeneous effect of the CEL on educational attainment of women depending on residence status and whether their birth mother had been abused by her partner. Moreover, heterogeneous effect tests also indicate that the effect of education differs by personal income and labour market status of women. Chapter 4 is the first study to investigate the causal effect of the husband`s education on violent and abusive behaviour against their wife. To do this, I use a natural experiment, an education reform increasing compulsory schooling from five to eight years in Turkey, as an instrument to obtain endogeneity-robust estimates. I find that the husband`s education lowers the probability of suffering physical, emotional and economic violence as well as experiencing socially unacceptable behaviour. The only aspect of violence not affected by the spouse`s education is sexual violence. I also find that women whose mothers or whose husbands’ mothers experienced domestic violence are more likely to suffer violence themselves.

8. Is The Presence of Subscapularis Tear with Supraspinatus Tear Related to Scapular Morphology?

Abstract

Aim: In previous studies it has been shown that scapular morphology is associated with supraspinatus tears. However, the relationship between subscapularis tears and scapular morphology is controversial. The aim of this study is to investigate whether or not subscapularis tear accompying supraspinatus tear is related to scapular morphology. Materials and Method: Records and imaging of 679 shoulder arthroscopy surgeries performed for rotator cuff tear in our orthopedics and traumatology clinic between 2016 and 2020 were retrospectively analyzed. 162 patients with isolated full-thickness supraspinatus tear and 83 patients with subscapularis tear accompanying supraspinatus tear were included in the study. The critical shoulder angle (CSA), glenoid version (GV), glenoid inclination (GI) and critical coracoid process angle (CCPA) of the patients were measured. Results: In the patient group with isolated supraspinatus tear and in the group with subscapularis tear accompanying supraspinatus tear; The mean age was 53.8±8.9 and 61.1±8.4 respectively, and there was a significant difference (P<0.001); the mean CSA values were 36.9°±4.25° and 35.6°±5.1°respectively, (P=0.32); GI mean values were 7.72°±5.4° and 8.4°±5.3°respectively, (P=0.51); GV mean values were 2.1°±4,5° and 2.5°±4.7° respectively, (P = 0.85); mean CCPA values were 21.4°±4.5° and 22.3°±4.9°respectively, (P=0.73). There was no significant difference between the groups in terms of gender (P=0.59). In the logistic regression analysis performed by using criteria of the measured angles, age and gender, it was observed that advanced age had an effect on the subscapularis tear accompanying the supraspinatus tear, independent of the other parameters (P < 0.001 OR:1.09). Conclusion: There appears not to be a significant difference between isolated supraspinatus tears and subscapularis tear accompanying supraspinatus tear, in terms of radiological parameters regarding scapular morphology. Age seems to be the most important factor in subscapularis tear accompanying supraspinatus tears. In patients with supraspinatus tear, an increase in age by one unit (in years) increases the odds of subscapularistear by1.09 times. [ABSTRACT FROM AUTHOR]

9. Hard Superconductivity of a Soft Metal in the Quantum Regime

Abstract

Superconductivity is inevitably suppressed in reduced dimensionality. Questions of how thin superconducting wires or films can be before they lose their superconducting properties have important technological ramifications and go to the heart of understanding coherence and robustness of the superconducting state in quantum-confined geometries. Here, we exploit quantum confinement of itinerant electrons in a soft metal to stabilize superconductors with lateral dimensions of the order of a few millimeters and vertical dimensions of only a few atomic layers. These extremely thin superconductors show no indication of defect- or fluctuation-driven suppression of superconductivity and sustain supercurrents of up to 10% of the depairing current density. The extreme hardness of the critical state is attributed to quantum trapping of vortices. This study paints a conceptually appealing, elegant picture of a model nanoscale superconductor with calculable critical state properties. Abstract

Bu araştırmanın amacı, 40-65 yaş arasındaki bireylerin fiziksel aktivite düzeyini ve uyku kalitesini incelemek ve fiziksel aktivite düzeyi ile uyku kalitesi arasında ilişki olup olmadığını saptamaktır. Araştırmaya, İstanbul ili, Asya yakasından 12 ilçede (Ümraniye, Pendik, Üsküdar, Kadıköy, Maltepe, Kartal, Ataşehir, Sultanbeyli, Sancaktepe, Beykoz, Tuzla, Çekmeköy) yer alan seçilmiş spor merkezlerine üye 40-65 yaş arası 600 (313 Erkek, 287 Kadın) birey katılmıştır. Ölçüm aracı olarak, Pittsburgh uyku kalitesi indeksi anketi (PUKİ) ve Uluslararası fiziksel aktivite anketi (IPAQ) kullanılmıştır. Verilerin analizinde; tanımlayıcı istatistik uygulanmış, normallik testlerinden ve varyanslarından homojenlik test edilerek parametrik testlerin kullanılmasına karar verilmiştir. Tüm ölçek maddelerinde cinsiyetler arası karşılaştırmada bağımsız iki grupta t-testi; ikiden fazla grupların karşılaştırılmasında ise tek yönlü varyans analizi (ANOVA) kullanılmıştır. Tüm ölçek maddelerinin yüzde/frekans dağılımları yapılmıştır. Bağımlı değişkenler arasındaki ilişkilerin incelenmesinde pearson korelasyonu kullanılmıştır. Araştırmadan elde edilen bulgulara göre, fiziksel aktivite düzeyi ile uyku kalitesi arasında genel olarak istatistiksel açıdan anlamlı bir ilişki bulunmamıştır. Abstract

This study aims to examine the effects of total, foreign and domestic tourism demand on sectoral employment in regions, based on the level 2 of the Nomenclature of Territorial Units for Statistics (NUTS2) classification determined by the Turkish Statistics Institute (TurkSTAT). For this purpose, we study the impact of tourism demand on overall and sectoral employment in the regions of Turkey between 2004 and 2013, using the Random Coefficients Regression model developed by Swamy. The main results of the study indicate that the contribution of tourism demand, both foreign and domestic to employment, differs across the regions and sectors. Also, the results show that tourism's employment contribution to the industrial sector is the highest. Moreover, the employment contribution of tourism demand is negative for some regions. The employment contribution of domestic tourists' stays is greater than that of foreigners. Based on the results of this study, we can conclude that tourism demand can be used as an effective tool to reduce differences in regional developmental level and increase the contribution to both regional and sectoral employment. There is an urgent need to either develop new tourism policies or revise existing ones so that the county and regions can benefit from investing in the tourism sector. Abstract

Job satisfaction is affected by many factors. A consistent negative correlation between Machiavellianism and Job satisfaction was shown by many previous studies. To estimate this association in a sample of Turkish people, 361 physicians were selected among the total number of 5,959 working in Ankara City Center. A self-administered questionnaire including the Mach IV scale and Job Satisfaction scale developed by Kasapoglu was applied to the subjects. Student t test and Pearson correlation were used for analysis. The correlation of -.25 between Job Satisfaction and Machiavellianism scores was significant and consistent with the mean scores on Job Satisfaction between the Machiavellian and Nonmachiavellian groups. Although the present association is small, the inverse value is consistent for Mach IV scale scores with job satisfaction in the literature.

40. Ball and chain effect: Is Turkey's growth rate constrained by current account deficit?

Abstract

Persistent and unsustainable current account deficits imply continuous foreign borrowing. In this study, we investigate the causal relationship between Turkish current account deficits (CAD) and growth, by using both traditional Granger and Frequency Domain Causality Tests developed by Breitung and Candelon (BC hereafter) over the period of first quarter of 2002 to the first quarter of 2017, since there is scarcely any definitive empirical study out there yet, systematically exploring the causal relationship between GDP growth and current account over short- and longer run. The results of the study indicate that rising CAD unidirectionally causes growth in both short- and medium-run. Thus, since CAD is financed mostly by short-term debt-creating inflows and Turkey is considered to be the most credit-dependent country across all emerging markets, while Turkey's excessive reliance on foreign credit comes at the cost of extremely volatile real GDP growth rates, our result raises many doubts about sustainability and stability of future growth trajectory in Turkey.

One of the key characteristics of the world economy in the last few decades has been the rise of foreign direct investments (FDI), which represent the major and most desirable form of capital flows from developed to developing and transition countries. The aim of this paper is to investigate specificities of FDI inflow to six small open transition economies of the western Balkans over the 2004-2014 period. Formal econometric tests carried out in the paper imply that-contrary to widespread ideology-FDI inflows exhibit at best no impact whatsoever or indeed a statistically significant negative impact on both GDP and GNP (cumulative) growth rates in the western Balkans. After exploring stylized facts and contrasting them with theoretical predictions, the article goes on to debunk several misconceptions in regard to motives, modes of operation and economic effects of FDI confronted with inconsistent, partial or outright detrimental government policies. The authors therefore call for a considerably different attitude towards FDI and the growth-generating concept in the Balkans.

48. Osteoid Osteoma at the Proximal Diaphysis of the Fifth Metatarsal A Case Report

Abstract

The foot is rarely the focus of osteoid osteoma, and only a few of those cases are related to the fifth metatarsal. The present case demonstrates that atypical symptoms with suspicious findings on plain radiographs that are not associated with trauma must be analyzed carefully to determine the nature of the lesion and perform the precise treatment to obtain and sustain the cure. A 29-year-old man presented to the outpatient clinic with a 2-year history of chronic pain in the lateral aspect of his left forefoot. The onset was not related to trauma, surgery, local infection, osteomyelitis, or another entity regarding the proximal fifth metatarsal. The patient noted that the pain was aggravated at night and typically subsided with the use of salicylates or other nonsteroidal anti-inflammatory drugs. Initial plain radiographs demonstrated cortical thickening and a lytic lesion at the proximal diaphysis of the fifth metatarsal. Because the pain relief was transient, we suspected an osteoid osteoma lesion, and subsequent magnetic resonance imaging manifested pathognomonic signs of subperiosteal osteoid osteoma. Diagnosis was followed by planning of the surgery that ended the patient's symptoms.

Abstract

Even though income inequality one of the most commonly analyzed topics in economics, there are few studies aiming to examine the convergence in income inequality. By using a balanced panel of 98 countries over the period of 1995 and 2015, we provide empirical evidences of the beta convergence that explains why there is a decrease in inequality levels in countries that have high inequality levels at the start of the study period and an increase in inequality levels for countries that have low inequality levels at the first year of the sample period. The results of the study support the convergence hypothesis in inequality and provide some explanation of the reasons and the tendencies of income inequalities in countries. Abstract

Long years of studies on IQ and delinquency have generated competing explanations. In this study, to test these competing explanations, we revisited Wolfgang et al. 's (1973) birth cohort dataset of 9,944 boys whose ages ranged between 10 and 18 years old. We conducted a Structural Equation Model (SQM) by using an SPSS AMOS software. This model enabled us to see the strength of the direct and indirect relationship between endogenous and exogenous variables as well as total effects on delinquency. Overall results suggest that environmental variables significantly affect the relationship between IQ and delinquency through schooling time. To be more specific, the direct impact of IQ level on offense is weak and ignorable, and indirect effects through schooling length and socio-economic status are insignificant. [ABSTRACT FROM AUTHOR]
