1. Towards improving the measurement of unsafe abortion : substantive estimates and methodological insights from Zambia

Abstract

Background: Measuring unsafe abortion is essential to understand the magnitude of the problem and monitor progress in women’s reproductive health. However, legal and societal constraints in high-burden contexts foster underreporting of induced abortions which makes obtaining accurate estimates challenging. My PhD examines the methodological challenges around defining and measuring unsafe abortions using Zambia as my country context. Methods: First, I conducted interrupted time series analysis on admissions for abortion-related complications and deaths from 2007-2015 at University Teaching Hospital (UTH), Lusaka to assess the impact of key contextual changes. Second, I collected data from women hospitalized for abortion-related complications in three provinces to estimate the incidence of abortion-related near-miss in 2014. Third, I compared estimates of the incidence of induced abortion in the three provinces using data from 3 methodological approaches. Results: The prevalence of unsafe and induced abortion is high in Zambia. Following the release of clinical guidelines in May 2009, there was an immediate decline in the absolute number of abortion complications by 86 cases (p=0.003). The abortion-related near-miss incidence rate was 72 per 100,000 women, and it was feasible to apply the adapted WHO near-miss criteria in Zambia. Estimates of the incidence of induced abortion per 1000 women ranged from 30 to 80. There was variation in the proportion of women estimated to seek facility care for abortion-related complications in each approach. Conclusion: The burden of unsafe abortion is high in Zambia despite its liberal law. Although there is no gold standard method to measure the burden of unsafe abortion, my findings suggest there is scope to improve use of available data to describe the burden of the most unsafe abortions and evaluate the impact of interventions on abortion-related indicators in restrictive contexts.

Abstract

Background: Datura contains a mixture of anticholinergic agents (alkaloids such as atropine, scopolamine and hyoscyamine) that are responsible for its pharmacological actions. The objective was to evaluate the effect of Datura stramonium in the testes of wistar rats. Methods: Twenty (20) adult male rats weight ranging from 106 g-180 g were distributed into five groups (A-E) of four (4) animals each. Animals were administered with Datura extract once daily for 21 days as Group A-50 mg/kg, Group B -100 mg/kg, Group C -200 mg/kg, Group D -400 mg/kg. While, Group E were given distilled water daily. Testes and blood samples were collected from all the groups, for the histological and biochemical activities. Result: Results showed overall, increase in body weight with siginificant differences in sperm count in all the treatment groups. Administration of high dose of Datura extract was found to have adverse effect on the histological findings in the testes. Extract of Datura stramonium plant is toxic since single extraction shows presence of scopolamine. Abuse of the flower of the plant pose cholinergic poisoning due to scopolamine, significantly reduces male fertility evidenced by increase in the number of abnormal sperm motility and altered testicular morphology. 5. Perspectives of TB survivors and policymakers on post-TB disability

Abstract

BACKGROUND: An international multistakeholder participatory workshop was hosted in the Gambia, West Africa, in November 2021. OBJECTIVES: To explore the experiences, challenges and recommendations of workshop participants on health and wellbeing after TB treatment. METHODS: An exploratory, descriptive, qualitative approach was used for data collection through facilitator-guided group discussions. Workshop participants included adolescent and adult TB survivors, and representatives of TB advocacy groups and the policy sector. Discussions were audio-recorded and transcribed verbatim, and the data were analysed using a deductive thematic approach. RESULTS: Overall, 38 participants (22 women) from six West African countries participated in the workshop, comprising 33 TB survivors and advocacy group representatives and 5 participants from the policy sector. Although some TB survivors noted improved ability to carry out physical activities, others continued to experience detrimental effects on their family life, social interactions, physical health and ongoing stigma. Policymakers emphasised the lack of data and clear guidelines on post-TB disability. CONCLUSIONS: Some TB survivors continue to suffer detrimental effects of the illness even after treatment completion. However, available data on post-TB disability is inadequate to support policy adoption. Therefore, there is an urgent need for increased advocacy, awareness and research to bridge knowledge gaps.

6. Metal Accumulation in Ekiti State's Three Major Dams' Water and Sediments, the Ecological Hazards Assessment and Consequences on Human Health

Abstract

Water is indispensable to life. Consequently, water and sediment contamination poses severe ecological threats to life. Thus, this investigation aimed to evaluate metal deposition in the sediments and surface water in Ekiti State's three dams and to analyze its potential ecological effects on man's bodily, social, and mental well-being. Metal levels in sediments and dam water were determined using Atomic Absorption Spectroscopy (AAS). Average values of the metals in Egbe, Ero, and Ureje dams, except for K, Mn, and Pb (in Ureje dam), were lower than the acceptable boundaries of local and foreign establishments. The values of the risk quotient (HQ) on the skin and consumption contacts with all metals (except Mn for ingestion exposure for children) were less than one in the Egbe, Ero, and Ureje dams for both adults and children. Consumption HQ values were higher than skin HQ values in the three dams for children and grown-ups. The total hazard index (HI) posed adverse non-carcinogenic risk to children in the catchment area of the dams while the adults were not affected by the non-carcinogenic hazard. The highest cancer hazard was found in the Ureje dam, while the lowest was in the Ero dam. Further, adults were prone to higher cancer risk than children. Using multiple pollution indices revealed that the sediments in Egbe, Ero, and Ureje dams were less contaminated by harmful metals in dry and wet periods. There is a need to reduce current polluting anthropogenic activities around the dams.

10. Effect of gamma irradiation and cooking on the physico-chemical properties, nutrients, and anti-nutrients compositions of egusi melon (Citrullus vulgaris) seeds.

Abstract

The effect of gamma irradiation and cooking on the physico-chemical and functional properties of “egusi” melon as well as its impact on quality attributes of the oil were investigated. The seeds of “egusi” melon were divided into five portions. One part was cooked, one irradiated at 5KGy, another was irradiated at 10KGy, and one part was cooked and irradiated at 10KGy as a double treatment method. The last part was left as is (raw) for reference. Irradiation was done using Cobalt-60 as gamma radiation source. Proximate compositions, mineral contents, amino acid contents, oil quality attributes, as well as anti-nutritional factors were determined for all samples using standard methods. Results revealed that gamma irradiation caused a decrease in values of moisture from the raw sample (5.80) with cooked, 10KGy and 5KGy irradiated samples of “egusi” melon showing reduced moisture contents of 4.27, 4.27 and 4.14% respectively. Protein decreased from 32.20% in raw “egusi” melon seeds to 29.77% and 29.47% in the 10KGy and 5KGy irradiated samples respectively. Mineral content of “egusi” melon seeds were affected by irradiation at 10 and 5kGy where the values of potassium increased from 14.93 in the raw to 20.88 in the 10KGy irradiated and 22.83 in the 5KGy irradiated samples. For the quality attributes of “egusi” melon oil, gamma irradiation caused significant decrease (P≤0.05) in iodine value and significant increase in acid and peroxide values. The treatments had profound effects on the Amino Acid Contents of “egusi” melon. Cooking increased the level of amino acids except for glutamic acid which decreased from 14.26 to 13.63 when cooked. Irradiation also had this effect, increasing in all but glutamic acid (14.26 to 13.83) but the combined treatment (cooking and Irradiation) showed an increase in glutamic acid content. Lysine, Histidine, Valine, and Leucine decreased when subjected to the combined treatment. The study showed that irradiation only caused little changes in the nutrient composition of “egusi” melon at low dosage highlighting its potential as a preservation and disinfection treatment for “egusi” melon. 11. Phytochemical, Antioxidant and Antimicrobial Analyses of Pterocarpus mildbraedii Stem Extracts.

Abstract

Phytochemical screening, antioxidant and antimicrobial activities of n-hexane, ethyl acetate and methanol extracts of Pterocarpus mildbraedii stem were studied in this research. Pterocarpus mildbraedii is used in folk medicine to treat fever, convulsions and respiratory disorders. The air-dried and milled plant materials were extracted with soxhlet apparatus at a temperature of 80 0C using n-hexane, ethyl acetate and methanol successively. The preliminary screening of the extracts was carried out using standard methods, and the free radical scavenging capacity using 2, 2 diphenyl-1-picryhydrazyl (DPPH) was determined to evaluate the antioxidant activity. Likewise, the antimicrobial screening was also carried out using the Broth dilution method. Phytochemical screening of the extracts (nhexane, ethyl acetate and methanol) revealed the presence of alkaloids, tannins, flavonoids, steroids, terpenes and phenol. However, tannin was not found in the n-hexane extract, while saponin and cardiac glycoside were also absent in all the extracts. The extracts were concentration dependent on the DPPH free radicals scavenging. The IC50 values were 0.8546, 0.8953 and 0.9081 µg/mL for the methanol, n-hexane and ethyl acetate extracts respectively. The antimicrobial screening showed that the methanol extract was still active at a concentration of 12.5 mg/mL. The result of this study suggests that the stem of Pterocarpus mildbraedii may be a good source of antioxidants and antimicrobial agents. Abstract

Milk and foods made from milk is manufactured into more stable dairy products of worldwide value, such as butter, cheese, ice cream, and yoghurt. Consumption of contaminated milk or dairy products by pathogens causes human gastrointestinal infection, which leads to diarrheal disease in human and hospitalization or death in severe cases especially among elderly and children. An assessment of milk and dairy products was designed to determine the microbiological quality of milk and dairy products consumed in Abakaliki, Nigeria. Culture techniques were usedfor isolation of enteric bacteria from retail dairy productsand disk diffusion method were used to determine the Antibiotic Resistance profile of isolates. Bacteria pathogens isolated were characterized and identified using morphological and biochemical techniques. SPSS and Chi-square test were used for the analysis of the study,P-value of 0.02 indicates a significant difference between the bacteria pathogens counts. A total of 161 pathogenic bacteria were isolated from 100 dairy products. Salmonellaspp heard (26.1%), Escherichia coli (44.1%) and Shigella spp. (29.8%). All identified isolates were found to be 100% susceptible to ciprofloxacin and gentamycin, with 66.7% for ofloxacin. Augmentin, ampicillin, chloramphenicol and spectinomycin was 100% resistant. Data obtained confirm that milk and dairy products retailed in Abakaliki pose a serious public health threat to consumers due to the presence of pathogenic bacteria. Standard and good storage conditions, as well as environmental and personnel hygiene should be practiced to prevent contamination of milk and dairy products for the safety of consumers. Abstract

The kinetics of bleaching process of oxidation of typical dye with a bleaching agent - sodium hypochlorite in aqueous medium was studied. The kinetic runs were executed using spectrophotometer to changes in concentration under pseudo first order whereby concentration of NaOCl was in large excess compared with the concentration of dye. The factors affecting the reaction rate that were studied include the concentration of dye and oxidant, temperature, ionic strength and pH of the bleaching reaction medium and the presence of a variable valence metal ion The result shows that the rate of oxidation increases with increasing in temperature, ionic strength and pH. Increasing in substrate and oxidant concentrations also increase the rate of oxidation. Higher observed rate constant k1 was obtained in the presence of Fe(III)ion. The Arrhenius activation energy for the oxidation in the absence and presence of Fe(III)ion are 56.21 kJmol-1 and 51.21 kJmol-1 respectively. The result of thermodynamic parameters such as the lowering of activation energy (Ea) and the higher value of second rate constants k2 in the presence of Fe (III) ion provide further support for Fe (III) ion enhancement of rate of oxidation. &nbsp

Abstract

Studies have shown the topical domestic use of hydraulic brake fluid and it has been reported by domestic users as efficacious against skin infections, especially dermatophytosis. Hydraulic brake fluid ointment is a combination of hydraulic brake fluid and a 30 g weight of simple ointment formulation. Its MIC on T. rubrum was determined using the micro-broth dilution method and a solid culture medium gave three different volumes of 0.2, 0.4, and 0.8 ml which were used to prepare percentage formulations of 30 g simple ointment. A total of 28 male albino rats (pathogen-free, 6-8 weeks old) weighing between 180-210 g, randomly divided into 7 groups of 4 rats each, was used for this experiment. The first and second groups received 30 g simple ointment administered constituting the negative (without dermatophytosis) and positive (with dermatophytosis) control groups. The third, fourth and fifth groups received 0.7, 1.3 and 2.7% formulation of the test agent in the simple ointment, respectively; the sixth group was topically given 0.2% ketoconazole gel; the standard group while the seventh group received 0.25 ml (equivalent to a drop) of hydraulic brake fluid (neat) without a simple ointment base applied on the infected paws of the group. Application was from the 10th day of post-infection and lasted for 2 weeks. Clinical response and fungal scores were significantly lower (p < 0.05) in T. rubrum - induced albino rats treated with 1.3, 2.7, and 0.25 ml (neat) concentrations of the test agent compared to infected non-treated group administered simple ointment alone. Abstract

Background The development of a fast and accurate, non-sputum-based point-of-care triage test for tuberculosis (TB) would have a major impact on combating the TB burden worldwide. A new fingerstick blood test has been developed by Cepheid (the Xpert MTB Host Response [MTB-HR] prototype), which generates a “TB score” based on messenger RNA (mRNA) expression of 3 genes. Here we describe the first prospective findings of the MTB-HR prototype. Methods Fingerstick blood from adults presenting with symptoms compatible with TB in South Africa, The Gambia, Uganda, and Vietnam was analyzed using the Cepheid GeneXpert MTB-HR prototype. Accuracy of the Xpert MTB-HR cartridge was determined in relation to GeneXpert Ultra results and a composite microbiological score (GeneXpert Ultra and liquid culture) with patients classified as having TB or other respiratory diseases (ORD). Results When data from all sites (n = 75 TB, 120 ORD) were analyzed, the TB score discriminated between TB and ORD with an area under the curve (AUC) of 0.94 (95% confidence interval [CI], .91–.97), sensitivity of 87% (95% CI, 77–93%) and specificity of 94% (88–97%). When sensitivity was set at 90% for a triage test, specificity was 86% (95% CI, 75–97%). These results were not influenced by human immunodeficiency virus (HIV) status or geographical location. When evaluated against a composite microbiological score (n = 80 TB, 111 ORD), the TB score was able to discriminate between TB and ORD with an AUC of 0.88 (95% CI, .83–.94), 80% sensitivity (95% CI, 76–85%) and 94% specificity (95% CI, 91–96%). Conclusions Our interim data indicate the Cepheid MTB-HR cartridge reaches the minimal target product profile for a point of care triage test for TB using fingerstick blood, regardless of geographic area or HIV infection status.

Abstract

Parkia filicoidea, Welw (Fabecea) plant parts are claimed to be used as herbal medicine in the management of inflammation. This claim however has not been substantiated, scientifically, hence the need for this study. The powdered stem bark of Parkia filocoidea was extracted by hot aqueous decoction, cooled and strained. Oral acute toxicity study was carried out in rats using Lorke’s method. Test doses of 100, 200 and 400 mg/kg of the extract were evaluated for antiinflammatory activity using Carrageenan- induced paw edema, dextran- induced paw edema and formalin- induced paw edema models of inflammation studies in albino rats. Indomethacin (10 mg/kg) and distilled water (2 ml/kg) were used as the positive and negative controls respectively. The oral acute toxicity study revealed no mortality or signs of toxicity in both phases of the study, even after 14 days. Doses of 100, 200 and 400 mg/kg of Parkia filocoidea extract produced significant (p<0.05) anti-inflammatory properties in the acute inflammatory models (carrageenan- and dextraninduced paw edema) but had no significant inhibitory effect in the sub-acute inflammatory model (formalin-induced paw edema) in rats when compared with the control. The effect of the extract is comparable to indomethacin in the acute models. The result is in tandem with the claim of the ethno medical use of the extract of Parkia filicoidea in the treatment of inflammatory conditions. [ABSTRACT FROM AUTHOR]

28. Bioaccumulation and depuration of zinc in the tissues of Heterobranchus longifilis exposed to zinc oxide nano-particles.

Abstract

Industries use zinc oxide nano-particles (ZnO-NPs) to produce most commercial and medicinal goods, but indiscriminate discharge of their effluents into the aquatic environment may accumulate in the tissues of the fish. The ability of these metal oxides to accumulate in the tissue of fish due to their nano size could lead to a reduction in the nutritional value of the fish. Therefore, this study aimed to evaluate the rate of zinc bioaccumulation in tissues of Heterobranchus longifilis. Juveniles of H. longifilis were exposed to sub-lethal concentrations (0.0, 6.00, 8.00, 10.00, 12.00 mg/l) of ZnO-NPs for 60 d and depurated for 30 d. At the end of each experiment, fish were sacrificed and analyzed for bioaccumulation. The increasing order of concentration of zinc was as follow: bone< muscle < blood < skin < kidney < liver < GIT < gills having 0.068mg/g to 0.263 mg/g from muscle to gills. That was above the permissible limits (0.050 mg/g) in fish tissues, except in the bone with 0.054 mg/g, which was still within the acceptable limits. Gills accumulated the highest (0.263 mg/g) content of zinc, and bone had the lowest (0.054 mg/g). After the depuration period, the content of zinc in the tissues decreased in all the tissues but still remained above the maximum permissible limits in gills, GIT, kidney, liver and skin (0.175 mg/g, 0.162 mg/g, 0.066 mg/g, 0.071 mg/g and 0.073 mg/g respectively). However, the bone, blood and muscle were able to depurate the zinc content to values (0.031 mg/g, 0.034 mg/g, and 0.044 mg/g, respectively) below the acceptable limits of Zn in fish tissues. The results indicated that zinc from ZnO-NPs bioaccumulated in H. longifilis tissues, but the fish could depurate the metal naturally. BACKGROUND: The COVID-19 pandemic has caused major disruption to healthcare services globally and has impacted on tuberculosis (TB) patients and TB diagnosis and treatment services both in low- and high-income countries. We therefore explored the perspectives of members of regional and international TB control and research networks to further understand TB service disruptions and compared the experiences of members from West African and European countries. METHODS: This cross-sectional, explorative descriptive study was conducted from May to July 2020 using an open online survey with target respondents from both West African and European countries. The survey comprised discrete questions exploring challenges faced with TB screening, diagnosis, treatment, prevention, and changes implemented. Additionally, respondents were asked to provide recommendations for remedial actions. RESULTS: We analysed responses from 124 respondents based in 29 countries located in Europe and West Africa. About half of the respondents reported challenges in delivering routine TB services during the COVID-19 pandemic, with over one third reporting having some form of guidance issued regarding maintaining delivery of routine TB services. Respondents emphasised the need for strengthening TB services especially in light of COVID-19 pandemic. Considerable similarities were found between the challenges experienced by TB professionals in both West African and European settings. Responses also highlighted the hidden challenges faced in some countries prior to the COVID-19 pandemic, especially in some West African settings where staff shortages and laboratory issues predated COVID-19. Abstract

In some low and middle-income countries, 10–20% of patients presenting with a persistent cough have tuberculosis (TB). Once TB is excluded, health service provision for alternative diagnoses is limited. We prospectively studied patients with two Xpert-negative sputum results presenting to a TB clinic in The Gambia. Of 239 patients, 108 did not have TB; 65/102 (6 were lost to follow-up) had alternative diagnoses, 24.6% of which were non-respiratory; 37/102 had no diagnosis, 27.0% of whom were HIV-1-positive; 37.8% had a history of TB and 24.3% smoked. We highlight the need for general health service integration with TB platforms and exploration of non-TB patients with chronic respiratory symptoms.

Abstract

Penile agenesis is complete absence of the penis in an otherwise normal phenotypic and genotypic male at birth that results from failure of development of the genital tubercle. It is an extremely rare anomaly that may be associated with anomalies in other organ systems, the extent and severity of which may affect the prognosis. The management is challenging and may have far reaching implications for the individual and family. While gender reassignment with bilateral orchidectomy and feminising genitoplasty has been carried out for most patients, significant psychosexual and social issues related to the male identity may occur due to foetal or postnatal sex steroid imprinting. We report a neonate with penile agenesis with bilateral renal agenesis and anorectal malformation.

45. Private information, institutional distance, and the failure of cross-border acquisitions: Evidence from the banking sector in Central and Eastern Europe \ud

Abstract

In this paper, we develop an information theory-based framework about cross-border acquisitions in the financial intermediation industry. We argue that even though “soft” information embedded in customer relationships of local banks can, in principle, help multinational banks (MNBs) overcome informational disadvantage in host countries, the cost of verification of this private information may, paradoxically, make local banks with significant customer relationships unattractive for cross-border acquisition. Further, we propose that the relationship between the amount of customer information embedded in an incumbent bank and the likelihood of its acquisition by a MNB is modified by the institutional distance between the home and host countries of the MNB. Specifically, the strength of the negative relationship increases with institutional distance between home and host countries because it increases the verification cost of private information with institutional distance. Our hypotheses find support in the context of Central and Eastern Europe.

Abstract

Phyllanthus amarus is a medicinal plant with immense benefits. Several experimental investigations have explored and establish its phytochemical constituents. This study was carried out to evaluate the antioxidant potentials and inhibitory effects of the hydro-alcoholic extract of the leaves on two major enzymes involved in the hydrolysis of extracellular ATP- Nucleoside triphosphate diphosphohydrolase (NTPDases) and 5?-nucleotidase in rat tissues using in-vitro models. The extract shows a dose dependent scavenging abilities against free radicals such as 2,2 diphenyl-2-picrylhydrazyl (DPPH), hydrogen peroxide (H2O2), hydroxyl radical, superoxide radical, and nitric oxide(NO) radicals. The ferric reducing power, Vitamin C content and total phenol content also increases as the concentration increases. The extract of Phyllanthus amarus showed significant inhibition (P< 0.05) of NTPDase and 5?-nucleotidase enzymes in rat tissues with increasing concentrations. Research has shown that targeting these purinergic enzymes could be a novel target for cancer immunotherapy and have been projected to be valuable tools in clinical therapeutics such as in the treatment of chronic pain, immune system diseases and cancer, as they would prolong the physiological effects of extracellular nucleotides or simultaneously administered nucleotide analogues. The hydro- alcoholic extract of Phyllanthus amarus with its high radical scavenging ability and inhibitory effects on the purinergic enzymes could be explored in the treatment of chronic pain, cancer and immune system related diseases.

Abstract

In this work, a new hybrid method has been proposed which extends K-means algorithm to categorical domain and mixed-type attributes. Also proposed is a new dissimilarity measure that uses relative cumulative frequency-based method in clustering objects with mixed values. The dissimilarity model developed could serve as a predictive tool for identifying attributes of objects in mixed datasets. It has been implemented using JAVA programming language and MATLAB. Experiments on real-world datasets show that the new hybrid algorithm is more efficient and more robust when compared with existing ones in terms of accuracy and time complexity. This tool can be used in a variety of applications such as in agro-based industries, in clinical datasets and in general information retrieval system (IRS).
