This article discusses a retrospective cohort study that aimed to predict relapse in patients with chronic spontaneous urticaria (CSU) who were treated with omalizumab. CSU is a condition that affects a small percentage of the population and significantly impacts patients' quality of life. Omalizumab is the only registered therapy for CSU, but its discontinuation remains uncertain. The study found that approximately 30% of patients achieved long-term remission after 24 weeks of therapy, and most relapse episodes occurred within the first 6 weeks after treatment discontinuation. Factors such as higher Urticaria Activity Score (UAS7) after 24 weeks, longer time to treatment, and prior relapses increased the risk of relapse, while Hashimoto's disease favored remission. The findings can help determine appropriate treatment duration and withdrawal regimens. Further research is needed to optimize omalizumab withdrawal and identify additional predictive factors for long-term remissions and relapse rates in CSU patients. [Extracted from the article]

(1): Atopic dermatitis and psoriasis vulgaris are chronic, inflammatory diseases. Clinical presentation usually leads to a proper diagnosis, but sometimes neither clinical examination nor histopathological evaluation can be conclusive. Therefore, we aimed to build up a novel diagnostic tool and check it for accuracy. The main objective of our work was to differentiate between healthy skin (C), atopic dermatitis (AD) and psoriasis vulgaris (PV) biopsies on the base of involucrin (IVL) and human β-defensin-2 (hBD-2) concentrations and their mRNA, as well as mRNA for TPP2 and PSMB8. (2): ELISA for IVL and hBD-2 proteins and Real-time PCR for the relative expression of mRNA for: IVL (IVL mRNA), hBD-2 (hBD-2 mRNA), PSMB8 (PSMB8 mRNA) and TPP2 (TPP2 mRNA), isolated from skin biopsies taken from AD and PV patients and healthy volunteers were performed. (3): hBD-2 mRNA and PSMB8 mRNA correlated with some parameters of clinical assessment of inflammatory disease severity. hBD-2 mRNA expression, exclusively, was sufficient to distinguish inflammatory skin biopsies from the healthy ones. Background: Non-melanocytic benign skin tumours encompass a diverse group of lesions, classified based on their cellular origin, such as epidermal, vascular, fibrous, neural, muscle, and adnexal tumours. Though they often reveal solitary lesions, multiple skin tumours focus on genodermatoses. Each syndrome exhibits distinct clinical characteristics and potential complications, including cutaneous and extra-cutaneous malignancies, some of which are potentially life-threatening. Diagnosing genetic syndromes is complex and requires numerous histopathological and immunohistochemistry tests due to similarities between the adnexal tumours and basal cell carcinoma upon pathology. Methods: To illustrate the clinical practice, we conducted a retrospective case study that included eleven patients with genodermatoses referred to a tertiary dermatology clinic from September 2018 to April 2024. We have also conducted a research study on available treatment modalities in this setting. Results: Five patients with excellent aesthetic results were treated using a recently approved FDA plasma device. After searching SCOPUS and PubMed database records, we assessed 96 original articles to present current knowledge regarding the dermato-surgical approach. Conclusions: Multiple skin tumours, especially on the face, may significantly affect patients' quality of life and have psychological consequences. An appropriate treatment selection tailored to the patient's needs should be provided. There is no standardised treatment for multiple benign tumours in genodermatoses, and selected methods with varying efficacy are employed. The paper presents recommendations concerning the surveillance of patients after the treatment of squamous-cell carcinoma (SCC), basal-cell carcinoma (BCC) and Merkel-cell carcinoma (MCC) based on the current European and American recommendations. This overview discusses the methodology and detailed recommendations concerning the post-treatment surveillance, with special attention to the clinical examination, dermatoscopy, imaging diagnostics and patient education. The recommendations emphasise the significance of early monitoring for recurrences, and early detection of new skin cancers, adapted to individual risk factors in a patient and the characteristics of primary cancer. Actinic keratosis is a common skin condition that has the potential to develop into malignant neoplastic lesions. Factors that are known to increase the likelihood of developing actinic keratosis include older age, male gender, UV exposure, and immunosuppressive treatment. Understanding the pathogenesis and significance of actinic keratosis as an initial form of the neoplastic process associated with a sequence of lesions arising on photodamaged skin has led to the concept of field cancerization. The choice of the optimal therapeutic pathway depends on the number of actinic keratosis lesions and the degree of photodamage in the skin, as well as the advancement of skin lesions. The clinical presentation of skin lesions also allows monitoring of the patient's condition over time and assessing response to treatment, and facilitates rational decisions regarding potential therapy adjustments. This article outlines the guidelines for the diagnosis and treatment of actinic keratosis in Poland and presents an evidence-based management regimen that incorporates the practical clinical experience of dermatology experts. Hidradenitis suppurativa is a chronic, recurrent, inflammatory skin disease that affects hair follicles, typically developing after puberty. It is characterized by painful, deep-seated inflammatory lesions that appear in anatomical regions where apocrine glands are located, mainly in the axillar, inguinal, and anogenital areas. The pathogenesis of hidradenitis suppurativa is multifactorial, with follicular occlusion representing the initial stage of the disease and subsequently leading to the formation of neutrophilic abscesses and infiltration by various inflammatory cells. The treatment of hidradenitis suppurativa requires a multidisciplinary approach, involving dermatologists and dermatologic surgeons, as well as physicians of other medical specialties. The primary pharmacological treatments for hidradenitis suppurativa involve antibiotics and biologic disease-modifying drugs, which can be used either in monotherapy or in combination with surgical interventions. Scabies is a highly infectious dermatosis. It is caused by the mite Sarcoptes scabiei var. hominis. The available estimates indicate more than 400 million cases of scabies annually. The main route of infection is direct skin contact. The main symptoms such as pruritus appear 3-6 weeks after infection. Additionally, papules, erythema, erosions, excoriations, and crusts may be visible. One of the rarest forms of scabies is Norwegian scabies. This is the most severe variant. Its clinical picture may resemble psoriasis. In the article, we present the case of a 21-year-old patient with Down syndrome who was admitted to the department of dermatology with a diagnosis of psoriasis. During hospitalization, due to clinical doubts, it was decided to extend the diagnostics. Basing on the microscopic examination, Norwegian scabies was diagnosed. The treatment involved oral ivermectin and topical 5% permethrin, which resulted in a remission of the skin lesions and the resolution of itching. [ABSTRACT FROM AUTHOR]

Background: Although the role of melanoma risk factors is well documented, their correlation with patients' age is less frequently analyzed. Method: The analysis was performed among 189 melanoma patients in different age groups, including <30 years, 31–60 years, and >60 years, to investigate the risk factors, topography, and coexistence of morphological features of 209 melanomas (dermoscopic and histopathological). Results: Among the youngest age group, no correlation with the presence of estimated risk factors was found. The most common dermoscopic pattern was spitzoid and multicomponent asymmetric. The group of middle-aged patients was the most diverse in terms of the occurrence of risk factors, solar lentiginosis, dermoscopic patterns, topography, histological subtypes, and invasiveness of melanomas. The oldest group characterized a strong correlation between solar lentiginosis, NMSC comorbidity, the prevalence of facial melanomas, the dermoscopic pattern of melanoma arising on chronic sun-damaged skin, and regression. Conclusion: The findings regarding the presence of age-specific features in melanoma patients, especially in the youngest and middle-aged groups, might be helpful for clinicians and to target secondary prevention efforts. Superficial mucoceles (SM) are small, benign, translucent vesicles, which develop in the oral mucosa, mainly on the lower lip, due to a rupture of minor salivary gland ducts and the extravasation of saliva. The use of immune checkpoint blockade treatment may lead to dermatologic immune-related adverse events in 40-50% of patients and with severe dermatologic immune adverse events of Common Terminology Criteria for Adverse Events of grade G3-G4 in 1-2% of patients. The present study described the case of a patient with squamous cell carcinoma treated with cemiplimab with the adverse effect of Stevens-Johnson syndrome. The patient developed multiple SM, which made it challenging for the patient to speak and eat. Ablative treatment using a plasma device (Plasma IQ) and electrocoagulation were used to remove all lesions, which achieved a precise and timely therapeutic effect without any remainder of scarring. Through the publication of the present case report and literature review, the present article aimed to enhance the understanding of this condition, providing valuable diagnostic and therapeutic information about the spectrum of mucosal involvement among drug-related toxicities of current oncological treatment. Introduction: Infliximab (IFX) is a monoclonal antibody that binds to and neutralizes TNF-a. IFX (Remicade) was approved by the U.S. Food and Drug Administration in 2006 for the treatment of severe plaque psoriasis. In 2013 two infliximab biosimilars: Remsima and Inflectra were also registered. The introduction of biosimilar drugs is associated with a significant reduction in treatment costs. Aim: To evaluate the efficacy of treatment with biosimilar IFX with non-medical switch option in patients with plaque psoriasis under the drug program "Treatment of moderate and severe plaque psoriasis" of the Ministry of Health in Poland. Material and methods: The group of 91 adult patients with moderate to severe plaque psoriasis, unresponsive or with contraindications to the standard treatment were qualified to the drug program (in 2016-2018). Efficacy of treatment with biosimilar IFX was evaluated using the Psoriasis Area and Severity Index, body surface area and Dermatology Life Quality Index scoring performed at week 0, 14, 46 and 94. Results: The mean change in PASI, DLQI, and BSA scores at week 14 was 89.92%, 93.75% and 90.91%, respectively. By week 14, 83.52% of patients achieved PASI75, 49.45% PASI = 90 and 26.37% PASI100. At week 46, 84.62% of patients achieved PASI75, 54.95% PASI = 90, and 21.98% PASI100. At week 94 of therapy, 80.22% of patients achieved PASI75, 48.35% PASI = 90, and 18.68% PASI100. At week 94 of therapy, PASI100 was maintained by 37.5% of patients who achieved PASI100 at week 14. Conclusions: 94-week therapy with biosimilar infliximab results in high and sustained clinical efficacy in patients with moderate to severe psoriasis. [ABSTRACT FROM AUTHOR]

Neuroendocrine neoplasms (NENs) are a heterogeneous group of rare tumours derived from peptidergic neurons and specialized neuroendocrine cells capable of secreting various peptides or amines. These cells may be present in endocrine tissue or diffused in the tissues of the digestive or respiratory system. The article reviews the characteristic features of NENs, with particular emphasis on skin manifestations, such as necrolytic migratory erythema (NME), tongue inflammation, angular cheilitis, venous thrombosis and alopecia in glucagonoma; "flushing", "lion face", pellagra skin symptoms, "scleroderma-like features without Raynaud's phenomenon" in carcinoid tumours. The paper also presents the clinical picture of the neuroendocrine tumour of the skin - Merkel cell carcinoma. The aim of this study was to draw attention to the need for precise and comprehensive diagnosis of the patients, with particular emphasis on skin lesions as a revelator of neuroendocrine tumours.
