Despite reporting an overall normal life, survivors of heritable retinoblastoma face numerous physical and psychosocial issues. In particular, reproductive decision-making is often complex and difficult. This study aims to examine survivors' reflections on passing on heritable retinoblastoma to their children, how survivors approach their reproductive choices, and how the healthcare system can optimize counseling and support. Semi-structured interviews with Danish adult survivors of heritable retinoblastoma were qualitatively analyzed to explore their experiences. Participants were recruited from the Retinoblastoma Survivorship Clinic, Aarhus University Hospital, Denmark. Thematic data analysis was conducted followed by a condensing process specifically for the subthemes relating to reproductive choices. A common subtheme for all participants was a strong wish to avoid passing on retinoblastoma to their children. The participants emphasized the various medical, practical, emotional, and moral issues impacting their final reproductive choice in the process of family planning to conceive a child unaffected by retinoblastoma. Some had no option other than to conceive naturally and hope for an unaffected baby; while others weighed the pros and cons of choosing natural conception with prenatal testing and then considering termination of pregnancy (in case of an affected fetus) versus choosing fertility treatment with preimplantation genetic testing to achieve an unaffected pregnancy. Several participants underlined the complexity of their decisions, and also expressed feelings of guilt, both toward their affected child, and guilt for putting their partner through many difficult decisions and obstacles due to their genetic condition. Our findings demonstrate how one family-planning decision is not unequivocally "better" or easier than another. Background Tumor hypoxia is associated with resistance to radiotherapy and chemotherapy. In head and neck squamous cell carcinoma (HNSCC), nimorazole, an oxygen mimic, combined with radiotherapy (RT) enabled to improve loco-regional control (LRC) in some patients with hypoxic tumors but it is unknown whether this holds also for radio- chemotherapy (RCTx). Here, we investigated the impact of nimorazole combined with RCTx in HNSCC xenografts and explored molecular biomarkers for its targeted use. Methods Irradiations were performed with 30 fractions in 6 weeks combined with weekly cisplatin. Nimorazole was applied before each fraction, beginning with the first or after ten fractions. Effect of RCTx with or without addition of nimorazole was quantified as permanent local control after irradiation. For histological evaluation and targeted gene expression analysis, tumors were excised untreated or after ten fractions. Using quantitative image analysis, micromilieu parameters were determined. Results Nimorazole combined with RCTx significantly improved permanent local control in two tumor models, and showed a potential improvement in two additional models. In these four models, pimonidazole hypoxic volume (pHV) was significantly reduced after ten fractions of RCTx alone. Our results suggest that nimorazole combined with RCTx might improve TCR compared to RCTx alone if hypoxia is decreased during the course of RCTx but further experiments are warranted to verify this association. Differential gene expression analysis revealed 12 genes as potential for RCTx response. When evaluated in patients with HNSCC who were treated with primary RCTx, these genes were predictive for LRC. Conclusions Nimorazole combined with RCTx improved local tumor control in some but not in all HNSCC xeno-grafts. We identified prognostic biomarkers with the potential for translation to patients with HNSCC.

Editorial In summary, ESTRO clinical and other guidelines are soundly based, but always require validation, careful and consistent implementation, and focused education and training on their use, as well as local monitoring of that. As guidelines develop and older ones become superseded, there should be a clear mechanism to inform the community of the status of specific guidelines, especially when they have been replaced by new or updated versions. An important scientific aim for the future is to further advance guidelines and their individual statements into fully evidence-based instruments. These, in principle, could be directly linked to growing data-bases, allowing feedback mechanisms and thus continuous optimization. Another challenging research topic is the interplay of guidelines with growing opportunities and demands of personalized approaches of treatment.

This issue includes a perspective contribution by Lefresne et al. [1] on face-to-face scientific conferences versus virtual meetings and the implications for climate change. Generally, it is the policy of the Green Journal not to publish contributions with non-scientific content. In this specific case, the editors believe that the contribution by Lefresne and colleagues raises an important point that needs careful consideration and discussion in our scientific community. As scientists, we are at the forefront of research and strive to positively shape the future for humankind. Therefore, it is of key importance that the environmental impact of scientific activities is considered, and that our community develops and contributes to solutions to prevent further exacerbation of climate change with its substantial impact on health-related issues. This includes that pros and cons of face-to-face scientific meetings are carefully weighed.

Item does not contain fulltext, PURPOSE: Osteoradionecrosis (ORN) of the mandible is a serious complication of head and neck radiotherapy. This study aims to investigate the effect of hyperbaric oxygen (HBO) treatment on ORN in two randomized, controlled multicentre trials. METHODS AND MATERIALS: Patients with ORN with indication for surgical treatment were randomised to either group 1: surgical removal of necrotic mandibular bone supplemented by 30 pre- and 10 postoperative HBO exposures at 243 kPa for 90 min each, or group 2: surgical removal of necrotic bone only. Primary outcome was healing of ORN one year after surgery evaluated by a clinically adjusted version of the Common Toxicity Criteria of Adverse Events (CTCAE) v 3.0. Secondary outcomes included xerostomia, unstimulated and stimulated whole salivation rates, trismus, dysphagia, pain, Activities of Daily Living (ADL) and quality of life according to EORTC. Data were combined from two separate trials. Ninety-seven were enrolled and 65 were eligible for the intent-to-treat analysis. The 33% drop-out was equally distributed between groups. RESULTS: In group 1, 70% (21/30) healed compared to 51% (18/35) in group 2. HBO was associated with an increased chance of healing independent of baseline ORN grade or smoking status as well as improved xerostomia, unstimulated whole salivary flow rate, and dysphagia. Due to insufficient recruitment, none of the endpoints reached a statistically significant difference between groups. ADL data could only be obtained from 50 patients. CONCLUSION: Hyperbaric oxygen did not significantly improve the healing outcome of osteoradionecrosis after surgical removal of necrotic bone as compared to standard care (70% vs. 51%). This effect is not statistically significant due to the fact that the study was underpowered and is therefore prone to type II error.

Sex-based differences in physiological traits may be influenced by both evolutionary and environmental factors. Here we used male and female flies from >80 Drosophila species reared under common conditions to examine variance in a number of physiological traits including size, starvation, desiccation and thermal tolerance. Sex-based differences for desiccation and starvation resistance were comparable in magnitude to those for size, with females tending to be relatively more resistant than males. In contrast thermal resistance showed low divergence between the sexes. Phylogenetic signal was detected for measures of divergence between the sexes, such that species from the Sophophora clade showed larger differences between the sexes than species from the Drosophila clade. We also found that sex-based differences in desiccation resistance, body size and starvation resistance were weakly associated with climate (annual mean temperature/precipitation seasonality) but the direction and association with environment depended on phylogenetic position. The results suggest that divergence between the sexes can be linked to environmental factors, while an association with phylogeny suggests sex-based differences persist over long evolutionary time-frames.

A look through almost 2000 abstracts submitted for the upcom- ing ESTRO 2022 meeting together with a glance back on the 2021 papers published in Radiotherapy and Oncology gives one a good impression of the (current) key focus areas in radiation oncology: Almost all of this work relates to optimal delivery of radiation ther- apy in terms of technology, quality assurance and morbidity reduc- ing approaches. Thus, at present the research questions considered as most relevant for radiotherapy of e.g. lung and oesophageal can- cer are not related to tumour control, but to the late risk of cardiac disease in the patients who are lucky to survive their cancer long enough to develop such problems [1]. The same scenario is found in the patient cohort that constitutes the largest indication for radiotherapy in Europe: women with early breast cancer [2]. In these examples, and in many other situations where radiotherapy is applied with a curative intent, less focus has currently been given to the aim or indication of the treatment, namely the control of loco-regional malignant disease. Of course, since Holthusen’s seminal paper in 1936 [3] the overall aim of radiotherapy, as stated over and over again by all teachers in the field (including the authors of this editorial), is uncomplicated tumour control, i.e. loco-regional tumour control without severe normal tissue damage (therapeutic ratio). This implies that rigorous study of the effects of radiotherapy on normal, non-tumour, tissues is an absolute necessity. Yet, the prescription of radiotherapy in clinical practice is done to kill tumour cells for local and loco-regional control. If the effects of radiotherapy on tumours are shifting out of focus, it might be taken for granted that the indication, dose, fractionation, and potential multidisciplinary interactions in this field are fully understood, and what remains is the fine tuning of the associated risk of morbidity.

Biological modeling for anti-cancer treatments using mathematical models can be very supportive in gaining more insight into dynamic processes responsible for cellular response to treatment, and predicting, evaluating and optimizing therapeutic effects of treatment. This review presents an overview of the current status of biological modeling for hyperthermia in combination with radiotherapy (thermoradiotherapy). Various distinct models have been proposed in the literature, with varying complexity; initially aiming to model the effect of hyperthermia alone, and later on to predict the effect of the combined thermoradiotherapy treatment. Most commonly used models are based on an extension of the linear-quadratic (LQ)-model enabling an easy translation to radiotherapy where the LQ model is widely used. Basic predictions of cell survival have further progressed toward 3 D equivalent dose predictions, i.e., the radiation dose that would be needed without hyperthermia to achieve the same biological effect as the combined thermoradiotherapy treatment. This approach, with the use of temperature-dependent model parameters, allows theoretical evaluation of the effectiveness of different treatment strategies in individual patients, as well as in patient cohorts. This review discusses the significant progress that has been made in biological modeling for hyperthermia combined with radiotherapy. In the future, when adequate temperature-dependent LQ-parameters will be available for a large number of tumor sites and normal tissues, biological modeling can be expected to be of great clinical importance to further optimize combined treatments, optimize clinical protocols and guide further clinical studies.
