Abstract

There are different BCR-ABL1 fusion genes that are translated into proteins that are different from each other, yet all leukemogenic, causing chronic myeloid leukemia (CML) or acute lymphoblastic leukemia. Their frequency has never been systematically investigated. In a series of 45503 newly diagnosed CML patients reported from 45 countries, it was found that the proportion of e13a2 (also known as b2a2) and of e14a2 (also known as b3a2), including the cases co-expressing e14a2 and e13a2, was 37.9% and 62.1%, respectively. The proportion of these two transcripts was correlated with gender, e13a2 being more frequent in males (39.2%) than in females (36.2%), was correlated with age, decreasing from 39.6% in children and adolescents down to 31.6% in patients ≥ 80 years old, and was not constant worldwide. Other, rare transcripts were reported in 666/34561 patients (1.93%). The proportion of rare transcripts was associated with gender (2.27% in females and 1.69% in males) and with age (from 1.79% in children and adolescents up to 3.84% in patients ≥ 80 years old). These data show that the differences in proportion are not by chance. This is important, as the transcript type is a variable that is suspected to be of prognostic importance for response to treatment, outcome of treatment, and rate of treatment-free remission.

Abstract

A case report is presented in which the rare occurrence of tongue cancer in a young person is linked to habitual use of marijuana and tobacco. The 23-year-old man had an extremely painful cancer located on the tongue, but not extending to the floor of the mouth. The patient smoked 20 cigarettes per day, did not drink alcohol, and was a frequent marijuana smoker. Poor oral hygiene, untreated dental disease, other systemic conditions, and infections (such as syphilis) have previously been associated with tumors of the tongue, but were apparently not involved in this case. The patient had approximately half of his tongue removed (a hemiglossectomy), and the muscles, major veins and lymph nodes on both sides of his neck were dissected and removed ''en bloc'', known as a bilateral neck dissection. Despite this surgical course, and a review of the tissue by pathologists which showed containment of the disease within the removed tissues, the disease recurred approximately one year later. The patient was treated with radiotherapy, but the disease persisted in the area of the neck. While by itself one case does not point to marijuana as a cause, the medical literature suggests that marijuana use, perhaps potentiated by tobacco use, may be involved in the production of carcinoma. It is particularly unusual for a young person to experience tongue cancer, and this case is presented both to indicate a potential effect of the use of marijuana, and to point out the extremely aggressive nature of this disease. The authors believe that the course of treatment in this case may have been too conservative, and suggest that radiation therapy be used in all similar cases despite a finding of negative resection margins (when all the tumor is contained within the specimen removed). Abstract

Intraoperative nerve monitoring (IONM) aimed at reducing the injuries of recurrent laryngeal nerve during thyroidectomy is controversial. We conducted a meta-analysis to assess the incidence of nerve injuries with or without IONM. Studies published from January 1994 to February 2012 in English language on humans were identified. Heterogeneity of studies was checked by the Higgins test. Summary estimates of predictive values of injury were made using the Mantel-Haenszel test based on the fixed-effects model. Publication bias was assessed by a funnel plot and Egger's method. Eight articles were selected accounting a total of 5257 nerves at risk. IONM revealed a significant impact in preventing transient injuries (positive predictive value = 5% [95% CI: 2-8], negative = 96% [95% CI: 91-100], relative risk = 0.73 [95% CI: 0.54-0.98], p = 0.035), whereas they failed to demonstrate effect on permanent injuries (positive predictive value = 2% [95% CI: 0.6-3.8], negative 99% [95% CI: 97-100], relative risk = 0.73 [95% CI: 0.44-1.23], p = 0.235). This meta-analysis demonstrated the merit of IONM in preventing transient injury during thyroidectomy. No advantage was found in permanent injuries.L'utilità del monitoraggio intraoperatorio dei nervi periferici (IONM), allo scopo di ridurre il danno dei nervi ricorrenti nel corso della chirurgia tiroidea, rappresenta ancora un aspetto controverso. Abbiamo, quindi, compiuto una metaanalisi, allo scopo di definire l'incidenza di danno nervoso con e senza IONM. Sono stati dapprima identificati gli studi eseguiti dal gennaio 1994 fino al febbraio 2012 in lingua inglese. L'eterogeneità degli studi presi in considerazione è stata valutata con il test di Higgins. Una valutazione complessiva del valore predittivo di danno è stata eseguita utilizzando il test di Mantel-Haenszel, basato sul modello effetti-fissi. Eventuali problemi legati alla pubblicazione sono stati valutati con un funnel plot e il metodo di Egger. Sono stati, alla fine, selezionati 8 lavori, per un numero complessivo di tronchi nervosi a rischio pari a 5.257.
