High energy states in the self-conjugate 16O nucleus were investigated by analyzing 13C(3He,α)12C reactions in the bombarding energy range 1.4–2.2 MeV. Thanks to a low-threshold and high-resolution array, we succeed in measuring cross sections of the decay channel where an α particle is emitted in coincidence with a 12C nucleus in the Hoyle state, leading to 4α particles in the final channel. The analysis of angular distributions indicates the occurrence of two low-spin resonant states, respectively with Jπ=2+ and 3−, at energies of ≈ 24.1 and 24.5 MeV. In this energy window, the observed branching ratios for the transition involving the emission of the Hoyle state are well larger than calculations based on barrier penetration, signaling the possible occurrence of high-energy states in 16O with a large cluster component.

BackgroundCoronavirus disease 2019 (COVID-19) vaccine effectiveness (VE) studies are increasingly reporting relative VE (rVE) comparing a primary series plus booster doses with a primary series only. Interpretation of rVE differs from traditional studies measuring absolute VE (aVE) of a vaccine regimen against an unvaccinated referent group. We estimated aVE and rVE against COVID-19 hospitalization in primary-series plus first-booster recipients of COVID-19 vaccines.MethodsBooster-eligible immunocompetent adults hospitalized at 21 medical centers in the United States during December 25, 2021-April 4, 2022 were included. In a test-negative design, logistic regression with case status as the outcome and completion of primary vaccine series or primary series plus 1 booster dose as the predictors, adjusted for potential confounders, were used to estimate aVE and rVE.ResultsA total of 2060 patients were analyzed, including 1104 COVID-19 cases and 956 controls. Relative VE against COVID-19 hospitalization in boosted mRNA vaccine recipients versus primary series only was 66% (95% confidence interval [CI], 55%-74%); aVE was 81% (95% CI, 75%-86%) for boosted versus 46% (95% CI, 30%-58%) for primary. For boosted Janssen vaccine recipients versus primary series, rVE was 49% (95% CI, -9% to 76%); aVE was 62% (95% CI, 33%-79%) for boosted versus 36% (95% CI, -4% to 60%) for primary.ConclusionsVaccine booster doses increased protection against COVID-19 hospitalization compared with a primary series. Comparing rVE measures across studies can lead to flawed interpretations of the added value of a new vaccination regimen, whereas difference in aVE, when available, may be a more useful metric.

The structure of 74Se at low energy was investigated via spectroscopy of internal conversion electrons at the INFN Legnaro National Laboratories (LNL). A set of internal K-conversion coefficients and monopole transition strengths was measured. A large ρ2(E0;22+→21+)⋅103=210(130) value was deduced. This result, in addition to a low upper limit for the 03+→02+ electron transition, casts in doubt a simple interpretation of the 74Se low-lying structure, in particular the recently proposed spherical, vibrational character. New microscopic beyond-mean-field calculations generally agree with the experimental results and are capable of producing a large ρ2(E0;22+→21+) value, even if still a factor ≈7 smaller than the experiment. Triaxiality and a complex shape-coexistence and mixing scenario seem responsible for this unexpected experimental result.

We report some preliminary experimental results on 13C(3He,α)12C reactions at low bombarding energies, in the range 1.4 – 2.2 MeV. The reconstruction of kinematic quantities in the final channel was performed by using a high-performance, low-threshold hodoscope, allowing for the particle identification even for low energy ejectiles. We succeeded in measuring detailed angular distributions, in absolute values, even for the transition leading to the emission of 12C in the Hoyle state, then resulting into 4 α particles in the final channel. The analysis of angular distributions as a function of energy suggests the occurrence of two low-spin resonant states, respectively with Jπ = 1− and 2+, at excitation energies of ≈ 24.1 and 24.5 MeV.

Malnutrition is a common condition in cancer patients which is usually associated with functional limitations, as well as increased morbidity and mortality. Based on the support of the young sections of Italian Association of Medical Oncology (AIOM), Italian Association of Radiotherapy and Clinical Oncology (AIRO) and Italian Society of Surgical Oncology (SICO) merged into the NutriOnc Research Group, we performed a multidisciplinary national survey with the aim to define the awareness of nutritional issues among healthcare professionals delivering anticancer care. The questionnaire was organized in four sections, as follows: Knowledge and practices regarding Nutritional Management of cancer patients; Timing of screening and assessment of Nutritional Status; Nutritional Treatment and prescription criteria; Immunonutrition and educational topics. The modules focused on esophagogastric, hepato-bilio-pancreatic and colorectal malignancies. Overall, 215 physicians completed the survey. As regards the management of Nutritional Status of cancer patients, many responders adopted the ERAS program (49.3%), while a consistent number of professionals did not follow a specific validated nutritional care protocol (41.8%), mainly due to lack of educational courses (14.5%) and financial support (15.3%). Nearly all the included institutions had a multidisciplinary team (92%) to finalize the treatment decision-making. Cancer patients routinely underwent nutritional screening according to 57.2% of interviewed physicians. The timing of nutritional assessment was at diagnosis (37.8%), before surgery (25.9%), after surgery (16.7%), before radiochemotherapy (13.5%) and after radiochemotherapy (7%). Most of the responders reported that nutritional status was assessed throughout the duration of cancer treatments (55.6%). An important gap between current delivery and need of nutritional assessment persists. The development of specific and defined care protocols and the adherence to these tools may be the key to improving nutritional support management in clinical practice.

BackgroundAlthough serum sodium concentration, particularly hyponatremia, has been shown to be a prognostic marker of survival in metastatic renal cell carcinoma (mRCC), the impact of normal sodium levels has not been investigated. Objective. To determine the best cut-off level of pregnant women’s first fasting plasma glucose (FFPG) test results for the prediction of subsequent onset of gestational diabetes mellitus (GDM) and to examine the association between FFPG and maternal and neonatal outcomes in a large Caucasian population. Methods. 1437 medical records of women with singleton pregnancies followed up between 2015 and 2018 were retrospectively analyzed. Data on FFPG tested in the first trimester and 75 g oral glucose tolerance test (OGTT) findings performed according to IADPSG criteria and Italian guidelines were collected and evaluated. The women’s clinical and metabolic characteristics (age, prepregnancy body mass index (BMI), previous pregnancies complicated by GDM, timing of delivery, and gestational hypertension) were also recorded. The fetal variables considered were being large for gestational age (LGA) or small for gestational age (SGA), macrosomia, and hypoglycemia. Results. Among the 1437 pregnant women studied, 684 had a normal glucose tolerance (NGT) and 753 developed GDM. Undifferentiated connective tissue disease (UCTD) is characterized by signs and symptoms suggestive of a connective tissue disease (CTD), but not fulfilling criteria for a specific CTD. Although UCTD is probably the most common rheumatic disease diagnosed in pregnant women, data about disease course during pregnancy and perinatal outcomes are very limited. Compared to other CTDs, UCTD seems to have milder clinical manifestations in pregnancy. Its natural history is related to disease activity at conception. In fact, if the disease is in a state of remission or minimal activity at conception, pregnancy outcomes are generally good. On the contrary, patients who become pregnant in a moment of high disease activity and/or who have multiple antibodies positivity show an increased risk of disease flares, evolution to a definite CTD and obstetric complications, such as fetal growth restriction, preeclampsia and preterm birth. Therefore, a preconception assessment is essential in women with UCTD to evaluate maternal and fetal risks, to initiate interventions to optimize disease activity, and to adjust medications to those that are least harmful to the fetus. The aim of the present study was to review the available literature about pregnancy course, maternal and fetal outcomes and therapeutic approaches of pregnant women with UCTD.

Objectives: Immune checkpoint inhibitors (ICIs) have led to a paradigm shift in non-small cell lung cancer (NSCLC) treatment. We investigated absolute eosinophil count (AEC) as a predictor of clinical outcomes and toxicity in NSCLC patients receiving ICIs. Materials and Methods: AEC was retrospectively collected at baseline and during treatment from 158 advanced NSCLC patients treated with single agent anti-PD1/anti-PDL1 monoclonal antibody in first or subsequent line of therapy at Medical Oncology Unit, Careggi University Hospital, Florence (Italy), between January 2016 to October 2020. Results: We found a significant association between high baseline AEC (≥130/μL) and better clinical outcomes. The response rates were 64.4% and 35.6% for patients with high and low AEC, respectively (p = 0.009). The high-AEC group showed a significantly longer PFS and OS than the low-AEC group (mPFS = 7.0 months, 95% CI 5.0–10.0 vs 2.5 months, 95% CI 2.0–4.0, p = 0.007 and mOS = 9.0 months, CI 95% 7.0–15.0 vs 5.5 months, 95% CI 4.0–8.0, p = 0.009, respectively). An increased risk of immune-related adverse events (irAEs) was reported in the high-AEC group (p = 0.133). The study of heavy ion nuclear reactionis an important tool to observe and disentangle different and competing mechanisms, which may arise in the different energy regimes. In particular, at relatively low bombarding energy, it is quite interesting the comparison between pre-equilibrium and thermal emission of light charged particles from hot nuclear systems [1-6]. Indeed, the nuclear structure of the interacting partners can be strongly correlated to the dynamics, especially at energies close to the Coulomb barrier, and this effect emerges when some nucleons or clusters of nucleons are either emitted or captured. In particular, a major attention has been devoted, in the last years, to the possible observation of cluster structure effects in the competing nuclear reaction mechanisms, especially when fast processes are involved. At this purpose, the four reactions 16O+30Si at 111 MeV, 16O+30Si at 128 MeV, 18O+28Si at 126 MeV, 19F+27Al at 133 MeV have been measured to study the onset of pre-equilibrium in an energy range where, for central collisions, complete fusion is expected to be the predominant mode. Experimental data were collected using the GARFIELD + RCo array [7], fully equipped with digital electronics at the LegnaroNational Laboratories. The comparison between experimental data and different model predictions have been performed: in particular, both dynamical models based either on Stochastic Mean Field (TWINGO) or Anti-symmetrized Molecular Dynamics and fully statistical models (GEMINI++) have been considered. Simulated events are filtered through a software replica of the apparatus, to take into account all possible distortions of the experimental distributions due to the finite size of the apparatus.

In the last few years the advent of targeted therapies against oncogenic drivers significantly improved the survival of non small cell lung cancer (NSCLC) patients with a favourable toxicity profile. Therefore, genetic testing, including at least EGFR mutations and ALK/ROS1 rearrangements, should be performed in all NSCLC patients (in particular with adenocarcinoma) who received a diagnosis of advanced disease. This review focuses on novel druggable oncogenic drivers, such as MET exon 14 mutations/MET amplification, RET fusions, BRAF V600E mutations, KRAS G12C mutations, NTRK rearrangements, and HER2 alterations.

ObjectiveMaternal characteristics and OGTT values of pregnancies complicated by gestational diabetes mellitus (GDM) were evaluated according to treatment strategies. The goal was to identify different maternal phenotypes in order to predict the appropriate treatment strategy.MethodsWe conducted a retrospective study among 1,974 pregnant women followed up for GDM in a tertiary referral hospital for high-risk pregnancies (Careggi University Hospital, Florence, Italy) from 2013 to 2018. We compared nutritional therapy (NT) alone (n = 962) versus NT and insulin analogues (n = 1,012) group. Then, we focused on different insulin analogues groups: long acting (D), rapid acting (R), both D and R. We compared maternal characteristics of the three groups, detecting which factors may predict the use of rapid or long-acting insulin analogue alone versus combined therapy.ResultsAmong women included in the analysis, 51.3% of them needed insulin therapy for glycemic control: 61.8% D, 28.3% combined D and R, and 9.9% R alone. Age >35 years, pre-pregnancy BMI >30, family history of diabetes, previous GDM, altered fasting plasma glucose (FPG), hypothyroidism, and assisted reproductive technologies (ART) were identified as maternal variables significantly associated with the need of insulin therapy. Altered 1-h and 2-h glucose plasma glucose level at OGTT, age >35 years, and previous GDM were found as independent predicting factors for the use of combined therapy with rapid and long acting analogues for glycemic control. The economic success of superintensive olive plantations is mainly due to the full mechanization of the harvesting and pruning. While the advantage of straddling machines is undoubted, winter mechanical pruning determines falls in productions. This is due to the indiscriminate suppression of both fertile leafy shoots destined to fruiting and growth, and exhausted parts of the plant. To reduce this damage, an innovative device has been developed, applied to a pruning machine, able to selectively cut the “aged” parts of vegetation. The selection is achieved by an air’s fluid dynamic action obtained throughout defined and directional air jets able to push the young and flexible shoots upwards and downwards; in this way they are saved by the cut, regenerating at least one year in advance the new fruiting hedge. Tests were carried out on the cultivar Arbequina, Tosca and Sikitita, in three superintensive olive groves located in the province of Grosseto, Rome and Latina, assessing the amount of leaves, shoots and branches, as well as fruits present at harvest, preserved from the pruning thanks to the action of the air flow, respectively for the East and West side of the rows. To get a profile of the biomass distribution along the cross section of the tree canopy, in the Grosseto farm a trial was also carried out to better assess leaves, shoots and branches distribution in the canopy. The statistical data analyses immediately evidenced two different populations due to the selective pruning. The work highlighted the remarkable effectiveness of the air jet in safeguarding the flexible and leafy vegetation and allowed to quadruple leaf surface and production.

The simulation of heavy ion collisions in the Fermi energy region is a challenge for the theoretical models; in particular it is difficult to obtain a coherent description in all the impact parameter range and to reproduce all the experimental observables. In this contribution we will show the very good job done by the dynamical model AMD [1] followed by the statistical code GEMINI [2, 3] as an afterburner. The model is able to reproduce the main characteristics of peripheral and semiperipheral collisions, although some discrepancies still persist.

The study of the emitted particles, comparing pre-equilibrium and thermal components, is a useful tool to examine the nuclear structure of emitters. Possible clustering effects, which may change the expected decay chain probability, could be highlighted on the competition between different reaction mechanisms. The NUCL-EX collaboration (INFN, Italy) has carried out an extensive research campaign on pre-equilibrium emission of light charged particles from hot nuclei. In this framework, the reactions 16O+30Si, 18O+28Si, 19F+27Al at 7 AMeV and 16O+30Si at 8 AMeV have been carried out using the GARFIELD+RCo array at Legnaro National Laboratories. Some anomalies in the α-particle emission channels have been evidenced in the measurement reported above, showing in an exclusive way the observed effects related to the entrance channels. The experimental results are compared to model prediction, for which the same filtering and complete event selection have been applied.

Four α-particles as a final state of 16O* quasi-projectile decayproduced in peripheral 16O+12C reactions at 130 MeV is thoroughly studied. The differentdecay channels leading to the four α-particles final state are reconstructed by carrying out an event-by-event analysis of α correlations in the population of intermediate 8Be and 12C. Although small, a non negligible contribution due to 8Begs evaporation is found.A comparison between predictions of an accurate Hauser-Feshbach decay code and branching ratios of the different decay channels is performed. Significant deviations are observed, among these the Hoyle state population which is considerably lower than the one predicted according to the statistical model, thus suggesting possible structure effects in the Coulomb barrier and/or in the transmission coefficients.

In recent years, a number of both theoretical and experimental investigations have been performed focusing on the zirconium isotopic chain. In particular, state-of-the-art Monte Carlo shell-model calculations predict shape coexistence in these isotopes. In this context, the 94Zr nucleus, which is believed to possess a nearly spherical ground state, is particularly interesting since the purported deformed structure is basedon the low-lying 02+ state, making it amenable for detailed study. In order to provide definitive conclusionson the shapes of the low-lying states, two complementary experiments to study 94Zr by means of low-energy Coulomb excitation were performed. This data will allow the quadrupole moments of the 21,2+ levels to be extracted as well as for the deformation parameters of the 01,2+ states to be determined and, thus, definitive conclusions to be drawn on the role of shape coexistence in this nucleus for the first time. The first experiment was performed at the INFN Legnaro National Laboratory with the GALILEO-SPIDER setup, which, for the first time, was coupled with 6 lanthanum bromide scintillators (LaBr3:Ce) in order to maximize the γ-ray detection effciency. The second experiment was performed at the Maier-Leibnitz Laboratory (MLL) in Munich and used a Q3D magnetic spectrograph to detect the scattered 12C ions following Coulomb excitation of 94Zr targets.

Maternal nutrition during pregnancy and lactation influences the growth and potential development of the fetus and contributes to the maturity of a healthy baby. The lack of sufficient calories, of macro- and micronutrients, can lead to deficiencies in building materials for the development and growth of the fetus: moreover there is growing evidence that the maternal nutritional status can alter the epigenetic state of the fetal genome, therefore alterations in nutrition during crucial periods of fetal development may result in developmental adaptations that permanently change the physiology, the metabolism of the offspring, and, as a consequence, predispose these individuals to diseases as adults. In this review we collected indications for the recommended intake of calories and nutrients for a healthy diet during pregnancy and lactation, also analyzing some nutritional choices that may increase the risk of nutritional deficiencies and the way to prevent them. Pregnancy-related dietary changes should begin prior to conception, with appropriate modifications throughout pregnancy and lactation, and appropriate supplementation of vitamins and minerals.
