1. Innovation adoption and diffusion in nursing : a comparative case study design

Abstract: Background: Innovation adoption and diffusion (IAD) in healthcare refers to the essential process of integrating novel practices into systems’ critical functions. IAD is often slow and unsuccessful, leading to patients not receiving optimal care in a timely manner. IAD research has been dominated by descriptive or prescriptive theories with limited benefit to practice applications; these theories are often simplistic and assume IAD to follow a linear and predictable pathway despite emerging evidence portraying healthcare organisations as complex adaptive systems. Therefore, contemporary IAD research is increasingly shifting in favour of complexity theory replacing linear theories. Nonetheless, nursing research in the field is still trailing behind other disciplines on this shift. Aim: This study applied complexity theory to evaluate the process of innovation adoption and diffusion in frontline nursing practice. Objectives: The study explored why some nursing units succeed in innovation adoption and diffusion while others, within the same organisation, fail. Methodology: Drawing on constructivism, this study applied a theoretical framework built on complexity theory as the grand theory, complex adaptive system (CAS) as the middle-range theory, and Atun’s framework at the micro level. The theoretical framework requires a systems thinking approach capable of exploring the system as a whole rather than a collection of isolated, separate individual components. Design: A Comparative case study design was adopted, forming a 2x2 matrix incorporating two cases with four units of investigation. The two cases were two innovations selected as representatives for the phenomenon of IAD in frontline nursing: WHO 5 Moments for Hand Hygiene (5MHH) and Intentional Nursing Rounding (INR). Each innovation (case) was evaluated through its adoption and diffusion into two comparable units of investigation (wards); a ward of successful adoption and diffusion and an unsuccessful comparator ward. The study deployed multiple data collection methods incorporating non-participatory observation for thirty hours in each unit, and semi-structured interviews with nurses across the nursing hierarchy. Results: The study was able to identify several manifestations of complexity in frontline nursing and demonstrate how these impact adoption and diffusion behaviours. Manifestations of complexity were identified in attributing clinical outcomes to staff compliance; in the discrepancy between evidence, policy, performed, practice, and records; and through the disparity in staff values and priorities across the hierarchy. Existing organisational arrangements of clinical governance (policy design, dissemination, monitoring and clinical documentation) did not facilitate IAD at the frontline. Conclusion: Complexity offers an explanation for the variation in frontline innovation behaviours. Future strategies are recommended to improve IAD in frontline nursing through enhancing the adaptability of the innovation, engaging the informal social communication network, recognising frontline priorities, acknowledging staged and gradual improvement in compliance over time, and maintaining patient interest at the core of all frontline practice innovations.

3. High-intensity versus low-level laser therapy in treatment of patients with subacromial impingement syndrome: a randomized, double-blind, controlled trial.

Abstract: To compare the effects of High-Intensity Laser Therapy (HILT) versus Low-Level Laser Therapy (LLLT) on shoulder pain and disability, shoulder Range of Motion (ROM), Pain Pressure Threshold (PPT), and sleep quality of patients having Subacromial Impingement Syndrome (SAIS). Forty-two patients with SAIS were randomly assigned into three groups, the HILT group (n = 14), the LLLT group (n = 14), and control group (n = 14). All groups received an exercise program consisted of shoulder muscles stretching and strengthening exercises. Along with the exercise program, the HILT group received HILT at 810 nm /980 nm, and LLLT group received LLLT at 904 nm, three times weekly for three weeks. The primary outcome was the change in shoulder pain and disability measured by Shoulder Pain and Disability Index (SPADI). Secondary outcomes included shoulder ROM using a standard goniometer, PPT measured by pressure algometer, and sleep quality by Pittsburgh Sleep Quality Index (PSQI). Measurements were taken both before and after a 3-week intervention to assess the outcomes. After 3-wk intervention, both the HILT and LLLT groups exhibited significant improvements in all parameters when compared to the control group (p < 0.05). Additionally, there were clinically significant differences between groups supported by a large ES favoring the HILT group for SPADI pain (ηp2 = 0.71), SPADI disability (ηp2 = 0.54), SPADI total (ηp2 = 0.82), PPT (ηp2 = 0.63), and PSQI (ηp2 = 0.42). The combination of HILT with exercises proved to be more beneficial in enhancing pain and function, PPT, and sleep quality compared to LLLT combined with exercises in treating patients with SAIS. Abstract: Objective To analyze how Pilates exercises affect standing, walking, and balance in children with diplegic cerebral palsy throughout a 10-week program. Methods We included 40 children aged 7–9 years with diplegic cerebral palsy, and randomly allocated them into two groups of the same size: conventional therapy group (group A) and conventional therapy+Pilates group (group B). We administered the same conventional physical therapy program to both groups for 45 minutes, with group B receiving additional Pilates exercises for 45 minutes. Both groups attended the intervention program three times/week for 10 weeks. All participants received 60 min of intensive motor learning approaches three days/week for three successive months. These approaches included constraint-induced movement therapy (CIMT), in which children wore a splint or sling on the unaffected upper limb, as well as hand-arm bimanual intensive training (HABIT) that requires the use of both hands during specific play-based activities. The control group received training on a horizontal surface while the child sat in front of an elbow-height table while the study group (vertical surface training) sat or stood in front of a wall/mirror/board. 6. Smartphone usage duration is not associated with increased pain or proprioception deficit in young adults with chronic mechanical low back pain: a cross-sectional study

Abstract: Abstract Background Smartphone use has been associated with pain in the upper quadrant; however, the relationship between usage duration and low back pain is still unclear. This study investigated the association between continuous smartphone use up to 30-min and back pain severity and proprioception acuity in patients with chronic low back pain. Fifty-eight patients with chronic mechanical LBP played a game for 10- and 30-min. In each session, pain and back repositioning errors were measured at baseline and immediately after task completion. Results Pain significantly but slightly increased following smartphone use, regardless to the duration (after 10 min: mean increase = 0.75 ± 1.17, P value < 0.001, 95% CI 0.44–1.06; after 30 min: mean increase = 0.96 ± 1.93, P value < 0.001, 95% CI 0.44–1.46). However, changes in perceived pain scores were not significantly different between the two tested durations (P value = 0.42). Proprioception repositioning error was not significantly different within the same testing session (mean change = 0.08 ± 1.83, 0.13 ± 1.77, P value = 0.73, 0.58, 95% CI − 0.40–0.56, − 0.60–0.33, for the 10 and 30 min, respectively). The changes in proprioception were not significant between the two-tested durations (P value = 0.56). Further, smartphone addiction did not significantly affect changes in pain and proprioception after game playing, regardless of the duration (P > 0.05). Conclusions These findings show that smartphone use slightly increases back pain immediately after continuous use; with no effect on back proprioception within the duration tested in this study. Changes in pain and proprioception were not influenced by smartphone addiction.

8. Prevalence of lumbosacral radiculopathy among physiotherapists of pediatric rehabilitation

Abstract: Physiotherapists (PTs) are at a significant risk of having musculoskeletal problems as a result of their jobs such as Work Related Musculoskeletal Disorders (WRMDs). WRMDs are described as injuries that involve a wide spectrum of inflammatory or degenerative diseases and disorders that result in pain or functional impairment, according to the World Health Organization. The aim of current study was to determine the prevalence of lumbosacral radiculopathy among pediatric physiotherapists in the ministry of health in Cairo, Egypt. Material and methods: two hundred participants included in this study from both sex with age ranged from 25-40 years. participants were assigned to Single group related measurement design. Specific questionnaire based on Roland Morris questionnaire (for back pain) and Modified Roland Morris questionnaire (for leg pain) (RMQ, RMQ-L) respectively. Results: there is a statically significant association between Age, BMI, working years, work hours per day and number of patients/day and lumbosacral radiculopathy; while sex had no significant association with lumbosacral radiculopathy. Subjects with age 30-37 years were 4.01 times more likely to have lumbosacral radiculopathy compared with subjects with age 25-29 years. Subjects working > 4h/day were 4.57 times more likely to have lumbosacral radiculopathy compared with subjects working 2-4 h/day. The results of this study showed that the point prevalence of lumbosacral radiculopathy was 30.5% (p > 0.05). Conclusion: according to this results, lumbosacral radiculopathy is common among Egyptian pediatric physiotherapists, Making Cairo's pediatric physiotherapists at a high-risk group, which necessitate appropriate intervention to manage such problem.

9. Core stability exercises versus biodex balance training on patients with mechanical low back pain

Abstract: Objectives: To compare the impact of core stability exercises versus Biodex balance training on postural stability and sway among patients suffering from mechanical low back pain. Methods: The study was conducted in the outpatient clinic of the Faculty of Physical Therapy, Modern University for Technology and Information, between March and June 2023. This randomized, double?blind, pre?post measurement study included forty-eight patients have been participated in this study and divided into three groups, Group A (control group) (n = 18) were given stretching as well as strengthening exercises. Group B (n = 18) were given core stability exercises plus stretching as well as strengthening exercises. Group C (n = 18) were given Biodex balance training (limit of stability, maze, and random control modes training) plus stretching as well as strengthening exercises. The Biodex Balance System were utilized to quantify dynamic balance indices at baseline and six weeks following therapy. Statistical analysis among groups for eye open firm, eye closed firm, eye open foam, and eye closed foam revealed from the Modified clinical test of sensory interaction and balance which were the main outcome indicator. Results: The Within-Group analysis revealed substantial differences regarding postural stability as well as control of postural sway for groups B and C, both prior to and following the treatment program (p value < 0.05). The between-group analysis revealed statistically substantial variations across all variables following the therapy, with Group C exhibiting further pronounced differences compared to Groups A and B (p < 0.05). Conclusion: Enhancement in all measures of dynamic balance was shown following core stability training activities and Biodex balance training, with a greater degree of improvement seen after Biodex balance training.

Group A: C5-6 antero-posterior cervical mobilization and ultrasound therapy. Group B: C5-6 lateral glide cervical mobilization and Ultrasound. Group C: C5-6 Postero-anterior cervical mobilization and US. The Visual Analogue Scale measured pain severity level and Lafayette Manual Muscle Tester measured shoulder external rotators and abductors isometric muscle strength. Results. The Wilcoxon test revealed a statistically significant difference in shoulder external rotators, abductors strength, and pain reduction between pre, immediate post-mobilization, 10 minutes post-mobilization, and 30 minutes post-mobilization within the three groups. But, the immediate post-mobilization and 10 minute post-mobilization median values of external rotators were statistically significant differences among the three groups, while the 30 minute post-mobilization median values of external rotators were not. For Abductors: Among the three groups, there were no statistically significant differences in abductors’ immediate post-mobilization, 10 minute post-mobilization, or 30 minute post-mobilization, but there were statistically significant differences in pain scores immediate post-mobilization, 10 minute post-mobilization, and 30 minute post-mobilization. Conclusion. The study found that all three kinds of cervical mobilization are useful in reducing pain and strengthening the external rotators and abductors muscles and that the most effective method is antero-posterior in treating rotator cuff tendinitis.

14. Effect of High Power Laser Therapy on Patients with Chronic Discogenic Sciatica: A Randomized Clinical Trial

Abstract: Back Ground: Sciatica is a frequent clinical illness that may be intensely painful, disabling, and life-altering. Objectives: The goal of this research was to see how high-power laser therapy affects the intensity of pain, angle of straight leg raise, six-minute walk test and electrophysiological studies in patients with chronic discogenic sciatica. Methods: A lumbar disc herniation at L5-S1 caused chronic unilateral sciatica in thirty-six male patients were included in this study. The patients have been divided into 2 identical groups [the control group (G1) and the study group (G2)] randomly. Participants in the control group (G1) got a designed physiotherapy program that included ultrasound, stretching exercise, back muscles strengthening exercise and sciatic nerve flossing technique. While patients in the study group (G2) got the same designed physiotherapy program as G1 plus high power laser therapy (HPLT). The outcome measures were latency and amplitude of Hoffmann reflex (H-reflex) for evaluating S1 nerve root function, visual analog scale (VAS) for evaluating pain level, angle of straight leg raise (SLR) and six-minute walk test (6MWT) for assessing functional impairment. Both groups were assessed before and after 4 weeks of therapy. Results: The findings demonstrated that both groups showed a significant reduction in pain level and latency of H-reflex following therapy. The angle of SLR, 6-minute walking distance (6MWD), and the amplitude of the H-reflex all improved significantly in both groups. After therapy, the study group's VAS and H reflex latency were significantly decreased than the control group's (p < 0.01). In addition, the study group's 6MWD, angle of SLR, and H reflex amplitudes were significantly increased than the control group's (p < 0.01) following therapy. Conclusion: High power laser therapy (HPLT) is a successful treatment for patients suffering from chronic discogenic sciatica.

Abstract: A common Carpal tunnel syndrome (CTS) is aneuropathic entrapment for the upper limbs with middle-aged people being at higher risk. To study how sensory rehabilitation impacts pain and functional outcome of hand in carpal tunnel syndrome patients. Twenty females with CTS, aged 25-45 years were enrolled. They were allocated at random to two groups; Group I: provided for sensory rehabilitation in addition to a standard physical therapy program and Group II: received only the standard physical therapy program only. Pre and post-treatment assessment included Visual Analogue Scale (VAS) and the Boston Carpal Tunnel Syndrome Questionnaire (BCTQ). In both post-treatment groups, the decrease of mean VAS and BCTQ scores was statistically significant. No substantial difference between the two groups in the measured parameters pre or post treatment. Adding sensory rehabilitation has nearly the same efficacy as the standard physical therapy program alone in decreasing pain, enhancing hand function in mild and moderate patients with CTS.

Abstract: Background Smartphone use has been associated with pain in the upper quadrant; however, the relationship between usage duration and low back pain is still unclear. This study investigated the association between continuous smartphone use up to 30-min and back pain severity and proprioception acuity in patients with chronic low back pain. Fifty-eight patients with chronic mechanical LBP played a game for 10- and 30-min. In each session, pain and back repositioning errors were measured at baseline and immediately after task completion. Results Pain significantly but slightly increased following smartphone use, regardless to the duration (after 10 min: mean increase = 0.75 ± 1.17, P value < 0.001, 95% CI 0.44–1.06; after 30 min: mean increase = 0.96 ± 1.93, P value < 0.001, 95% CI 0.44–1.46). However, changes in perceived pain scores were not significantly different between the two tested durations (P value = 0.42). Proprioception repositioning error was not significantly different within the same testing session (mean change = 0.08 ± 1.83, 0.13 ± 1.77, P value = 0.73, 0.58, 95% CI − 0.40–0.56, − 0.60–0.33, for the 10 and 30 min, respectively). The changes in proprioception were not significant between the two-tested durations (P value = 0.56). Further, smartphone addiction did not significantly affect changes in pain and proprioception after game playing, regardless of the duration (P > 0.05). Conclusions These findings show that smartphone use slightly increases back pain immediately after continuous use; with no effect on back proprioception within the duration tested in this study. Changes in pain and proprioception were not influenced by smartphone addiction.

Abstract: Primary immunodeficiency (PID) is characterized by recurrent and often life-threatening infections, autoimmunity and cancer, and it poses major diagnostic and therapeutic challenges. Although the most severe forms of PID are identified in early childhood, most patients present in adulthood, typically with no apparent family history and a variable clinical phenotype of widespread immune dysregulation: about 25% of patients have autoimmune disease, allergy is prevalent and up to 10% develop lymphoid malignancies1–3. Consequently, in sporadic (or non-familial) PID genetic diagnosis is difficult and the role of genetics is not well defined. Here we address these challenges by performing whole-genome sequencing in a large PID cohort of 1,318 participants. An analysis of the coding regions of the genome in 886 index cases of PID found that disease-causing mutations in known genes that are implicated in monogenic PID occurred in 10.3% of these patients, and a Bayesian approach (BeviMed4) identified multiple new candidate PID-associated genes, including IVNS1ABP. We also examined the noncoding genome, and found deletions in regulatory regions that contribute to disease causation. In addition, we used a genome-wide association study to identify loci that are associated with PID, and found evidence for the colocalization of—and interplay between—novel high-penetrance monogenic variants and common variants (at the PTPN2 and SOCS1 loci). This begins to explain the contribution of common variants to the variable penetrance and phenotypic complexity that are observed in PID. Abstract: Background: Pulmonary arterial hypertension (PAH) is a rare disease with an emerging genetic basis. Heterozygous mutations in the gene encoding the bone morphogenetic protein receptor type 2 ( BMPR2 ) are the commonest genetic cause of PAH, whereas biallelic mutations in the eukaryotic translation initiation factor 2 alpha kinase 4 gene ( EIF2AK4 ) are described in pulmonary veno-occlusive disease/pulmonary capillary hemangiomatosis. Here, we determine the frequency of these mutations and define the genotype-phenotype characteristics in a large cohort of patients diagnosed clinically with PAH. Methods: Whole-genome sequencing was performed on DNA from patients with idiopathic and heritable PAH and with pulmonary veno-occlusive disease/pulmonary capillary hemangiomatosis recruited to the National Institute of Health Research BioResource–Rare Diseases study. Heterozygous variants in BMPR2 and biallelic EIF2AK4 variants with a minor allele frequency of sorting intolerant from tolerant predictions) were identified as potentially causal. Phenotype data from the time of diagnosis were also captured. Results: Eight hundred sixty-four patients with idiopathic or heritable PAH and 16 with pulmonary veno-occlusive disease/pulmonary capillary hemangiomatosis were recruited. Mutations in BMPR2 were identified in 130 patients (14.8%). Biallelic mutations in EIF2AK4 were identified in 5 patients with a clinical diagnosis of pulmonary veno-occlusive disease/pulmonary capillary hemangiomatosis. Furthermore, 9 patients with a clinical diagnosis of PAH carried biallelic EIF2AK4 mutations. These patients had a reduced transfer coefficient for carbon monoxide (K co ; 33% [interquartile range, 30%–35%] predicted) and younger age at diagnosis (29 years; interquartile range, 23–38 years) and more interlobular septal thickening and mediastinal lymphadenopathy on computed tomography of the chest compared with patients with PAH without EIF2AK4 mutations. However, radiological assessment alone could not accurately identify biallelic EIF2AK4 mutation carriers. Patients with PAH with biallelic EIF2AK4 mutations had a shorter survival. Conclusions: Biallelic EIF2AK4 mutations are found in patients classified clinically as having idiopathic and heritable PAH. These patients cannot be identified reliably by computed tomography, but a low K co and a young age at diagnosis suggests the underlying molecular diagnosis. Genetic testing can identify these misclassified patients, allowing appropriate management and early referral for lung transplantation.

Telomere length is a risk factor in disease and the dynamics of telomere length are crucial to our understanding of cell replication and vitality. The proliferation of whole genome sequencing represents an unprecedented opportunity to glean new insights into telomere biology on a previously unimaginable scale. To this end, a number of approaches for estimating telomere length from whole-genome sequencing data have been proposed. Here we present Telomerecat, a novel approach to the estimation of telomere length. Previous methods have been dependent on the number of telomeres present in a cell being known, which may be problematic when analysing aneuploid cancer data and non-human samples. Telomerecat is designed to be agnostic to the number of telomeres present, making it suited for the purpose of estimating telomere length in cancer studies. Telomerecat also accounts for interstitial telomeric reads and presents a novel approach to dealing with sequencing errors. We show that Telomerecat performs well at telomere length estimation when compared to leading experimental and computational methods. Furthermore, we show that it detects expected patterns in longitudinal data, repeated measurements, and cross-species comparisons. We also apply the method to a cancer cell data, uncovering an interesting relationship with the underlying telomerase genotype. Publisher PDF

Abstract: Purpose: In preclinical models, radioimmunotherapy with (131)I-A5B7 anti-carcinoembryonic antigen (CEA) antibody ((131)I-A5B7) combined with the vascular disruptive agent combretastatin-A4-phosphate (CA4P) produced cures unlike either agent alone. We conducted a phase I trial determining the dose-limiting toxicity (DLT), maximum tolerated dose, efficacy, and mechanism of this combination in patients with gastrointestinal adenocarcinomas. EXPERIMENTAL DESIGN: Patients had CEA of 10 to 1,000 microg/L, QTc < or =450 ms, no cardiac arrhythmia/ischaemia, and adequate hematology/biochemistry. Tumor was suitable for blood flow analysis by dynamic contrast enhanced-magnetic resonance imaging (MRI). The starting dose was 1,800 MBq/m(2) of (131)I-A5B7 on day 1 and 45 mg/m(2) CA4P given 48 and 72 hours post-(131)I-A5B7, then weekly for up to seven weeks. Results: Twelve patients were treated, with mean age of 63 years (range, 32-77). Two of six patients at the first dose level had DLTs (grade 4 neutropenia). The dose was reduced to 1,600 MBq/m(2), and CA4P escalated to 54 mg/m(2). Again, two of six patients had DLTs (neutropenia). Of ten assessable patients, three had stable disease and seven had progressive disease. Single-photon emission computed tomography confirmed tumor antibody uptake in all 10 patients. DCE-MRI confirmed falls in kinetic parameters (K(trans)/IAUGC(60)) in 9 of 12 patients. The change of both pharmacokinetic parameters reached a level expected to produce efficacy in one patient who had a minor response on computed tomography and a reduced serum tumor marker level. Conclusions: This is believed to be the first trial reporting the combination of radioimmunotherapy and vascular disruptive agent; each component was shown to function, and myelosuppression was dose-limiting. Optimal dose and timing of CA4P, and moderate improvements in the performance of radioimmunotherapy seem necessary for efficacy.

44. Lack of weight recording in patients being administered narrow therapeutic index antibiotics: a prospective cross-sectional study.

Abstract: Objectives: Patient weight is a key measure for safe medication management and monitoring of patients. Here we report the recording of patient's body weight on admission in three hospitals in West London and its relationship with the prescription of antibiotic drugs where it is essential to have the body weight of the patient. Methods: A prospective cross-sectional study was conducted in three teaching hospitals in West London. Data were collected during March 2011-September 2011 and July 2012-August 2012, from adult admissions units, medical and surgical wards. Data from each ward were collected on a single day to provide a point prevalence data on weight recording. Patient medication charts, nursing and medical notes were reviewed for evidence of weight and height recording together with all the medication prescribed for the patients. An observational study collecting data on the weight recording process was conducted on two randomly selected wards to add context to the data. Results: Data were collected on 1012 patients. Weight was not recorded for 46% (474) of patients. Eighty-nine patients were prescribed a narrow therapeutic antibiotic, in 39% (35/89) of these weight was not recorded for the patient. Intravenous vancomycin was the most commonly prescribed antibiotic requiring therapeutic monitoring. In total 61 patients were receiving intravenous vancomycin and of these 44% (27/61) did not have their weight recorded. In the observational study, the most frequently identified barrier to weight not being recorded was interruptions to the admission process. Conclusions: Despite the clinical importance of body weight measurement it is poorly recorded in hospitalised patients, due to interruptions to the workflow and heavy staff workloads. In antibiotics a correct, recent patient weight is required for accurate dosing and to keep drugs within the narrow therapeutic index, to ensure efficacy of prescribing and reduce toxicity.
