Studies prove that the way to increase motivation in students is to create a positive atmosphere in the classroom. Creating a positive atmosphere involves an attitude towards students, listening to students, understanding, considering their opinion, courtesy, humor, justice, etc. There are always students who find mathematics a dry, boring, difficult subject. Therefore, our goal is to help students overcome difficulties and show them the attractive side of mathematics. And this will happen if the teaching will be student-oriented

Background Given the susceptibility of organs to ischaemic injury, alternative preservation methods to static cold storage (SCS), such as normothermic machine perfusion (NMP) are emerging. The aim of this study was to perform a comparison between NMP and SCS in liver transplantation with particular attention to bile duct lesions. Methods The outcomes of 59 consecutive NMP-preserved donor livers were compared in a 1 : 1 propensity score-matched fashion to SCS control livers. Postoperative complications, patient survival, graft survival and bile duct lesions were analysed. Results While patients were matched for cold ischaemia time, the total preservation time was significantly longer in the NMP group (21 h versus 7 h, P Conclusion The use of NMP allowed for a significantly prolonged organ preservation with a lower rate of observed ischaemic-type bile duct lesions.

Normothermic machine perfusion (NMP) allows for ex vivo viability and functional assessment prior to liver transplantation (LT). Hyperspectral imaging represents a suitable, non-invasive method to evaluate tissue morphology and organ perfusion during NMP. Liver allografts were subjected to NMP prior to LT. Serial image acquisition of oxygen saturation levels (StO2), organ hemoglobin (THI), near-infrared perfusion (NIR) and tissue water indices (TWI) through hyperspectral imaging was performed during static cold storage, at 1h, 6h, 12h and at the end of NMP. The readouts were correlated with perfusate parameters at equivalent time points. Twenty-one deceased donor livers were included in the study. Seven (33.0%) were discarded due to poor organ function during NMP. StO2 (p < 0.001), THI (p < 0.001) and NIR (p = 0.002) significantly augmented, from static cold storage (pre-NMP) to NMP end, while TWI dropped (p = 0.005) during the observational period. At 12–24h, a significantly higher hemoglobin concentration (THI) in the superficial tissue layers was seen in discarded, compared to transplanted livers (p = 0.036). Lactate values at 12h NMP correlated negatively with NIR perfusion index between 12 and 24h NMP and with the delta NIR perfusion index between 1 and 24h (rs = −0.883, p = 0.008 for both). Furthermore, NIR and TWI correlated with lactate clearance and pH. This study provides first evidence of feasibility of hyperspectral imaging as a potentially helpful contact-free organ viability assessment tool during liver NMP.

Normothermic machine perfusion (NMP) has become a clinically established tool to preserve livers in a near-physiological environment. However, little is known about the predictive value of perfusate parameters toward the outcomes after transplantation.Fifty-five consecutive NMP livers between 2018 and 2019 were included. All of the livers were perfused on the OrganOx metra device according to an institutional protocol. Transplant and perfusion data were collected prospectively.Forty-five livers were transplanted after NMP. Five livers stem from donors after circulatory death and 31 (68.9%) from extended criteria donors. Mean (SD) cold ischemia time was 6.4 (2.3) h; mean (SD) total preservation time was 21.4 (7.1) h. Early allograft dysfunction (EAD) occurred in 13 of 45 (28.9%) patients. Perfusate aspartate aminotransferase (P = 0.008), alanine aminotransferase (P = 0.006), lactate dehydrogenase (P = 0.007) and their development over time, alkaline phosphatase (P = 0.013), and sodium (P = 0.016) correlated with EAD. Number of perfusate platelets correlated with cold ischemia time duration and were indicative for the occurrence of EAD. Moreover, von Willebrand Factor antigen was significantly higher in perfusates of EAD livers (P 0.001), and Δ von Willebrand factor antigen correlated with EAD. Although perfusate lactate and glucose had no predictive value, EAD was more likely to occur in livers with lower perfusate pH (P = 0.008). ΔPerfusate alkaline phosphatase, Δperfusate aspartate aminotransferase, Δperfusate alanine aminotransferase, and Δperfusate lactate dehydrogenase correlated closely with model for early allograft function but not liver graft assessment following transplantation risk score. Bile parameters correlated with extended criteria donor and donor risk index.Biomarker assessment during NMP may help to predict EAD after liver transplantation. The increase of transaminases and lactate dehydrogenase over time as well as platelets and vWF antigen are important factors indicative for EAD.

Engineering support of the natural and anthropogenic solid dust particles concentration measurement in the surface layer of the atmosphere implies the development and creation of a certain number of complex, but easy to manufacture and operate devices. The task takes on particular importance when considering dust as a means of transportation for various viruses and infections. The technical solutions we are considering boil down to the implementation of precision photometric measurements of light scattering by dust particles in liquid samples, the concentration of dust in which we increase to reliably measurable by photometry. The carried out research made it possible to formulate problems and propose solutions for a monitoring network. Technical solutions are implemented in working models and tested in urban dustiness of Tbilisi. (Abstract)

ACE Inhibitors represent the first class of anti hypertensive agents that was designed and developed on the basis of a well-defined physiopathol ogical axis of arterial hypertension, a vascular disorder that is now becoming one of the major causes of morbidity/mortality, not only in developed societies but also in the highly populated developing countries. An angiotensin-converting enzyme (ACE) inhibitor, the carboxylalkyl compounds derivated lizinopril is apharmaceutical drug, used primarily for the treatment of hypertension (elevated blood pressure) and congestive heart failure. Purpose of investigation was – comparison of in vitro outlet of 5 mg tablets of ”Lizinocor” containing Lizinopril , produced by the Georgian pharmaceutical company “GMP” and its analogue ”Zestril” using method of high-performance liquid chromatography. According to the received results average percent quantity outlet of ”lizinocor” is 97,65 %, ”Zestril” – 95, 88%;. Inclination in comparison with Zestril is 1,85 %, (norm ± 5 %). ”Lizinocor” 5 mg tablets containing Lizinopril produced by the Georgian pharmaceutical company ”GMP” are characterized by good outlet quality.

Significant progress has now been made in diagnosing, treating, reducing complications and lethality at liver abscesses. This is due to the displacement from the arsenal of surgery of complex and traumatic abdominal or trans thoraces approach to abscesses, which themselves created the problem of the postoperative period, complicated the course of an already severe infectious pathology. Modern minimally invasive technologies have significant advantages, especially with regard to interventional sonography, that is, the combined properties of diagnosis and treatment. Purpose of the study. To objectify the choice of the method of surgical intervention of liver abscesses on the basis of a new self-developed ultrasound semiotics of the stage of the formation of abscesses. Materials and methods. A comprehensive examination of 62 patients with liver abscesses was based on the study of the clinical picture of the liver abscesses, a bacteriological study of the contents, CT scan, and especially ultrasound, on the basis of which the ultrasound semiotics of the stages of the formation of abscesses was developed and implemented. Results and discussion. According to the possibilities of ultrasound diagnosis of abscesses of the liver from the first signs of local inflammation in the liver to the final stage of formation of the pyogenic capsule, we identified four stages of their development: 1 stage – local inflammatory changes of the parenchyma without its destruction; Stage 2 – destructive changes of parenchyma without signs of distinct boundaries; Stage 3 – clear boundaries of destructive changes with a non-expressed capsule; Stage 4 – clear formation of a capsule with complete destruction of tissues. The proposed ultrasonic semiotics allows us to determine the method of operation. Thus, at stages 1 and 2, and in some cases in 3 stages, there is no alternative to puncture-drainage methods in residual postoperative cavities. At stages 3 and 4, there are more advantages in laparoscopic operations, in some cases laparotomic. Conclusions. the proposed classification of ultrasonic semiotics stages of the formation of liver abscesses most pathogenetically corresponds to the morphological changes of the abscess zone and, most importantly, is adapted to the choice of the method of operation.

Sensitive, accurate and efficient Liquid Chromatographic Method is developed for the quantitative determination of Nitrofuran residues in fish samples. It was found that the coefficient of variation for all four test substances of the Nitrofuran group is from 0.03 ìg / ml less than 2%, respectively the detection minimum was considered to be 0.03ìg / ml; 376 nm wavelength is specific for Nitrofurans; For the quantification of nitrofuran residues in a fish sample validation of the developed Liquid Chromatographic Method revealed complete compliance of the method (Guidance for Industry Bioanalytical Method Validation U.S. Department of Health and Human Services Food and Drug Administration Center for Drug Evaluation and Research (CDER) Center for Veterinary Medicine (CVM) May 2001) with the following validation characteristics of the requirements: detection minimum, specificity, accuracy, correctness and linearity.

Organ transplantation survival rates have continued to improve over the last decades, mostly due to reduction of mortality early after transplantation. The advancement of the field is facilitating a liberalization of the access to organ transplantation with more patients with higher risk profile being added to the waiting list. At the same time, the persisting organ shortage fosters strategies to rescue organs of marginal donors. In this regard, hypothermic and normothermic machine perfusion are recognized as one of the most important developments in the modern era. Owing to these developments, novel non-invasive tools for the assessment of organ quality are on the horizon. Hyperspectral imaging represents a potentially suitable method capable of evaluating tissue morphology and organ perfusion prior to transplantation. Considering the changing environment, we here discuss the hypothetical combination of organ machine perfusion and hyperspectral imaging as a prospective feasibility concept in organ transplantation.

In case of mechanical cholestasis, along with stagnation in the veins and bile ducts, lymph production increases, which contributes to the accumulation of ascitic fluid. In the peribiliaryareas, where the extramural biliary glands are located, an accumulation of lymphatic vessels has been observed in all the preparations.In these same areas, a contrast-enhanced microtomography showed transition of the ink-gelatin contrast agents (using gelatin and India ink) from the extramural part of the mucous glands to the lumen of the lymphatic capillaries and blood vessels.Lymphobiliary connections were visualized during the preparation of the sections of the portal triad of the lobar and sectoral portal tracts under a binocular microscope. More reliable evidence of the existence of lymphobiliary connections in the portal triad region was revealed on histological preparations stained with traditional methods.; Thus, anatomically, the area of the portal triad appears to be one of the alternative areas of lymphobiliary communications, which further confirms its high adaptive capacity in case of stagnation of bile.

The coronavirus disease 2019 (COVID-19) pandemic has changed life on a global scale. The numbers of transplantations have plummeted as a result of fear of disease transmission, recipient coronavirus disease 2019 infection, priority shift, and resource limitations. Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) complicates transplantation because donor testing, (re)allocation of limited resources, and recipient testing may exceed permissible ischemia times. Normothermic machine perfusion (NMP) helps safely prolong liver preservation up to 38 hours. Additional time is essential under the current circumstances. Here we present the case of a 29-year-old liver transplant recipient in whom prolonged liver preservation required for SARS-CoV-2 screening was accomplished through NMP. Donor and recipient test results for SARS-CoV-2 were negative, and intensive care unit capacity was eventually available. The surgical procedure and postoperative course were uneventful. NMP can extend preservation times in liver transplantation while awaiting SARS-CoV-2 test results and available intensive care unit capacity.

The aim of the study is to improve the results of uncomplicated hypertensive crises treatment at the prehospital stage by studying the state of systemic hemodynamics, determining the balance of the autonomic nervous system, and markers relating to the systemic inflammatory response in patients with hypertension in uncomplicated hypertensive crises. In 2017-2019, an open, prospective and comparative study was conducted and consisted of 206 patients with documented stage II HT, of which 131 patients applied for E.M.S. in the "Municipal Institution Zaporizhzhya Station of Emergency (Ambulance) Medical Assistance of " of the Zaporizhzhia regional Council and 75 people who had a stable course of the disease and were examined on an outpatiently on the basis of the "Primary health care Center No. 10". Almost healthy 31 people were examined on the basis of the MI "Regional medical exercises dispensary" of the Zaporizhzhia regional Council. The indicators of systemic inflammatory response among the examined individuals in the study were analyzed. An increase in the concentration of serum proinflammatory markers was demonstrated depending on the stage and degree of BP increase. Increased concentrations of catecholamines are associated with increased blood levels of proinflammatory cytokines such as IL-6 and TNF-б.the level of CRP may reflect the inflammatory process in the intima of blood vessels. Interleukin-1в (IL-1в), which is considered an "early response" factor, has been proven to be involved in energy homeostasis and inflammation.

Mitochondria sense changes resulting from the ischemia and subsequent reperfusion of an organ and mitochondrial reactive oxygen species (ROS) production initiates a series of events, which over time result in the development of full-fledged ischemia-reperfusion injury (IRI), severely affecting graft function and survival after transplantation. ROS activate the innate immune system, regulate cell death, impair mitochondrial and cellular performance and hence organ function. Arresting the development of IRI before the onset of ROS production is currently not feasible and clinicians are faced with limiting the consequences. Ex vivo machine perfusion has opened the possibility to ameliorate or antagonize the development of IRI and may be particularly beneficial for extended criteria donor organs. The molecular events occurring during machine perfusion remain incompletely understood. Accumulation of succinate and depletion of adenosine triphosphate (ATP) have been considered key mechanisms in the initiation; however, a plethora of molecular events contribute to the final tissue damage. Here we discuss how understanding mitochondrial dysfunction linked to IRI may help to develop novel strategies for the prevention of ROS-initiated damage in the evolving era of machine perfusion.

The paper shows the use of the Jupyter data analysis and machine learning environment in clinical studies. The use of tools and various libraries for the Python language allows us to identify statistically significant patterns in a large body of information quickly and with minimal costs. Data visualization tools allow making the results convenient for perception and analysis.
