Background Physiotherapy is a major cornerstone of enhanced rehabilitation after surgery (ERAS) and reduces the development of atelectasis after thoracic surgery. By initiating physiotherapy in the post-anaesthesia care unit (PACU), the aim was to evaluate whether the ultra-early initiation of rehabilitation (in the first hour following tracheal extubation) would improve the outcomes of patients undergoing elective thoracic surgery. Methods A case–control study with a before-and-after design was conducted. From a historical control group, patients were paired at a 3:1 ratio with an intervention group. This group consisted of patients treated with the ultra-early rehabilitation programme after elective thoracic surgery (clear fluids, physiotherapy, and ambulation). The primary outcome was the incidence of postoperative atelectasis and/or pneumonia during the hospital stay. Results After pairing, 675 patients were allocated to the historical control group and 225 patients to the intervention group. A significant decrease in the incidence of postoperative atelectasis and/or pneumonia was found in the latter (11.4 versus 6.7 per cent respectively; P = 0.042) and remained significant on multivariate analysis (OR 0.53, 95 per cent c.i. 0.26 to 0.98; P = 0.045). A subgroup analysis of the intervention group showed that early ambulation during the PACU stay was associated with a further significant decrease in the incidence of postoperative atelectasis and/or pneumonia (2.2 versus 9.5 per cent; P = 0.012). Conclusions Ultra-early rehabilitation in the PACU was associated with a decrease in the incidence of postoperative atelectasis and/or pneumonia after major elective thoracic surgery.

A preliminary survey on the diversity of gliding squirrels was conducted from September 2019 until February 2020 in several locations within Merapoh, Pahang, Peninsular Malaysia using standard line transect method. The survey areas include a pristine forest, logged-over forest and agro-forested plantations. Gliding squirrels were spotted using a headlamp, binoculars (8x30 magnification) and a Digital SLR affixed with a telephoto lens (Nikon D500 + Nikkor 200-500 F5.6 VR) as they are mainly nocturnal. The highlight of this short survey was two new distributional records of flying squirrels for Merapoh Forest Complex. These two species were Spotted giant gliding squirrel (Petaurista elegans) and Temminck’s gliding squirrel (Petinomys setosus). Other gliding squirrels recorded in this survey include the Red Giant gliding squirrel (Petaurista petaurista), Horsfield’s gliding squirrel (Iomys horsfieldii) and Red-cheeked gliding squirrel (Hylopetes spadiceus). Apart from gliding squirrel, a total of eleven (11) species of other squirrels were also recorded within this survey. Continuous and systematic diversity surveys on gliding squirrels are needed as it is still understudied in Malaysia especially Merapoh.

For patients with brain tumors identification of diagnostic and prognostic markers in easy accessible biological material such as plasma or cerebrospinal fluid (CSF) would greatly facilitate patient management. MIC 1/GDF15 (growth differentiation factor 15) is a secreted protein of the TGF beta superfamily and emerged as a candidate marker exhibiting increasing mRNA expression during malignant progression of glioma. Determination of MIC 1/GDF15 protein levels by ELISA in the CSF of a cohort of 94 patients with intracranial tumors including gliomas meningioma and metastasis revealed significantly increased concentrations in glioblastoma patients (median 229 pg/ml) when compared with control cohort of patients treated for non neoplastic diseases (median below limit of detection of 156 pg/ml p < 0.0001 Mann Whitney test). However plasma MIC 1/GDF15 levels were not elevated in the matching plasma samples from these patients. Most interestingly patients with glioblastoma and increased CSF MIC 1/GDF15 had a shorter survival (p = 0.007 log rank test). In conclusion MIC 1/GDF15 protein measured in the CSF may have diagnostic and prognostic value in patients with intracranial tumors.

Cloud Computing is emerging today as a commercial infrastructure that eliminates the need for maintaining expensive computing hardware. Through the use of virtualization, clouds promise to address with the same shared set of physical resources a large user base with different needs. Thus, clouds promise to be for scientists an alternative to clusters, grids, and supercomputers. However, virtualization may induce significant performance penalties for the demanding scientific computing workloads. In this work we present an evaluation of the usefulness of the current cloud computing services for scientific computing. We analyze the performance of the Amazon EC2 platform using micro-benchmarks and kernels. While clouds are still changing, our results indicate that the current cloud services need an order of magnitude in performance improvement to be useful to the scientific community.
