- Abstract
Despite the absence of a pattern of use or not of vitamin D in osteoporotic patients by health authorities, vitamin D remains widely prescribed by doctors. This dissonance can give rise to inappropriate prescriptions. This study made it possible to take stock of the prescription of vitamin D in patients taking anti-osteoporotic drugs in Occitanie, as well as to evaluate the prescription methods for cholecalciferol.

Glucocorticoids (GCs) are widely used by several specialties for the treatment of a variety of diseases and conditions. The unfavorable effect of oral GCs on bone health is well-documented. The ensuing from their use glucocorticoid-induced osteoporosis (GIOP) is the most common cause of medication-induced osteoporosis and fractures. It is uncertain, however, if, and in what extent, GCs administered by other routes affect the skeleton. In the present review, we quote current evidence on the effect of inhaled GCs, epidural and intra-articular steroid injections, and topical GCs on bone outcomes. Although evidence is limited and weak, it seems that a small proportion of the administered GCs may be absorbed, enter the systemic circulation, and adversely affect the skeleton. Potent GCs, higher doses, and longer treatment duration seem to infer the greater risk for bone loss and fractures. There are scarce data, and only for inhaled GCs, regarding the efficacy of antiosteoporotic medications in patients receiving GCs through routes other than oral. Further studies are needed to clarify the relationship between GC administration through these routes and bone outcomes and to help establishing guidelines for the optimal management of such patients. The cost of osteoporotic fractures in France is estimated at nearly 7 billion euros for 2019, or 2.6% of healthcare expenditure. The average cost of a hip fracture in France is around 22,000 euros in the first year. Direct costs are the most important, represented by hospitalization and nursing home. The cost of assessing and managing osteoporosis represents less than 1% of the average annual cost. In the future, the financial and societal impact of osteoporotic fractures is set to increase, due to the growing number of fragility fractures associated with an ageing population, coupled with a lack of assessment and pharmacological management of osteoporosis.

Osteoporosis is the most common bone disease, affecting one in three women and one in five men over the age of 50 worldwide, ultimately affecting nearly 70% of women over the age of 80. The clinical consequence of osteoporosis, which also constitutes its major complication, is a fragility fracture. Globally, it is estimated that osteoporosis is responsible for 9 million fractures each year, or one fracture every 3 seconds. Therefore, its screening in general medicine appears essential. This observation leads to several questions: what is the prescribing attitude of general practitioners? What importance do they give to this screening in their practice? What are the possible obstacles to this screening? [ABSTRACT FROM AUTHOR]

- Abstract
The treatment of prostate cancer with androgen deprivation therapy (ADT) causes a decrease in bone density as a side effect. For prescribers of this treatment, knowledge of the prevention of osteoporosis and fracture risk is important, as well as knowledge about the indication for follow-up testing or indication for additional lifestyle measures and therapy. In this article we discuss the relationship of ADT with osteoporosis and fracture risk based on the literature and give recommendations based on the Dutch prostate cancer guideline and the recently published Dutch guideline on osteoporosis and fracture prevention. BONE fractures in old age , *CANCER patient care , *BONE health , *GERIATRIC assessment , *CANCER chemotherapy - Abstract
The occurrence of bone fractures is frequent in the elderly population, and in cancer patients, especially with bone metastases. The growing incidence of cancer associated with an aging population implies important health challenges, including bone health. Decisions on cancer care in older adults have to take into account older adults' specificities. Screening tools as G8 or VES 13 and evaluating tools as comprehensive geriatric assessment (CGA) do not include bone-related items. Bone risk assessment is indicated according to identification of geriatric syndromes such as falls, history, and the oncology treatment plan. Some cancer treatments disrupt bone turnover and decrease bone mineral density. This is mainly caused by hypogonadism, induced by hormonal treatments and some chemotherapies. Treatments can also cause direct (i.e. chemotherapy, radiotherapy or glucocorticoids) or indirect toxicity through electrolyte disorders (i. e. some chemotherapies or tyrosine kinase inhibitors) on bone turnover. Bone risk prevention is multidisciplinary. Certain interventions proposed in the CGA aim to improve bone health and reduce the risk of falling. It is also based on the drug management of osteoporosis, and the prevention of complications from bone metastases. Management of fractures, related or not to bone metastases, relates to the concept of orthogeriatrics. It is also based on the benefit-risk ratio of the operation, access to minimally invasive techniques, prehabilitation or rehabilitation, but also the prognosis related to cancer and geriatric syndromes. Bone health is essential in older cancer patient's care. Bone risk assessment should be part of CGA in routine use and specific decision-making tools should be developed. Bone event management must be integrated throughout the patient's care pathway and onco-geriatrics multidisciplinarity should include rheumatological expertise. [ABSTRACT FROM AUTHOR]

OSTEOPOROSIS , *SKELETON , *CONSENSUS (Social sciences) , *BONE density , *FRACTURE fixation - Abstract
Osteoporosis is the most common weakening disease of the skeleton. It affects more than 200 million people worldwide. Since the international consensus conference of Hong Kong in 1993, that of Amsterdam in 1996, and the NIH in 2001, osteoporosis has been defined by a progressive decrease in bone mass and qualitative alterations of bone tissue, therefore of bone strength, predisposing the patient to an increased risk of fractures [1]. In 1994, the WHO defined diagnostic criteria for densitometric osteoporosis based on the measurement of bone mineral density (BMD) by DXA. Postmenopausal osteoporosis occurs in a specific hormonal context. It is a real silent epidemic, until fracture complications appear [2 , 3]. The development and greater availability of non-invasive methods for measuring bone mineral content have contributed to facilitating the screening and assessment of fracture risk. Osteoporotic pelvic ring fractures show a continuously increasing incidence. In addition to the fracture morphology, the clinical situation of the patients is also crucial for therapy. The osteoporotic fracture working group of the spine section of the German Society for Orthopaedics and Trauma Surgery (DGOU) has developed a score for the treatment decision that is based on the OF-Pelvis classification and on essential clinical parameters. The score includes the clinical parameters of mobilization ability, pain situation, fracture-related neurological deficits and risk factors for an increased surgical complication rate. In addition, the fracture morphology is determined according to the OF-Pelvis classification including the fracture modifiers. For each parameter, points are awarded or deducted depending on the characteristics and the total is then calculated. If a parameter cannot be determined, no point are awarded. A score below 8 means a recommendation for conservative therapy, while a score above 8 means a recommendation for surgical therapy. If there are exactly 8 points, conservative or surgical treatment is possible. The OF-Pelvis score was evaluated retrospectively in 5 trauma centers based on records and imaging in 107 patients and compared with the therapy actually carried out. There was an overall agreement of 91 % between the therapy recommendation of the OF-Pelvis score and the therapy actually carried out (conservative versus surgical). Collecting the score took about 5 minutes. The OF-Pelvis score is an easy-to-use decision-making aid for determining the indication for conservative or surgical treatment of osteoporotic pelvic ring fractures. [ABSTRACT FROM AUTHOR]

PELVIC fractures , *JOINT instability , *OSTEOPOROSIS , *SACRAL fractures , *COMPUTED tomography , *EDEMA - Abstract
As the population ages, the incidence of fractures of the sacrum and pelvic ring is increasing. This represents a challenge for outpatient and inpatient care facilities. The Osteoporotic Fractures Working Group of the Spine Section of the German Society for Orthopaedics and Trauma (DGOU) has developed the OF-Pelvis classification for these injuries with a view to making it reliable and easy to use. The OF-Pelvis classification divides these injuries into 5 categories. From OFP 1 to OFP 5, the injury severity and instability grade increases. Category OFP 1 corresponds to evidence of edema in the sacrum or pelvic ring without evidence of fracture on computed tomography. Category OFP 2 corresponds to a unilateral or bilateral fracture of the anterior pelvic ring on CT-scan. An OFP 3 fracture corresponds to a unilateral posterior sacral fracture with facultative fracture of the anterior pelvic ring. An OFP 4 fracture presents with a bilateral posterior sacral fracture. The highest severity is an injury which is transiliosacral or iliac and corresponds to the OFP 5 category. Three modifiers can be assigned. These indicate a higher degree of instability or further injuries. An avulsion of the transverse process of LWK 5 corresponds to modifier 1. Modifier 2 is assigned if there is dislocation of the fracture. Evidence of an edema in a location of the pelvic ring where no fracture is detectable on CT-scan corresponds to modifier 3. Initial testing of the assessment of categories and modifiers by multiple investigators showed substantial to near perfect agreement (Fleiss' kappa = 0.764-0.790 for the categories, Fleiss' kappa = 0.646-0.629 for the modifiers). The OF-Pelvis seems to be a reliable tool for the classification of osteoporotic sacral and pelvic ring fractures which can also be an aid in clinical practice for physicians not specialized in the treatment of such fractures due to its reliability and simplicity. OLDER patients , *OSTEOPOROSIS , *ALGORITHMS - Abstract
Osteoanabolic osteoporosis drugs have become better available. The osteoanabolic therapeutic principle has a stronger, faster-onset fracture-reducing effect than the antiresorptive preparations. It has also been newly recognized that the significance of a first fragility fracture as a risk factor is time-dependent: the less time that has elapsed since the first fracture, the higher the resulting re-fracture risk. Patients older than 65 years whose index fragility fracture occurred less than two years before are therefore grouped in a separate "Imminent Fracture Risk" category. These innovations were implemented by updating the osteoporosis therapy guideline. According to this guideline, patients in the "Imminent Fracture Risk" category should be offered osteoporosis therapy as soon as possible, in order to avoid as many fractures as possible. We are critical of an overly strict implementation of this algorithm in very old fracture patients. Our own data indicate that more than 30 % of this subpopulation do not experience the effect of a newly started osteoporosis therapy. We advocate a clinically based indication for osteoporosis therapy. For this, we propose a "Question Surprise" modified for osteological purposes. "Would I be surprised if I had to treat the same patient for a fracture again in a year?" If the question is answered with "No," then that patient could be a candidate for specific osteoporosis treatment. [ABSTRACT FROM AUTHOR]

Osteoporose ,Fractuur ,Ouderen ,Behandeling ,Medicine - Abstract

Introduction: Every day dental implants are being sought by young adults and elderly people. With the increase in this demand, more people with osseointegration problems due to diseases such as osteoporosis have emerged, due to which there has been a greater need in the search for resolutions for these deficits. Materials and Methods: For this literature review, articles found in PubMed, Scielo and google academic databases were used, with 842 articles found, 83 selected and 29 used. Conclusion: It is concluded that the greatest difficulty in implant dentistry in patients with osteoporosis is osseointegration and to improve the vitamin and hormonal deficits that cause it, drugs and vitamin supplements are necessary. BASKETBALL players , *BONE health , *OSTEOPOROSIS , *SQUAT (Weight lifting) , *AEROBIC capacity , *DUAL-energy X-ray absorptiometry - Abstract
The purpose of this study was to compare bone health parameters (bone mineral content [BMC], bone mineral density [BMD], geometric indices of femoral neck [FN] strength (cross-sectional area [CSA], cross-sectional moment of inertia [CSMI], section modulus [Z], buckling ratio [BR] and strength index [SI]) and composite indices of FN strength (compression strength index [CSI], bending strength index [BSI], and impact strength index [ISI])) in middle-aged inactive men (n = 25) and middle-aged former basketball players (n = 18). The participants of the study were 43 middle-aged men; their ages ranged from 38 to 52 years. Dual-energy X-ray absorptiometry was used to evaluate body composition, BMD and geometric indices of FN strength. Composite indices of FN strength (CSI, BSI, and ISI) were calculated. Physical performance tests were used to evaluate the vertical jump (VJ), horizontal jump (HJ), triple jump (TJ), maximum power (MP) of the lower limbs (watts), maximal squat strength, maximal bench press strength, maximal leg extension strength, maximal barbell curl strength, maximal triceps extension strength, sprint performance (10 meters), and maximum oxygen consumption (VO 2 max, L/min). WB BMC, WB BMD, L1-L4 BMD, CSA, CSMI and Z were significantly higher in former basketball players compared to inactive men. MP, TJ, VJ, maximal squat strength and maximal bench press strength were significantly higher in former basketball players compared to inactive men. The present study suggests that former basketball practice is associated with greater bone health parameters in middle-aged men. [ABSTRACT FROM AUTHOR]

Ocular inflammatory reactions secondary to treatment with bisphosphonates are a rare entity. Their physiopathological mechanism is still poorly understood but seems to be linked to an immunological reaction mediated by some cytokines. Observation: We report the case of a 63-year-old patient followed for severe osteoporosis and treated with zoledronate. The patient presented 6 weeks after the initiation of treatment with a decrease in visual acuity diagnosed as acute anterior uveitis. Conclusion: Acute anterior uveitis is an important entity to be aware of because a good evaluation and rapid management can reduce complications involving the visual prognosis. [ABSTRACT FROM AUTHOR]
