Abstract

BackgroundAdolescence is a critical period of maturation when nutrient needs are high, especially among adolescents entering pregnancy. Using individual-level data from 140,000 participants, we examined socioeconomic, nutrition, and pregnancy and birth outcomes for adolescent mothers (10-19 years) compared to older mothers in low and middle-income countries.MethodsThis study was conducted between March 16, 2018 and May 25, 2021. Data were obtained from 20 randomised controlled trials of micronutrient supplementation in pregnancy. Stratified analyses were conducted by age (10-14 years, 15-17 years, 18-19 years, 20-29 years, 30-39 years, 40+ years) and geographical region (Africa, Asia). Crude and confounder-adjusted means, prevalence and relative risks of pregnancy, nutrition and birth outcomes were estimated using multivariable linear and log-binomial regression models with 95% confidence intervals.FindingsAdolescent mothers comprised 31.6% of our data. Preterm birth, small-for-gestational age (SGA), low birthweight (LBW) and newborn mortality followed a U-shaped trend in which prevalence was highest among the youngest mothers (10-14 years) and then reduced gradually, but increased again for older mothers (40+ years). When compared to mothers aged 20-29 years, there was a 23% increased risk of preterm birth, a 60% increased risk of perinatal mortality, a 63% increased risk of neonatal mortality, a 28% increased risk of LBW, and a 22% increased risk of SGA among mothers 10-14 years. Mothers 40+ years experienced a 22% increased risk of preterm birth and a 103% increased risk of stillbirth when compared to the 20-29 year group.InterpretationThe youngest and oldest mothers suffer most from adverse pregnancy and birth outcomes. Abstract

BackgroundIn 2013, an estimated 2.8 million newborns died and 2.7 million were stillborn. A much greater number suffer from long term impairment associated with preterm birth, intrauterine growth restriction, congenital anomalies, and perinatal or infectious causes. With the approaching deadline for the achievement of the Millennium Development Goals (MDGs) in 2015, there was a need to set the new research priorities on newborns and stillbirth with a focus not only on survival but also on health, growth and development. We therefore carried out a systematic exercise to set newborn health research priorities for 2013-2025.MethodsWe used adapted Child Health and Nutrition Research Initiative (CHNRI) methods for this prioritization exercise. We identified and approached the 200 most productive researchers and 400 program experts, and 132 of them submitted research questions online. These were collated into a set of 205 research questions, sent for scoring to the 600 identified experts, and were assessed and scored by 91 experts.ResultsNine out of top ten identified priorities were in the domain of research on improving delivery of known interventions, with simplified neonatal resuscitation program and clinical algorithms and improved skills of community health workers leading the list. The top 10 priorities in the domain of development were led by ideas on improved Kangaroo Mother Care at community level, how to improve the accuracy of diagnosis by community health workers, and perinatal audits. The 10 leading priorities for discovery research focused on stable surfactant with novel modes of administration for preterm babies, ability to diagnose fetal distress and novel tocolytic agents to delay or stop preterm labour.ConclusionThese findings will assist both donors and researchers in supporting and conducting research to close the knowledge gaps for reducing neonatal mortality, morbidity and long term impairment. WHO, SNL and other partners will work to generate interest among key national stakeholders, governments, NGOs, and research institutes in these priorities, while encouraging research funders to support them. We will track research funding, relevant requests for proposals and trial registers to monitor if the priorities identified by this exercise are being addressed.

27. Build the cradle later : an examination of perinatal care and mortality in village Nepal

Abstract

Background: As perinatal and neonatal mortality move to the foreground of the debate on how best to improve child survival in poor countries, there appears a pressing need to test potential interventions. Implicit in testing is the ability to document birth and neonatal outcomes in rural communities. The thesis sets perinatal events in the context of the child survival literature and critically examines current knowledge about practices, outcomes and interventions. This knowledge is found to be limited, particularly in terms of practices and outcomes in rural settings in developing countries. Methods: Two methods are described to document perinatal events in villages in Makwanpur district, Nepal. (1) a census of women of reproductive age, collecting information about previous pregnancies and most recent live births (2) a prospective system of registration that tracked women through childbirth and infants through the neonatal period. Both methods yielded information on pregnancies, birth outcomes, care practices and health care seeking patterns. Results: The census collected information from 12,170 women, of whom 4867 had given birth in the preceding two years. The prospective surveillance collected information on 3522 pregnancies over two years. The thesis presents results under two broad themes: (1) a description of the birth experience of women in rural Makwanpur, its outcomes, practices and care seeking patterns (2) a comparison of the two methods of data collection. Poverty was the norm in rural Makwanpur and only a quarter of participants were literate. 30% of pregnant women had any antenatal care, which tended to be both late and limited, and 95% gave birth at home. Three percent of women were helped by trained health workers and there were compromises in hygiene and warmth at the time of delivery. Breastfeeding rates were high. The prospective registration process suggested a neonatal mortality rate of 37 per thousand live births, a figure higher than that from the census (27 per thousand), and thus higher than would be estimated by existing methodologies. Discussion: Antenatal, delivery, postnatal and neonatal care practices and care seeking are compared with existing knowledge from other studies. Their implications for programmatic intervention are considered. The issue of recall-based data collection is discussed as it bears upon the evaluation of public health interventions. The feasibility of registration systems is assessed and recommendations made for increasing reliability, expansion and replication, and reducing system costs. 386 words.

Abstract

This study seeks to investigate the impact of perceived structural injustice on participation in collective action to address violence against women. We will test the this by running an individually randomized controlled experiment. We will expose women living in informal settlements in Mumbai, India to either: A) a facilitated discussion of the structural roots of violence against women B) a facilitated discussion on identifying types of abuse C) no discussion. Afterwards, we will ask them if they are willing to participate in activities to raise awareness in their own neighbourhood and observe if they indeed do so. The study will be nested inside a larger cluster-randomized controlled trial of a community-based violence prevention programme in informal settlements in Mumbai, India called the SNEHA-TARA trial (Daruwalla et al., 2019). We hypothesize that relative to conditions B and C, condition A will promote greater amounts of awareness-raising behaviour and this difference will be mediated by perceived structural injustice. We also hypothesize that this effect will be greater in intervention areas of the underlying TARA trial compared to control areas. Contrasting condition A with conditions B and C allows us to partially account for experimenter demand. If simply the act of being asked by an experimenter is the main reason for participating in awareness-raising activities, there should be no difference between A and C. If simply the act of having a discussion on violence against women (as opposed to discussing structural injustice) is the reason, there should be no difference between A and B.

Abstract

A growing number of public health interventions involve community members in activism to prevent violence against women (VAW), but the psychological drivers of participation are poorly understood. In this project, we developed a new scale for measuring three proposed drivers of participation in collective action to address VAW in the context of urban informal settlements in Mumbai, India: perceived legitimacy, perceived efficacy, and collective action norms. Using a household survey of over 2,500 men and women, we examined its item, construct, and criterion validity. A valid scale would provide useful insight into modifiable beliefs and attitudes that community mobilisation interventions can address to inspire activism in low-resource contexts.
