1. Factores de riesgo y significado clínico de las metástasis en los ganglios linfáticos retropancreáticos en pacientes con cáncer gástrico

Abstract

Antecedentes: La incidencia de metástasis en ganglios linfáticos retropancreáticos en pacientes con cáncer gástrico no es despreciable. Objetivo: Determinar los factores de riesgo de metástasis en los ganglios linfáticos (GL) retropancreáticos e investigar su importancia clínica. Método: Se analizaron retrospectivamente los datos clínicos patológicos de 237 pacientes con cáncer gástrico entre junio de 2012 y junio de 2017. Resultados: Hubo 14 pacientes (5.9%) que presentaron metástasis de GL retropancreático. La mediana de supervivencia de los pacientes con y sin metástasis del GL retropancreático fue de 13.1 y 25.7 meses, respectivamente. Según el análisis univariado, se encontró que el tamaño tumoral ≥ 8 cm, Bormann tipo III/IV, tipo indiferenciado, presencia de invasión angiolinfática, profundidad de invasión (pT4), estadio N3 y metástasis GL No. 3, 7, 8, 9 y 12p se asociaba con metástasis de GL retropancreático. Según el análisis multivariante, se encontró que el tamaño tumoral ≥ 8 cm, Bormann tipo III/IV, tipo indiferenciado, pT4, estadio N3, metástasis de GL No. 9 y metástasis de GL No. 12p eran variables pronósticas independientes para la metástasis de GL retropancreático. Conclusiones: La metástasis del GL retropancreático es un factor de mal pronóstico para el cáncer gástrico. El tamaño del tumor ≥ 8 cm, el tipo III/IV de Bormann, el tumor indiferenciado, el estadio pT4, N3 y las metástasis de GL No. 9 y 12p son factores de riesgo de metástasis en los GL retropancreáticos.

Abstract

BACKGROUND: Abdominal surgery can lead to postoperative intra-abdominal adhesions (PIAAs) with significant morbidity and mortality. This study compares the use of honey with a standard bioresorbable membrane (Seprafilm tm) to prevent the formation of PIAAs in rats. METHODS: Thirty rats underwent laparotomy, and PIAAs were induced by scraping the cecum. The animals were divided into three groups, each containing ten rats. Group 1 (control) represented the cecal abrasion group, with no intraperitoneal administration of any substance. Group 2 (honey group) underwent cecal abrasion and intraperitoneal administration of honey. Group 3 (Seprafilm tm group) underwent cecal abrasion and intraperitoneal Seprafilm tm application. RESULTS: Group 1 exhibited higher adhesion scores for adhesions between the abdominal wall and the organs. Groups 2 and 3 had decreased adhesive attachments to the intra-abdominal structures. Compared to group 1, the incidence of adhesion formation was lower in both group 2 (p=0.001) and group 3 (p=0.001). The incidence of fibrosis was also lower in group 2 (p=0.016) and group 3 (p=0.063) compared to group 1. There was no significant difference between the histopathological fibrosis scores for the rats in group 2 and those in group 3 (p= 0.688). CONCLUSION: This study suggests that both honey and Seprafilm tm decrease the incidence of PIAAs in the rat cecal abrasion model. Although the mechanism of action is not clear, intraperitoneal administration of honey reduced PIAAs. The outcome of this study demonstrates that honey is as effective as Seprafilm tm in preventing PIAAs.

Abstract

In this study, a vibration isolator is designed as a periodic structure, which is constructed with embedded inertial amplification mechanisms. At first, various compliant inertial amplification mechanism configurations are proposed. Among them, the one with the widest vibration isolation frequency band is selected via conducted parametric studies. Then, the determined mechanism is utilized as the unit cell (i.e., the repetitive building block) of the periodic structure. Finally, the displacement transmissibility (i.e., Frequency Response Function (FRF)) plots are presented to demonstrate the vibration isolation performance of the designed periodic structure., Bu çalışmada, iç içe gömülmüş atalet artırımı mekanizmalarıyla oluşturulmuş bir periyodik yapı, titreşim yalıtıcısı olarak tasarlanmıştır. İlk olarak, çeşitli esnek atalaet artırımı mekanizma konfigürasyonları önerilmiştir. Yapılan parametrik çalışmalar neticesinde, bu mekanizmalardan en geniş titreşim yalıtımı frekans aralığına sahip olanı belirlenmiştir. Abstract

Bu çalışmada amaç, zeytinyağı sanayii yan ürünü olan karasuyun Macar fiğ-tritikale silajlarına farklı dozlarda ilavesinin kimyasal kompozisyon, fermentasyon, fiziksel, mikrobiyolojik özellikleri ve in vitro sindirilebilirlikleri üzerine etkisini incelemektir. Araştırma silajları belirtilen şekilde oluşturulmuştur: (i) %100 Macar fiğ-tritikale silajı (kontrol); (ii) %95 Macar fiğ-tritikale + %5 karasu; (iii) %90 Macar fiğ-tritikale + %10 karasu; (iv) %85 Macar fiğ-tritikale + %15 karasu; (v) %80 Macar fiğ-tritikale + %20 karasu. Silolama dönemi 56 gün devam etmiştir. Sonuç olarak, karasu ilavesiyle silajların koku, organik madde, pH ve suda çözünebilir karbonhidrat içerikleri azalmış; kuru madde, ham kül, ADF, ham yağ ve Fleig puanı artmıştır. Ham protein, ham selüloz, NDF, amonyak azot, laktik asit bakterileri, in vitro kuru madde ve organik madde sindirilebilirlikleri bakımından görülen farklılıklar önemli bulunmamıştır. Araştırma silajlarında sülfit indirgeyen anaeroblar, Listeria spp., Enterobactericeae ve maya içeriğine rastlanmamıştır. Küf içeriği, %15 karasu içeren grupta bir örnekte saptanmıştır. Araştırma sonunda, Macar fiğ-tritikale karışımlarından oluşan silajlara %20 düzeyine kadar karasu ilavesinin herhangi bir olumsuz etkisinin olmadığı sonucuna varılmıştır.

Abstract

Abstract Background Neoadjuvant treatment is the standard treatment in locally advanced ESCC. However, the optimal chemotherapy regimen is not known. Method This is a retrospective observational cohort study conducted with propensity score matching. Patients with resectable ESCC from 13 tertiary centers from Türkiye were screened between January 2011 and December 2021. We compared the efficacy and safety of neoadjuvant chemoradiotherapy with the CF and the CROSS regimens in patients with ESCC. Results Three hundred and sixty‐two patients were screened. Patients who received induction chemotherapy (n = 72) and CROSS‐ineligible (n = 31) were excluded. Two hundred and fifty nine patients received neoadjuvant chemoradiotherapy. After propensity score matching (n = 97 in both groups), the mPFS was 18.4 months (95% CI, 9.3–27.4) and 25.7 months (95% CI, 15.6–35.7; p = 0.974), and the mOS was 35.2 months (95% CI, 18.9–51.5) and 39.6 months (95% CI 20.1–59.2; p = 0.534), in the CF and the CROSS groups, respectively. Abstract

Early detection strategies for pancreatic ductal adenocarcinoma are needed. This article describes a high‐sensitivity platform for detection of epithelial cells shed from preneoplastic lesions at a high risk of becoming malignant and a combination of technologies that could be used for early detection of disease in high‐risk patients., Background. Recent work has demonstrated early shedding of circulating epithelial cells (CECs) from premalignant intraductal papillary mucinous neoplasms (IPMNs). However, the potential use of CECs as a “liquid biopsy” for patients with IPMNs has been limited by antigen dependence of CEC isolation devices and the lack of robust detection biomarkers across CEC phenotypes. Materials and Methods. We utilized a negative depletion microfluidic platform to purify CECs from contaminating leukocytes and coupled this platform with immunofluorescence, RNA in situ hybridization, and RNA sequencing (RNA‐seq) detection and enumeration. Results. Using established protein (EpCAM, cytokeratins) and novel noncoding RNA (HSATII, cytokeratins) biomarkers, we detected CECs in 88% of patients bearing IPMN lesions. RNA‐seq analysis for MUC genes confirm the likely origin of these CECs from pancreatic lesions. Conclusion. Our findings increase the sensitivity of detection of these cells and therefore could have clinical implications for cancer risk stratification. Implications for Practice. This work describes a high‐sensitivity platform for detection of epithelial cells shed from preneoplastic lesions at high risk of malignant transformation. Further research efforts are underway to define the transcriptional programs that might allow discrimination between circulating cells released from tumors that will become malignant and cells released from tumors that will not. After further refinement, this combination of technologies could be deployed for monitoring and early detection of patients at high risk for developing new or recurrent pancreatic malignancies.

Abstract

INTRODUCTION: Glomerulonephritis is still the primary cause among the diseases causing end stage renal disease. Helicobacter pylori (HP), also having a local proinflammatory effect on gastric mucosa, can trigger a local and systemic inflammatory response, and consequently have a role in the development of extragastrointestinal defects. MATERIAL AND METHODS: The study was composed of patients diagnosed with primary glomerulonephritis who had dyspeptic complaints throughout the diagnosis. Patients who received endoscopic biopsy upon the determination of pathologic findings in their upper gastrointestinal endoscopy were HP positive in their biopsy material. A triple eradication therapy was initiated for HP. RESULTS: The study included 14 female and 19 male patients, 33 in total, whose biopsy material was determined to be HP positive. Before the eradication for HP, we found serum albumin to be 34.0 (19.0-51.0) g/l, serum total protein 58.6 ±12.9 g/l, serum creatinine 0.9 (0.5-1.2) and proteinuria 3069 (652-12392) mg/day in 24-hour urine. After the eradication, however, serum albumin was found to be 40 (20-52) g/l, serum total protein 62.3 ±11.1 g/l, serum creatinine 1.02 (0.6-1.29) mg/dl and proteinuria was 2850 (172-15181) mg/day in 24-hour urine. A comparison of the results showed that a statistically significant difference is established between the serum albumin, total protein and creatinine values (p = 0.001, p = 0.001 and p = 0.021, respectively), but not between proteinuria values in 24-hour urine (p = 0.990). CONCLUSIONS: Patients with primary glomerulonephritis, HP eradication treatment has an effect on serum albumin levels.

Abstract

In this study, inertial amplification induced phononic band gaps are attained by performing structural optimization on a compliant unit cell mechanism of a one-dimensional periodic structure. First of all, stop band characteristics of the lumped parameter model of the unit cell mechanism is discussed. Next, the distributed parameter model of the compliant unit cell is presented. In order to obtain wide and deep inertial amplification induced stop bands, both size and topology optimization methods are utilized considering the distributed parameter model of the unit cell mechanism. The band gap characteristics of the infinite periodic size and topologically optimized mechanisms are compared. Moreover, vibration transmissibility of the finite periodic size and topologically optimized mechanisms are calculated and the effect of number of unit cells is discussed. Finally, a parametric study is carried out to demonstrate the effect of topology optimization design space volume fraction on the band gap limits.

Abstract

Gastrointestinal stromal tumors (GISTs) are the most common mesenchymal tumors arising from the gastrointestinal tract. GISTs can occur in any segment of the gastrointestinal tract from the distal esophagus to the anus, but they are most common in the stomach. The presence of c-KIT staining is a strong and universal marker of GISTs. Approximately 70 % patients with GISTs have some clinical symptoms, including gastrointestinal bleeding, manifested as melena, hematemesis, or bright red blood per rectum. Most GISTs are incidentally detected during endoscopic examinations. Endoscopic features are usually insufficient for the diagnosis of GISTs. Endoscopic ultrasonography (EUS) has become an important diagnostic tool for the evaluation of subepithelial lesions such as GISTs. EUS-guided sampling of GISTs is the preferred technique for histological diagnosis. The capability of endoscopic ultrasonography-guided fine-needle aspiration (EUS-FNA) for biopsy of GISTs is better than any other well-accepted method. The EUS-guided Tru-cut core biopsy technique provides a grossly visible sample of tissue, but it has some technical disadvantages. The curative treatment of GISTs is still complete surgical resection. However, optimal endoscopic treatment along with imatinib remains controversial. Advanced resection techniques by endoscopic devices, including endoscopic submucosal resection, endoscopic submucosal tunnel dissection, and endoscopic-laparoscopic combined approach, have been used in the treatment of GISTs.

Abstract

Acromegaly is associated with an increased prevalence of colonic polyps. The aim of this study was to evaluate the prevalence and recurrence rate of colonic polyps in acromegalic patients. Ninety-six acromegalic patients and 100 irritable bowel syndrome patients (IBS) were enrolled in the study. Twenty patients who were cured exclusively by surgery, and 20 patients that could not be hormonally controlled were re-examined colonoscopically after 36 months. Twenty-nine of 96 acromegalic patients (30.2%) had colonic polyps. In the IBS group, 10 (10.0%) had colonic polyps. The prevalence of colonic polyps was significantly higher in acromegaly. The group of acromegalic patients with and without polyps did not differ significantly with regard to plasma GH, IGF-I, fasting insulin levels and glycemic status. The presence of colonic polyps was correlated with increased patient age and male gender. We did not observe a difference in terms of polyp recurrence frequencies in the patients cured by surgery compared to uncontrolled patients. Acromegalic patients have a higher prevalence of colonic polyps than that of control subjects. We could not identify any factors that could predict polyps within the acromegalic patients - but age and male sex.

Abstract

Background Familial Mediterranean Fever (FMF) is a systemic relapsing autoinflammatory disorder. Adenosine deaminase (ADA) is an enzyme widely distribute in tissues and body fluids. Circulating levels of ADA have been shown to increase in several inflammatory conditions. This study was designed to evaluate the serum ADA in patients with FMF during acute attacks and attack-free periods. Methods The study groups comprised 23 FMF patients in attack-free period (male/female: 11/12), 30 FMF patients in attack period (male/female: 11/19) and 20 healthy control (male/female:10/10). The groups were similar for age, gender and disease duration. Results The mean age of FMF patients in attack-free period, patient with acute attack were 34.3±11.7 and 29.4±11.1 respectively. The disease durations were 13.1±10.2 and 8.2±7.6 years for patients in attack-free periods and patients with acute FMF attack, respectively. Patients with acute attack had significantly higher ADA levels than both patients with attack-free periods and healthy controls (for each, p Conclusion In this study we demonstrated that FMF patients with acute attacks had higher serum ADA levels than attack-free periods and healthy controls. It is likely that ADA may have a role in the cytokine network of the inflammatory cascade of FMF. Also, elevated ADA levels may be a part of the activated Th1 response in the disease. ADA may be used as a supportive marker to differentiate FMF attacks from attack-free periods. Further larger-scale studies are needed to support this result.

Abstract

The aim of this study is to obtain a wide and deep phononic gap at low frequencies in a two-dimensional periodic solid structure with embedded inertial amplification mechanisms. Size and shape optimizations are performed on the building block mechanism of the periodic structure to maximize phononic gap (stop band) width and depth. It is shown that shape optimization offers a wider and deeper gap, when both size and shape optimized mechanisms have the same mass and stiffness values. Analysis of the shape optimized mechanism is carried out using two different finite element models, one using beam elements and the other using shell elements. Both models produced similar results for the stop band width and depth. A two-dimensional periodic structure is constructed with the shape optimized building block mechanisms. Moreover, experimental and numerical frequency response results of this periodic structure are obtained. The matching frequency response results indicate that the two-dimensional periodic structure has a wide and deep phononic gap for in-plane excitations. Furthermore, due to proper selection of the out-of-plane thickness of the periodic structure, out-of-plane vibration modes do not occur within the phononic gap. (C) 2015 Elsevier Ltd. Abstract

The purpose of this retrospective study was to evaluate the effects of Dyna Locking Trochanteric (DLT) nail (UI Corporation 529-1, Yonghyun-dong, Uijungbu Kyunggi-Do, Korea 480-050) in 31-A1 and 31-A2 intertrochanteric femur fractures. Eighty-seven patients (63 female and 24 male, mean age 77 years) were treated. The mean duration of followup was 16.6 +/- 3.1 months (range, 12-24 months). There were 36 cases of 31-A1 and 51 cases of 31-A2 fractures. The duration of surgery, fluoroscopy screening time, the blood loss, the hospital stay, the tip-apex distance were significantly less in the 31-A1 fracture group compared with the 31-A2 fracture group (p0.001). Cut-out was observed in 13 patients (14.9%) above 80 years old with 31-A2 fractures. Especially in elderly patients with unstable 31-A2 fractures, the wedge wing on the neck screw does not work properly. Therefore further biomechanical investigations and also prospective clinical studies with larger series are needed to improve the device.

Abstract

Active noise control in a one dimensional acoustic duct, in which fluid medium inside the duct has a mean flow velocity, is studied. The acoustic duct model with general boundary conditions is solved in Laplace domain and infinite dimensional system transfer functions are obtained. For controller designs, appropriate microphone, and noise canceling source locations are determined. Low order finite dimensional transfer function approximations of actual system transfer functions are obtained. It is found that, in a selected frequency range, approximations represent actual system in a satisfactory way. By using approximated system transfer functions, finite dimensional, low order, optimal H2 and H∞ controllers are synthesized via linear matrix inequalities method. Closed loop frequency response and time domain simulations show that the controllers successfully suppress unwanted sound, which propagates along the duct.

25. The effect of Helicobacter pylori eradication on proteinuria in patients with primary glomerulonephritis

Abstract

Introduction Glomerulonephritis is still the primary cause among the diseases causing end stage renal disease. Helicobacter pylori (HP), also having a local proinflammatory effect on gastric mucosa, can trigger a local and systemic inflammatory response, and consequently have a role in the development of extragastrointestinal defects. Material and methods The study was composed of patients diagnosed with primary glomerulonephritis who had dyspeptic complaints throughout the diagnosis. Patients who received endoscopic biopsy upon the determination of pathologic findings in their upper gastrointestinal endoscopy were HP positive in their biopsy material. A triple eradication therapy was initiated for HP. Results The study included 14 female and 19 male patients, 33 in total, whose biopsy material was determined to be HP positive. Before the eradication for HP, we found serum albumin to be 34.0 (19.0–51.0) g/l, serum total protein 58.6 ±12.9 g/l, serum creatinine 0.9 (0.5–1.2) and proteinuria 3069 (652–12392) mg/day in 24-hour urine. After the eradication, however, serum albumin was found to be 40 (20–52) g/l, serum total protein 62.3 ±11.1 g/l, serum creatinine 1.02 (0.6–1.29) mg/dl and proteinuria was 2850 (172–15181) mg/day in 24-hour urine. A comparison of the results showed that a statistically significant difference is established between the serum albumin, total protein and creatinine values (p = 0.001, p = 0.001 and p = 0.021, respectively), but not between proteinuria values in 24-hour urine (p = 0.990). Conclusions Patients with primary glomerulonephritis, HP eradication treatment has an effect on serum albumin levels.

27. Analysis of surgical complications of thyroid diseases: results of a single institution

Abstract

The purpose of this study is to compare the complications of different types of thyroidectomy for benign and malignant thyroidal diseases.Between January 2001 and March 2006, 982 patients underwent thyroidectomy in single institute. A retrospective analysis was performed on demographic characteristics and pathology reports of patients, complications of surgery as well as the follow-up of patients. Sixty patients could not be fully followed up and were excluded.The types of the surgical procedures were as follows; 451 (48.9%) total, 137 (14.9%) subtotal, 60 (6.5%) near-total thyroidectomy and 274 (29.8%) lobectomy with isthmectomy. In the benign group, temporary recurrent laryngeal nerve injury (RLNI) was the most common complication in patients with toxic diffuse goiter (TDG 2%) while this complication occurred in patients with differentiated thyroid cancer (DTC 1.5%) in the malign group. Permanent RLNI in benign thyroidal diseases was seen more commonly in patients with toxic multinodular goiter (1.3%). In benign thyroidal diseases, temporary hypoparathyroidism (THPT) was mostly found in patients with TDG (8%), whereas in malignant thyroidal diseases this was found more in patients with DTC (2%). Permanent HPT (PHPT) in benign thyroidal diseases was observed more commonly in patients with multinodular goiter (0.9%). In malignant thyroidal diseases, it was more frequently observed in patients with DTC (0.5%). Infective complications after thyroid surgery are rarely observed and have a low incidence (0.4%).RLNI and HPT are the most common complications of after thyroid surgery but they may be avoided with an accurate technique (Tab. 3, Ref. 15). 28. Yield and Some Quality Properties of Binary Alfalfa-Grass Mixtures in Different Mixture Ratios

Abstract

The aim of this investigation was to determine appropriate perennial grasses and their most suitable mixture ratios, which can be grown with alfalfa in binary mixtures. The research was conducted during 2009-2011 in Isparta (37.50 °N, 30.32 °E), located in the Western Mediterranean climate conditions. In the study, alfalfa was planted as a binary mixture with perennial ryegrass (Lolium perenne L.), orchardgrass (Dactylis glomerata L.), meadow fescue (Festuca pratensis Huds.), and smooth bromegrass (Bromus inermis Leyss.) in a randomized block design with three replications. Sowing ratios of alfalfa in the mixtures varied 20, 30, and 40%, and grass species ratios were 80, 70, and 60%. In the study, significant differences were found among the applications regarding hay yield, crude protein content, crude protein yield, NDF, ADF, land equivalent ratio (LER), and grass ratio in the hay (GR). The highest hay yield was obtained from alfalfa + orchardgrass and alfalfa + smooth bromegrass binary mixtures (25.98 and 25.78 t ha-1, respectively). Alfalfa + perennial ryegrass and alfalfa + meadow fescue mixtures gave the highest crude protein contents as 14.93 and 14.80%, respectively. The highest LER values were observed on perennial ryegrass and smooth bromegrass binary mixtures, and the highest grass ratios were determined in orchardgrass and smooth bromegrass mixtures made with alfalfa. Increasing the alfalfa sowing ratio in the mixtures positively affected all quality characteristics such as crude protein, NDF, and ADF. It was concluded that orchardgrass and smooth bromegrass binary mixtures with alfalfa produced high yield and quality hay and that the mixture should contain 40% alfalfa and 60% grass.

Abstract

Stress fracture of the femoral neck is a rare complication of total knee arthroplasty. We report a case of femoral neck fracture following revision arthroplasty of the ipsilateral knee. Contributing factors may have been the patient's osteoporosis and a period of immobilisation before the revision arthroplasty. The patient was treated successfully with hemiarthroplasty of the hip. In patients who have undergone total knee arthroplasty, complaints of groin pain that radiates to the involved knee should alert to the possibility of a stress fracture of the femoral neck.

Abstract

Squamous cell cancer of the lung occurs with early dissemination but clinically significant metastases localized to the small bowel are rare. Small bowel metastasis is a fatal complication usually seen in advanced stages of the disease. In the present study, we report a case of a 65-year-old patient who was admitted to the emergency department with acute abdominal pain and during evaluation was found to have squamous cell carcinoma metastasis to small bowel leading to perforation. We also discuss the current management possibilities in the guidance of current literature.

31. Free intra-abdominal hemorrhage after open-heart surgery: an unusual gastrointestinal complication

Abstract

Gastrointestinal complications after open-heart surgery are rare. Many preoperative, intraoperative, and postoperative factors may predispose patients to these complications or cause them. Our patient was a 64-year-old woman who underwent aortic valve replacement due to aortic stenosis. Free intra-abdominal hemorrhage occurred on the 2nd postoperative day. During exploratory laparotomy, it was determined that the hemorrhage was from a vein near the falciform ligament of the liver and from a bleeding laceration of the splenic capsule. The complication was repaired surgically. To our knowledge, intra-abdominal hemorrhage of both liver and spleen after open-heart surgery has never been reported before, even in large patient series. We report the case and present our ideas regarding the cause of the bleeding.

Abstract

Conventional x-ray is still the most important diagnostic and follow-up tool in developmental dysplasia of the hip joint, where hip joint ultrasonography cannot be performed. Artifacts that are seen because of technical errors can cause difficulties in diagnosis and and even can lead to wrong diagnoses and treatments. Herein, we report a diagnostic problem caused by an x-ray artifact in a child who is followed up with developmental dysplasia of the hip joint.

Abstract

Summary: Background: Early death after cancer surgery is higher in low-income and middle-income countries (LMICs) compared with in high-income countries, yet the impact of facility characteristics on early postoperative outcomes is unknown. The aim of this study was to examine the association between hospital infrastructure, resource availability, and processes on early outcomes after cancer surgery worldwide. Methods: A multimethods analysis was performed as part of the GlobalSurg 3 study—a multicentre, international, prospective cohort study of patients who had surgery for breast, colorectal, or gastric cancer. The primary outcomes were 30-day mortality and 30-day major complication rates. Potentially beneficial hospital facilities were identified by variable selection to select those associated with 30-day mortality. Adjusted outcomes were determined using generalised estimating equations to account for patient characteristics and country-income group, with population stratification by hospital. Findings: Between April 1, 2018, and April 23, 2019, facility-level data were collected for 9685 patients across 238 hospitals in 66 countries (91 hospitals in 20 high-income countries; 57 hospitals in 19 upper-middle-income countries; and 90 hospitals in 27 low-income to lower-middle-income countries). The availability of five hospital facilities was inversely associated with mortality: ultrasound, CT scanner, critical care unit, opioid analgesia, and oncologist. Abstract

Background: Laparoscopic cholecystctomy has become the treatment of choice for symptomatic gallstones. The potential risks have dissuaded some surgeons from using the laparoscopic procedure in patients with previous abdominal surgery. Therefore, we aimed to investigate the effect of previous abdominal surgery on the feasibility and safety of laparoscopic cholecystectomy. Methods: This study included 600 well-documented patients with gallstones who underwent laparoscopic cholecystctomy at our surgical department between May 2000 and January 2004. The patients were classified into 3 groups: group 1, patients without a history of previous abdominal surgery (n=408); group 2, patients with a history of upper abdominal surgery (n=92); group 3, patients with a history of lower abdominal surgery (n=100). The data were collected and analyzed for open conversion rates, operative times, perioperative and postoperative complications, and hospital stay. Results: Of the 600 study patients, 192 had undergone previous abdominal surgery (92 upper, 100 lower). Conversion rate, hospital stay, and complication rates were similar in each group. Mean operating time was the longest (57±9.8 min) in patients with previous upper abdominal surgery (P0.05). Conclusion: Previous abdominal surgery is not a contraindication to safe laparoscopic cholecystectomy. However, previous upper abdominal surgery is associated with a prolonged operation time.

Abstract??We sought to investigate the frequency and distribution of colorectal cancer (CRC) in patients by age and to evaluate whether there is a difference between young (<40 years of age) and older patients (?40 years of age) with regard to cancer localizations. From a total of 5165 colonoscopies, 314 (6.0%) cases were identified to have colorectal carcinoma. Forty-one (13%) of 314 CRC patients were young, with a mean age of 31.1?5.7 years. When cancer localizations were compared with reference to age, it was seen that CRCs in young patients were mostly localized at the right colon, versus at the left colon and rectum (P=013) in patients >40 years of age. Tumor localizations in colon cancer patients change with age. In our study, young patients tended to have right-sided colon tumors, but those in patients >40 years of age were frequently localized at the left colon and rectum. [ABSTRACT FROM AUTHOR]

ABSTRACT PURPOSE: To investigate whether topically administered hemostatic agents ankaferd blood stopper and microporous polysaccharide hemospheres can decrease epidural fibrosis after laminectomy in rats. METHODS: Eighteen adult male Sprague-Dawley rats were equally and randomly divided into three groups. In the treatment groups, ankaferd blood stopper and microporous polysaccharide hemospheres topically administrated upon duramater surface after laminectomy. Fibroblast count, epidural fibrosis and arachnoidal involvement were evaluated and graded histopathologically. RESULTS: Our data revealed that the count of fibroblasts, the grading of epidural fibrosis and arachnoideal involvement in the rats treated with microporous polysaccharide hemospheres were significantly less than the control group. Although the arachnoideal involvement in ankaferd blood stopper group were significantly less than the control group, there were no statistical differences when comparing the grading of epidural fibrosis and the fibroblasts count between the treatment groups and the control group. CONCLUSION: The ankaferd blood stopper and microporous polysaccharide hemospheres reduced epidural fibrosis and arachnoideal involvement after laminectomy in rats.

Abstract

Background. In the follow up of chronic hepatitis B infection (HBV), a significant correlation between quantitative HBsAg titer measurement and HBV DNA level, and moreover with intrahepatic covalently closed circular DNA was already shown. However, besides their impact on long-term follow up, they are really expensive methods, and not available widespread. We aimed to investigate the utility of qualitative measurement of HBsAg titer in prediction of virologic response at the end of the first year of anti-viral treatment in chronic HBV infection.Material and methods. A total of 70 patients receiving anti-viral therapies for chronic HBV infection were included into the study. The patients were evaluated according to Hbe Ag status and response to treatment. The determinations used in the study (biochemical, virologic responses, primary non-response) were accepted as it was described in AASLD.Results. Qualitative HBsAg titer increased significantly in both HBeAg positive and negative patients (p values 0.002 and < 0.000). Increasing of HBsAg titer in first three months is more dramatic in responder group, however the difference was disappeared at the sixth and twelve moths on follow up. Similarly, a fast increasing in anti-HBe titer in HBeAg negative chronic HBV patients was related with higher response at the end of first year therapy. However, the changings at the 12th month of anti-viral treatment were similar in both responder and non-responder groups.Conclusion. In conclusion, the fast increase in qualitative measurement of HBsAg titer seemed to be a predictor of higher anti-viral medication success in chronic HBV patients. However, this meaningful increasing was disappeared on the follow up, particularly after the six months examination.

Abstract

This study was performed to determine suitable perennial forage species and their mixtures for the establishment of short-term artificial pastures under rainfed conditions in Turkey. The study was conducted from 2008 through 2010. Alfalfa (Medicago sativa L.), sainfoin (Onobrychis sativa Lam.), brome grass (Bromus inermis Leys.), intermediate wheatgrass (Agropyron intermedium (Host). Beauv.), crested wheatgrass (Agropyron cristatum L. Gaertn.) and their binary and ternary mixtures were used as experimental material. The study found significant differences in yield and quality among the forage mixtures investigated. Sainfoin + bromegrass + crested wheatgrass and sainfoin + crested wheatgrass mixtures gave the highest dry matter yield (8.36 and 7.75 t/ha, respectively). Binary and ternary mixtures of alfalfa + grasses had higher crude protein levels and lower values of ADF and NDF content than mixtures of sainfoin + grasses. Pure alfalfa (56.64%) and binary mixtures of alfalfa + grasses (53.53 to 54.28%) had the highest TDN values. The relative feed values of the mixtures ranged from 95.64 to 112.58. The results of the study indicated that alfalfa and sainfoin binary mixtures with grasses may both be used to establish artificial pastures in similar ecologies owing to their high forage yield and quality.

Abstract

The results of the acute exacerbations of hepatitis B virus are varied from silent to severe acute hepatitis. HBsAg seroconversion induced by either antiviral drugs or occurred spontaneously, as a targeted end point of HBV infection management is infreaquent. The affect of the severe hepatitic flare on HBsAg seroconversion was reported before, however the predictive markers of HBsAg seroconversion are not clear yet. The reasons of the spontaneously HBV re-activations, or its usual results are not known well. We, herein reported a case, with the high serum HBV DNA titer during an acute spontaneously induced severe exacerbation of HBV infection which spontaneously resulted in total HBV recovery.

Abstract

Purpose. We investigated the effect of eritoran, a Toll-like receptor 4 antagonist, on retinochoroidal inflammatory damage in an endotoxin-induced inflammatory rat model. Methods. Endotoxin-induced inflammatory model was obtained by intraperitoneal injection of 1.5 mg/kg lipopolysaccharide (LPS). Group 1 had control rats; in groups 2-3 LPS and 0.5 mg/kg sterile saline were injected; and in groups 4-5 LPS and 0.5 mg/kg eritoran were injected. Blood samples were taken and eyes were enucleated after 12 hours (h) (groups 2 and 4) or 24 hours (Groups 3 and 5). Tumor necrosis factor-α (TNF-α) and malondialdehyde (MDA) levels in the serum and retinochoroidal tissue and nuclear factor kappa-B (NFκB) levels in retinochoroidal tissue were determined. Histopathological examination was performed and retinochoroidal changes were scored. Results. Eritoran treatment resulted in lower levels of TNF-α, MDA, and NFκB after 12 h which became significant after 24 h. Serum TNF-α and retinochoroidal tissue NFκB levels were similar to control animals at the 24th h of the study. Eritoran significantly reversed histopathological damage after 24 h. Conclusions. Eritoran treatment resulted in less inflammatory damage in terms of serum and retinochoroidal tissue parameters.

48. The Effect of Stoma Site Marking on Stomal Complications: A Long-term Retrospective Study.

Abstract

Objective: To identify the effect of stoma site marking on stoma-related complications., Methods: The study sample included 639 individuals with stomas who were followed up in a stomatherapy unit in Turkey between January 1, 2017, and June 20, 2021. Researchers collected patient data from nursing records. Data were evaluated using number, percentage, χ2, and logistic regression tests., Results: Of the individuals with stomas, 60.6% (n = 387) were men, and 72.6% (n = 464) had a cancer diagnosis. Their mean age was 60.16 (SD, 14.81) years. The stoma site was marked preoperatively in of 67.1% of patients (n = 429), and 17.1% (n = 109) developed stoma-related complications. The complication rate was higher in individuals with unmarked stoma sites (25.7%; P = .000), emergency surgeries (25.0%; P = .006), colostomies (23.9%; P = .042), and permanent stomas (28.3%; P = .002). The three most common complications were peristomal skin problems (56.9%), mucocutaneous separation (13.8%), and edema (9.2%)., Conclusions: The incidence of stoma-related complications in the postoperative period was higher in individuals with unmarked stoma sites.
