Abstract

Background Several cancer therapies have been associated with cardiovascular harm in early-phase clinical trials. However, some cardiovascular harms do not manifest until later-phase trials. To limit interdisease variability, we focused on breast cancer. Thus, we assessed the reporting of cardiovascular safety monitoring and outcomes in phase 2 and 3 contemporary breast cancer clinical trials. Methods and Results We searched Embase and Medline records for phase 2 and 3 breast cancer pharmacotherapy trials. We examined exclusion criterion as a result of cardiovascular conditions, adverse cardiovascular event reporting, and cardiovascular safety assessment through cardiovascular imaging, ECG, troponin, or natriuretic peptides. Fisher's exact test was utilized to compare reporting. Fifty clinical trials were included in our study. Patients were excluded because of cardiovascular conditions in 42 (84%) trials. Heart failure was a frequent exclusion criterion (n=31; 62% trials). Adverse cardiovascular events were reported in 43 (86%) trials. Cardiovascular safety assessments were not reported in 23 (46%) trials, whereas natriuretic peptide and troponin assessments were not reported in any trial. Abstract

To determine the association of placental pathology with the severity of necrotizing enterocolitis (NEC) in preterm infants. This single-center matched case-control study included infants with NEC (n = 107) and gestational age and birth weight-matched controls (n = 130), born between 2013 and 2020. Placentas were evaluated according to the Amsterdam Placental Workshop Group Consensus Statement. Acute histologic chorioamnionitis with the fetal response was significantly more common in infants with surgical NEC vs. medical NEC (35.4% vs. 15.3%; p = 0.02). On regression model, infants with multiple placental pathologies (OR 2.16; 95% CI 1.01 − 4.73; p = 0.04) and maternal vascular malperfusion (OR 2.2; 95% CI 1.12 − 4.51; p = 0.02) had higher odds of either medical or surgical NEC than controls. Infants with multiple placental lesions, including placental inflammatory and vascular lesions, were at higher risk of medical or surgical NEC in the postnatal period. [ABSTRACT FROM AUTHOR]

Abstract

Background:Although African Americans (AAs) have worse coronary heart disease (CHD) outcomes compared with whites, few studies have examined the relationship between cigarette smoking and CHD risk among AAs. We evaluated the relationship between cigarette smoking and incident CHD in the Jackson Heart Study (JHS).Methods and Results:JHS participants who completed the first study visit (2000-2004, n=5,306) were classified by self-reported baseline smoking status into current, past (smoked at least 400 cigarettes/life) or never smokers. Current smokers were further classified by smoking intensity [number of cigarettes smoked per day (1-19 or ?20)] and followed for incident CHD (through 2012). CHD events were adjudicated by the JHS. Hazard ratios for incident CHD for each smoking group compared to never smokers were estimated with adjusted Cox proportional hazard regression models. At baseline there were 693 (13%) current, 986 (19%) past and 3579 (68%) never smokers. After adjusting for clinical covariates, the CHD risk in current smokers was significantly higher compared to never smokers (HR 2.05, 95% CI 1.31-3.22) and past smokers also had higher risk for CHD compared to never smokers (HR 1.65, 95% CI 1.18-2.33) (Table). Among current smokers, we did not observe a dose-response effect for CHD risk with smoking intensity.Conclusions:In a large prospective cohort of AAs, current and past cigarette smoking was associated with an increased risk of CHD.

Abstract

Background:Hypertension in pregnancy has been associated with increased risk for albuminuria in the future. However, there are limited data assessing the relationship between hypertensive diseases in pregnancy and renal function later in life. Therefore, we evaluated the relationship between hypertensive diseases in pregnancy and renal function later in life in the Genetic Epidemiology Network of Arteriopathy (GENOA) study.Methods and Results:This is a cross-sectional analysis of 961 women from the GENOA study (mean age 60 ? 9 years, 56 % African American). Participants were classified as nulliparous women (n= 81), women with a history of normotensive pregnancies (n= 741) and a history of a hypertensive pregnancy (n= 139). We compared adjusted associations among these groups with standardized measurements of renal function using generalized estimating equations. After adjusting for risk factors, women with a history of hypertensive pregnancy had lower measured glomerular filtration rate (GFR) by normalized iothalamate clearance and Cystatin C estimated GFR compared to women with a history of normotensive pregnancies (Table).Conclusions:A history of hypertensive pregnancy is associated with a risk for GFR decline (estimated and measured) in later life compared to women with a history of normotensive pregnancies. However, this findings should be interpreted cautiously and requires further study.

43. Abstract 14071: Association of Aortic Stiffness With Subclinical Left Ventricular Remodeling in African Americans

Abstract

Introduction:The relations between aortic stiffness and regional left ventricular (LV) structure and function are not well-established in African Americans.Hypothesis:Greater aortic stiffness is associated with subclinical adverse LV remodeling.Methods:Jackson Heart Study participants without prevalent cardiovascular disease underwent 1.5T cardiovascular magnetic resonance (n=1106). Phase contrast images of the cross-sectional aorta at the pulmonary bifurcation and the diaphragm were acquired to measure aortic pulse wave velocity in the aortic arch (PWV-A) and full ascending and descending thoracic aorta (PWV-T). Short-axis steady state free precession images were acquired to measure LV mass, end-diastolic volume, ejection fraction, and circumferential strain (global and regional). We used multivariable linear regression to relate PWV-A and PWV-T to LV structure and function, and studied effect modification by risk factors including diabetes. We considered p?0.05/14 tests=0.0036 significant.Results:Characteristics of the sample are presented in the TABLE.Greater LVM was associated with higher PWV-A (B=0.17, 95% CI 0.06-0.28, p=0.002). More positive (worse) circumferential strain at the LV base, but not mid or apical segments, was associated with higher PWV-T (B=0.14, 95% CI 0.07-0.20, p<0.001). After accounting for multiple testing, global circumferential strain had a borderline association with PWV-T (p=0.006), but ejection fraction was not associated with PWV-T (p=0.05). We observed effect modification with worse LV basal strain in individuals with diabetes vs. no diabetes (B=0.24, 95% CI 0.10-0.37, p=0.001 vs. B=0.10, 95% CI 0.02-0.17, p=0.01, respectively).Conclusions:In our cross-sectional study, aortic stiffness was associated with adverse subclinical LV remodeling and regional dysfunction in African Americans. Future studies may evaluate the effect of modulation of aortic stiffness on LV structure and function.

Abstract

Background:Previous studies have shown the beneficial effects of physical activity (PA) on cardiovascular disease (CVD) mortality and morbidity, which may be mediated by decreased inflammation and atherosclerosis. The purpose of this study was to investigate the relationships between PA and biomarkers of inflammation representing different domains of the inflammatory cascade associated with atherosclerotic CVD.Methods:We investigated the cross-sectional associations of PA with 17 biomarkers (high sensitivity CRP, serum amyloid A, ICAM, VCAM, IL-6, IL-18, TNF receptor-1, TNF receptor-2, MCP-1, E-selectin, P-selectin, Myeloperoxidase, RAGE, MMP-2, MMP-9, TIMP-1 and TIMP-2) among GENOA study participants (African Americans recruited from Jackson, MS and whites from Rochester, MN). Hypertension before age 60 years in at least 2 siblings in a sibship was required for enrollment. PA index was calculated by weighting self-reported hours of activity: 3*heavy activity hours/day+ 2*moderate activity hours/day + slight activity hours/day. The relationships between PA index and biomarkers were evaluated using adjusted generalized estimating equations accounting for familial clustering.Results:Among 2702 participants (mean age 61?10 years, 64.5% women, 54.1% African American) 5 of 17 biomarkers (high sensitivity CRP, serum amyloid A, IL-6, MCP-1, and TIMP-1) were inversely associated with PA index at a Bonferroni adjusted p-value threshold (Table, p<0.003).Conclusion:PA was negatively associated with several biomarkers of inflammation and more associated with systemic inflammation than specific domains of inflammatory cascades. Further investigation is warranted into the mechanistic insights of the potential beneficial effects of PA on inflammation and atherosclerosis.

Abstract

Background:Few studies have examined the relationship between hypertensive diseases in pregnancy with cardiac structure and function later in life. Therefore, we evaluated the relationship between hypertensive diseases in pregnancy with cardiac structure and function later in life in the Genetic Epidemiology Network of Arteriopathy (GENOA) study.Methods and results:In this cross-sectional analysis, we investigated 1013 African American women with echocardiographic measurements from the GENOA study (Phase II, 2000-05; Jackson, MS). Participants were classified as nulliparous women (n= 61), women with a history of normotensive pregnancies (n= 780), a history of a hypertensive pregnancy (n= 152), or a history of preeclampsia (n= 20). We compared adjusted associations among this groups with echocardiographic measurements of cardiac structure and function using generalized estimating equations. The mean age of participants was 62 ? 9.5 years at the time of echocardiography. After adjusting for clinical covariates, women with a history of hypertensive pregnancy had higher left atrial systolic dimension compared to women with a history of normotensive pregnancies (Table, P= 0.013). There were no significant differences in left ventricular ejection fraction, left ventricular mass index, relative wall thickness, diastolic posterior wall thickness, left ventricular internal diastolic dimension, left ventricular internal systolic dimension, end diastolic volume, and end systolic volume dimensions among this groups.Conclusions:A history of hypertension in pregnancy is associated with higher left atrial systolic dimension compared to women with a history of normotensive pregnancies. However, given the multiple comparisons considered, this finding should be interpreted cautiously and requires further study.

Abstract

Background: Increased hsCRP (high-sensitivity C-reactive protein), a marker of inflammation, is associated with incident cardiovascular events. We aim to determine whether the baseline or trajectory of hsCRP levels over time predicts incident heart failure (HF) hospitalization., Methods: JHS (Jackson Heart Study) participants' (n=3920 Black adults) hsCRP levels were measured over 3 visits (from 2000 to 2013). We assessed the association of hsCRP at baseline (visit 1) with incident HF hospitalization using Cox proportional hazards models. Furthermore, we assessed the association of the trajectory of hsCRP over repeated measurements (visits 1-3) with incident HF using joint models. Hazard ratios are reflective of an increase in hsCRP by 1 SD on a log2 scale. We also assessed the association of change in hsCRP between visit 1 and visit 3 with Cox proportional hazards models by grouping patients by low (<2 mg/L) and high (≥2 mg/L) hsCRP levels. The 4 groups were low-to-low (referent), low-to-high, high-to-low, and high-to-high., Results: Mean baseline age of participants was 54±13 years, and 63.8% were women. Over a median follow-up of 12 years, 308 (7.9%) participants were hospitalized with incident HF. Baseline hsCRP was not associated with incident HF (adjusted hazard ratio, 1.08 [95% CI, 0.96-1.22]). However, increasing hsCRP levels over repeated measures were associated with a higher risk of incident HF overall (adjusted hazard ratio, 1.22 [95% CI, 1.03-1.44]) and HF with preserved ejection fraction (adjusted hazard ratio, 1.30 [95% CI, 1.02-1.65]) but not HF with reduced ejection fraction ( P >0.05). Furthermore, changes in hsCRP from low-to-high and high-to-low levels were associated with incident HF ( P <0.05)., Conclusions: While baseline hsCRP was not associated with incident HF, an increasing trajectory of hsCRP over time was associated with increased risk for incident HF (particularly HF with preserved ejection fraction).
