Abstract

We present the case of a 28-year-old woman with oto-spondylo-megaepiphyseal dysplasia, which is a rare skeletal disorder, who underwent bilateral total hip arthroplasty. Full-scale three-dimensional plaster model of the acetabulum and the femur provided us with a feasible preoperative plan. Pre- and postoperative three-dimensional motion analyses proved a significant improvement in her ambulation and confirmed the efficacy of total hip arthroplasty. In conclusion, full-scale three-dimensional plaster models of the bone and three-dimensional motion analyses were useful for total hip arthroplasty in patients with skeletal dysplasia.

Abstract

Objective We aimed to assess the diagnostic utility of the linguistically validated Japanese version of the Leeds Assessment of Neuropathic Symptoms and Signs Pain Scale ( LANSS-J) as a screening tool for neuropathic pain in the clinical setting. Methods Patients with neuropathic pain or nociceptive pain who were 20 to 85 years of age were included. Sensitivity and specificity using the original cutoff value of 12 were assessed to evaluate the diagnostic utility of the LANSS-J. Sensitivity and specificity with possible cutoff values were calculated, along with area under the receiver operating characteristic curve. We then evaluated agreement regarding assessment of the LANSS-J by two investigators. We used the intraclass correlation coefficient ( ICC) for the total score and Cohen's kappa coefficient for each item. Results Data for patients with neuropathic pain ( n = 30) and those with nociceptive pain ( n = 29) were analyzed. With a cutoff of 12, the sensitivity was 63.3% (19/30) and the specificity 93.1% (27/29). Sensitivity improved substantially with a cutoff of ≤ 11 (≥ 83.3%, 25/30). High specificity (93.1%, 27/29) was sustained with a cutoff of 9 to 12. The ICC for the total score was 0.85, indicating sufficient agreement. Kappa coefficients ranged from 0.68 to 0.84. Conclusions The LANSS-J is a valid screening tool for detecting neuropathic pain. Our results suggest that employing the original cutoff value provides high specificity, although a lower cutoff value of 10 or 11 (with its high specificity maintained) may be more beneficial when pain attributed to neuropathic mechanisms is suspected in Japanese patients. Abstract

平成9年8月に発生した「ウラン廃棄物屋外貯蔵ピット問題」を契機に、旧東海製錬所における製錬事業の終了に伴い残された旧鉱さいたい積場および原料鉱石の状況、各施設の解体後の状況を調査した。この結果、事業所構内の限定された場所に埋設している残材等があることが判明したため、調査結果とその後の調査計画をとりまとめ、平成9年9月18日に「旧東海製錬所に関連する残材等の調査について」を国、関係自治体等に報告するとともに、公表した。その後、この調査計画に従って実施した結果、鉱さいや鉱石等の残材の埋設場所を特定した。また、平成10年6月に発生した「プルトニウム燃料工場屋外器材ピット問題」における廃棄物(一般器材等)保管状況の改善措置に係る水平展開として、再処理施設から発生した非放射性器材や焼却灰の処置状況等の調査についても実施した。これらの結果から周辺環境への影響の観点からも問題のないことが確認できた。, None

Abstract

プルトニウム燃料第二開発室湿式回収室(F-104室)に設置している、湿式回収工程脱硝設備の解体撤去を実施した。本設備はグローブボックス12基(グローブボックス総容積107m/SUP3)からなり、過去に実施した解体撤去のうちでも設備規模が最大のものである。また、グローブボックス内の残留プルトニウム汚染が5.4 Ci/cm/SUP2と極めて高く、作業の安全確保及び作業員の放射線被ばくの防止を確実に実施する必要があった。グローブボックスの解体は昭和63年1月の室内間仕切り工事の後、非汚染物の撤去からはじめ、平成元年1月にトラブルゼロで完了した。安全確保・向上のために実施した事前放射線モニタリング、簡易フードの設置等の各種対策や技術改良が有効であったことを確認した。, None

Abstract

ABSTRACT To prevent periprosthetic osteolysis and subsequent aseptic loosening of artificial hip joints, we recently developed a novel acetabular highly cross-linked polyethylene (CLPE) liner with graft polymerization of 2-methacryloyloxyethyl phosphorylcholine (MPC) on its surface. We investigated the wear resistance of the poly(MPC) (PMPC)-grafted CLPE liner during 20 million cycles in a hip joint simulator. We extended the simulator test of one liner to 70 million cycles to investigate the long-term durability of the grafting. Gravimetric, surface, and wear particle analyses revealed that PMPC grafting onto the CLPE liner surface markedly decreased the production of wear particles and showed that the effect of PMPC grafting was maintained through 70 million cycles. 29. Characteristics of M-Gtfi, A New Inhibitor of Streptococcus Mutans Glucosyltransferase

Abstract

M-GTFI, originally screened as an inhibitor of Streptococcus mutans glucosyltransferase, strongly inhibited α-glucosidase, in a non-competitive manner especially when the synthetic substrate p-nitrophenyl-α-D-glucopyranoside was used. It also inhibited β-glucosidase, β-amylase and, to a lesser extent, β-glu-curonidase.The inhibitor was stable in neutral and alkaline pH ranges and dependency of the inhibition on pH and temperature was not observed. Some proteinases and polysaccharides-hydrolyzing enzymes as well as human saliva did not inactivate the inhibitor.There was a correlation between the release of sulfate anions from the inhibitor molecule on incubation with HCI (0.2 N) at 100°c and loss of inhibitory properties of the molecule. It is suggested that the presence of sulfate ester linkages in the inhibitor molecule play an important role in the inhibition process.

Abstract

M-GTFI, an inhibitor of glucosyltransferase from S. mutans was produced by Micromonospora narashinoensis strain No. 731. The isolation procedure for M-GTFI was improved and established for spectro-scopic analyses, and some properties of the inhibitor were investigated. The structure of M-GTFI was shown to be trisodium [2-sulphonato-9-undecenyll-oxacyclotriacont-3-en-2-one, 16, 18-his sulp-honate. The chemical structure of M-GTFI was therefore similar to that of izumenolide which is a β-lactamase inhibitor containing sulfate ester groups in its molecule.The inhibitory characteristics of M-GTFI were parallel to that of other inhibitory compounds containing sulphate esters but the spectrum of activity was wider.

打投したコンクリートは数時間後に発熱する。放熱が少ない場合には,コンクリート内の温度が上昇し,場合によってはこれが原因でクラックが発生する。原子力関連施設ではマスコンクリートの使用が多くなり単位セメント量が多いため発熱量も大きく,場合によってはクラックの発生等が予想される。原子力施設では遮へい,気密性の要求から極力クラックの原因を取り除く必要がある。今回,プルトニウム転換技術開発施設及び再処理工場第2排気筒の建設工事において,マスコンクリートの発熱を実測する機会が得られたので,これらデータをもとに一連の熱解析,熱応力解析を試み結果をまとめた。, None
