1. Impact of dose reductions on adjuvant abemaciclib efficacy for patients with high-risk early breast cancer: analyses from the monarchE study.

Abstract

In monarchE, adjuvant abemaciclib significantly improved invasive disease-free survival (IDFS) and distant relapse-free survival (DRFS), with sustained benefit beyond the 2-year treatment period. Abemaciclib dose reductions were allowed to proactively manage adverse events. Exploratory analyses to investigate the impact of dose reductions on efficacy were conducted. Across the three patient subgroups as defined by relative dose intensity (≤66%, 66-93%, ≥93%), the estimated 4-year IDFS rates were generally consistent (87.1%, 86.4%, and 83.7%, respectively). In the time-dependent Cox proportional hazard model, the effect of abemaciclib was consistent at the full dose compared to being reduced to a lower dose (IDFS hazard ratio: 0.905; 95% confidence interval: 0.727, 1.125; DRFS hazard ratio: 0.942; 95% confidence interval: 0.742, 1.195). These analyses showed that the efficacy of adjuvant abemaciclib was not compromised by protocol mandated dose reductions for patients with node positive, hormone receptor positive, human epidermal growth factor 2-negative, high-risk early breast cancer.

Abstract

Clinical trials frequently include multiple end points that mature at different times. The initial report, typically based on the primary end point, may be published when key planned co-primary or secondary analyses are not yet available. Clinical trial updates provide an opportunity to disseminate additional results from studies, published in JCO or elsewhere, for which the primary end point has already been reported.Two years of adjuvant abemaciclib combined with endocrine therapy (ET) resulted in a significant improvement in invasive disease-free survival (IDFS) and distant relapse-free survival (DRFS) that persisted beyond the 2-year treatment period in patients with hormone receptor-positive, human epidermal growth factor receptor 2-negative, node-positive, high-risk early breast cancer (EBC). Here, we report 5-year efficacy results from a prespecified overall survival (OS) interim analysis. In the intent-to-treat population, with a median follow-up of 54 months, the benefit of abemaciclib was sustained with hazard ratios of 0.680 (95% CI, 0.599 to 0.772) for IDFS and 0.675 (95% CI, 0.588 to 0.774) for DRFS. This persistence of abemaciclib benefit translated to continuous separation of the curves with a deepening in 5-year absolute improvement in IDFS and DRFS rates of 7.6% and 6.7%, respectively, compared with rates of 6% and 5.3% at 4 years and 4.8% and 4.1% at 3 years. With fewer deaths in the abemaciclib plus ET arm compared with the ET-alone arm (208 v 234), statistical significance was not reached for OS. No new safety signals were observed. In conclusion, abemaciclib plus ET continued to reduce the risk of developing invasive and distant disease recurrence beyond the completion of treatment. The increasing absolute improvement at 5 years is consistent with a carryover effect and further supports the use of abemaciclib in patients with high-risk EBC.

3. Promoting Women in Mathematics through Creating a Learning Community and Encouraging Double Majors

Abstract

The Mathematical Sciences Department at Shenandoah University focused on increasing the participation of women in the mathematics program by fostering a welcoming learning community and promoting double majors. Data from the past decade is presented. During this time, the program saw an increase in women undergraduate mathematics majors from 11% to as high as 71%. This paper describes the initiatives taken to make these changes.

4. Understanding Frequency of and Time Spent on Editing in Writing Center Tutorials

Abstract

This longitudinal study serves as a model for examining and understanding the role of editing and its contributing factors in writing center tutorials. The study quantifies the frequency of and time devoted to editing in over 4,000 writing center tutorials and finds these quantities differ in statistically significant ways when broken down by user groups (gender, student standing, course level, and primary language); tutoring modes (face-to-face, synchronous-online, and asynchronous); and time of student visit relative to project due date. Analysis identifies contributing factors that influence frequency of and time spent on editing in tutorials by these groups, including group membership and their associated editing practices and tutorial habits. The study's findings fill an important gap in our understanding of what takes place in writing center tutorials and have significant implications for tutor training and marketing of writing center services.

5. Multivalent Interactions Drive the Toxoplasma AC9:AC10:ERK7 Complex To Concentrate ERK7 in the Apical Cap.

Abstract

The Toxoplasma inner membrane complex (IMC) is a specialized organelle that is crucial for the parasite to establish an intracellular lifestyle and ultimately cause disease. The IMC is composed of both membrane and cytoskeletal components, further delineated into the apical cap, body, and basal subcompartments. The apical cap cytoskeleton was recently demonstrated to govern the stability of the apical complex, which controls parasite motility, invasion, and egress. While this role was determined by individually assessing the apical cap proteins AC9, AC10, and the mitogen-activated protein kinase ERK7, how the three proteins collaborate to stabilize the apical complex is unknown. In this study, we use a combination of deletion analyses and yeast two-hybrid experiments to establish that these proteins form an essential complex in the apical cap. We show that AC10 is a foundational component of the AC9:AC10:ERK7 complex and demonstrate that the interactions among them are critical to maintaining the apical complex. Importantly, we identify multiple independent regions of pairwise interaction between each of the three proteins, suggesting that the AC9:AC10:ERK7 complex is organized by multivalent interactions. Together, these data support a model in which multiple interacting domains enable the oligomerization of the AC9:AC10:ERK7 complex and its assembly into the cytoskeletal IMC, which serves as a structural scaffold that concentrates ERK7 kinase activity in the apical cap. IMPORTANCE The phylum Apicomplexa consists of obligate, intracellular parasites, including the causative agents of toxoplasmosis, malaria, and cryptosporidiosis. Hallmarks of these parasites are the IMC and the apical complex, both of which are unique structures that are conserved throughout the phylum and required for parasite survival. The apical cap portion of the IMC has previously been shown to stabilize the apical complex. Here, we expand on those studies to determine the precise protein-protein interactions of the apical cap complex that confer this essential function. We describe the multivalent nature of these interactions and show that the resulting protein oligomers likely tether ERK7 in the apical cap. This study represents the first description of the architecture of the apical cap at a molecular level, expanding our understanding of the unique cell biology that drives Toxoplasma infections.

7. Environmental chemical exposures and mental health outcomes in children: a narrative review of recent literature

Abstract

Background: Mental health is an important factor for children’s overall wellbeing. National health statistics show that millions of children are diagnosed with mental health disorders every year, and evidence from studies on chemical pollutants like lead and bisphenols indicate that environmental exposures are linked to mental health illnesses in youth. However, the relationship between children’s mental health and the environment is not well understood. This paper aims to review recent literature on prenatal and/or childhood environmental chemical exposures and mental health problems related to mood, anxiety, and behavior. This work also identifies areas of insufficient data and proposes suggestions to fill the data gaps.Methods: A narrative review was performed by searching Google Scholar and PubMed for literature published in the last 6 years (2017–2022), using search terms related to children, mental health, and environmental chemical exposure. Additional relevant studies were identified by screening the references in these papers.Results: A total of 29 studies are included in this review and results are summarized by chemical category: heavy metals, endocrine-disrupting chemicals, and pesticides. The majority of studies reported positive and significant associations between chemical exposures and child mental health outcomes including internalizing and externalizing behaviors.Conclusion: This review demonstrates that there is a growing body of literature that suggests developmental exposure to some environmental chemicals increases a child’s risk of mood, anxiety, and behavior problems. Future research should expand on these findings to understand cumulative impacts, chemical mixtures, neurotoxic mechanisms, sex differences, and windows of vulnerability.

Abstract

We analyze the impact of a proposed tidal instability coupling $p$-modes and $g$-modes within neutron stars on GW170817. This non-resonant instability transfers energy from the orbit of the binary to internal modes of the stars, accelerating the gravitational-wave driven inspiral. We model the impact of this instability on the phasing of the gravitational wave signal using three parameters per star: an overall amplitude, a saturation frequency, and a spectral index. Incorporating these additional parameters, we compute the Bayes Factor ($\ln B^{pg}_{!pg}$) comparing our $p$-$g$ model to a standard one. We find that the observed signal is consistent with waveform models that neglect $p$-$g$ effects, with $\ln B^{pg}_{!pg} = 0.03^{+0.70}_{-0.58}$ (maximum a posteriori and 90% credible region). By injecting simulated signals that do not include $p$-$g$ effects and recovering them with the $p$-$g$ model, we show that there is a $\simeq 50\%$ probability of obtaining similar $\ln B^{pg}_{!pg}$ even when $p$-$g$ effects are absent. We find that the $p$-$g$ amplitude for 1.4 $M_\odot$ neutron stars is constrained to $\lesssim \text{few}\times10^{-7}$, with maxima a posteriori near $\sim 10^{-7}$ and $p$-$g$ saturation frequency $\sim 70\, \mathrm{Hz}$. This suggests that there are less than a few hundred excited modes, assuming they all saturate by wave breaking. For comparison, theoretical upper bounds suggest a $p$-$g$ amplitude $\lesssim 10^{-6}$ and $\lesssim 10^{3}$ modes saturating by wave breaking. Thus, the measured constraints only rule out extreme values of the $p$-$g$ parameters. They also imply that the instability dissipates $\lesssim 10^{51}\, \mathrm{ergs}$ over the entire inspiral, i.e., less than a few percent of the energy radiated as gravitational waves., Comment: 7 pages, 2 figures

Abstract

The MAJORANA collaboration is constructing the MAJORANA DEMONSTATOR at the Sanford Underground Research Facility at the Homestake gold mine, in Lead, SD. The apparatus will use Ge detectors, enriched in isotope \nuc{76}{Ge}, to demonstrate the feasibility of a large-scale Ge detector experiment to search for neutrinoless double beta decay. The long half-life of this postulated process requires that the apparatus be extremely low in radioactive isotopes whose decays may produce backgrounds to the search. The radioassay program conducted by the collaboration to ensure that the materials comprising the apparatus are sufficiently pure is described. Abstract

Neutrinoless double beta decay searches play a major role in determining neutrino properties, in particular the Majorana or Dirac nature of the neutrino and the absolute scale of the neutrino mass. The consequences of these searches go beyond neutrino physics, with implications for Grand Unification and leptogenesis. The \textsc{Majorana} Collaboration is assembling a low-background array of high purity Germanium (HPGe) detectors to search for neutrinoless double-beta decay in $^{76}$Ge. The \textsc{Majorana Demonstrator}, which is currently being constructed and commissioned at the Sanford Underground Research Facility in Lead, South Dakota, will contain 44 kg (30 kg enriched in $^{76}$Ge) of HPGe detectors. Its primary goal is to demonstrate the scalability and background required for a tonne-scale Ge experiment. This is accomplished via a modular design and projected background of less than 3 cnts/tonne-yr in the region of interest. Mastery-based testing is an assessment scheme that challenges students to provide complete solutions to problems derived from clear course concepts. Students are allowed multiple attempts to demonstrate mastery, which helps create a classroom environment where students value persistence toward thorough understanding. In this paper, we describe in detail the benefits and implementation of mastery-based testing in college mathematics courses. We also summarize student response data that show positive reactions to this testing method.

Abstract

The {\sc Majorana} collaboration is searching for neutrinoless double beta decay using $^{76}$Ge, which has been shown to have a number of advantages in terms of sensitivities and backgrounds. The observation of neutrinoless double-beta decay would show that lepton number is violated and that neutrinos are Majorana particles and would simultaneously provide information on neutrino mass. Attaining sensitivities for neutrino masses in the inverted hierarchy region, $15 - 50$ meV, will require large, tonne-scale detectors with extremely low backgrounds, at the level of $\sim$1 count/t-y or lower in the region of the signal. The {\sc Majorana} collaboration, with funding support from DOE Office of Nuclear Physics and NSF Particle Astrophysics, is constructing the {\sc Demonstrator}, an array consisting of 40 kg of p-type point-contact high-purity germanium (HPGe) detectors, of which $\sim$30 kg will be enriched to 87% in $^{76}$Ge. The {\sc Demonstrator} is being constructed in a clean room laboratory facility at the 4850' level (4300 m.w.e.) of the Sanford Underground Research Facility (SURF) in Lead, SD. It utilizes a compact graded shield approach with the inner portion consisting of ultra-clean Cu that is being electroformed and machined underground. Abstract

Advanced gravitational wave detectors, currently under construction, are expected to directly observe gravitational wave signals of astrophysical origin. The Einstein Telescope, a third-generation gravitational wave detector, has been proposed in order to fully open up the emerging field of gravitational wave astronomy. In this article we describe sensitivity models for the Einstein Telescope and investigate potential limits imposed by fundamental noise sources. A special focus is set on evaluating the frequency band below 10Hz where a complex mixture of seismic, gravity gradient, suspension thermal and radiation pressure noise dominates. We develop the most accurate sensitivity model, referred to as ET-D, for a third-generation detector so far, including the most relevant fundamental noise contributions., Comment: 13 pages, 7 pictures

The Mathematical Sciences Department at Shenandoah University focused on increasing the participation of women in the mathematics program by fostering a welcoming learning community and promoting double majors. Data from the past decade is presented. During this time, the program saw an increase in women undergraduate mathematics majors from 11% to as high as 71%. 19. Understanding Frequency of and Time Spent on Editing in Writing Center Tutorials.

Abstract

This longitudinal study serves as a model for examining and understanding the role of editing and its contributing factors in writing center tutorials. The study quantifies the frequency of and time devoted to editing in over 4,000 writing center tutorials and finds these quantities differ in statistically significant ways when broken down by user groups (gender, student standing, course level, and primary language); tutoring modes (face-to-face, synchronous-online, and asynchronous); and time of student visit relative to project due date. Analysis identifies contributing factors that influence frequency of and time spent on editing in tutorials by these groups, including group membership and their associated editing practices and tutorial habits. The study's findings fill an important gap in our understanding of what takes place in writing center tutorials and have significant implications for tutor training and marketing of writing center services. Abstract

Structural Topology Optimization typically features continuum-based descriptions of the investigated systems. In Part 1 we have proposed a Topology Optimization method for discrete systems and tested it on quasi-static 2D problems of stiffness maximization, assuming linear elastic material. However, discrete descriptions become particularly convenient in the failure and post-failure regimes, where discontinuous processes take place, such as fracture, fragmentation, and collapse. Here we take a first step towards failure problems, testing Discrete Element Topology Optimization for systems with nonlinear material responses. The incorporation of material nonlinearity does not require any change to the optimization method, only using appropriately rich interaction potentials between the discrete elements. Three simple problems are analysed, to show how various combinations of material nonlinearity in tension and compression can impact the optimum geometries. We also quantify the strength loss when a structure is optimized assuming a certain material behavior, but then the material behaves differently in the actual structure. For the systems considered here, assuming weakest material during optimization produces the most robust structures against incorrect assumptions on material behavior. Such incorrect assumptions, instead, are shown to have minor impact on the serviceability of the optimized structures.

22. Environmental chemical exposures and mental health outcomes in children: a narrative review of recent literature.

Abstract

Background: Mental health is an important factor for children's overall wellbeing. National health statistics show that millions of children are diagnosed with mental health disorders every year, and evidence from studies on chemical pollutants like lead and bisphenols indicate that environmental exposures are linked to mental health illnesses in youth. However, the relationship between children's mental health and the environment is not well understood. This paper aims to review recent literature on prenatal and/or childhood environmental chemical exposures and mental health problems related to mood, anxiety, and behavior. This work also identifies areas of insufficient data and proposes suggestions to fill the data gaps. Methods: A narrative review was performed by searching Google Scholar and PubMed for literature published in the last 6 years (2017-2022), using search terms related to children, mental health, and environmental chemical exposure. Additional relevant studies were identified by screening the references in these papers. Results: A total of 29 studies are included in this review and results are summarized by chemical category: heavy metals, endocrine-disrupting chemicals, and pesticides. The majority of studies reported positive and significant associations between chemical exposures and child mental health outcomes including internalizing and externalizing behaviors. Conclusion: This review demonstrates that there is a growing body of literature that suggests developmental exposure to some environmental chemicals increases a child's risk of mood, anxiety, and behavior problems. Future research should expand on these findings to understand cumulative impacts, chemical mixtures, neurotoxic mechanisms, sex differences, and windows of vulnerability. [ABSTRACT FROM AUTHOR]

Abstract

Study DesignSecondary analysis of a randomized controlled trial.ObjectiveTo identify preoperative physical variables associated with favorable postoperative outcome in individuals undergoing laminectomy or laminotomy for degenerative central lumbar spinal stenosis.Summary of Background DataClinical or condition specific variables have most commonly been studied as predictors of postoperative outcome in lumbar spinal stenosis. If associated to favorable postoperative outcome, modifiable physical variables would inform prehabilitation interventions for patients with degenerative central lumbar spinal stenosis.MethodsPatients awaiting surgery for central lumbar spinal stenosis were recruited to participate in a randomized controlled trial. Following baseline data collection of demographics, clinical portrait and physical testing, participants were randomized to either 6-week active prehabilitation program or hospital standard care. Complete baseline and postoperative data were obtained from 58 participants which were included in the present analysis. Favorable postoperative outcome was determined based on two outcome measures. Favorable outcome was defined as a decrease of ≥30% on the Numerical Rating Scale for leg pain intensity and a decrease of ≥30% on the Oswestry Disability Index for low back disability. Baseline physical variables were used to conduct binary logistic regression.ResultsSixty percent of participants were determined as having a favorable postoperative outcome. None of the included physical variables were found to be predictors of a favorable postoperative outcome based on leg pain intensity and low back pain-associated disability [trunk flexors muscle strength (OR = 0.73; 95%CI (0.02–27.12)] lumbar extensors muscle endurance [OR= 1.09; 95%CI (0.95–1.24)] total ambulation time [OR = 1.00 95%CI (0.99–1.01)] lumbar active range of motion in extension [OR = 1.08; 95%CI (0.95–1.23)] and knee extensors muscle strength [OR=1.02; 95%CI (0.98–1.06)].ConclusionResults show that none of the investigated variables, all related to low back and lower limbs physical capacity, were predictors of postoperative recovery. Further testing in larger cohort is needed to assess the full potential of physical outcome measures as predictors of postoperative recovery.

Abstract

There exists a never-ending “arms race” between malware analysts and adversarial malicious code developers as malevolent programs evolve and countermeasures are developed to detect and eradicate them. Malware has become more complex in its intent and capabilities over time, which has prompted the need for constant improvement in detection and defence methods. Of particular concern are the anti-analysis obfuscation techniques, such as packing and encryption, that are employed by malware developers to evade detection and thwart the analysis process. In such cases, malware is generally impervious to basic analysis methods and so analysts must use more invasive techniques to extract signatures for classification, which are inevitably not scalable due to their complexity. In this article, we present a hybrid framework for malware classification designed to overcome the challenges incurred by current approaches. The framework incorporates novel static and dynamic malware analysis methods, where static malware executables and dynamic process memory dumps are converted to images mapped through space-filling curves, from which visual features are extracted for classification. The framework is less invasive than traditional analysis methods in that there is no reverse engineering required, nor does it suffer from the obfuscation limitations of static analysis. On a dataset of 13,599 obfuscated and non-obfuscated malware samples from 23 families, the framework outperformed both static and dynamic standalone methods with precision, recall and accuracy scores of 97.6%, 97.6% and 97.6% respectively.

Abstract

Multihadronic e+e- annihilation events at a center-of-mass energy of 29 GeV have been studied with both the original (PEP 5) Mark II and the upgraded Mark II detectors. Detector-corrected distributions from global shape analyses such as aplanarity, Q2-Q1, sphericity, thrust, minor value, oblateness, and jet masses, and inclusive charged-particle distributions including x, rapidity, p, and particle flow are presented. These distributions are compared with predictions from various multihadron event models which use leading-logarithmic shower evolution or QCD matrix elements at the parton level and string or cluster fragmentation for hadronization. The new generation of parton-shower models gives, on the average, a better description of the data than the previous parton-shower models. The energy behavior of these models is compared to existing e+e- data. The predictions of the models at a center-of-mass energy of 93 GeV, roughly the expected mass of the Z0, are also presented. © 1988 The American Physical Society.

46. How Odd is Betelgeuse?

Abstract

The role of massive stars (those with masses greater than eight solar masses) in the chemical enrichment of galaxies and renewed star formation forms an important field of fundamental research in modern astrophysics. The complex nature of massive star evolution produces formidable challenges in furthering our understanding of the processes driving massive star evolution. Our ever-changing picture is further complicated by the relatively recent realisation that the vast majority of massive stars are formed in multiple star systems. In close binaries, interactions between the stars can lead to an exchange of matter, dramatically altering the structure and evolution of the component stars and the binary system as a whole. Depending on the initial binary configuration, this transfer of mass may be stable or unstable, producing results varying from wide binaries with massive secondaries, to common envelopes with tight binaries or even stellar mergers. A fraction of binary systems will be disrupted when the primary star explodes as a supernova, producing a runaway secondary star. Alpha-Orionis, commonly known as Betelgeuse, is believed to be the product of such a scenario. We synthesise a realistic population of binary systems containing massive stars and simulate the evolution of these systems accounting for mass transfer through Roche lobe overflow. We analyse the probability for each system to be disrupted after the supernova explosion of the primary for a range of supernova kicks and determine which fraction of these systems will produce a secondary star in the estimated mass range of Betelgeuse. We also investigate which mass transfer channel is most likely to produce a Betelgeuse-like star. We find that ~ 14% and ~ 25% of binaries will produce a secondary in the mass ranges 13-18 M_sun and 11-20 M_sun, respectively, allowing for the large uncertainties in determining the mass of Betelgeuse. Of these Betelgeuse candidates, approximately 75% emerge from systems follow, Mellan hösten 2019 och våren 2020, fängslades både professionella och amatöra astronomer av de besynnerliga händelserna av Betelgeuse. De lever snabbt och dör unga, deras livslängd mäts i miljoner år istället för miljarder - bara ett ögonblick på kosmologiska tidsskalor. Att komplicera frågor ytterligare, studier tyder på att de flesta massiva stjärnor inte bildas isolerat; snarare är de födda i system som innehåller två (eller fler) stjärnor. När dess stjärnor utvecklas och påverkar varandra, utbyter de material, som förändra deras struktur och ytterligare utveckling. Abstract

This paper presents a signal processing system that: (1) provides an efficient and reliable instrument for investigating the activity of neuronal assemblies in the brain; and (2) demonstrates the feasibility of generating the command signals of prostheses using the activity of relevant neurons in disabled subjects. The system operates online, in a fully automated manner and can recognize the transient waveforms of several neurons in extracellular neurophysiological recordings. Optimal algorithms for detection, classification, and resolution of overlapping waveforms are developed and evaluated. Full automation is made possible by an algorithm that can set appropriate decision thresholds and an algorithm that can generate templates on-line. The system is implemented with a fast IBM PC compatible processor board that allows on-line operation.
