BACKGROUND: Self-management programs have beneficial effects on asthma control, but their implementation in clinical practice is poor. Mobile health (mHealth) could play an important role in enhancing self-management.OBJECTIVE: To assess the clinical effectiveness and technology acceptance of myAirCoach-supported self-management on top of usual care in patients with asthma using inhalation medication.METHODS: Patients were recruited in 2 separate studies. The myAirCoach system consisted of an inhaler adapter, an indoor air-quality monitor, a physical activity tracker, a portable spirometer, a fraction exhaled nitric oxide device, and an app. The primary outcome was asthma control; secondary outcomes were exacerbations, quality of life, and technology acceptance. In study 1, 30 participants were randomized to either usual care or myAirCoach support for 3 to 6 months; in study 2, 12 participants were provided with the myAirCoach system in a 3-month before-after study.RESULTS: In study 1, asthma control improved in the intervention group compared with controls (Asthma Control Questionnaire difference, 0.70; P = .006). A total of 6 exacerbations occurred in the intervention group compared with 12 in the control group (hazard ratio, 0.31; P = .06). Asthma-related quality of life improved (mini Asthma-related Quality of Life Questionnaire difference, 0.53; P = .04), but forced expiratory volume in 1 second was unchanged. In study 2, asthma control improved by 0.86 compared with baseline (P = .007) and quality of life by 0.16 (P=.64). Participants reported positive attitudes toward the system.DISCUSSION: Using the myAirCoach support system improves asthma control and quality of life, with a reduction in severe asthma exacerbations. The daily energy requirements of animals are determined by a combination of physical and physiological factors, but food availability may challenge the capacity to meet nutritional needs. Western gorillas (Gorilla gorilla) are an interesting model for investigating this topic because they are folivore-frugivores that adjust their diet and activities to seasonal variation in fruit availability. Observations of one habituated group of western gorillas in Bai-Hokou, Central African Republic (December 2004-December 2005) were used to examine seasonal variation in diet quality and nutritional intake. We tested if during the high fruit season the food consumed by western gorillas was higher in quality (higher in energy, sugar, fat but lower in fibre and antifeedants) than during the low fruit season. Food consumed during the high fruit season was higher in digestible energy, but not any other macronutrients. Second, we investigated whether the gorillas increased their daily intake of carbohydrates, metabolizable energy (KCal/g OM), or other nutrients during the high fruit season. Intake of dry matter, fibers, fat, protein and the majority of minerals and phenols decreased with increased frugivory and there was some indication of seasonal variation in intake of energy (KCal/g OM), tannins, protein/fiber ratio, and iron. Intake of non-structural carbohydrates and sugars was not influenced by fruit availability. Gorillas are probably able to extract large quantities of energy via fermentation since they rely on proteinaceous leaves during the low fruit season. Macronutrients and micronutrients, but not digestible energy, may be limited for them during times of low fruit availability because they are hind-gut fermenters. We discuss the advantages of seasonal frugivores having large dietary breath and flexibility, significant characteristics to consider in the conservation strategies of endangered species.

Batterham et al. report that the gut peptide hormone PYY3-36 decreases food intake and body-weight gain in rodents, a discovery that has been heralded as potentially offering a new therapy for obesity. However, we have been unable to replicate their results. Although the reasons for this discrepancy remain undetermined, an effective anti-obesity drug ultimately must produce its effects across a range of situations. The fact that the findings of Batterham et al. cannot easily be replicated calls into question the potential value of an anti-obesity approach that is based on administration of PYY3-36.

The forestomach of ruminants and camelids does not only allow a differential excretion of fluids and small particles but also a differential excretion of small and large particles. The question whether larger particles of different size classes are also retained for different time periods, or whether simply a particle-size threshold exists above which all particles of a size higher than this threshold are retained in an undifferentiated manner, has not been addressed so far. We determined the mean retention time (MRT) of different-sized large particles (10 and 20mm) in three banteng (Bos javanicus) on two forage only diets, grass and grass hay. We used cerium-mordanted fibre (10mm) and lanthanum-mordanted fibre (20mm) as particle markers, mixed in the food. Average total tract MRT for large and very large particles at the grass diet was 58 and 56h and at the grass hay diet 66 and 64h, respectively. Very large particles moved slightly faster than large particles through the gut of the banteng. Three interpretations are possible: Very large particles are resubmitted to rumination sooner than large particles; ingestive mastication of the particle markers could have reduced the difference in the size of the particle markers; alternatively, particle retention may be governed by a threshold, above which all particles of a size higher than this threshold are retained in an undifferentiated manner. In order to test these possibilities, experiments with fistulated animals would have to be performed

Die Erzeugung von Belchdurchzügen (Kragen) an Rohren und Profilen wird derzeitig ausschließlich mit Stempel und Matrize realisiert. Jedoch stößt das Verfahren bei Halbzeugen mit ungünstigem Längen-Durchmesser Verhältnis sowie gekrümmten Bauteilen, bedingt durch die Positionierung der Matrize im Inneren des bauteils, an seine Verfahrensgrenzen. Aus diesem Grunde wurde am Fraunhofer-Institut für Werkzeugmaschinen und Umformtechnik ein Verfahren entwickelt, bei dem mit Hilfe des Innenhochdruck-Umformens (IHU) Blechdurchzüge ohne Matrize hergestellt werden können.
