Glioblastoma (GBM) heterogeneity, aggressiveness and infiltrative growth drastically limit success of current standard of care drugs and efficacy of various new therapeutic approaches. There is a need for new therapies and models reflecting the complex biology of these tumors to analyze the molecular mechanisms of tumor formation and resistance, as well as to identify new therapeutic targets. We established and screened a panel of 26 patient-derived subcutaneous (s.c.) xenograft (PDX) GBM models on immunodeficient mice, of which 15 were also established as orthotopic models. Sensitivity toward a drug panel, selected for their different modes of action, was determined. Best treatment responses were observed for standard of care temozolomide, irinotecan and bevacizumab. Matching orthotopic models frequently show reduced sensitivity, as the blood-brain barrier limits crossing of the drugs to the GBM. Molecular characterization of 23 PDX identified all of them as IDH-wt (R132) with frequent mutations in EGFR, TP53, FAT1, and within the PI3K/Akt/mTOR pathway. Their expression profiles resemble proposed molecular GBM subtypes mesenchymal, proneural and classical, with pronounced clustering for gene sets related to angiogenesis and MAPK signaling. Subsequent gene set enrichment analysis identified hallmark gene sets of hypoxia and mTORC1 signaling as enriched in temozolomide resistant PDX. In models sensitive for mTOR inhibitor everolimus, hypoxia-related gene sets reactive oxygen species pathway and angiogenesis were enriched. Our results highlight how our platform of s.c. GBM PDX can reflect the complex, heterogeneous biology of GBM. Combined with transcriptome analyses, it is a valuable tool in identification of molecular signatures correlating with monitored responses. Available matching orthotopic PDX models can be used to assess the impact of the tumor microenvironment and blood-brain barrier on efficacy. Our GBM PDX panel therefore represents a valuable platform for screening regarding molecular markers and pharmacologically active drugs, as well as optimizing delivery of active drugs to the tumor.

Das Ziel dieser Arbeit war die Entwicklung und Herstellung von „Trojan Horse“- Liposomen zur systemischen Therapie von Hirntumoren und Metastasen durch eine Verbesserung des aktiven Transportes durch die BHS. Um dieses Ziel zu erreichen, wurde ein oberflächenmodifiziertes liposomales Trägersystem entwickelt, das an einen Vertreter der LDL Rezeptor Familie (LRP) im Gehirn bindet und einen endozytotischen Transport über die BHS auslöst. Um dieses Ziel zu erreichen wurden die Liposomen hergestellt, charakterisiert und anschließend umfassend in in vitro und in vivo Experimenten getestet. Einführend wurde der Einfluss der Membraneigenschaften der Vesikel auf die zelluläre Aufnahme und die Transzytose in vitro untersucht. Es wurden mehr als 25 verschiedene Liposomen mit der Lipidfilm-Hydratations- und Extrusionstechnik hergestellt und dann charakterisiert. Die Ergebnisse der zellulären Aufnahmeversuche zeigten, dass vor allem die Oberflächenladung und die Membranfluidität der Vesikel, die durch die Zusammensetzung bedingt war, eine entscheidende Rolle spielten. Positiv geladene Liposomen (L2) und die negativ geladene Formulierung (L3), die das Helferlipid DOPE enthielt, wurden am besten von den Zellen aufgenommen. In den folgenden in vitro und in vivo Experimenten wurden fünf repräsentative Liposomenformulierungen (L1-L5) mit unterschiedlicher zellulärer Aufnahmerate verwendet. In den anschließenden Transzytose Experimenten zeigte sich, dass der transzelluläre Transport von einem hydrophilen Markermolekül positiv mit der Membranfluidität korrelierte und fluide, DOPE und OPP enthaltene Liposomen (L4) am effizientesten transportiert wurden. Um den aufgezeigten Effekt des verbesserten Transportes über Zellbarrieren durch fluide Liposomen zu steigern, wurden die Liposomen L4 mit einem 19-mer-Peptid als Ligand ausgestattet. Dieses Peptid wurde vom LRP spezifischen Aprotinin abgeleitet und aktiviert ein rezeptorvermitteltes Targeting. Diese Oberflächenveränderung steigerte die zelluläre Aufnahme der fluiden, ligandtragenden Liposomen L4-LG deutlich um 60 % und erhöhte die Transzytose um mehr als 50 % im Vergleich zu den Vesikeln L4 ohne Ligand. Zur Untersuchung des therapeutischen Effektes dieser Liposomen in vivo wurden humane Brustkrebszellen intrazerebral und s.c. in Mäuse transplantiert. Diese wurden i.v. mit freiem Mitoxantron oder mit MTO haltigen Liposomen (ohne/mit Ligand) behandelt. Die Liposomen L4-LG hatten die höchste Wirksamkeit und führten zu einer signifikanten Reduktion des Tumorvolumens um 88 % für die s.c. und um 73 % für die intrazerebral wachsenden Tumoren im Vergleich zur unbehandelten Kontrollgruppe. Eine etwas schwächere, jedoch nicht signifikant verschiedene Wirkung hatten auch die ligandenfreien Liposomen L4 was darauf hindeutet, dass eher ein „slow release“ den verbesserten therapeutischen Effekt bedingte und das Targetingsystem noch optimierbar ist. Des Weiteren konnten mit allen liposomalen Formulierungen die durch freies MTO hervor gerufenen Nebenwirkungen (gastrointestinale Toxizität, Dehydratation) verringert werden. Die Ergebnisse der pharmakokinetischen Studie zur zeitlichen Verteilung und Umsetzung von MTO wurden mit Hilfe eines 2 -Kompartiment-Modells berechnet und zeigten für die ligandenhaltigen Liposomen L1-LG und L4-LG im Vergleich zum freien MTO eine längere Verweildauer im Körper. Diese Ergebnisse korrelieren sehr gut mit den Resultaten der therapeutischen Studien. Die Daten zeigen, dass eine verbesserte Therapie von Hirnmetastasen mit „Trojan Horse“- Liposomen, die einen aktiven Transport über die BHS mittels LRP nutzen, möglich ist. Die entwickelten Liposomen können als Grundlage für eine Plattformtechnologie zur systemischen Behandlung von Hirntumoren eingesetzt werden, die dazu geeignet ist, auch auf andere Rezeptoren angewendet zu werden und/oder um andere Arzneistoffe ins Gehirn zu transportieren., The aim of this work was the design and development of Trojan horse liposomes to treat brain tumors and metastases systemically by an improvement of the active passage of these drug transporters through the BBB. To achieve this aim, a surface modified liposomal carrier system was designed to target a receptor of the LDL family (LRP) in the brain and to mediate an endocytotic transport through the BBB. Therefore, liposomes were produced, characterized and subsequently tested in vitro and in vivo in extensive experiments. First, the effects of liposomal membrane properties on cellular uptake and transcytosis were investigated in vitro. More than 25 different liposome formulations were prepared by lipid film hydration and extrusion technology and characterized. Liposomal uptake by the cells seems to be mediated by different mechanisms. It is mainly depending on surface charge and membrane fluidity based on the lipids used for preparation. Liposomes with a positive charge (L2) and the negative charged formulation (L3) which contains the helper lipid DOPE were taken up in highest amounts. In the following in vitro and in vivo experiments representative liposomes were selected (L1-L5), which showed different uptake rates. In a next step, transcytosis experiments proved the positive correlation of the transcellular transport of a hydrophilic marker molecule with the membrane fluidity and showed that fluid liposomes containing DOPE and OPP (L4) were the most efficient vesicles for the transcytosis. To increase the previous effect of the improved transport across cellular barriers by fluid liposomes, the L4 liposomes were equipped with a 19-mer peptide ligand. This peptide was derived from the LRP-specific aprotinin and activates a receptor-mediated targeting. This surface modification increased the cellular uptake of the fluid, ligand-containing liposome L4-LG significantly by 60 %. Transcytosis was increased by more than 50 % compared to the L4 vesicles without ligand. To test the therapeutic effect in vivo, human breast cancer cells were transplanted into the brain and s.c. in mice. Mice were treated i.v. with free Mitoxantron or MTO entrapped formulations (without/with ligand). L4-LG liposomes showed the best efficiency and led to a significant reduction in the s.c. tumor volume by 88 % and 73 % of intracerebrally growing tumors, respectively, in comparison to the untreated control group. A slightly decreased, but not significantly different effect was shown by the ligand-free liposomes L4, what suggests that mostly the slow release improved the therapeutic effect and that the targeting system is still improvable. In addition, all tested liposomal formulations reduced the side effects caused by free MTO (gastrointestinal toxicity, dehydration). The results of the pharmacokinetic study of the time dependent distribution and transformation of MTO were calculated with the help of a 2-compartment model. For ligand-containing L1-LG and L4-LG liposomes, they showed longer retention in the body, in comparison to the free MTO. These results correlate very well with the results of therapeutic trials. All these data show that an improved therapy of brain metastasis is possible with “Trojan horse” liposomes that use active transport across the BBB by LRP. These vesicles could be used as a platform technology for systemic treatment of brain tumors, which can be adapted to target other receptors and/or to carry other drugs into the brain.
