1. Estimates of female genital mutilation/cutting in the Netherlands: a comparison between a nationwide survey in midwifery practices and extrapolation-model

Contains fulltext : 221017.pdf (publisher's version ) (Open Access), BACKGROUND: Owing to migration, female genital mutilation or cutting (FGM/C) has become a growing concern in host countries in which FGM/C is not familiar. There is a need for reliable estimates of FGM/C prevalence to inform medical and public health policy. We aimed to advance methodology for estimating the prevalence of FGM/C in diaspora by determining the prevalence of FGM/C among women giving birth in the Netherlands. METHODS: Two methods were applied to estimate the prevalence of FGM/C in women giving birth: (I) direct estimation of FGM/C was performed through a nationwide survey of all midwifery practices in the Netherlands and (II) the extrapolation model was adopted for indirect estimation of FGM/C, by applying population-based-survey data on FGM/C in country of origin to migrant women who gave birth in 2018 in the Netherlands. RESULTS: A nationwide survey among primary care midwifery practices that provided care for 57.5% of all deliveries in 2018 in the Netherlands, reported 523 cases of FGM/C, constituting FGM/C prevalence of 0.54%. The indirect estimation of FGM/C in an extrapolation-model resulted in an estimated prevalence of 1.55%. Possible reasons for the difference in FGM/C prevalence between direct- and indirect estimation include that the midwives were not being able to recognize, record or classify FGM/C, referral to an obstetrician before assessing FGM/C status of women and selective responding to the survey. Also, migrants might differ from people in their country of origin in terms of acculturation toward discontinuation of the practice. This may have contributed to the higher indirect-estimation of FGM/C compared to direct estimation of FGM/C. CONCLUSIONS: The current study has provided insight into direct estimation of FGM/C through a survey of midwifery practices in the Netherlands. Our findings suggest that about 41,000 women had undergone FGM/C, of which 37% had Type III (infibulation). In total 4,190 girls are estimated to be at risk of FGM/C in the next 20 years, of whom 394 were first-generation girls. CONCLUSION: These findings show the urgency to develop appropriate strategies and policies to prevent FGM/C, to protect girls and women at risk of the practice, and to provide adequate services and support for those affected by FGM/C in the Netherlands.

Il tema è di particolare rilievo, in quanto il 60% dei msna presenti in Italia ha 17 anni e dunque ha la necessità di acquisire competenze e abilità per inserirsi nel mondo nel lavoro, per trovare casa e peri la gestione della vita autonoma.

Abstract

The role of the personal ideal family size for international migrants has rarely been studied in the current debate on fertility and migration in the European context. It is not known to which extent the reduction of fertility observed among immigrants who settle in a country where fertility is lower than in their country of origin is the result of a change in fertility norms among those immigrants. The study of migrants’ ideals family size has the potential to shed light on fertility norms without the interference of economic conditions and migration-related disruptive phenomena. Due to the complexity of its migration context, Italy is an interesting destination country for studying changes in migrants’ ideal family sizes. This paper uses data from the survey of the Italian National Institute of Statistics (ISTAT) on immigrant families conducted in 2011-12. We compare the personal ideal family size of women of reproductive age with the prevalent norm in their country of origin, applying a multinomial logistic regression analysis. Results show that the country of origin has an important role in the determination of immigrants’ ideal family sizes. Women from countries where large families are the ideal are more likely to show a lower personal ideal family size compared to their non-migrant co-nationals, while women from countries where two children are considered ideal mostly share the same norm. The occurrence of fertility preferences expressed in a non-numeric form (e.g. “Up to God”) changes between women with different countries of origin. This study confirms that conformity with the ideal of the country of origin is more likely among women who migrated as adults. At the same time, the number of years spent in the destination country is not significantly associated with a shift away from the norms prevalent in the country of origin. Abstract

In un contesto in cui le sensazioni e gli stati d’animo, nonché le posizioni preconcette, sembrano avere il sopravvento sulle valutazioni ragionate e documentate in merito alla questione dell’immigrazione in Italia, appare opportuno fornire al dibattito elementi utili per ancorare le riflessioni e le decisioni pubbliche a un quadro di riferimento certo, garantito dalle statistiche disponibili e dai risultati delle ricerche scientifiche. In particolare questo primo capitolo si occupa di introdurre tutti i temi trattati nel corso del volume sulla questione delle migrazioni internazionali, prendendo in esame le popolazioni immigrate in Europa e in Italia nella storia recente, valutando fattori quali la dinamica demografica, i flussi, le tipologie migratorie e tutte le questioni relative, dal comportamento demografico, al lavoro, all’integrazione.

Abstract

Questo capitolo analizza i comportamenti demografici degli stranieri in Italia, a partire dalla disamina delle dinamiche demografiche più recenti, per passare poi a considerare le condizioni di salute sia per quanto riguarda la salute percepita, che le malattie croniche, che le limitazioni alle attività. Si passa dunque a considerare la mortalità, sia quella complessiva, che quella infantile, che per causa. Altro fattore rilevante da tenere in considerazione è inoltre la nuzialità e l’incidenza dei matrimoni misti nel nostro paese nel corso degli ultimi anni, questione collegata ai comportamenti riproduttivi. Per descrivere gli effetti prodotti dalle migrazioni internazionali del nuovo millennio su dimensione e caratteristiche delle persone che vivono in Italia, è possibile confrontare la popolazione residente alla data più recente (o popolazione reale) con quella attesa alla stessa data in assenza di migrazioni. Nelle regioni settentrionali e centrali le migrazioni (sia interne che internazionali) hanno frenato in modo leggermente più incisivo che a livello nazionale l’invecchiamento demografico, mentre nelle regioni meridionali e insulari tale processo risulterebbe addirittura accelerato, anche se in misura minima, a causa dell’emigrazione dei giovani soprattutto verso le regioni centro-settentrionali ma anche verso l’estero.
