9. Stage I corpus cancer: is teletherapy necessary?

Abstract

Surgical removal of the uterus and lymph nodes may prevent recurrence of uterine cancer without the need for radiation therapy. Researchers evaluated the outcomes of 396 endometrial cancer patients who underwent extensive surgical staging and removal of the uterus and lymph nodes. Surgical complication rates were not excessive. Almost all (97%) patients with stage I disease survived for at least five years after surgery. Radiation therapy after surgery may not be necessary when thorough tumor staging is performed even for patients with stage IB and IC cancer and may not reduce survival rates.

10. Incisional bowel herniations after operative laparoscopy: a series of nineteen cases and review of the literature

Abstract

The use of large surgical openings during laparoscopy may lead to hernias after surgery at the incision sites. Researchers evaluated records of laparoscopic surgeries at various hospitals to determine the rate of bowel herniations at incision sites. Twenty-one bowel herniations in 19 patients occurred. The total number of laparoscopic surgeries could not be determined. Herniations may develop in both umbilical and non-umbilical incision sites. The risk of herniations may increase when surgical openings are 10 millimeters or more in size, when screws are used to anchor the opening to the abdominal wall, and when surgeons overuse the opening. Smaller instruments and close follow-up of patients may reduce the rate of complications.

Abstract

There is an increased risk of infection following extensive pelvic surgery. The incidence of infection following prophylactic single and multiple doses of antibiotics was studied among 116 patients before surgery to treat gynecological cancer. Patients were given a single dose of antibiotic administered in the operating room 45 minutes before surgery. Additional doses were given 8 and 16 hours later. Two types of hysterectomies were performed. There was no difference between the overall surgical site infection rate with either operation type (4.3 and 4.5 percent) or antibiotic treatment schedule. Patients that developed a hospital-acquired infection were more likely to have had hospital stays of longer than 10 days. A single dose of antibiotics was effective in reducing the risk of infection after extended pelvic surgery for the treatment of gynecological cancer. 13. Use of serum CA 125 measurement in posttreatment surveillance of early-stage endometrial carcinoma

Abstract

The level of CA 125, a cancer antigen or protein found in the blood, is used to monitor patients with advanced ovarian and endometrial cancer. The measurement is a way of assessing whether the cancer cells have spread to other areas of the body or if the disease has regressed. To determine the value of CA 125 measurements for monitoring the status of early endometrial cancer once treatment has been initiated, patients treated for endometrial adenocarcinoma (stages I and II) were studied. Of the 125 patients with early endometrial cancer, 123 had normal CA 125 levels before treatment. The disease returned in 13 patients. All the patients with cancer cells that had spread to the pelvis (one patient), abdomen (four patients), and lungs (two patients), had high CA 125 levels. None of the six patients with cancer cells recurring in the vagina had high CA 125. Intestinal blockage occurred in four patients receiving pelvic radiation as part of the cancer treatment. They, too, had high CA 125 but no evidence that cancer had returned. CA 125 is a useful measurement when monitoring patients after treatment for early endometrial cancer. However, a high CA 125 may be mistaken for return of cancer when radiation treatment has caused intestinal injury. Conversely, cancer cells appearing in the vagina may not produce an elevated CA 125. 49. Predictive factors for carboplatin hypersensitivity reactions in gynecologic cancers: Effect of BRCA status.

Abstract

Introduction: Carboplatin hypersensitivity reactions have been reported to occur in up to 16% of patients with gynecologic cancers. Several predisposing factors have been suggested including presence of BRCA1/2 mutation, however, contribution of these mutations to reaction development has not been extensively studied. The purpose of this study was to determine if there is an association between BRCA1/2 mutation status and the development of carboplatin hypersensitivity reactions. Methodology: Eligible patients were women aged 18 years or older with a diagnosis of ovarian, fallopian tube, uterine, endometrial, or primary peritoneal cancer who attempted to receive at least one dose of carboplatin. The primary outcome was the effect of BRCA1/2 status on the development of carboplatin hypersensitivity reactions with regard to: reaction frequency, timing, and severity. Secondary outcomes included identification of additional risk factors that may help identify predisposition to carboplatin hypersensitivity reaction., Results: A total of 44 patients were included in this study. Five patients (38%) in the reaction group and 4 patients (31%) in the no reaction group had a documented mutation in one or both BRCA genes (p = 1.00). No significant differences were found in terms of reaction severity or symptoms, and timing of reaction after dose administration. Incidence of thyroid disorder was significantly higher among patients who experienced a hypersensitivity reaction (1 (4%) vs 10 (45%); p = 0.004)., Conclusion: BRCA mutation status was not associated with an increased risk of carboplatin hypersensitivity in our patient population. Further investigation into thyroid dysfunction as a risk factor for reaction development is warranted.

Abstract

Mesonephric adenocarcinoma is a rare tumor, accounting for <1% of cervical cancers. Well-differentiated mesonephric adenocarcinoma can be difficult to distinguish from diffuse mesonephric hyperplasia. Herein, we report a case of well-differentiated mesonephric adenocarcinoma with an FGFR2 mutation not previously reported in the literature. Nonselective tyrosine kinase inhibitors or FGFR2 inhibitors may represent options for targeted therapy.
