Mental health legislation is vital for safeguarding the rights of individuals with mental disorders, a vulnerable segment of the population. This article explores the evolution and impact of Romania's mental health laws, which have developed to align with European standards while addressing the unique challenges of the Romanian mental health system. By examining recent legislative advancements and new policies, such as the National Mental Health Strategy 2020-2030 and the Mental Health and Wellbeing Act of 2021, the article highlights efforts to improve mental health care and reduce stigma. Comparisons with mental health legislation in other European Union countries, including Germany, France, Spain and others, provide a broader context for understanding Romania's progress and ongoing needs. The role of media in perpetuating stigma and the importance of public education and legislative reform are discussed as crucial components for fostering a supportive environment for individuals with mental disorders. Furthermore, the article addresses the biopsychosocial implications of stigmatization and the necessity of a societal commitment to enhance mental health understanding and support. The conclusion emphasizes that continuous legislative updates, public awareness campaigns, and integrated community-based services are essential for creating an inclusive and respectful society for those affected by mental disorders. Alcohol use disorder is a medical condition marked by a diminished capacity to halt or manage alcohol consumption, even in the face of detrimental social, professional, or health outcomes. It encompasses various terms such as alcohol abuse, dependence, addiction, and colloquially, alcoholism. Research has consistently highlighted the significant toll that alcoholism can take on the spouses of individuals struggling with alcohol addiction. Living with a spouse who struggles with alcoholism can be incredibly challenging and emotionally taxing. It can have significant impacts on one's mental and physical health, as well as on the dynamics of the relationship and family life. The typologies of wives of alcoholics categorize the various coping and behavioral patterns these individuals may adopt in response to their spouse's alcoholism, including enabling the behavior, adopting a victim mindset, compensating by being overly competent (the hero), displaying oppositional behavior (the rebel), or withdrawing emotionally and socially to protect themselves. The dynamics of alcoholism in a marriage can erode trust, communication, and intimacy, often leading to a breakdown of the relationship if left unaddressed. Seeking professional help, such as therapy or support groups, can be crucial for both spouses to navigate the challenges of alcoholism and work towards healing and recovery, either individually or as a couple. Suicide represents a critical health problem and one of the most challenging complications of psychiatric disorders. People with alcohol use disorder face vulnerability to suicidal ideation, attempts, and completed suicide. Both suicide and alcohol use disorder are complex phenomena influenced by a multitude of factors, including biological, psychological, social, and environmental determinants. The aim of this review is to assess risk factors that are related to suicide in patients with alcohol use disorder, in order to better understand the underlying mechanisms. 7. Assessment of self-injurious behavior in major depressive disorder: a case report

- Abstract
Introduction: Self-harm and depression are two related mental health issues that often co-occur and can have serious impacts on individuals and their well-being. Major Depressive Disorder (MDD) is a complex and multifaceted mental health condition that is prevalent among young adults. It is a significant public health concern that affects individuals, families and society as a whole, contributing to considerable social, economic, and health-related costs. Despite the high prevalence of depression in young adults, there is a lack of understanding of its etiology, risk factors, and optimal treatment strategies. Self-harm behavior is a concerning manifestation of MDD that can have serious consequences, including injury, infection, or even death. Methods: In this article, we report a case of an MDD patient with self-harm behavior and discuss the assessment method and treatment options. Our aim is to raise awareness of these pathologies among psychiatrists and the general population and to highlight the importance of early diagnosis and effective treatment in reducing the risk of self-harm behavior and suicide. Conclusion: In light of the case presented, it is evident that early detection and targeted intervention are crucial in mitigating the risk of self-injurious behavior in MDD patients. The utilization of a comprehensive assessment methodology, inclusive of a thorough review of the patient’s clinical history and utilization of standardized rating scales, proved indispensable in the tailoring of an individualized treatment regimen. The amalgamation of pharmacotherapy and psychotherapy proved to be a successful strategy in the amelioration of depressive symptoms and consequent self-harm behavior. As such, we trust that this case report will serve to sensitize clinicians to the importance of early identification and prompt management of these pathologies, in the quest for enhanced mental health outcomes.

This article describes the case of a 48-year-old female patient who was involuntary admitted by the authorities for a psychotic episode characterized by delusional ideas of grandeur, filiation and reference. The patient firmly believed that she was not only a member of the royal family of Monaco, but also had a special familial connection to the ruling dynasty. This case highlights the complexities of psychotic disorders and the presence of multiple delusional themes that can significantly impact an individual's perception of reality and functioning. These delusions profoundly affected the patient's daily life and interpersonal relationships. Her strong conviction in her royal filiation led to strained interactions with family members, who were unable to validate her claims. The patient's preoccupation with her imagined connection to the royal family overshadowed her personal responsibilities, causing disruptions in her overall functioning. This paper underscores the intricate nature of psychotic disorders and the multifaceted delusions that can arise within them. Understanding the interplay between these delusional themes is vital in guiding diagnosis and treatment strategies. By implementing a comprehensive approach that includes medication, psychotherapy, and a supportive environment, healthcare professionals can assist patients in regaining a more accurate perception of reality and improve overall functioning and well-being. Schizoaffective disorder represents a psychiatric condition characterized by symptoms of both schizophrenia and mood disorders, such as bipolar disorder or depression. Together, these two conditions have a major impact on individuals and affect many aspects of their lives. Moreover, practicians often have difficulties in diagnosing and treating the patients suffering from this disorder. In patients with psychiatric conditions, including schizoaffective disorder, the lack of insight is frequently a significant problem. Objectives: This paper aims to provide a general perspective upon the evolution of schizoaffective disorder and to emphasize the patients' limited capacity to recognise their condition and the need for treatment. Methods: This article will present the clinical case of a 37-year old woman, who is brought by the police to the ER of "Socola" Institute of Psychiatry for psychomotor agitation, emotional lability, mood swings, delusions of prejudice and persecution, impulsive behavior and aggressiveness towards her parents, especially her father. Conclusions: Impairment of insight is considered an important characteristic of psychiatric disorders, including schizoaffective disorder. It is crucial for physicians to develop trusting relationships with both the patient and the patient's family in order to improve the quality of life of both patients and their families. Although the modern era has brought new ways of diagnosis and treatment to the medical field, and especially to the neurosciences, psychiatric disorders accompanied by aggressive behavior remain a mental health problem, but also an issue that targets society in general. Psychiatric illnesseswhich have an aggressive componentrepresent a source of social disruption and a threat to public health. It is noteworthy to mention that these disorders are among the leading causes of disability worldwide, accounting for a third of all years lost todisability. Around 33% of the world's population will develop a mental health issue at some point in their lives and an important number of them will build up a hostile demeanor, as psychiatric disorders and aggression are often seen as inextricably linked. In order to apply the principles of prophylaxis, i.e., prophylactic, curative and rehabilitative treatment of psychiatric pathology in current practice, it was necessary to collect data targetingcases of psychiatric disordersaccompanied by aggressive behavior, taking into account external and internal factors of the individuals. 15. NATURE'S METRONOME: ADIPOKINES' VITAL RHYTHM IN DIGESTIVE DISEASE PATHOGENESIS.

Introduction. Adipokines are signaling molecules secreted by adipose tissue that play a key role in regulating metabolism and inflammation. Some examples of adipokines include leptin, adiponectin, resistin, and visfatin. Focusing on the first two mentioned hormones, leptin is involved in regulating appetite and energy expenditure, while adiponectin plays a role in insulin sensitivity and antiinflammatory processes. The dysregulation of adipokine secretion has been linked to the development of various diseases, including obesity, type 2 diabetes, and cardiovascular disease. Recent research has suggested that adipokines may also be involved in the pathophysiology of digestive diseases, such as inflammatory bowel disease (IBD) and non-alcoholic fatty liver disease (NAFLD). Aim. The aim of this study was to investigate the association between adipokines and the risk of digestive diseases. Methods. A systematic review and meta-analysis were conducted using MEDLINE, Scopus, and Cochrane Library databases. Studies that reported on the association between adipokines and digestive diseases were included. The quality of the included studies was assessed using the Cochrane Risk of Bias Tool. The meta-analysis was performed using a random-effects model. Results. A total of 25 studies were included in the meta-analysis, with 16 studies focusing on IBD and 9 studies focusing on NAFLD. The results showed that adipokines such as leptin and adiponectin are involved in the pathogenesis of IBD. Leptin has proinflammatory effects, and its levels were found to be elevated in patients with IBD, whereas adiponectin has anti-inflammatory effects, and its levels were found to be decreased in IBD patients. Similarly, adipokines such as adiponectin, visfatin, and leptin have been shown to play a role in the development of NAFLD. Decreased levels of adiponectin have been detected in NAFLD patients, while leptin and visfatin levels were found to be increased in NAFLD patients compared to healthy controls. Conclusion. This meta-analysis provides evidence of an association between adipokines and the risk of digestive diseases. The findings suggest that dysregulation of adipokines may play a role in the pathogenesis of IBD and NAFLD. Further research is needed to determine the mechanisms underlying this association and to explore the potential use of adipokines as biomarkers or therapeutic targets for digestive diseases.
