Abstract

Artificial Intelligence (ai) systems are precious support for decision-making, with many applications also in the medical domain. The interaction between mds and ai enjoys a renewed interest following the increased possibilities of deep learning devices. However, we still have limited evidence-based knowledge of the context, design, and psychological mechanisms that craft an optimal human–ai collaboration. In this multicentric study, 21 endoscopists reviewed 504 videos of lesions prospectively acquired from real colonoscopies. They were asked to provide an optical diagnosis with and without the assistance of an ai support system. Endoscopists were influenced by ai (OR=3.05), but not erratically: they followed the ai advice more when it was correct (OR=3.48) than incorrect (OR=1.85). Endoscopists achieved this outcome through a weighted integration of their and the ai opinions, considering the case-by-case estimations of the two reliabilities. This Bayesian-like rational behavior allowed the human–ai hybrid team to outperform both agents taken alone. We discuss the features of the human–ai interaction that determined this favorable outcome.

Abstract

Background: The cause of collagenous colitis (CC) and lymphocytic colitis (LC) is unknown and epidemiological risk factors for CC and LC are not well studied. The aim was to evaluate in a case-control study epidemiological risk factors for CC and LC. Methods: In all, 120 patients with CC, 70 with CL, and 128 controls were included. For all cases and controls information was prospectively recorded. A binary logistic regression analysis was performed separately for CC and LC. Results: Independent associations observed with the diagnosis of CC were: current smoking (odds ratio [OR], 2.4), history of polyarthritis (OR, 20.8), and consumption of lansoprazole (OR, 6.4), low-dose aspirin (OR, 3.8), beta-blockers (OR, 3.6), and angiotensin II receptor antagonists (OR 0.20). In the case of LC they were: current smoking (OR, 3.8), associated autoimmune diseases (OR, 8), and consumption of sertraline (OR, 17.5), omeprazole (OR 2.7), low-dose aspirin (OR, 4.7), and oral antidiabetic drugs (OR, 0.14). Conclusions: The consumption of drugs, current smoking, and associated autoimmune diseases were independently associated with the risk of microscopic colitis. (Inflamm Bowel Dis 2013; 19: 411-417)

Abstract

International audience; OBJECTIVES: The Selecting Therapeutic Targets in Inflammatory Bowel Disease (STRIDE) program was initiated by the International Organization for the Study of Inflammatory Bowel Diseases (IOIBD). It examined potential treatment targets for inflammatory bowel disease (IBD) to be used for a "treat-to-target" clinical management strategy using an evidence-based expert consensus process. METHODS: A Steering Committee of 28 IBD specialists developed recommendations based on a systematic literature review and expert opinion. Consensus was gained if ≥75% of participants scored the recommendation as 7-10 on a 10-point rating scale (where 10=agree completely). RESULTS: The group agreed upon 12 recommendations for ulcerative colitis (UC) and Crohn's disease (CD). The agreed target for UC was clinical/patient-reported outcome (PRO) remission (defined as resolution of rectal bleeding and diarrhea/altered bowel habit) and endoscopic remission (defined as a Mayo endoscopic subscore of 0-1). Histological remission was considered as an adjunctive goal. Clinical/PRO remission was also agreed upon as a target for CD and defined as resolution of abdominal pain and diarrhea/altered bowel habit; and endoscopic remission, defined as resolution of ulceration at ileocolonoscopy, or resolution of findings of inflammation on cross-sectional imaging in patients who cannot be adequately assessed with ileocolonoscopy. Biomarker remission (normal C-reactive protein (CRP) and calprotectin) was considered as an adjunctive target. CONCLUSIONS: Evidence- and consensus-based recommendations for selecting the goals for treat-to-target strategies in patients with IBD are made available. Abstract

International audience; BACKGROUND: & Aims: There is a need for a scoring system that provides a comprehensive assessment of structural bowel damage, including stricturing lesions, penetrating lesions, and surgical resection, that measures disease progression. We developed the Lémann Index and assessed its ability to measure cumulative structural bowel damage in patients with Crohn's disease (CD). METHODS: We performed a prospective, multicenter, international, cross-sectional study of patients with CD evaluated at 24 centers in 15 countries. Patients were stratified based on CD location and duration. All patients underwent clinical examination and abdominal magnetic resonance imaging analyses. Upper endoscopy, colonoscopy, and pelvic magnetic resonance imaging analyses were performed according to suspected disease locations. The digestive tract was divided into 4 organs and subsequently into segments. For each segment, investigators collected information on previous surgeries, predefined strictures, and/or penetrating lesions of maximal severity (grades 1-3), and then provided damage evaluations ranging from 0.0 (no lesion) to 10.0 (complete resection). Overall level of organ damage was calculated from the average of segmental damage. Investigators provided a global damage evaluation (from 0.0 to 10.0) using calculated organ damage evaluations. Predicted organ indexes and Lémann index were constructed using a multiple linear mixed model, showing the best fit with investigator organ and global damage evaluations, respectively. An internal cross-validation was performed using bootstrap methods. RESULTS: Data from 138 patients (24, 115, 92, and 59 with upper tract, small bowel, colon/rectum, and anus CD location, respectively) were analyzed. According to validation, the unbiased correlation coefficients between predicted indexes and investigator damage evaluations were 0.85, 0.98, 0.90, 0.82 for upper tract, small bowel, colon/rectum, anus, respectively, and 0.84 overall. CONCLUSIONS: Conclusions: In a cross-sectional study, we assessed the ability of the Lémann index to measure cumulative structural bowel damage in patients with CD. Provided further successful validation and good sensitivity to change, the index should be used to evaluate progression of CD and efficacy of treatment.

Abstract

Background: Magnetic resonance enterography is increasingly utilized for assessment of luminal Crohn's disease activity. The Magnetic Resonance Index of Activity and the London Index are the most commonly used outcome measures in clinical trials. We assessed the reliability of these indices and several additional items., Methods: A consensus process clarified scoring conventions and identified additional items based on face validity. Four experienced radiologists evaluated 50 images in triplicate, in random order, at least 1 month apart, using a central image management system. Intra- and interrater reliability were assessed by calculating and comparing intraclass correlation coefficients., Results: Intrarater intraclass correlation coefficients (95% confidence intervals) for the Magnetic Resonance Index of Activity, London, and London "extended" indices and a visual analogue scale were 0.89 (0.84 to 0.91), 0.87 (0.83 to 0.90), 0.89 (0.85 to 0.92), and 0.86 (0.81 to 0.90). Corresponding interrater intraclass correlation coefficients were 0.71 (0.61 to 0.77), 0.67 (0.55 to 0.75), 0.70 (0.61 to 0.76), and 0.71 (0.62 to 0.77). Reliability for each index was greatest in the terminal ileum and poorest in the rectum. All 3 indices were highly correlated with the visual analogue scale; 0.79 (0.71 to 0.85), 0.78 (0.71 to 0.84), and 0.79 (0.72 to 0.85) for the Magnetic Resonance Index of Activity, London, and the London "extended" indices, respectively., Conclusions: "Substantial" interrater reliability was observed for all 3 indices. Abstract

Endoscopy is currently considered the reference standard for the evaluation of disease activity and severity in patients with inflammatory bowel disease (IBD). However, information provided by endoscopy is limited to the mucosal surface and cannot always be complete. Cross-sectional imaging techniques are gaining acceptance for the assessment of patients with Crohn's disease (CD) and ulcerative colitis (UC). Overall diagnostic accuracy of ultrasonography (US), computed tomography (CT) and magnetic resonance imaging (MRI) for the evaluation of colonic lesions in patients with IBD is high. The main limitation of US is its high dependence on expertise of the examiner and the anatomic location of lesions, and its main strengths are wide availability safety and low cost. CT is highly accurate for the evaluation of patients with IBD, but due to safety issues related to radiation exposure this technique should be reserved for urgent situations in which US is not diagnostic. Technological advances in the area of MRI have allowed optimization for the assessment of patients with IBD, mainly CD. Available evidence suggests that magnetic resonance colonography (MRC) has a high diagnostic accuracy for the detection of disease activity, location, severity, and complications, particularly for penetrating and stricturing lesions, characteristic of CD. MRC can provide a valuable guidance for performing medical and surgical treatment with maximized efficacy and safety. Overall, MRI findings accurately reflect disease activity and provide reliable information for decision-making and patient care optimization.

Abstract

Pregnant primiparous rats were exposed to low immobilization stress or ACTH injections (one unit) throughout pregnancy. We measured the following parameters in the female offsprings: puberty (vaginal opening and first oestrous), oestrous cycle and sexual behaviour. Compared to the controls, female sexual receptivity measured by means of lordosis and the lordosis quotient (LQ) increased in both the experimental groups, but the puberty parameters of the offspring did not alter.
