4. Adherence assessment of eye drops in patients with glaucoma using 8 item Morisky Score: a cross sectional study.

Objective: To determine adherence to glaucoma eye drops in patients from Glaucoma Project (Ministry of Health) by Morisky scale of 8 items. Methods: We conducted a cross-sectional study through a questionnaire applied to 237 patients diagnosed with glaucoma regularly enrolled in the Glaucoma Project at Institutode Olhos de Maceio, adapting the Morisky scale, already validated in Portuguese, for eyedrops. The variables were adherence to antiglaucoma eyedrops, age, sex, race, glaucoma diagnosis time, eyedrops number used, systemic diseases, education, subjective vision (categorized in bad, reasonable or good) and the factors correlated with poor adherence. Statistical analysis between variables was performed with the statistical test Chi-square test for categorical variables and test Mann- Whitney test for continuous outcomes, considering a 5% significance level. Results: The rate of adherence to the glaucoma drops was 54%. The age and number of drops (p = 0.02 and 0,03 respectively), and quality of vision (p<0,001) were statistically significant for the inappropriate use of the therapy and the most common reason both adherent group as the non-adherent was forgetfulness (23% and 76,15% respectively). Conclusion: Using the Morisky scale adapted to antiglaucoma eyedrops the adherence was 54%. [ABSTRACT FROM AUTHOR]

Objective: To evaluate how much decreases intraocular pressure (IOP) with bimatoprost 0,03% (Glamigan, Germed, Brazil) in patiens with primary open angle glaucoma (POAG) and ocular hypertension (OH) and possible side effects. Study: Controlled and randomized study. Methods: There were evaluated 66 eyes of 35 patients with age of 18 years old or more diagnosed with POAG and OH. All the patients had Glamigan® as first drop for treatment used once daily (at night) and 30 days later they had IOP measured by Goldmann tonometry. Results: The study population had 35 (66 eyes) patients with primary open angle glaucoma (POAG) and ocular hypertension (OH), 16 male patients (45,7%) and 19 female (54,3%). The mean age was 66,7 years old and the maximum was 84 years old. The mean decrease in IOP was 6,5 mmhg after 30 days using the drops. Seventeen patients had conjunctival redness (26,6%), 10 (15,6%) had pain, 8 (12,5%) had burning, 5 (7,8%) had itch and 2 (3,1%) had blurring vision. Conclusion: Glamigan® was efficient when evaluating IOP decrease. The most correlated side effects were conjunctival redness (26,6%) and pain (15,6%). Purpose: To study the technique of eye drop instillation in glaucoma patients and identify independent factors that may influence their performance. Methods: In this cross-sectional study, 71 consecutive patients with glaucoma or ocular hypertension, self-administering topical anti-glaucoma medications for ≥6 months were evaluated. All patients instilled a tear substitute into the eye with the worst eyesight using the technique normally used at home. The following parameters were evaluated: age, number of years receiving treatment with ocular hypotensive eye drops, time spent to instill the first drop, number of drops instilled, correct location of the eye drops, contact of the bottle with the eye, closing of the eyelids or occlusion of the tear punctum, and asepsis of the hands. Results: The mean age of the patients was 66 ± 10.8 years, and patients were on ocular hypotensive drugs for 11.3 ± 7.3 (range, 2-35) years. Only 28% of the patients were able to correctly instill the eye drops (squeeze out 1 drop and instill it into the conjunctival sac without bottle tip contact). Touching the tip of the bottle to the globe or periocular tissue occurred in 62% of the patients. In 49% of the patients, the eye drops fell on the eyelids or cheek. Two or more drops were squeezed by 27% of the patients. Conclusions: The majority of glaucoma patients were unable to correctly instill eye drops. Age was an independent factor associated with eye drop instillation performance. RESUMO Objetivo: Avaliar a técnica de instilação de colírio em portadores de glaucoma e identificar fatores independentes que pode influenciar o desempenho. Métodos: Neste estudo transversal 71 pacientes consecutivos com glaucoma ou hipertensão ocular que auto instilam seus colírios há pelo menos 6 meses, foram avaliados. Todos os pacientes instilaram um colírio lubrificante no olho de pior visão utilizando a mesma técnica de instilação de colírio que utilizam rotineiramente em casa. Foram avaliados parâmetros como: idade, número de anos em tratamento com colírios hipotensores oculares, tempo gasto para instilação da primeira gota, número de gotas instiladas, localização correta do colírio, contato do frasco com o olho, fechamento de pálpebras ou oclusão do ponto lacrimal e assepsia das mãos. Resultados: A idade média dos pacientes foi de 66 ± 10,8 anos. Os pacientes esta vam em tratamento com colírios hipotensores oculares por, em média, 11,3 ± 7,3 anos (variando de 2 a 35 anos). Apenas 28% dos pacientes foram capazes de instilar corretamente o colírio (instilação de 1 gota em saco conjuntival sem contato com a ponta do frasco). Contato da ponta do frasco com o olho ou tecido periocular ocorreu em 62% dos pacientes. Em 49% dos casos, o colírio caiu nas pálpebras ou fora do saco lacrimal na primeira tentativa. Duas ou mais gotas foram instiladas por 27% dos pacientes. Conclusão: A maioria dos pacientes com glaucoma é incapaz de instilar o colírio corretamente. A idade é um fator independente que influencia o desempenho da instilação de colírio.

Purpose: Ultraviolet (UV) acts as low-dose ionizing radiation. Acute UVB exposure causes photokeratitis and induces apoptosis in corneal cells. Astaxanthin (AST) is a carotenoid, present in seafood, that has potential clinical applications due to its high antioxidant activity. In the present study, we examined whether topical administration of AST has preventive and therapeutic effects on UV-photokeratitis in mice. Methods: C57BL/6 mice were administered with AST diluted in polyethylene glycol (PEG) in instillation form (15 μl) to the right eye. Left eyes were given vehicle alone as controls. Immediately after the instillation, the mice, under anesthesia, were irradiated with UVB at a dose of 400 mJ/cm2. Eyeballs were collected 24 h after irradiation and stained with H&E and TUNEL. In an in vitro study, mouse corneal epithelial (TKE2) cells were cultured with AST before UV exposure to quantify the UV-derived cytotoxicity. Results: UVB exposure induced cell death and thinning of the corneal epithelium. However, the epithelium was morphologically well preserved after irradiation in AST-treated corneas. O soro autólogo tem sido adotado como uma nova abordagem para tratar síndrome do olho seco porque contém vitaminas, alguns fatores de crecimento e fibronectina que são considerados importantes contribuintes para integridade corneana e conjuntival. Colírio de soro autólogo é produzido sem preservativo. O soro é não-alérgico e suas propriedades bioquímicas são similares à lágrima. O soro autólogo tópico tem sido relatado efetivo para o tratamento de olho seco grave relacionado a distúrbios da superfície ocular como na síndrome de Sjögren, ceratoconjuntivite límbica superior, doença do enxerto versus hospedeiro, síndrome de Stevens-Johnson, procedimentos cerato-refrativos, erosão corneana persistente ou recorrente, ceratopatia neurotrófica, úlcera de Mooren, ceratopatia associada à aniridia, e bolhas filtrantes após trabeculectomia. O objetivo do presente estudo é revisar a literatura recentemente publicada sobre doenças da superficie ocular tratadas com soro autológo tópico. Autologous serum has been used to treat dry eye syndrome for many years. It contains several growth factors, vitamins, fibronectin and other components that have been considered important for corneal and conjunctival integrity. Serum eye drops are usually prepared as an unpreserved blood solution. The serum is by nature well tolerated and its biochemical properties are somewhat similar to natural tears. Autologous serum eye drops have been reported to be effective for the treatment of severe dry eye-related ocular surface disorders (Sjögren's syndrome), and also other entities such as superior limbic keratoconjunctivitis, graft-versus-host disease, Stevens-Johnson syndrome, ocular cicatricial pemphigoid, recurrent or persistent corneal erosions, neurotrophic keratopathy, Mooren's ulcer, aniridic keratopathy, filtering blebs after trabeculectomy, and post-keratorefractive surgery. The purpose of this study is to review the recently published literature on ocular surface diseases treated with human autologous serum eye drops. A síndrome do olho seco constitui freqüentemente, grande fonte de frustração, tanto para os pacientes, como para os oftalmologistas que, não raramente, são vencidos pela persistência dos sintomas, apesar dos esforços para sua abordagem diagnóstica e terapêutica. O tema adquire sua real importância quando nos defrontamos com o fato do “olho seco” ser uma das queixas mais comuns na prática oftalmológica. Os autores apresentam uma revisão de aspectos essenciais para a compreensão da moderna abordagem desta síndrome. The dry eye syndrome is frequently the source of great frustration, not only for the patients but also for the eye specialists who, not rarely, are defeated by the persistence of the symptoms, despite the efforts made in their diagnostic and therapeutic approach. The subject acquires its real importance, when we are faced with the fact that “dry eye” is a very common complaint in ophthalmologic practice. The authors present an upto- date review about important aspects involved in this issue.

RESUMO Objetivo: Determinar a adesão aos colírios antiglaucomatosos em pacientes do Projeto Glaucoma (Ministério da Saúde) por meio da escala de Morisky de 8 itens. Métodos: Foi realizado um estudo transversal por meio de questionário aplicado a 237 pacientes diagnosticados com glaucoma inscritos regularmente no Projeto Glaucoma do Instituto de Olhos de Maceió, adaptando a escala de Morisky já validada em português para colírios. As variáveis foram a adesão aos colírios antiglaucomatosos, idade, sexo, raça, tempo de diagnóstico do glaucoma, número de colírios utilizados, doenças sistêmicas, escolaridade, perfil de visão (ruim, razoável ou boa) e os fatores correlacionados com a adesão. A análise estatística entre as variáveis foi realizada com os testes estatísticos do Quiquadrado para as variáveis categóricas e Teste U de Mann-Whitney para as contínuas, considerando um nível de significância de 5%.
