Abstract Background The emergence of multidrug-resistant strains of Plasmodium falciparum poses a great threat of increased fatalities in cases of cerebral and other forms of severe malaria infections in which parenteral artesunate monotherapy is the current drug of choice. The study aimed to investigate in a mouse model of human cerebral malaria whether a trioxaquine chemically synthesized by covalent linking of a 4,7-dichloroquinoline pharmacophore to artesunate through a recent drug development approach termed ‘covalent bitherapy’ could improve the curative outcomes in cerebral malaria infections. Methods Human cerebral malaria rodent model, the C57BL/6 male mice were infected intraperitoneally (ip) with Plasmodium berghei ANKA and intravenously (iv) treated with the trioxaquine from day 8 post-infection (pi) at 12.5 and 25 mg/kg, respectively, twice a day for 3 days. Treatments with the trioxaquine precursors (artesunate and 4,7-dichloroquine), and quinine were also included as controls. In vivo safety evaluation for the trioxaquine was done according to Organization for Economic Co-operation and Development (OECD) guidelines 423, where female Swiss albino mice were orally administered with either 300 or 2000 mg/kg of the trioxaquine and monitored for signs of severity, and or mortality for 14 days post-treatment. Results The trioxaquine showed a potent and a rapid antiplasmodial activity with 80% parasite clearance in the first 24 h for the two dosages used. Long-term parasitaemia monitoring showed a total parasite clearance as the treated mice survived beyond 60 days post-treatment, with no recrudescence observed. Artesunate treated mice showed recrudescence 8 days post-treatment, with all mice in this group succumbing to the infection. Also, 4,7-dichloroquinoline and quinine did not show any significant parasitaemia suppression in the first 24 h post-treatment, with the animals succumbing to the infection. Conclusion Covalent bitherapy proves to be a viable source of urgently needed new anti-malarials for management of cerebral malaria, and this polypharmacology approach could be a potential strategy to protect artesunate from parasite resistance and in potentially improving clinical outcomes in severe forms of malaria infections.

Background We explore the spatial distribution of comorbidity with trachoma and potentially visual-impairing non-communicable diseases (NCDs) and their risk factors among Kenyan pastoralists. Methods Using a cross-sectional study design, we recruited and clinically examined 262 study participants for NCDs (hypertension or diabetes) and trachoma using the World Health Organization grading system. Network models estimated interactions and risks linked with trachoma and NCDs while the Poisson point process determined their spatial distribution. Results Of the 262 participants, 140 (53%) had trachoma, with >71% of these cases identified among females and those >60 y of age. A total of 36 trachoma cases co-occurred with hypertension (26%) and diabetes (0.01%). NCDs were frequent among those with recurring trachoma (21%) and trachomatous trichiasis (14.3%). Trachoma and NCDs clustered together in <1 km distances (R=0.18, p=0.02). In network analysis, age was strongly associated with trachoma and NCDs. Trachoma was linked with geographic location while diabetes was linked with water source distances. Education level became the central risk factor. Conclusions We demonstrate a twin trachoma–NCD burden that is higher among elderly pastoralists in southern Kenya. Attenuating adverse population-level visual impairment, including integrating the trachoma SAFE strategy with NCD comprehensive care, amplifies the benefits from economies of scale, accelerating realization of Sustainable Development Goal 3 and universal health coverage in hard-to-reach areas. In Yatta sub-County, a semi-arid land, there is scanty information on the causes and effects of climate change, as well as agricultural adaptation strategies. This scanty information assessment of climate related risks, and decision making about appropriate adaptation measures. A survey was conducted in two wards of Yatta, Kenya, to identify opportunities for building farmer capacity in dealing with climate variability. A semistructured questionnaire was administered to 60 households randomly distributed in the two wards and data was analyzed using Statistical Package for Social Scientists (SPSS). Results showed that farmers in the region were aware of climate change (98%) with the known indicators reported to be erratic and low rainfall (50%), drought (33%), and rising temperatures (14%). Farmers mentioned deforestation (73%) as the main cause of climate change, and reported major impacts on crop production in the region, such as the introduction of drought tolerant crops (45%) reduced yields (43%) and changes in planting time (38%). As a result, farmers prepared the land early using organic and inorganic fertilizers, planted early-maturing crop varieties and carried out water and soil conservation practices. Even though farmers in Yatta sub-county are aware of climate change its impacts and different coping and adaptation strategies, crop production in the region continues to decline. There is need, therefore, to increase farmer's capacity to better adapt to the effects of climate change to ensure sustainable agricultural production and improved food security. Proximate analysis for moisture, crude protein, crude fat and total ash was carried out on dagaa (Rastrineobola argentea), a small pelagic fish specie found in Lake Victoria. The first phase of the study involved sampling of fresh, sundried (for 1 day, 2 days, 3 days, 4 days) and retail market dagaa. The second phase of the study involved pre-washing of fresh dagaa with selected solutions namely, salted solution (3% NaCl), chlorinated solution (100 ppm) or potable tap water (control) and thereafter oven-drying the respective pre-washed samples at 30°C (31hrs), 40°C (23hrs) or 50°C (15hrs). Results showed that the crude protein composition of fresh dagaa (74.4% dry weight basis, dwb) was significantly higher (p<0.05) than values in market samples (62.5% dwb). After oven-drying at 30°C, the salted-wash treatments resulted to significantly lower (p<0.05) crude protein content of 60.4% (dwb) when compared with the chlorinated (64.6% dwb) and control-wash treatments (64.1% dwb).The crude fat content in fresh dagaa (14.8% dwb) was significantly higher (p<0.05) than levels in market samples (13.9% dwb).The salted-wash treatments showed significantly lower (p<0.05) crude fat content (15.9% dwb) than the chlorinated (17.0% dwb) and control (16.9% dwb) wash treatments after ovendrying at 30°C.The total ash content in fresh dagaa (10.3% dwb) was significantly lower (p<0.05) than levels in market samples (13.5% dwb).The salted and chlorinated-wash treatments exhibited significantly higher (p<0.05) total ash content (19.9%, 16.7% dwb, respectively) than the control-wash treatment (15.9% dwb) after drying at 40°C. In this study, oven-drying of dagaa at 40°C after washing with chlorinated (100ppm) solution was suggested with regard to the optimal retention of the crude protein and fat levels of the dried dagaa.These conditions are achievable at the local community level through use of solar driers whereas sodium hypochlorite products are accessible to most of the households involved in dried fish processing. BackgroundWorldwide, the number of emerging and re-emerging infectious diseases is increasing, highlighting the importance of global disease pathogen surveillance. Traditional population-based methods may fail to capture important events, particularly in settings with limited access to health care, such as urban informal settlements. In such environments, a mixture of surface water runoff and human feces containing pathogenic microorganisms could be used as a surveillance surrogate.MethodWe conducted a temporal metagenomic analysis of urban sewage from Kibera, an urban informal settlement in Nairobi, Kenya, to detect and quantify bacterial and associated antimicrobial resistance (AMR) determinants, viral and parasitic pathogens. Data were examined in conjunction with data from ongoing clinical infectious disease surveillance.ResultsA large variation of read abundances related to bacteria, viruses, and parasites of medical importance, as well as bacterial associated antimicrobial resistance genes over time were detected. Significant increased abundances were observed for a number of bacterial pathogens coinciding with higher abundances of AMR genes. Vibrio cholerae as well as rotavirus A, among other virus peaked in several weeks during the study period whereas Cryptosporidium spp. and Giardia spp, varied more over time.ConclusionThe metagenomic surveillance approach for monitoring circulating pathogens in sewage was able to detect putative pathogen and resistance loads in an urban informal settlement. Thus, valuable if generated in real time to serve as a comprehensive infectious disease agent surveillance system with the potential to guide disease prevention and treatment. The approach may lead to a paradigm shift in conducting real-time global genomics-based surveillance in settings with limited access to health care.
