2. Lay consultation for treatment-seeking decisions in slums of Nigeria : a mixed methods study

Abstract

Background Informal networks are important for social support for slum residents of Low and Middle-Income Countries (LMICs), where maintaining health is crucial for household income and formal healthcare provision is scarce. Consulting personal or online-based networks (known as lay consultation) during illness can influence personal treatment decisions, but there was scarce evidence from slums. This thesis aimed to (i) examine the use, characteristics and influence of lay consultation networks in slums of Nigeria, and (ii) explore experiences of using the networks. Methods A mixed-methods approach was adopted. A systematic review was initially conducted to synthesize evidence on lay consultation for symptoms and illnesses in slums of LMICs. A cross-sectional survey was conducted among adults in two slums in Nigeria to describe their lay consultation networks. This was followed by interviews to explore experiences of lay consultation in more depth. Key findings Most participants spoke to lay consultants during illness. The lay consultants used per illness episode were few and were mostly close network members. Use of online sources of advice was scarce largely due to poor access to smart devices and poor digital health literacy. Lay consultation was multi-dimensional, involving casual reporting of health concerns as part of daily bonding, exchanging advice and information about health, emotional and practical support. Lay consultation networks significantly influenced participants' choice of health care services. However, lay consultation had negative aspects, including facilitating the transfer of unhelpful advice, discriminating and stigmatizing people with certain characteristics and intruding on a patient's privacy, contributing to worse health outcomes. Participants were strongly agentic in how they selected lay consultants and adopted advice from others. Conclusion This thesis highlights the potential of engaging lay consultation networks in health education interventions and creating a formal health resource as cost-effective means to support treatment decisions in slums.

8. Location and media dynamics in information sharing in Ibadan, Southwestern Nigeria

Abstract

This paper focuses on information needs and sharing among Ibadan residents, taking into cognizance the people’s social environment and media accessibility. Five hundred copies of a questionnaire were administered among household heads and 12 key informant interviews were conducted in Ibadan, south-western Nigeria. Most respondents in the rural (57%) and urban (93%) areas possessed radio which was mainly used for information sharing and entertainment purposes. Mobile phones (93%) and television sets (76%) were possessed by urban dwellers more than their rural counterparts, whereas Internet facilities were more accessed (42%) than possessed (36%) by urban dwellers. Both rural and urban dwellers attributed inability to fully utilize the media to factors such as poor electricity supply, network uncertainties, promotion of immorality and frequent bad news projected by radio and television. Findings in this study emphasised the social and environmental differences in the adoption of the media for information sharing among rural and urban dwellers. Thus, the goal of the media is better achieved when the factors that determine the use of media in different zones are properly examined and messages are designed to fit the nature of particular environments.Keywords: Information sharing, media sources, multimedia dynamics, mass communication

10. Age heaping among adults in Nigeria: evidence from the Nigeria Demographic and Health Surveys 2003-2013.

Abstract

Age, as a variable, represents a critical basis for demographic classification; thus, its misrepresentations or misreporting alter the accuracy of demographic estimates. This paper examines the extent and pattern of age heaping in the age data for adults, collected in the Nigerian Demographic Health Survey (NDHS). The study used the NDHS data for 2003, 2008, and 2013 to compute a Whipple's and Meyers' blended index for each survey year, by gender, geopolitical zones, states and place of residence. The analysis shows that age heaping was higher than the acceptable levels in all three data sets. The Whipple's index puts the rate of age heaping in the 2003 dataset at 271.3, whilst the rates declined slightly in the 2008 and 2013 datasets to reach 204.2 and 202.5 respectively. Similarly, the Myers' blended index portrayed that age heaping in the 2003 data was highest at 47.0 while the subsequent years were lower at 38.60 and 38.66, respectively. Digits ending in 0 and 5 were mostly reported in all three surveys and higher rates of age heaping were observed among males, the uneducated and rural dwellers. Age heaping was prominent in all three surveys, thus affecting the data quality gathered at these surveys.
