The tendency to continue or intensify gambling after losing (loss-chasing) is widely regarded as a defining feature of gambling disorder. However, loss-chasing in real gambling contexts is multifaceted, and some aspects are better understood than others. Gamblers may chase losses between multiple sessions or within a single session. Furthermore, within a session, loss-chasing can be expressed in the decision of (1) when to stop, (2) how much stake to bet, and (3) the speed of play after winning and losing. Using a large player-tracking data set (>2500 players, >10 million rounds) collected from the online commercial game Mystery Arena, we examined these three behavioral expressions of within-session loss-chasing. While the first two aspects (when to stop and how much stake to bet) have been examined previously, the current research is the first large-scale study to examine the effects of wins and losses on the speed of play in real gambling. The players were additionally assigned different involvement levels by the operator based on their gambling behavior on the operator's own platform, which further allowed us to examine group differences in loss-chasing. We found that after winning, both the high- and low-involvement groups were less likely to stop, and increased the stake amount, thus showing win-chasing instead of loss-chasing in these two facets. After losing, both groups played more quickly though, which may reflect an urge to continue gambling (as an expression of loss-chasing). Wins and losses had a smaller influence on the speed of play for the high-involvement players, suggesting that they might have reduced sensitivity to wins and/or losses. Introduction: Due to the COVID-19 pandemic, the ACGME recommended all interviews for the 2021 residency application cycle be held virtually. Because this is major shift from neurosurgical interviews in past years, this study aims to evaluate both applicant and interviewer satisfaction of conducting interviews virtually., Methods: For faculty, an 11-question online survey was sent to 116 United States neurosurgery training programs. A 14-question online survey was sent to 255 neurosurgery applicants. The resulting data were analyzed qualitatively and quantitatively., Results: From applicants, 118 responses were received. From faculty, 171 individual responses were received. Thirty-five percent (34.7%) of applicants agreed that they were satisfied with the virtual interview process as a whole. Although 44.5% of faculty disagreed with the statement "I would like to replace in-person interviews with virtual interviews in the future", 57.3% of faculty agreed that they were likely to implement virtual interviews in the future., Conclusions: Some things might be better assessed through in-person interviews, but there are clear benefits to virtual interviews. Future iterations of the interview process, incorporating virtual interviews, might help determine how and in which situations virtual interviews can be utilized in future residency application cycles., (Copyright © 2021 Elsevier Inc. Purpose : The authors aim to characterize oculofacial plastic surgery-related online interest that may be useful in forecasting demand and in designing patient-directed online resources. Methods : The authors queried Google Trends for over 100 oculofacial plastic surgery terms. The main outcome measure was the top 50 oculofacial plastic surgery-related search terms from 2004 to 2020. Secondary outcomes were trends, including seasonality, and search volume changes during the COVID-19 lockdown (March-May 2020) compared to 2018-2019. Terms were analyzed individually and in thematic categories; controlled against generic search terms to account for general internet traffic. Results : Between 2004 and 2020, searches for oculofacial plastic surgery altogether increased, surpassing the rate of internet traffic growth. One thematic category - eyelid malpositions - decreased month-over-month. The top five terms were "face lift," "Bell's palsy," "puffy eyes," "dark circles under eyes," and "chalazion." Eyelid neoplasms searches peaked in summer ( R 2  = 0.880) whereas cosmetic ( R 2  = 0.862), symptoms ( R 2  = 0.907), and surgeries ( R 2  = 0.140) peaked in winter. Overall, oculofacial-related searches decreased during the COVID-19 lockdown, although thyroid eye disease interest increased compared to 2018 or 2019 (+68.6%; adj. p = .005). Oculofacial plastic surgery interest in 2020 was inversely correlated to "COVID-19" searches (r = -0.76, p < .001). Conclusions : Oculofacial plastic surgery searches increased since 2004 at a pace greater than that ascribed to internet traffic growth. The most searched terms were "face lift," "Bell's palsy," "puffy eyes," "dark circles under eyes," and "chalazion." Almost all oculofacial-related searches decreased during the COVID-19 lockdown.

Preprints are becoming well established in the life sciences, but relatively little is known about the demographics of the researchers who post preprints and those who do not, or about the collaborations between preprint authors. Here, based on an analysis of 67,885 preprints posted on bioRxiv, we find that some countries, notably the United States and the United Kingdom, are overrepresented on bioRxiv relative to their overall scientific output, while other countries (including China, Russia, and Turkey) show lower levels of bioRxiv adoption. We also describe a set of 'contributor countries' (including Uganda, Croatia and Thailand): researchers from these countries appear almost exclusively as non-senior authors on international collaborations. Purpose: Depression is common in epilepsy, and is often under-detected and under-treated. The motivation to create a depression eHub for persons with epilepsy is to connect them to the best available online resources to effectively manage their depression. The study sought to: 1) identify facilitators and barriers to accessing resources related to management of epilepsy and/or depression and 2) discuss gaps in available resources (free and in the public domain) and 3) identify suggestions for future content., Methods: Semi-structured interviews were conducted with ten patients with epilepsy and a history of depression. Using inductive analysis, two team members engaged in a process of textual open-coding utilizing a conventional content analysis approach whereby content was conceptually clustered based on the research questions. A phenomenological framework was applied to describe the phenomenon of online health resource access and utilization from the perspective of people with epilepsy., Results: Facilitators to the use of online resources included information credibility, thoughtful organization, and accessibility of resources. Barriers included difficulties finding and piecing together information from many different sites. Patients reported difficulty having the motivation to seek out resources while depressed, which was compounded by feelings of stigma, social isolation, and lack of control. Gaps in resources included a lack of information about living with epilepsy day-to-day and resources for family and friends. Suggested content included information to raise awareness about epilepsy and depression; questionnaires to screen for symptoms of depression; stories of other patients with epilepsy and depression via video or moderated forums; current research and news; local community resources; and tools and strategies to manage depression in epilepsy., Conclusions: There is a gap in accessible resources for patients with epilepsy and depression as well as barriers that include epilepsy-related restrictions, depression-related impairments, lack of awareness, and stigmatization. The Houston-based health system has implemented new workflows and technology in 14 of its hospitals and across its care delivery network to make the payment process more patient-friendly and build consumer loyalty.

Objective: Content analysis of Internet-based diabetes forum text was used to examine the experiences encountered by students with diabetes transitioning into and through college., Participants: Forum posts (N = 238) regarding attending college with diabetes were collected and analyzed., Methods: Thematic coding was used to identify prominent topics, followed by analysis of theme distribution across college transitional stages. Three students with diabetes were recruited to review results and corroborate findings., Results: Twenty thematic categories were identified. Preparation for college involved efforts to move toward autonomous diabetes management. Transitioning in was marked by adjusting to a college lifestyle, then working to manage issues such as busy schedules and alcohol use as continuing students, and turning attention toward future career options and finances while transitioning out., Conclusions: As they move into and through college, students with diabetes negotiate developmental and diabetes-specific tasks within an environment that presents unique logistical, lifestyle, and psychological challenges.

Despite the growth of the digital era, rapid spread of information, and real-time communication, there are several urologists throughout the world who do not have access to urological scientific programs, academic training programs, or memberships to well-established urological organizations, associations, and societies. The International Young Urological Association (IYUA) is a non-profit organization that was created to specifically address and facilitate access for urologists that for geopolitical, economical, or other reasons would not otherwise have these opportunities available to them. The IYUA's non-competitive nature allows for urologists to use the IYUA as an initial platform to network with globally established organizations to deliver scientific training programs, courses, and meetings in less common locations. It is our objective to demonstrate the IYUA's mission and facets that may contribute to the increased academic and scientific education of professionals in Urology where the access to these resources can be challenging.

Purpose: The ability to track prescription opioid abusers' endorsement of compounds may be valuable in forecasting abuser's preferences. We developed a metric, referred to as the Endorsement Ratio (ERo), to explore the feasibility of quantifying the sentiment expressed by opioid abusers who post online about prescription opioid products., Methods: Internet posts written between April 1, 2010 and March 31, 2011 and related to six prescription opioid compounds (oxycodone ER, hydrocodone, hydromorphone, oxymorphone, morphine ER, and tramadol) were identified and qualitatively coded. A mixed effects multinomial logistic regression was employed to model the probability of observing endorsing, discouraging, mixed, or unclear messages per compound. Endorsement-to-discouragement ratio (ERo) and ratio of endorsement-to-discouragement ratios (RERo) were estimated for each compound., Results: In the study sample of 12 838 Internet posts, a ranking of endorsement could be established, with oxymorphone as most endorsed (ERo = 7.39), followed by hydromorphone (ERo = 5.02), hydrocodone (ERo = 3.53), oxycodone ER (ERo = 3.23), morphine ER (ERo = 2.71), and tramadol (ERo = 1.76). Relative rankings of products required that route of administration be considered., Conclusions: This study expands existing Internet monitoring approaches by developing a metric to estimate the endorsement of specific prescription opioid compounds as viewed through the lens of Internet communities. Route of administration must be taken into account when examining preferences of drug abusers for various products. This study did not assess whether the novel metric is valid with respect to classification of abuse rates of different drugs. Patient navigation programs are increasing throughout the USA, yet some evaluation measures are too vague to determine what and how navigation functions. Through collaborative efforts an online evaluation program was developed. The goal of this evaluation program is to make data entry accurate, simple, and efficient. This comprehensive program includes major components on staff, mentoring, committees, partnerships, grants/studies, products, dissemination, patient navigation, and reports. Pull down menus, radio buttons, and check boxes are incorporated whenever possible. Although the program has limitations, the benefits of having access to current, up-to-date program data 24/7 are worth overcoming the challenges. Of major benefit is the ability of the staff to tailor summary reports to provide anonymous feedback in a timely manner to community partners and participants. The tailored data are useful for the partners to generate summaries for inclusion in new grant applications.

Recruiting, retaining, and educating advanced practice nurses is essential to meet the growing need for advanced practice nurses in rural and urban communities. Through the support of Health Resources and Services Administration funding, the urban school of nursing expanded its MSN program and implemented the graduate curriculum on its rural campus by utilizing emerging online and distance education technologies. The purpose of this manuscript is to provide an overview of expanding an existing MSN program offered in an urban, traditional classroom setting to rural graduate nursing students via an online synchronous format. In addition, the article will describe the rural growth of the existing neonatal nurse practitioner program as an exemplar and the different methodologies that are being used in each program to engage the rural nurse practitioner students in clinical courses. In addition, strategies to address barriers related to rural nurse practitioner student recruitment and retention will be discussed.

Introduction: As the volume of data and analyses grows with time, so does the need to present this increasingly complex information in an accessible and clinically informative manner which is responsive to, and reflects the nature of, the enquiries made by those seeking to access the data., The Uk Renal Registry Interactive Data Portal: The UK Renal Registry (UKRR) now has a bespoke interactive data portal which provides a focussed point of access to a variety of graphical display formats and analyses of UKRR data including: · Centre-specific reports--a distillation of annual UKRR data including a colour-coded dashboard summary as well as both funnel plots and longitudinal statistical process control charts for a range of clinical parameters. · Interactive flash-based longitudinal Statistical Process Control charts on a per-centre and per-parameter basis allowing for a more detailed review of performance over time. These charts are the interactive correlates of those available in the centre-specific reports. · Rosling/Gapminder-style motion charts on a perparameter basis simultaneously detailing performance and activity data from multiple centres interactively over time (more details below). · An interactive graphical pivot chart solution using OLAP technology allowing users to design and export their own charts/analyses in real-time using UKRR data., Conclusion: This work builds strongly on the wealth of information arising from the high-quality validated UKRR datasets. Since "Budapest Open Access Initiative" signed in 2002, there have been more and more open-access journals coming out on Internet, thus brining the best condition to the researchers. It has made better the phenomenon of not having enough foreign language periodicals in our country. The present author has selected part of free biomedicine periodical websites: High Wire Press; PubMed Central; BioMed Central; BioLine International; Free Medical Journals Site; Scientific Electronic Library; Hindawi; Academic Journals Inc; Biomedical Journals from India. And then, analysis and demonstration are made with respect to the search channels of 3 main free electronic periodical websites. It is the intent of this paper to make things convenient for researchers to get information from the websites quickly.

Background: During the last decade, the Internet has become increasingly popular and is now an important part of our daily life. When new "Web 2.0" technologies are used in health care, the terms "Health 2.0" or "Medicine 2.0" may be used., Objective: The objective was to identify unique definitions of Health 2.0/Medicine 2.0 and recurrent topics within the definitions., Methods: A systematic literature review of electronic databases (PubMed, Scopus, CINAHL) and gray literature on the Internet using the search engines Google, Bing, and Yahoo was performed to find unique definitions of Health 2.0/Medicine 2.0. We assessed all literature, extracted unique definitions, and selected recurrent topics by using the constant comparison method., Results: We found a total of 1937 articles, 533 in scientific databases and 1404 in the gray literature. We selected 46 unique definitions for further analysis and identified 7 main topics., Conclusions: Health 2.0/Medicine 2.0 are still developing areas. Many articles concerning this subject were found, primarily on the Internet. However, there is still no general consensus regarding the definition of Health 2.0/Medicine 2.0. We hope that this study will contribute to building the concept of Health 2.0/Medicine 2.0 and facilitate discussion and further research.

Background: Augmenting validated paper versions of existing outcome measures with an equivalent online version may offer substantial research advantages (cost, rapidity and reliability). However, equivalence of online and paper questionnaires cannot be assumed, nor can acceptability to respondents. The aim was to test whether online and written versions of the Roland Morris Disability Questionnaire (RMDQ), a standard measure of functional disability in back pain, are equivalent at both group and individual levels to establish whether they can be used interchangeably., Methods: This is a within-participants equivalence study. 167 participants with back pain fully completed both the paper and online versions of the RMDQ in random order. Participants were recruited from a chiropractic clinic and patient support groups in Southern England. Limits of equivalence were pre-defined as 0.5 RMDQ points, the Bland-Altman range was calculated, and participants' comments were examined using content analysis., Results: The mean score difference was 0.03 (SD = 1.43), with the 95% Confidence Interval falling entirely within our limits of equivalence (-0.19 to 0.25). The Bland-Altman range was -2.77 to 2.83 RMDQ points. Participants identified unique advantages and disadvantages associated with each version of the RMDQ., Conclusions: The group and individual level data suggest that online and paper versions of the RMDQ are equivalent and can be used interchangeably. The Bland-Altman range appears to reflect the known measurement properties of the RMDQ. Furthermore, participants' comments confirmed the potential value to be had from offering them the choice of completing the RMDQ online or on paper.

Introduction/objective: Although the Internet, podcasts and multimedia have become a natural part of our life, these instruments still do not find widespread use in medical education. We aimed to increase the percentage of students benefiting from our lectures during their 6-month principal training period in dermatology by making lectures available online., Methods: To establish a baseline, we started to count and calculate the average percentage of students attending face to face classes. For the next semester, with a new student generation, we made recordings of about half of the lectures and made them available online. After this testing period, we informed the next new student population at the beginning of the following semester that all lectures would be recorded and made available online. Students' attendance was documented during these periods, and in addition, questionnaires were used to assess students' acceptance and use of the online lectures., Results: At the end of the project, 66% of the students (n = 256) indicated that they had used the online lectures, and 12% of all students stated that they were usually unable to attend lectures due to conflicting obligations, but could now participate thanks to the e-learning programme. An additional 44.9% of all respondents indicated that they welcomed the e-learning programme as a way to view specific lectures. The average attendance of face-to-face lectures did not decrease., Conclusion: Online lectures in dermatology were highly welcomed by our students and may be a good means to improve the education of students in dermatology.

On the basis of a study by the Institute of Medicine, the current health care system is facing several challenges that may be addressed by changes in health professions education. The study focused on integration of five core competencies into health professions education, one of which was informatics. This critical analysis investigates current use of technology and online instructional strategies in nursing education. It also explores the potential impact of integration of informatics into nursing education to increase the cognitive skills of nurses to promote evidence-based nursing. Advantages and disadvantages of using online education in the instruction of nursing students and recommendations for best online practices in nursing education are discussed.

Background: The feasibility and acceptability of teaching medical students to use PDA clinical decision support tools via a Web-based course have not been previously evaluated., Methods: A total of 119 third-year family medicine clerkship students completed a baseline survey on PDA use, attended an introductory PDA lecture, and were invited to voluntarily access a Web-based course through Blackboard. All students had been previously issued with PDAs in their second year., Results: At baseline, 95% of students reported having removed their PDA from its box, 59% reported using it weekly, and 71% had loaded medical applications. From August 2006--March 2007, 36 students accessed the course 610 times (range 8-54). The PDA cases comprised 63% of hits, course resources 30% of hits, and course information 6% of hits. Students evaluated the course equally to other clerkship didactics., Conclusions: It is feasible and acceptable to students to teach PDA decision support tools in an online course. In our setting, for the minority of students who chose to learn online, the format was successful and met their needs.
