Extrahepatic bile duct cancer is a high mortal malignancy. Angiopoietin (Ang) and its receptor Tie, which are known to contribute to angiogenesis, have recently been reported to participate in the proliferation and differentiation of malignant tumor cells. The aim of this study is to investigate the expression and the significance of Ang-1, 2 and Tie-2 in extrahepatic bile duct carcinoma cells. We used immunohistochemistry to study 119 cases of surgically resected human extrahepatic bile duct carcinoma, and Reverse Transcription-Polymerase Chain Reaction (RT-PCR) to confirm the expression of Ang-1, 2 and Tie-2 mRNA. Among these 119 cases, 52 (43.7%), 50 (42.0%) and 89 (74.8%) cases showed positive staining for Ang-1, 2 and Tie-2, respectively, in bile duct carcinoma cells. In 38 cases of normal mucosa, 6 (15.8%), 10 (26.3%) and 9 (23.7%) cases were positive for Ang-1, 2 and Tie-2, respectively. A case of carcinoma of the descending colon in a 27-year-old man is reported herein. The patient was referred to the University Hospital with a diagnosis of colonic obstruction on December 18, 1985. One month prior to this admission, he had emergency colostomy for obstruction of the descending colon at other hospital. One this admission, barium enema studies through a colostomy demonstrated a complete obstruction of the descending colon with an irregular contour. Colonofiberscopy also revealed carcinoma in this location. Left hemicolectomy was carried out on January 7, 1986. At operation, carcinoma showed extensive spreads onto the peritoneal and serosal surface. Grossly, the involved portion of the colon was strictured with a marked thickening of the wall. Histologically, carcinoma was poorly differentiated adenocarcinoma with extensive lymphatic and vascular invasion. Following operation, he was treated with adjuvant chemotherapeutic agents. At this writing six months after surgery, the patient is doing well., 論文

A case of mucinous cystadenoma of the appendix in a 63-year-old female is presented herein. The patient was seen in the University Hospital with a complaint of left lower quadrant abdominal mass on November 21, 1984. One year prior to this admission, she noticed a mass in the left lower quadrant of the abdomen. On admission, physical examination revealed a hard, movable mass, hen's egg in size, in the left lower quadrant. Barium enema studies demonstrated smooth displacement of the cecum by the mass. Ultrasonography and computed tomograpy showed a cystic mass with internal homogeneous material. Colonofiberscopy detected no abnormality of the cecum. At surgery, the mass appeared well circumscribed with no tumor spreads. Resection of the mass of the appendix was carried out. Grossly, the mass was 7.5×4.5×4.0cm. in size and cystic on cut section. The cyst was filled with gelatinous material. The cyst wall was 2mm. in thickness with inner smooth surface. Histologically, areas of the hyperplastic columnar epithelium with cellular stratification and atypism and hyperplastic glands with mucus columnar cells were scatteredly found. The cyst wall was hyalimzed with focal calcification with no evidence of tumor invasion. She has been doing well with no evidence of recurrence of the tumor 1.5 years since surgery.

A 48-year-old man was admitted to the University Hospital for surgery of the colon on October 16, 1985. About one month prior to this admission, the patient was seen in a local medical clinic with a complaint of abdominal pain. Barium enema studies at the clinic demonstrated a complete obstruction of the transverse colon, and then he was diagnosed to have carcinoma of the colon. On admission, the patient appeared not critically ill. Physical examination revealed tenderness in the upper abdomen. Laboratory data and values of tumor markers were whithin normal limits. On October 28, he was operated on. Extensive spreads to the liver, omentum and regional lymph nodes were noted. Right henucolectomy was carried out. Grossly, the involved colon revealed carcinoma of scirrhous type(Borrmann's 4th type). Histologically, carcinoma showed moderately differentiated tubular adenocarcinoma with marked vascular and lymphatic invasion. The patient was treated with adjuvant chemotherapeutic agents(Mitomycin C, 5-fluorouracil), but expired with clinical manifestations of peritonitis carcinomatosa four months after surgery.

A case of leiomyoblastoma of the pylorus showing prolapse into the duodenal bulb and cholelithiasis in a 56-year-old female is described herein. The patient was hospitalized to the University Hspital for further investigation with a diagnosis of a polypoid tumor of the pylorus on April 18, 1983. Upper gastrointestinal series demonstrated a polypoid tumor in the pylorus to the duodenal bulb. Her gastrofiberscopy revealed a submucosal tumor overlying the pylorus and obstructing the outlet of the stomach. She was also diagnosed to have cholelithiasis. At surgery, the polypoid tumor showed no evidence of tumor spreading onto the serosa and to the lymph nodes. Following cholecystectomy for cholelithiasis, antral gastrectomy was carried out. The polypoid tumor was located over the pyloric ring and pedunculated, with projection into the duodenal bulb. It had a smooth mucosal surface, and was oval in shape and 5×3×2cm. in size. On cut section, the tumor appeared lobulated, locally necrotic and circumscribed between the mucosa and the muscularis propriae. The pathological report was leiomyoblastoma. At this writing, she has been doing well about three years since surgery. This is the second reported case showing prolapse into the duodenum and the first of the pylorus in Japan.
