Abstract

In 2007, the Care of the Dying clinical coordinated Pathway (CDP) was adapted from the United Kingdom Liverpool Care Pathway (LCP) and implemented in a tertiary hospital in Singapore to improve care in oncology and subsequently renal patients. With concerns about its use after the Neuberger review, an audit of patients on the CDP was performed to determine if the use of such a pathway should be continued locally.A two-year retrospective audit of CDP use was conducted. Aspects of communication, initiation of CDP, patient monitoring, medications, nutrition, and hydration were assessed.In the 111 patient records reviewed, there were documented extent of care discussions with 94% of caregivers and 29% of patients before CDP initiation. Of the 88% of CDPs initiated within office hours, 90% fulfilled the inclusion criteria. All patients were monitored at least every eight hours. Seventy-three patients (66%) were prescribed opioids or sedatives and subsequently monitored appropriately, albeit 56% had no documented prior discussion with caregivers regarding medication use. Indications for opioid use were documented for all patients and only one patient had documentation of excessive sedation. Oral feeding and parenteral hydration were continued in 85% and 74% of patients, respectively.There was no documented compromise in medication safety, clinical monitoring, and provision of nutrition and hydration for patients on the CDP. However, documentation of important end-of-life decisions and conversations remain poor. Development of an alternative care tool encouraging systematic discussion and documentation of individualized end-of-life care plans should be considered.

Abstract

Proponents of physician-assisted suicide and euthanasia (PAS/E) assert that mentally competent terminally ill patients experiencing 'intractable' physical and/or existential suffering with no means of alleviating this suffering should be afforded the right to end their lives at a time and place of their choosing. A significant obstacle to legalising PAS/E has been the slippery slope argument that posits that gradual abuse of PAS/E is inevitable, leaving vulnerable patient groups of patients susceptible to its use. To distance this practice from the slippery slope, proponents of PAS/E proffer the concept of "Medical Aid-In-Dying" (MAID) that boasts of strict criteria for its use and forward evidence that its use in Oregon and the Netherlands has revealed no evidence of vulnerable patients being coerced into having PAS/E. This paper assesses the position and the assertions brought by proponents for the legalization of MAID in Singapore using an evidenced based socioculturally sensitive approach to contextualise these arguments. Socioculturally, prevailing Confucianinspired family-centric practices hinders informed consent and creates coercive influences upon the decisionmaking process that will hinder effective consent for MAID whilst clinical evidence highlights inherent inaccuracies within determinations of prognosis and intractability that compromise legitimate application of MAID. This paper highlights the need for socioculturally sensitive, evidenced-based evaluation of all extrapolated data to consider the impact of prevailing mores and laws upon its application within the local scene. Abstract

The article presents a perspective on withdrawing artificial hydration and nutrition (AHN) from patients in persistent vegetative state (PVS) in the context of Singapore. Within the Confucian-centric socio-cultural beliefs prevailing in Singapore, withdrawal of AHN are met with resistance and foster guilt among family members. AHN withdrawal therefore cannot be abrupt and unilateral but rather a multi-dimensional review that considers socio-cultural norms and familial and religious beliefs.
