Another frequent condition, especially among elderly people, is cardiac amyloidosis (CA), particularly the wild-type transthyretin cardiac amyloidosis (ATTRwt). Since both of these diseases are considered a marker of ageing, there is a significant proportion of elderly patients who associate both severe AS and CA. Recent studies reported a high prevalence of both severe AS and CA (AS-CA) in elderly patients referred for TAVR of 13– 16%, carrying a worse prognosis. The present case illustrates the diagnostic algorithm and the management of ATTRwt CA in an elderly patient with severe paradoxical low-flow low-gradient AS, accompanied by a review of the current literature about the red flags which help identifying CA in patients with severe AS, as well as the prognosis and management of these disease association.Keywords: aortic stenosis, cardiac amyloidosis, wild-type ATTR, transcatheter aortic valve replacement, conduction disorders

Contains fulltext : 202668.pdf (Publisher’s version ) (Open Access) Aims: The main aim of this study was to characterize changes in the left ventricular (LV) blood flow kinetic energy (KE) using four-dimensional (4D) flow cardiovascular magnetic resonance imaging (CMR) in patients with myocardial infarction (MI) with/without LV thrombus (LVT). Methods and results: This is a prospective cohort study of 108 subjects [controls = 40, MI patients without LVT (LVT- = 36), and MI patients with LVT (LVT+ = 32)]. All underwent CMR including whole-heart 4D flow. LV blood flow KE wall calculated using the formula: KE=12 rhoblood . Vvoxel . v2, where rho = density, V = volume, v = velocity, and was indexed to LV end-diastolic volume. Patient with MI had significantly lower LV KE components than controls (P < 0.05). LVT+ and LVT- patients had comparable infarct size and apical regional wall motion score (P > 0.05). The relative drop in A-wave KE from mid-ventricle to apex and the proportion of in-plane KE were higher in patients with LVT+ compared with LVT- (87 +/- 9% vs. 78 +/- 14%, P = 0.02; 40 +/- 5% vs. 36 +/- 7%, P = 0.04, respectively). The time difference of peak E-wave KE demonstrated a significant rise between the two groups (LVT-: 38 +/- 38 ms vs. LVT+: 62 +/- 56 ms, P = 0.04). In logistic-regression, the relative drop in A-wave KE (beta = 11.5, P = 0.002) demonstrated the strongest association with LVT. Conclusion: Patients with MI have reduced global LV flow KE. Additionally, MI patients with LVT have significantly reduced and delayed wash-in of the LV. The relative drop of distal intra-ventricular A-wave KE, which represents the distal late-diastolic wash-in of the LV, is most strongly associated with the presence of LVT.

Abstract

Current echocardiographic data reporting the impact of concomitant mitral regurgitation (MR) on outcome in patients who undergo transcatheter aortic valve replacement (TAVR) are conflicting. Using cardiovascular magnetic resonance (CMR) imaging, this study aimed to assess the impact of MR severity on cardiac reverse remodeling and patient outcome. 85 patients undergoing TAVR with CMR pre- and 6 m post-TAVR were evaluated. The CMR protocol included cines for left (LV) and right ventricular (RV) volumes, flow assessment, and myocardial scar assessment by late gadolinium enhancement (LGE). Patients were dichotomised according to CMR severity of MR fraction at baseline ('non-significant' vs 'significant') and followed up for a median duration of 3 years. Forty-two (49%) patients had 'significant MR' at baseline; they had similar LV and RV size and function compared to the 'non-significant MR' group but had greater LV mass at baseline. Abstract

Background - Despite the fact that assessment of right ventricular longitudinal strain (RVLS) carries important implications for patient diagnosis, prognosis, and treatment, its implementation in clinical settings has been hampered by the limited reference values and the lack of uniformity in software, method, and definition used for measuring RVLS. Accordingly, this study was designed to establish (1) the reference values for RVLS by 2-dimensional speckle-tracking echocardiography; and (2) their relationship with demographic, hemodynamic, and cardiac factors. Methods and Results - In 276 healthy volunteers (55% women; age, 18-76 years), free wall and septum RVLS (6 segments) and free wall RVLS (3 segments) using both 6- and 3-segment regions of interest were obtained. Feasibility of 6-segment RVLS was 92%. Free wall RVLS from 3- versus 6-segment regions of interest had similar values, yet 6-segment region of interest was more feasible (86% versus 73%; P<0.001) and reproducible. Reference values (lower limits of normality) were as follows: 6-segment RVLS, -24.7±2.6% (-20.0%) for men and -26.7±3.1% (-20.3%) for women; 3-segment RVLS, -29.3±3.4% (-22.5%) for men and -31.6±4.0% (-23.3%) for women (P<0.001). Free wall RVLS was 5±2 strain units (%) larger in magnitude than 6-segment RVLS, 10±4% larger than septal RVLS, and 2±4% larger in women than in men (P<0.001). At multivariable analysis, age, sex, pulmonary systolic pressure, right atrial minimal volume, as well as right atrial and left ventricular longitudinal strain resulted as correlates of RVLS values. Conclusions - This is the largest study providing sex- and method-specific reference values for RVLS. Our data may foster the implementation of 2-dimensional speckle-tracking echocardiography-derived RV analysis in clinical practice.

40. Left Atrial Function in Patients with Reentrant Paroxysmal Supraventricular Tachycardia with Narrow QRS Complex - The Role of Speckle Tracking Echocardiography.

Abstract

Objectives: We sought to investigate by speckle tracking technique the longitudinal function of the left atrium in patients with reentrant paroxysmal supraventricular tachycardia with narrow QRS complex, in order to identify early functional changes that these arrhythmias may induce even when paroxysmal. Methodology: 23 patients with reentrant paroxysmal supraventricular tachycardia with narrow QRS complex have been evaluated by echocardiographic speckle tracking techniques in the first week after pharmacological conversion to sinus rhythm. The subsequent electrophysiology study identified 13 of these patients with atrioventricular nodal reentrant tachycardia (AVNRT), and 10 patients with atrioventricular reentrant tachycardia (AVRT). The echocardiographic examinationby speckle tracking technique was performed considering current standards, in apical 4 and 2 chamber views, using narrow sector angles, single focus imaging and optimal depth. Left atrium contour was traced defining the region of interest and the longitudinal strain was obtained for 12 segments. The average longitudinal strain obtained in each apical view was represented by a curve allowing the peak-strain quantification. Results: The peak longitudinal strain values assessed in the apical 4 chamber view in the AVNRT group (most frequent value 16% (mod = 16%)) and in the AVRT group (most frequent value 8.5% (mod = 8.5%)(were significantily lower than the peak longitudinal strain values measured in the control group (most frequent value 39% (mod = 39%)), with no significant difference in peak longitudinal strain between the two different types of tachycardia groups (F = 30,19; p < 0,0001). In a similar manner, the peak longitudinal strain measured in the apical 2 chamber view were significantly lower in the tachycardia group than in the control group (most frequent value in the control group 38% (mod = 38%), in the AVNRT group 19% (mod = 19%), in the AVRT group 19% (mod = 19%)(F = 36,68; p < 0,0001). Conclusions: Tachycardia irrespective of the type - AVNRT or AVRT results in the impairment of left atrial mechanical function. Abstract

Objectives: To evaluate the epidemiology of arterial hypertension in the Balkan countries, based on the surveys conducted in these countries and comparing their methodology and results with those obtained from SEPHAR studies. Methods: The SEPHAR II was an arterial hypertension study conducted in Romania between October 2011 and March 2012. We compare the methodology and the results obtained from 9 other similar studies conducted in the last 18 years in the countries of the Balkan Peninsula: Bulgaria, Kosovo, Albania, Bosnia Herzegovina, Turkey, Croatia, Slovenia, Serbia, and Greece. Results: The prevalence of arterial hypertension in adult population of Romania was 40, 4%, similar with the prevalence of the disease in the other Balkan countries. In Romania we observed the highest awareness rate of the disease (69, 5%) from all the analysed countries. The proportion of treated hypertensive patients was higher in Romania (85%) than in the other countries. The adequate control of hypertension was obtained in about 25% of patients in Romania, which was greater than that observed in the other countries of Balkan Peninsula. Conclusions: The design of SEPHAR studies was similar to the homologues studies conducted in the other countries of the Balkan Peninsula. The arterial hypertension in Romania shared similar epidemiological characteristics with the other Balkan countries with only discrete differences. A high prevalence of hypertension has been observed in this region. Regarding the awareness, treatment and control of hypertension with treatment, Romania had a favourable position among the Balkan countries but was still far from an ideal situation. Abstract

It is universally known the link between physical inactivity and coronary heart disease. It is estimated that over 30% of coronary heart disease worldwide is due to physical inactivity. This evidence draws attention to the importance of exercise in primary prevention of heart disease and in secondary prevention measures, such as cardiac rehabilitation. International guidelines started to include exercise training beside pharmacological therapies, developing a specific type of training modality tailored for each subpopulation of cardiovascular patients and their risk factors. Many different types of exercise protocols, arranged for intensity, method, type, application, control and setting, exist and cardiopulmonary exercise test represents the gold standard for the assessment of functional evaluation in exercise prescription. In clinical practice, it is not always feasible to group patients into a specific category, due to the variety of their co-morbidities and risk factors: in secondary prevention, exercise intensity must yet be safe but also tailored to achieve the outcome goal of rehabilitation. The purpose of this report is to introduce the innovation of recent notions published in the scientific literature, trying to insert them into clinical practice giving a simple, but yet effective, instrument to the health care professionals. Abstract

Purpose: To investigate the relationship between two measures of regional myocardial workload (RMW): regional myocardial glucose metabolism, assessed by 18F-fluorodeoxyglucose (FDG) positron emission tomography (PET) and regional LV pressure-strain loop area. Both were evaluated in a chronic rapid pacing animal model of dilated cardiomyopathy (DCM) and LBBB-like mechanical dyssynchrony. Methods: Nine sheep were subjected to rapid RV pacing (180 bpm), developing LV dilatation and asymetric remodelling, with LBBB-like ventricular activation. After 8 weeks all animals underwent two FDG-PET scans: one under normal LV conduction (AAI pacing), another under LBBB-like activation (DDD pacing). Potential inhomogeneities in RMW represented by FDG-uptake, were then evaluated. Before the last PET scan, all animals were subjected to invasive pressure-volume and simultaneous echocardiographic circumferential strain (CS) analysis of the mid-ventricular segments. Segmental pressure-CS-loop area was then recorded as alternative measure of RMW and compared to PET results. Results: DDD and AAI pacing led to different, but reproducible patterns of inhomogeneous RMW distribution (Figure) with a significant decreased septal-to-lateral wall ratio of RMW in DDD mode compared to AAI, both in the FDG-uptake (0.87 ± 0.09 vs 1.05 ± 0.15, resp., p=0.005) and pressure-strain loops area (0.22 ± 0.18 vs 0.94 ± 0.23, resp., p=0.0001). A significant correlation was found between both measures (r=0.584, p=0.018). Conclusions: Our study indicates that RMW can be measured with both regional FDG-uptake and the area of regional pressure-strain loops. The influence of motion and partial volume on the reconstructed FDG-uptake in the asymmetrically remodelled hearts is currently being investigated.

50. Predictors of Increased Arterial Stiffness in Hypertensive Patients.

Abstract

Objectives: to evaluate arterial stiffness in hypertensive patients and to identify predictors of increased arterial stiffness. Method: 798 hypertensives identified in SEPHAR II survey were evaluated by BP and arterial stiffness measurements: aortic puls weave velocity (PWVao) and aortic augmentation index (AIXao), and a study questionnaire.Values above the 75th percentile of mean SBP' standard deviation (s.d.) defined increased SBP variability. Log(TG/HDL-cholesterol) defined atherogenic index of plasma (AIP). Results: Mean values of studied parameters were: PWVao-10.19±2.22m/s, AIXao - 39.17±16.59%, BP- 149.96±20.94/89.18±11.54, PP-60.99±17.95 mmHg, HR-73.75±10.89 bpm and SBP's.d -7.73±8.6 mmHg (24.9% of subjects with increased SBP variability), SUA-5.48±1.47 mg/dl and AIP - 0.38±0.34. Factors associated with both increased PWVao and AIXao values were: age group, visceral obesity, increased SUA and AIP levels, presence of at least 1 form of TOD and the lack of BP control. While DM, dislipidemias, increased SBP's variability, microalbuminuria and atrial fibrilation were associated only with increased PWVao, ischemic heart disease and stroke history was associated only with increased AIXao levels. Conclusions: Age above 60 years, visceral obesity, dislipidemia, increased SBP variability, SUA and AIP levels, the lack of optimal BP control and the presence of TOD may be considered as predictors of an increased arterial stiffness in hypertensive patients.
