Introduction: The prevalence of infection by different genotypes of human papillomavirus (HPV) varies among different geographic areas. The objective of the study is to determine the prevalence and distribution of HPV66 genotype among women with normal or abnormal Pap smear tests.

Amaç: Erken evre endometrioid tip endometrium kanserli olgularda uterus alt segment tutulumunun anjiyolenfatik invazyon, sağkalım ve lenf nodu tutulumu açısından prediktif değer olup olmadığının saptanması. Materyal ve Metod: Çalışma Gazi Üniversitesi Hastanesi Kadın Hastalıkları ve Doğum Kliniğinde 01 Ocak 2009 ile 15 Nisan 2017 tarihleri arasında Gazi Üniversitesi Tıp Fakültesi Jinekolojik Onkoloji Kliniğinde opere edilmiş (total abdominal histerektomi+bilateral salpingooferektomi±lenf nodu diseksiyonu) endometrioid tip endometrium kanserli evre 1a ve 1b olan 98 hasta ile retrospektif klinik çalışma formatında gerçekleştirildi. Endometrium kanseri nedeniyle opere olan (total abdominal histerektomi ve bilateral salpingo-ooferektomi (TAH+BSO) , bilateral pelvik lenf nodu disseksiyonu ± paraaortik lenf nodu disseksiyonu) Erken Evre Endometrioid Tip Endometrium Kanserli olgularda uterus alt segment tutulumunun anjiyolenfatik invazyon, sağkalım ve evreleme üzerine etkileri açısından prediktif değere sahip olup olmadıklarına bakıldı. Bulgular: Çalışmaya dahil edilen 98 hastanın 17(%17,3)‟si Evre 1a iken, 65 (%66,3)‟i Evre 1b, 7(%7,1)‟si Evre 3c1, 8(%8,2)‟i Evre 3c2 idi. Çalışmaya dahil edilen 98 hastanın 30(%30,6)‟unda sadece pelvik lenf nodu diseksiyonu, 59(%60,2)‟unda pelvik + paraaortik lenf nodu diseksiyonu yapılırken, sadece paraaortik lenf nodu diseksiyonu yapılan hasta yoktur. 76(%83,5) hastada lenf nodu metastazı saptanmazken, 7(%7,6) hastada sadece pelvik lenf nodu metastazı, 5(%5,4) hastada pelvik + paraaortik lenf nodu metastazı saptanmıştır. Çalışmaya dahil edilen 98 hastanın 42(%42,9)‟sinde myometrial invazyon derinliği ½‟den az, 56(%57,1)‟sında ½‟den fazla olarak saptanırken; myometrial invazyon olmayan hasta yoktu. Myometrial invazyon derinliği ½‟den fazla olan 56(%57,1) hastanın 25‟inde alt uterin segment tutulumu mevcuttu, myometrial invazyon derinliği ½‟den az olan 42(%42,9) hastada ise alt uterin segment tutulumu yoktu ve bu istatistiksel olarak anlamlı bulundu (p=0,030). Çalışmaya dahil edilen 98 hastanın 43(%43,8)‟ünde tümör çapı 4 cm‟den küçük, 55(%56,1)‟inde tümör çapı 4 cm‟den büyük olarak saptanmıştır. Tümör çapı 4 cm‟den büyük 55(%56,1) hastanın 38‟inde alt uterin segment tutulumu mevcuttu, tümör çapı 4 cm‟den küçük 43(%43,8) hastada ise alt uterin segment tutulumu yoktu ve bu istatistiksel olarak anlamlı bulundu (p=0,001). Ayrıca diğer prognostik faktörlerden olan; lenfovasküler alan invazyonu (p=0,852), tümör grade (p=0,392) ve yaş (p=0,543) ın alt uterin segment tutulumu ile ilişkisi olmadığı izlenmiştir. Sonuç: Endometrium kanseri nedeniyle lenf nodu diseksiyonu içeren tümör cerrahisi operasyonu olan hastaların patoloji sonuçları takiplerinde alt uterin segment tutulumu olan hastalarda myometrial invazyon derinliği (>1/2) istatistiksel olarak anlamlı bulunurken, tümör çapı büyüklüğü (>4cm) ve pelvik lenf nodu tutulumu da istatistiksel olarak anlamlı bulunmuştur. Ayrıca alt uterin segment tutulumu diğer prognostik faktörlerden LVAI, tümör grade ve yaŞ için belirleyici olarak izlenmedi. Aim: This study aims to determine the predictability of lower uterine segment to angiolymphatic invasion, survival and lymph node involvement for early stage endometrioid endometrial cancer. Materials and Methods: The study was performed retrospectively in Gazi University Hospital Obstetrics and Gynecology Clinic including 98 patients who were operated at Gazi University Medical Faculty Gynecological Oncology Clinic from january 2009 to april 2017 (total abdominal hysterectomy + bilateral salpingooferectomy ± lymph node dissection) and had diagnosed stage of 1a and stage 1b endometrioid type endometrial cancer. The purpose of this study to evalute the predictive value of lower segment involvement to survival, lymph node and angiolymphatic invasion in the cases which were opereted (total abdominal hysterectomy and bilateral salpingo-oophorectomy, bilateral pelvic lymph node dissection and paraaortic lymph node dissection) for early stage (stage 1a and 1b) endometrioid type endometrial cancer. Results: The study includes 98 patients with endometrioid type endometrial cancer. In the study 17(17.3%) patients were stage 1a, 65(66.3%) patients were stage 1b, 7(7.1%) patients were stage 3c1 and 8(8.2%) patients were stage 3c2. In the study we performed an operation to 30 patients (30.6%) containing only pelvic lymph node dissection, 59 (60.2%) patients pelvic + paraaortic lymph node dissection and we do not have any patients who were opereted including only paraaortic lymph node dissection. In 7 (7.6%) patients, only pelvic lymph node metastasis and in 5 (5.4%) patients, pelvic + paraaortic lymph node metastasis were detected. None of 76 (83.5%) patient had lymph node metastases. 42 (42.9%) patients of the study had less than ½ myometrial invasion depth and 56 (57.1%) had more than ½ and we did not detect any patients who has not myometrial invasion. We found 56 patients (57.1%) with a myometrial invasion depth greater than ½ and 25 of this patients had lower uterine segment involvement. 42 (42.9%) patients with less than ½ of the depth of myometrial invasion had no lower uterine segment involvement. The difference between the results was statistically significant (p = 0.030). In 43 patients (43.8%) the tumor diameter was less than 4 cm and 55 (56.1%) had tumor diameter greater than 4 cm. Lower uterine segment involvement was detected in 38 of 55 (56.1%) patients with a tumor diameter greater than 4 cm. There was no lower uterine segment involvement in 43 (43.8%) patients with a tumor diameter less than 4 cm. This was also statistically significant (p = 0.001). It was also observed that other prognostic factors; lymphovascular invasion (p = 0.852), tumor grade (p = 0.392) and age (p = 0.543) were not associated with lower uterine segment involvement. Conclusion: Myometrial invasion depth more than 50% was statistically significant as a prognostic factor for lower uterine segment involvement in follow-up pathology results including the patients who were operated with lymph node dissection due to endometrium carcinoma. The size of tumor diameter (> 4 cm) and pelvic lymph node invasion was also found to be statistically significant for lower uterine segment involvement. Furthermore, lower uterine segment involvement was not an independent prognostic factor for LVAI, tumor grade, and age.

Cesarean scar pregnancy is implantation of gestational sac in the previous cesarean scar. Its prevelance increased in last decade. A 31 year old woman, had two previous cesarean section, admitted to the emergency service with vaginal bleeding. A gestational sac which was implanted in the previous uterine incision was detected in ultrasonography. In literature treatment options such as minimal invasive surgery, and/or methotrexate injection, uterin artery embolization can be preferred.

Endometrium kanseri Gelişmiş ülkelerde kadın üreme sisteminin en sık görülen malignitesidir ve meme, akciğer ve kolorektal kanserden sonra kadınlarda en sık görülen dördüncü kanserdir. Endometrial biyopsi ya da fraksiyone küretaj sonuçları pozitif olan hastalarda tedavi cerrahi evreleme ile başlar. Cerrahi sırasında evreleme için kilit basamak uterin spesimenin açılarak invazyon derinliğinin gross incelenmesidir. İntraoperatif frozen inceleme gross incelemeye yardımcı ya da alternatif olarak kullanıma girmiş bir metoddur ancak literatürde frozen inceleme ile nihai patoloji sonucunu karşılaştıran çalışmalarda,özellikle erken evre ve düşük histolojik grade gruplarında tutarlılık yönünden çelişkili sonuçlar alınmıştır. Bu çalışmada kliniğimizde endometrium kanseri nedeniyle opere edilen hastaların sonuçları karşılaştırılarak preoperatif ve intraoperatif incelemeler ile nihai inceleme arasındaki tutarlılık düzeyinin ortaya konması amaçlanmıştır. Gazi Üniversitesi Tıp Fakültesi Kadın Hastalıkları ve Doğum anabilim dalında Ocak 2010 Nisan 2015 tarihleri arasında histerektomi ameliyatı yapılan hastalardan patoloji sonucu endometrium kanseri olarak raporlanan tüm hastalar hastane bilgi işletim sistemi ENLIL® üzerinden tarandı. Çalışmaya endometrial örneklemesi de Gazi Üniversitesi Tıp Fakültesi Patoloji Anabilim Dalı tarafından değerlendirilmiş olan 134 hasta dahil edildi. Preoperatif endometrial örnekleme sonuçları ve intraoperatif frozen inceleme sonuçları nihai patoloji sonuçlarıyla karşılaştırıldı. Endometrial örnekleme ile nihai patolojik sonuçlar arasında histopatolojik tanı yönünden %76,1, patolojik derece yönünden %66 oranında tutarlılık tespit edildi. Frozen inceleme sonuçları nihai patolojik sonuçlarla karşılaştırıldığında tutarlılık düzeyi histopatolojik tanıda %68,5, myometrial invazyon boyutunda %61,9 servikal uzanımda ise %93 olarak bulundu. Mevcut bulgular göz önüne alındığında frozen incelemenin hastalığın evresini öngörmede ve dolayısıyla cerrahi planlamasında etkinliğinin ve güvenilirliğinin düşük olduğu görülmektedir. Bu durumda endometrium kanseri nedeniyle operasyona alınacak hastaların, eğer uzun süreli operasyonu tolere edemeyecek mevcut bir komorbiditeleri yoksa, lenf nodu disseksiyonunu da içeren tam cerrahi evrelemeden geçirilmelerini önermekteyiz. Endometrial cancer is the most common malignancy of the female reproductive system in developed countries and the fourth most common cancer in women following breast, lung and colorectal cancers. The treatment begins with surgical staging for patients with a positive endometrial sampling result. During surgery the most crucial step is the opening of the uterin specimen in the operating room and gross examination of the deepest myometrial invasion. Intraoperative frozen section is used to assist or as an alternative to the gross examination. However controversial results have been reported in studies that evaluate the consistency of the frozen section and its comparison to the final pathologic resuls. In this study, it is our purpose to compare the preoperative, intraoperative and postoperative results of endometrial cancer patients and evaluate the results for consistency. Data of patients who underwent an hysterectomy and had a pathology result of endometrial cancer between January 2010 and April 2015 in Gazi University School of Medicine, Department of Obstetrics and Gynecology were obtained from ENLIL® hospital information management system. 134 patients who also had an endometrial sampling procedure in our clinic were included. Preoperative endometrial sampling and intraoperative frozen section results were compared to the final pathology results. Endometrial sampling results were 76,1%consistent with the final results for histopathologic diagnosis and 66%for histologic grade. Frozen section results were 68,5%consistent for histopathologic diagnosis, 61,9%for depth of myometrial invasion and 93%for servical involvement. In light of this recent information, it is apparent that frozen section is inefficient and unreliable in predicting the surgical stage of endometrial cancer. Therefore we suggest standard surgical staging including lymph node dissection for patients with endometrial cancer, unless the patient has any comorbidities that would make the complete staging impossible.

Over kanseri kadın genital sistemi kanserleri arasında en sık ölümle sonuçlanan kanser tipidir. Over kanserinin prognozu erken evrede iyi iken ileri evrelerde 5 yıllık sağ kalım oldukça düşüktür. Over kanseri ayırıcı tanısında kullanılan CA 125 başta endometriozis olmak üzere birçok benign ve malign hastalıkta yükselmektedir. CA 125 over kanserine özgül bir tümör belirteci olmadığından dolayı ayırıcı tanıda rolü sınırlıdır. Bu durum HE 4 gibi yeni tümör belirteçlerinin araştırılmasına yol açmaktadır. Bu çalışmanın amacı; over kitlesi saptanıp opere edilen hastalarda görüntüleme skoru, serum CA 125 düzeyi ve serum HE 4 düzeyinin over kanseri ayırıcı tanısındaki performansını karşılaştırmaktır. Çalışmamıza over kitlesi saptanıp Gazi Üniversitesi Kadın Hastalıkları ve Doğum bölümünde opere edilen 85 hasta dahil edilmiştir. Hastaların operasyon öncesi görüntüleme skorları, CA 125 düzeyi, HE 4 düzeyleri ve operasyon sonrası patoloji sonuçları prospektif olarak değerlendirilmiştir. Sonuç olarak over kanserini tanısında HE 4 düzeyi, CA 125 düzeyi ve görüntüleme skoruna göre daha duyarlı ve daha özgül bulunmuştur. HE 4 düzeyinin CA 125?in aksine endometriolarda yükselmediği görülmüştür. Ovarian cancer is the most common cause of the gynecologic cancer death. The prognosis of early stage ovarian cancer is favorable. On the other hand five year survival of advanced stage ovarian cancer is extremely low. CA 125 is commonly used as a tumour marker for patients suspicious of ovarian cancer. High CA 125 levels can be found in lots of benign and malign conditions like endometriosis. Due to low specifity of CA 125 for ovarian cancer, the convenience in differential diagnosis is restricted. The aim of this study is to compare effectiveness of imaging score, serum CA 125 and HE 4 levels in differential diagnosis of pelvic mass. This study include 85 patients who were diagnosed with pelvic mass and operated in our institution. Preoperative serum CA 125 and HE 4 levels, imaging scores and pathology results were evaluated prospectively for all patients. In conclusion, serum HE 4 levels found more sensitive and specific than serum CA 125 levels and imaging score in differential diagnosis of ovarian cancer. It was shown that Serum HE 4 levels rarely increases in endometriomas in contrast with CA 125
