1. Real-Time Monitoring System to Manage Mental Healthcare Emergency Unit

Abstract: Objectives Real-time relevant information helps guide the healthcare decision-making process in daily clinical practice as well as the management and optimization of healthcare processes. However, proprietary business intelligence suite solutions supporting the production of decision-making information requires investment that is out of reach of small and medium-sized healthcare facilities or those with limited resources, particularly in developing countries. This paper describes our experience in designing and implementing a real-time healthcare monitoring system solution to manage healthcare emergency units. Methods Through the use of free Business Intelligence tools and Python data science language we designed a real-time monitoring system, which was implemented to explore the Electronic Medical Records system of a university mental health emergency unit and render an electronic dashboard to support health professional daily practice. Results Three main dashboards were created to monitor patient waiting time, to access the clinical notes summary for the next waiting patient, and to obtain insights into activity during the last 24 hours. Conclusions The designed system could serve as a monitoring support model using free and user-friendly data science tools, which are good alternatives to proprietary business intelligence solutions and drastically reduce cost. Still, the key to success in decision-making systems is based on investment in human resources, business intelligence skills training, the organizational aspect of the decision-making process, and data production quality insurance.

Abstract: Objectives Monitoring healthcare activities is the first step for health stakeholders and health professionals to improve the quality and performance of healthcare services. However, monitoring remains a challenge for healthcare facilities, especially in developing countries. Fortunately, advances in business analytics address this need. This paper aims to describe the experience of a low-income healthcare facility in a developing country in using business analytics descriptive techniques and to discuss business analytics implementation challenges and opportunities in such an environment. Methods Business analytics descriptive techniques were applied on 3 years’ electronic medical records of outpatient consultation of the University Psychiatric Centre (CPU) of Casablanca. Statistical analysis was conducted to compare results over years. Results Over the 3 monitored years, the monthly number of computerized physician order entries increased significantly (p < 0.001). Physicians improved their personal recording over years. Schizophrenia as well as depressive and bipolar disorders were noted at the top of outpatient mental disorders. Antipsychotics are the most prescribed drugs, and a significant annual decrease in outpatient care wait time was noted (p < 0.001). Conclusions Business analytics allowed CPU to monitor mental healthcare outpatient activity and to adopt its business processes according to outcomes. However, challenges mainly in the organizational dimension of the decision-making process and the definition of strategic key metrics, data structuration, and the quality of data entry had to be considered for the optimal use of business analytics.

3. Dermatologists and burnout: Myth or reality?

Abstract: Background: Although dermatologists are known to have a good level of job satisfaction, they no longer rank among the happiest healthcare professionals. Our study aimed to estimate the prevalence of burnout among dermatologists and define its risk factors and methods of prevention. Materials and Methods: This was a multicenter, cross-sectional study conducted in Morocco over five months from February to June 2022. Results: A total of 313 responded to the survey, among which 70.95% suffered from burnout. The risk factors associated with burnout were the female sex (p < 0.001), being married (p = 0.025), having children (p = 0.001), full-time work (p = 0.05), lack of time for research (p < 0.001), excessive documentation (p < 0.001), confrontation with aggressive patients (p < 0.001) as well as insufficient remuneration at work (p < 0.001). Conclusion: Burnout among dermatologists is pervasive and costly for professional life. Institutions need to provide stress management interventions, and dermatologists must balance patient care with other interests such as research and teaching. [ABSTRACT FROM AUTHOR]

4. Seclusion in a Moroccan Adult Psychiatry Unit

Abstract: Despite significant advances in the pharmacological treatment of mental disorders, seclusion is still used in daily practice for patients hospitalized in Moroccan psychiatric institutions. Staff generally prefers to avoid using these procedures, as they limit the patient's freedom and undermine his or her dignity. However, it is sometimes necessary to do so to manage extreme episodes of dangerous behavior that threatens the safety of the patient and those around them. We aim in this study to identify the factors that impact the average duration of seclusion and to deduce, if possible, recommendations and measures to optimize or even reduce this time. This is a one-year retrospective study, conducted in the psychiatric department of IBN ROCHD University hospital in Casablanca-Morocco. The study includes data from 169 patients admitted during the entire year of 2018. Multivariate logistic regression analysis was employed to identify factors associated with the duration of seclusion. Based on the results of our study, it appears that the only socio- demographic factor that has a significant impact on the duration of the isolation is marital status. Also, for patients with a personal psychiatric history, the isolation was shorter. Furthermore, it was observed that patients who self-harmed resided longer in seclusion rooms. Regarding the medical diagnosis, it seems that schizoaffective disorder is the only diagnosis that leads to a longer period of isolation than others. Post-seclusion observations indicate that patients whose condition has deteriorated are placed in an extended placement. Although seclusion is not part of the patient's standard treatment, such an intervention may be implemented as an emergency treatment for patients exhibiting behavior that is dangerous to themselves or others. In addition, it would also be necessary to consider Morocco’s reality with its cultural aspects but also to deal with the lack of human and material resources from which our health care system suffers

5. Prevalence of Substance Use Among Students in The Faculty of Medicine and Pharmacy of Casablanca, Morocco

Abstract: Introduction: Substance use is a major public health problem. Medical students are an interesting population category to study, given the particularity of the stress factors to which they are exposed, predisposing them to the use of psychoactive substances. The objective of this study is to determine the prevalence of psychoactive substance use in this category, as well as the levels of dependence and risky use, and also to define the characteristics of use and the relationship between substance use and sociodemographic characteristics. Methods: Cross-sectional study with a descriptive aim, carried out in the form of a survey using a questionnaire, among 456 medical students in Casablanca, all levels combined. Results: The most widespread psychoactive substances are tobacco: 16.6%, chicha: 15.3%, cannabis: 14.9% and alcohol: 13.1%. The highest rate of risky use was noted with tobacco use: 30%, and cannabis use: 80%. More than half of the users had a family background of use, and one third of the users were repeaters. The age of initiation, for all substances, varied between 10 and 29 years. In our study, 18.6% of the students were poly-substance users. Psychiatric background was found in 42% of users vs 17% of non-users. Conclusion: The extent of psychoactive substance use among students is considerable, and the study has made it possible to identify certain socio-demographic risk factors.

6. L'évaluation du syndrome du Burnout chez les médecins en formation au CHU Ibn Rochd de Casablanca

Abstract: INTRODUCTION: Le Burnout est très répandu en milieu hospitalier et sa gravité est encore plus accrue chez les jeunes médecins. L'objectif est de déterminer la prévalence du burnout parmi les médecins en formation (internes et résidents) au CHU Ibn Rochd de Casablanca et chercher les facteurs associés. METHODES: étude transversale descriptive menée en 2013- 2014 auprès d'un échantillon de 300 médecins à l'aide d'un questionnaire auto-administré; le burnout était évalué par la version française du Maslash Burnout Inventory (MBI). RESULTATS: un total de 191 médecins a participé à l'étude (taux de réponse à 63,7%) avec une prédominance féminine à 79,1% et une moyenne d'âge de 26,7 ans (ET = 3). Les scores moyens des sous-dimensions du MBI: l'épuisement émotionnel, la dépersonnalisation et l'accomplissement personnel étaient respectivement de (33,7, 10,7), (12,2 , 6,5) et (30,6 , 8,3). Le burnout sévère touchait 31,8% des participants. Il était associé aux problèmes de communication au sein de l'équipe soignante (p = 0,01), l'accompagnement insuffisant (p = 0,05), l'insatisfaction des séniors (p = 0,01), la crainte de faire des erreurs médicales (p = 0,05), le recours à un psychothérapeute (p = 0,001), la consommation de psychotropes (p = 0,001), les troubles anxieux (p = 0,01), la dépression (p = 0,01) et les idées suicidaires (p = 0,05). Les facteurs protecteurs étaient représentés par: le sentiment d'équité au sein de l'équipe soignante (p = 0,01) et la pratique de loisirs (p = 0,05). Le changement d'orientation de carrière était associé au burnout sévère (p = 0,05). CONCLUSION: ces résultats rejoignent ceux des études précédentes et justifient l'intérêt d'un programme de prévention à différents niveaux.

8. Evaluation of Psychometric Properties of the Arabic Version of PSS Stress Measuring Scale in the Moroccan Population

Abstract: This study was conducted to examine the psychometric properties of the classic Arabic version of the 10-item Perceived Stress Scale (PSS10) in a large sample of Moroccan population. A cross-sectional survey was yielded between December 2013 and January 2014 following an accidental sampling. Individuals aged above 18, from different socioeconomic categories, were invited to take part in the study. The participants had completed a questionnaire including the classic Arabic versions of PSS10. Two data collection methods involving the paper questionnaire (PQ) and an electronic questionnaire (EQ) were solicited. The psychometric properties of PSS10 were examined by internal consistency, test–retest reliability, convergent validity, and factorial structure. A total of 1,099 individuals have participated in the study. Internal consistencies of the classic Arabic PSS10 for both of PQ and EQ were respectively α = 0.72 and α = 0.85; the test–retest reliability score was 0.91. In both of PQ and EQ, PSS10 scores were higher among females ( p < .01), and were positively and significantly correlated with the DASS21 subscales. The principal component analysis provided a two-factor structure. The percentages of variance explained by the two factors were 55.58 for EQ and 54.61 for PQ. The classic Arabic version of the PSS10 showed satisfactory psychometric properties in both data collection methods.

9. Prevalence of stress in Casablanca medical students: a cross-sectional study

Abstract: INTRODUCTION: recently, an important literature data has reported that medical students experience stress more than students in other disciplines. In contrast, there is a significant shortage of the stress impact on the academic performance. The primary purpose of our study was to determine the prevalence of stress among Casablanca Medical students and to investigate if there is an association between stress and academic skills. METHODS: a total of 275 participants studying at Casablanca Medical School were included. The study was conducted using a self-administered, anonymous questionnaire, which included four subscales on academic skills perception (Academic competence, Test competence, Time management and Strategic study habits) and a Test Anxiety scale to assess the degree of stress related to exams among medical students. RESULTS: the overall findings showed that 52.7% of respondents were stressed by examinations, and the highest stress prevalence was among the fifth-year medical students. Measures of comparative stress degrees between male and female students did not show any statistical significant differences (p=0.34). Correlation analysis revealed negative association between stress and academic competence (-0.394), test competence (-0.426), time management (-0.240), strategic study (-0.183) respectively (p=0.001). CONCLUSION: medical educators and psychologists have to increase clinical awareness of stress among medical students, by establishing strategies for stress management.

Méthodes:nous avons mené une étude transversale descriptive et analytique au niveau du Centre Hospitalier Universitaire Ibn Rochd de Casablanca et à la faculté de médecine et de pharmacie de Casablanca Maroc, durant l´année universitaire 2016-2017. L´enquête a porté sur un échantillon de 506 étudiants. Nous avons utilisé le questionnaire SCOFF-F(Sick, Control, One stone, Fat, Food; version française), un outil validé pour le dépistage des troubles du comportement alimentaire et l´échelle HAD(Hospital anxiety and depression)pour dépister les troubles anxieux et dépressifs. Résultats:le questionnaire SCOFF a détecté 127 personnes (soit 25,09%) sur 506 susceptibles de souffrir de troubles du comportement alimentaire. L´âge, le niveau d´études, l´utilisation des moyens de contrôle du poids, l´anxiété et la dépression sont des facteurs associés de façon significative au risque de présenter un trouble du comportement alimentaire chez les étudiants. Conclusion:nos résultats sont en accord avec les données de la littérature concernant la fréquence des troubles du comportement alimentaire en population étudiante et plus spécifiquement les étudiants en médecine. Mener d´autres études sur une population plus variée pourraient également être envisagées afin d´avoir une vision plus globale de la situation au Maroc.

Abstract: Resume Introduction Depuis l’avenement de la therapie antiretrovirale et l’allongement de l’esperance de vie des patients, la sante mentale est devenue une preoccupation pour les patients vivant avec le VIH (PvVIH) et pour les soignants. Objectif de notre travail : estimer la prevalence des troubles psychiatriques, etudier la nature de ces troubles et identifier les facteurs de risques sociodemographiques et medicaux influencant la sante mentale des PvVIH. Patients et methodes Etude transversale descriptive menee dans le service des Maladies Infectieuses de janvier 2015 an 2015 portant sur les PvVIH, vus a l’hopital de jour et ayant presente des troubles psychiatriques. Les donnees ont ete recueillies sur les fiches d’exploitation et analysees sur Excel. Resultats La prevalence des troubles est de 3,39 %. La moyenne d’âge est de 39,28 ans (24–58 ans). Le sexe feminin predomine (sex-ratio : 0,62). La plupart des patients sont analphabetes ou ayant un niveau d’etudes primaires (63 %), ont un travail (67 %). Douze patients (17 %) ont un antecedent psychiatrique et 42 patients (61 %) ont presente des troubles au cours du suivi. Les troubles psychiatriques sont domines par la depression (87 %) et l’anxiete (29 %). Les sources presumees des troubles sont la precarite materielle (84 %), le divorce (30 %) et la perte du travail (29 %). Le mode de contamination est essentiellement les rapports heterosexuels (87 %). La duree mediane d’infection VIH est de trois ans (un mois-18 ans). La majorite des patients (77 %) sont sous therapie antiretrovirale dont 64 % ont un taux des lymphocytes CD4 Conclusion Les manifestations psychiatriques sont frequentes au cours de l’infection a VIH. Elles sont dominees par la depression et sont responsables de l’echec therapeutique, d’ou la necessite de la collaboration entre psychiatre et infectiologue dans la prise en charge des PvVIH.

Abstract: BACKGROUND Monitoring health care activities is the first step for health stakeholders and health professionals to improve quality and performance of health care services. However, monitoring remains a challenge for health care facilities especially in developing countries. Fortunately, advances in business analytics address this need. OBJECTIVE To demonstrate the use of business analytics descriptive techniques on electronic medical records and to describe business analytics adoption challenges in the University Psychiatric Centre (CPU) of Casablanca, Morocco. METHODS Business analytics descriptive techniques were applied on three years electronic medical records of CPU outpatient consultation. Monitored mental health key metrics concern quantitative assessment of acts, mental health disorder’s frequency, psychiatric drugs prescription, health professional’s performance, patient geolocation and outpatient care wait time. Three annual feedback meetings based on business analytics reports took place. RESULTS Over the three monitored years, monthly Medical Record Order Entries (MROE) increased significantly. Physicians improved their personal recording. Schizophrenia, depression and bipolar disorders were noted at the top of diagnosis. Antipsychotics are the most prescribed drugs and an average decrease of 12 minutes per year in outpatient care wait time was noted. CONCLUSIONS Business analytics allowed CPU to monitor mental healthcare outpatient activity and to make informed decisions. Yet, challenges should be addressed to step forward in order to become a data-driven health care facility mainly in the organizational dimension of the decision-making process and in the definition of key metrics in response to strategic needs.

Abstract: Resume Bien que l’association du lithium avec les neuroleptiques soit une pratique courante en psychiatrie, il y a eu aussi un debat considerable quant a sa securite chez les patients. Nous rapportons ici le cas d’une jeune femme de 30 ans hospitalisee pour son quatrieme acces maniaque et qui apres stabilisation avait developpe brutalement, un mois apres son hospitalisation, un syndrome malin des neuroleptiques (SMN) associe a une intoxication au lithium. La prise en charge etait multidisciplinaire, l’evolution lente, et la patiente a garde un deficit neurologique residuel et permanent des membres inferieurs quatre mois apres resolution de l’episode. Abstract: International audience; There is consistent evidence that impulsivity is linked to tobacco consumption and to symptomatology in schizophrenia. In the current study, we propose a new integrative model of the relationship between impulsivity, psychopathological symptoms, and tobacco status in patients with paranoid schizophrenia. We investigated 33 paranoid schizophrenia patients and 37 healthy controls using a battery of psychopathological scales included the Structured Clinical Interview forDSM-IV(SCID), the five-factor model of the Positive and Negative Symptom Scale (PANSS), the Barratt Impulsiveness Scale (BIS-10), and the UPPS Impulsive Behavior scale (UPPS). Step-wise regression analysis revealed that positive factor of the PANSS and tobacco status contributed positively and significantly to the explained variance of impulsivity. In addition, non-planning impulsivity and sensation-seeking emerged as significant predictors of tobacco status, while smoking predicted non-planning impulsivity and sensation-seeking. Our results suggest that the relationship between sensation-seeking and tobacco use is reciprocal and proposes a new integrative model of the relationship between impulsivity, positive symptoms and tobacco status in patients with paranoid schizophrenia. However, the exact mechanisms for these relationships deserve further investigation.

Abstract: Lack of insight into illness is a prevalent and distinguishing feature of schizophrenia, which has a complex history and has been given a variety of definitions. Currently, insight is measured and treated as a multidimensional phenomenon, because it is believed to result from psychological, neuropsychological and organic factors. Thus, schizophrenia patients may display dramatic disorders including demoralization, depression and a higher risk of suicide, all of which are directly or indirectly related to a lack of insight into their illness, and make the treatment difficult. To improve the treatment of people with schizophrenia, it is thus crucial to advance research on insight into their illness. Insight is studied in a variety of ways. Studies may focus on the relationship between insight and psychopathology, may view behavioral outcomes or look discretely at the cognitive dysfunction versus anatomy level of insight. All have merit but they are dispersed across a wide body of literature and rarely are the findings integrated and synthesized in a meaningful way. The aim of this study was to synthesize findings across the large body of literature dealing with insight, to highlight its multidimensional nature, measurement, neuropsychology and social impact in schizophrenia. The extensive literature on the cognitive consequences of lack of insight and the contribution of neuroimaging techniques to elucidating neurological etiology of insight deficits, is also reviewed.

Abstract: The molecular epidemiology of stroke is critically lacking in the developing world. We explored the relationships between genetics polymorphism and risk for ischemic stroke among the residents of Casablanca, Morocco. Ninety-one stroke patients matched 1:2 for their age, gender, and ethnic background to 182 healthy controls who were genotyped for the prothrombin G20210A mutation and factor V (FV) Leiden and were assessed for conventional risk factors for stroke. No significant association was found between prothrombin gene mutation with stroke (p = .054). Regarding stroke subtypes, significant relationships between patients with a large artery disease subtype of stroke and this mutation was found compared to controls (p = .046). As a genetic risk factor to develop this event, a strong association was observed when adjusted for conventional vascular risk factors (adjusted OR, 4.3; p = .029). No FV Leiden was found. We suggest that prothrombin mutation but not FV Leiden should be considered as a modest genetic risk factor for large artery disease stroke subtype in the Moroccan population.

Abstract: Purpose The cardiovascular mortality of the psychiatric patients is higher than the general population. The antipsychotics and their propriety of prolongation of the QT interval seems to be a possible track. We tried to analyze the epidemiological, clinical, biological, therapeutic and especially electrocardiographic (ECG) data in those patients, to determine the prevalence of the syndrome of prolongation of the QT interval and investigate its main risk factors. Methods We conducted a cross-sectional study, at the university psychiatric center Ibn Rochd of Casablanca, in collaboration with the cardiology department, over one year, concerning 134 patients receiving an antipsychotic treatment, we realized12 derivations ECG, and assessed clinical, biological and pharmacological data. Results The mean age was 32, 48±10,9 year,82,8% men/17,2% women, the first psychiatric disorder was the paranoid schizophrenia 56%, hyperkalemia was present in1 patient, hyermagnesaemia (1), calcemia was normal, polytherapy has been found in the majority of cases(2.8±0.9), neuroleptics were prescribed for all patients(45.5% for long term), in association with benzodiazepines (25%) and anticholinergics (23%), antidepressant (21%) and lithium (10%).The mean dose antipsychotic chlorpromazine equivalent (CPZ eq) is 617.8±534, 3mg/d, 88% of patients are under moderate doses and only 6% receive very high doses >2000mg/d CPZ eq. the mean heart rate was 82.7±16.5 BPM, one patient had a bradycardia to 44 bpm. The mean QTc is 386.22±30,9ms, the prevalence of long QT syndrome (>440ms) in our series is 4.5% so lower than the literature. The mean value of QTc increases with age, and it is higher in female gender, the risk of QT prolongation is associated with multiple co-prescribed drugs, and gets higher with the number of prescribed drugs and with strong dosage. The risk increases also at the beginning of the treatment and in patients with long term treatment >6 months. Conclusion However, despite the proven toxicity of these treatments, it must be remembered that the QT prolongation does not necessarily induce a negative risk-benefit balance. This is to be assessed on a case-by-case, and in cooperation between psychiatrists, cardiologists, pharmacists, for a better control of the risk of sudden cardiac death from a psychiatric medication origin.

Le deuxième objectif est d'étudier les relations entre les performances au test de classification catégorielle de cartes du Wisconsin (WCST) et la durée et l'intensité des symptômes cliniques, ainsi que les différentes dimensions cliniques. Méthode: Quatre-vingt dix sujets ont participé à cette étude, répartis en 3 groupes de 30: un groupe de patients souffrant de schizophrénie, un groupe formé de leurs frères et sœurs, et un groupe témoin. L'intensité des symptômes a été évaluée par l'échelle des symptômes positifs et négatifs (PANSS), et le fonctionnement social par l'échelle globale du fonctionnement (EGF). Le WCST a été administré aux 3 groupes. Résultats: Les patients souffrant de schizophrénie et leurs frères et soeurs présentaient des performances significativement plus faibles au WCST que celles des témoins. L'analyse statistique a montré que le groupe des patients présentait un déficit des performances au WCST significativement plus important que les 2 autres groupes. Les frères et sœurs présentaient également des performances significativement plus faibles que celles des témoins. D'autre part, aucune relation significative n'a été trouvée entre la performance au WCST et les autres variables, notamment l'âge, le sexe, le niveau d'instruction, la durée de la maladie, le traitement, et les différents scores PANSS et EGF. Conclusion: Les patients souffrant de schizophrénie et leurs frères et sœurs non malades partagent les mêmes déficits des fonctions exécutives. Ces résultats suggèrent que les performances au WCST peuvent être considérées comme un marqueur de vulnérabilité à la schizophrénie chez les frères et sœurs de patients atteints de schizophrénie.

L’objectif de l’etude etait d’evaluer la denomination des objets et des actions chez des patients schizophrenes traites au Centre psychiatrique universitaire Ibn Rochd de Casablanca. Methodes Nous avons mene en 2012 une enquete cas-temoins, aupres d’un echantillon de patients diagnostiques comme schizophrenes selon les criteres du DSM-IV-TR. Les temoins ont ete recrutes parmi les patients vus en consultation de rhumatologie. La denomination des objets et des actions s’est faite a l’aide de 148 images noir et blanc. Un autre test avait consiste a nommer des legumes ou des mots commencants par la lettre M durant une minute. La comparaison du nombre de reponses correctes etait realisee par le test de Wilcoxon. Resultats Au total, 20 patients et 20 temoins ont participe a l’etude. La moyenne d’âge des patients etait 35,6 ans (ET = 10,3) et celle des temoins etait de 38,1 ans (ET = 10,5), 70 % des cas et 50 % des temoins etaient de sexe masculin. Les patients schizophrenes avaient reussi a denommer moins d’images comparativement aux temoins (72,3 versus 74, p = 0,003). Le nombre de legumes ainsi que le nombre de mots commencant par la lettre M cites etait egalement plus faible chez les cas compares aux temoins (9,8 versus 12,4, p = 0,005 et 8,0 versus 13,0, p Conclusion Cette etude montre un deficit de denomination des objets et des actions chez les schizophrenes.

Abstract: Introduction Le Burnout est très répandu en milieu hospitalier et sa gravité est encore plus accrue chez les jeunes médecins. L'objectif est de déterminer la prévalence du burnout parmi les médecins en formation (internes et résidents) au CHU Ibn Rochd de Casablanca et chercher les facteurs associés. Méthodes Étude transversale descriptive menée en 2013- 2014 auprès d'un échantillon de 300 médecins à l'aide d'un questionnaire auto-administré; le burnout était évalué par la version française du Maslash Burnout Inventory (MBI). Résultats Un total de191 médecins a participé à l'étude (taux de réponse à 63,7%) avec une prédominance féminine à 79,1% et une moyenne d'âge de 26,7ans (ET = 3). Les scores moyens des sous-dimensions du MBI: l'épuisement émotionnel, la dépersonnalisation et l'accomplissement personnel étaient respectivement de (33,7 ± 10,7), (12,2 ± 6,5) et (30,6 ± 8,3). Le burnout sévère touchait 31,8% des participants. Il était associé aux problèmes de communication au sein de l'équipe soignante (p < 0,01), l'accompagnement insuffisant (p < 0,05), l'insatisfaction des séniors (p = 0,01), la crainte de faire des erreurs médicales (p < 0,05), le recours à un psychothérapeute (p < 0,001), la consommation de psychotropes (p = 0,001), les troubles anxieux (p < 0,01), la dépression (p < 0,01) et les idées suicidaires (p < 0,05). Les facteurs protecteurs étaient représentés par: le sentiment d'équité au sein de l'équipe soignante (p < 0,01) et la pratique de loisirs (p < 0,05). Le changement d'orientation de carrière était associé au burnout sévère (p < 0,05). Conclusion Ces résultats rejoignent ceux des études précédentes et justifient l'intérêt d'un programme de prévention à différents niveaux. Morocco as an African and a developing country has always had a mental health policy defined by several glitches and failures, which had not helped him improve its mental health services quality. Nevertheless, huge improvements were achieved through time.ObjectivesThis report, aims to draw attention on how compulsory it is to think and act all together to promote mental health and provide patients with better health services in Morocco.MethodsThe National Human Rights Council conducted an information and investigation mission in Morocco's main mental health hospitals and facilities between March 27 and July 6, 2012.ResultsStructures are insufficient and inadequate in terms of geographical distribution, architecture and equipment. There is a big shortage of medical and paramedical staff and little interest is given to vulnerable groups. Nevertheless, huge improvements have also been achieved through time with mental health issues becoming a cornerstone of the ministerial program, the involvement of the NGOs, the construction of newer facilities, the implementation of an information gathering system and the presence of a substance use policy.ConclusionPsychiatry in Morocco has come a long way since it was firstly implemented in the country as a medical specialty. Undoubtedly, a lot has been done but much more remains to be achieved. A cross-sectional survey was yielded between December 2013 and January 2014 following an accidental sampling. Individuals aged above 18, from different socioeconomic categories, were invited to take part in the study. The participants had completed a questionnaire including the classic Arabic versions of PSS10. Two data collection methods involving the paper questionnaire (PQ) and an electronic questionnaire (EQ) were solicited. The psychometric properties of PSS10 were examined by internal consistency, test–retest reliability, convergent validity, and factorial structure. A total of 1,099 individuals have participated in the study. Internal consistencies of the classic Arabic PSS10 for both of PQ and EQ were respectively α = 0.72 and α = 0.85; the test–retest reliability score was 0.91. In both of PQ and EQ, PSS10 scores were higher among females ( p < .01), and were positively and significantly correlated with the DASS21 subscales. The principal component analysis provided a two-factor structure. The percentages of variance explained by the two factors were 55.58 for EQ and 54.61 for PQ. 28. La somnipathie chez les lycéens de Casablanca, Maroc

Abstract: Introduction Le trouble du sommeil ou la somnipathie est un dysfonctionnement des habitudes du sommeil pouvant nuire au bon fonctionnement physique, mental et emotionnel de l’individu. En effet, ce trouble medical, considere reellement serieux, se repercute negativement sur la sante de la personne sous forme de fatigue chronique, de troubles de l’attention, de la memoire, de la concentration et/ou de la vigilance. L’objectif de notre l’etude est de decrire les troubles du sommeil chez les lyceens a Casablanca et d’etudier les facteurs associes a ces troubles. Materiels et methodes Une etude transversale avait ete menee sur un echantillon de lyceens de Casablanca au cours de l’annee 2006. Un questionnaire auto-administre avait ete distribue a ces eleves evaluant les troubles du sommeil, les conditions de vie et les habitudes toxiques. Une analyse des associations entre les differents facteurs et les troubles du sommeil avaient ete etudiees par le test de Khi 2 . Resultats Au total, 2484 lyceens ont participe a l’etude avec un âge moyen de 17 ans (ET = 1,42) ; presque un sur deux des participants etaient de sexe masculin (53,2 %). La moyenne d’heures de sommeil etait de 7,86 (ET = 1,78). Environ un tiers des lyceens avait un sommeil difficile (31 %) et fatiguant (37 %), et un lyceen sur cinq se plaignait de reveil precoce (20 %) et de reve repete (20 %). Des associations statistiquement significatives avaient ete revelees entre les troubles du sommeil, le retentissement sur la vie social et scolaire, et les habitudes toxiques (consommation de tabac, alcool et psychotropes). Discussion et conclusion Ce travail avait montre une frequence elevee des troubles de sommeil chez les adolescents, ce qui suggere de mettre en place des cellules d’ecoute au sein des etablissements scolaires afin d’identifier rapidement ces troubles et d’assurer une prise en charge adequate.

Abstract: The predisposition to stroke might involve interactive effects among variants in several genes. We tested this hypothesis by examining the influence of polymorphisms in methylenetetrahydrofolate reductase (MTHFR) (C677T) and prothrombin (F2) (G20210A) as risk factors for stroke in Morocco. The polymerase chain reaction-restriction fragment length polymorphism methods were used to analyze DNA from 91 stroke patients and 182 controls. Association between the two polymorphisms and the risk of stroke was estimated by four-level models for the analysis of genetic interaction. Neither the MTHFR 677TT nor the F2 20210GA genotype showed any significant association compared to the MTHFR CC and F2 GG genotypes, respectively. An interactive effect between the MTHFR 677TT and F2 20210GA polymorphisms showed an increased risk of stroke. The odds ratios, in univariate and multivariate analysis, for the combined polymorphisms were 4.99 (95% CI, 1.75–14.2, P = 0.001) and 5.29 (95% CI, 1.63–17.1, P = 0.005), respectively.

Abstract: Lack of insight into illness is a prevalent and distinguishing feature of schizophrenia, which has a complex history and has been given a variety of definitions. Currently, insight is measured and treated as a multidimensional phenomenon, because it is believed to result from psychological, neuropsychological and organic factors. Thus, schizophrenia patients may display dramatic disorders including demoralization, depression and a higher risk of suicide, all of which are directly or indirectly related to a lack of insight into their illness, and make the treatment difficult. To improve the treatment of people with schizophrenia, it is thus crucial to advance research on insight into their illness. Insight is studied in a variety of ways. Studies may focus on the relationship between insight and psychopathology, may view behavioral outcomes or look discretely at the cognitive dysfunction versus anatomy level of insight. All have merit but they are dispersed across a wide body of literature and rarely are the findings integrated and synthesized in a meaningful way. The aim of this study was to synthesize findings across the large body of literature dealing with insight, to highlight its multidimensional nature, measurement, neuropsychology and social impact in schizophrenia. The extensive literature on the cognitive consequences of lack of insight and the contribution of neuroimaging techniques to elucidating neurological etiology of insight deficits, is also reviewed.

Abstract: This study's first objective was to confirm that patients with schizophrenia and their nonmentally ill siblings share the same impaired executive function when compared to healthy control subjects. The second objective was to study the relation between Wisconsin card sorting task (WCST) performance and the persistence and severity of clinical symptoms, as well as different clinical dimensions.Ninety subjects were involved in this study, divided in 3 groups of 30 each: one group of patients with schizophrenia, one group of their siblings, and a control group. Symptom severity was assessed with the Positive and Negative Syndrome Scale (PANSS), and social functioning was measured by the Global functioning scale (GFS). The WCST was administered to all 3 groups.Patients with schizophrenia and their siblings had a significantly lower WCST performance than control subjects. Statistical analysis showed that the patient group had a significantly greater impaired WCST performance than the 2 other groups. Siblings also had a significantly lower performance than the control subjects. Furthermore, no significant relation was found between WCST performance and other variables, including age, gender, education, illness duration, treatment, and different PANSS and GFS scores.Patients with schizophrenia and their nonmentally ill siblings share the same impaired executive function. These findings suggest that WCST performance can be considered a schizophrenia vulnerability marker in siblings of patients with schizophrenia.

34. Validity of the depressive dimension extracted from principal component analysis of the PANSS in drug-free patients with schizophrenia

Abstract: Depressive symptoms frequently occur during the course of schizophrenia. This study explored the relationships between the schizophrenia symptomatology and three measures of depression. Eighty-one drug-free inpatients with acute schizophrenia were assessed with the positive and negative syndrome scale (PANSS), the Calgary depression scale for schizophrenia (CDSS), and the Hamilton rating scale for depression (HAM-D). The depressive subscale of PANSS (PANSS-D) was also considered as a third scale for measuring depression. A principal component analysis (PCA) of PANSS items identified five clinical dimensions of schizophrenia called 'negative', 'positive', 'anxio-depressive', 'excitement', and 'disorganisation and others'. Our anxio-depressive dimension (PANSS-ad) was strictly identical with the PANSS-D. Scores on CDSS and HAM-D were highly inter-correlated and highly correlated with the PANSS-ad. Furthermore, while scores on CDSS were correlated only with this dimension, scores at HAM-D were also positively correlated with the negative dimension and negatively correlated with the excitement dimension. In conclusion, our results suggest that PANSS evaluation itself may be sufficient to give a correct approximation of the depression in patients with schizophrenia. However, depression scales are of course needed to assess specifically depressive symptoms in patients with schizophrenia; hence, the CDSS could be a more specific instrument than HAM-D.
