Traumatic brain injury (TBI) in children often causes cognitive and mental dysfunctions, as well as acute and chronic pain. Adult hippocampal neurogenesis plays a key role in cognition, depression, and pain. Adult hippocampal neurogenesis can be modulated by genetic and environmental factors, such as TBI and opioids. Buprenorphine (BPN), a semisynthetic opioid, is commonly used for pain management in children, however, the effects of BPN on adult hippocampal neurogenesis after pediatric TBI are still unclear. This study investigated the sex-specific effects of BPN on adult hippocampal neurogenesis during acute phase after pediatric TBI. Male and female littermates were randomized on postnatal day 20-21(P20-21) into Sham, TBI+saline and TBI+BPN groups. BPN was administered intraperitoneally to the TBI+BPN mice at 30 min after injury, and then every 6-12 h (h) for 2 days (d). Bromodeoxyuridine (BrdU) was administered intraperitoneally to all groups at 2, 4, 6, and 8-h post-injury. All outcomes were evaluated at 3-d post-BrdU administration. We found that TBI induced significant cognitive impairment, depression, and reduced adult hippocampal neurogenesis in both male and female mice, with more prominent effects in females. BPN significantly improved adult hippocampal neurogenesis and depression in males, but not in females. We further demonstrated that differential expressions of opioid receptors, transcription factors and neuroinflammatory markers at the neurogenic niche might be responsible for the differential effects of BPN in males and females. Background: The 2021 Centers for Disease Control and Prevention sexually transmitted infection treatment guidelines recommend extragenital testing for gonorrhea and chlamydia in men who have sex with men and for women based on reported behaviors and exposures. The "IWantTheKit (IWTK)" program is a free online platform for specimen self-collection and mail-in for combined chlamydia/gonorrhea testing. We sought to assess the additional diagnostic value of extragenital testing compared with genital testing only for chlamydia/gonorrhea and determine factors associated with a positive extragenital test result among IWTK users., Methods: From August 2013 to January 2022, 7612 unique IWTK users returned swabs for testing; 3407 (45%) users requested both genital and extragenital tests and were included in this analysis. Descriptive statistics were summarized for demographic characteristics, reported behaviors, and genital and extragenital test results, and data were stratified by gender and age group. A logistic regression model was used to estimate associations between factors and extragenital sexually transmitted infection positivity., Results: Chlamydia positivity rates were 4.7%, 2.4%, and 1.5% at genital, extragenital, and both sites, respectively; for gonorrhea, 0.4%, 1.1%, and 0.4% were positive at those sites, respectively. Among women, age 25 years and younger was significantly associated with extragenital chlamydia (odds ratio [OR], 4.0; P = 0.010). Being in high-risk quiz score group was associated with extragenital chlamydia (OR, 2.6; P = 0.005) and extragenital gonorrhea in men and women (OR, 8.5; P = 0.005)., Conclusions: Extragenital testing detected additional chlamydia and gonorrhea cases in the IWTK user population that would have been missed by genital-only testing, especially for women younger than 25 years and people reported to be at high risk., Competing Interests: Conflict of Interest and Sources of Funding: None declared., (Copyright © 2022 American Sexually Transmitted Diseases Association. The goal of this study was to assess awareness of, interest in, and sources of knowledge about pre-exposure prophylaxis (PrEP) among 1,464 HIV-uninfected patients from public STD clinics in Baltimore. Logistic regression analyses were conducted to examine factors associated with PrEP awareness and interest. Results suggest PrEP awareness and interest in knowing more about PrEP was highest among men who reported sex with men or with both men and women. White patients were more likely to be aware of PrEP than African American patients, and those reporting consistent condom use were more likely to have heard about PrEP than those who never used condoms. The major source of knowledge about PrEP was health care providers. Findings can inform future prioritization of locally targeted PrEP programs to enhance access to and uptake of PrEP among those most at risk for HIV transmission.

Background and Objective: When treating pronounced dentoalveolar compensation of a skeletal Class III malocclusion, preoperative decompensation frequently requires the extraction of maxillary lateral teeth and retraction of the incisors. In this context, maximum anchorage of the maxillary molars is frequently necessary to attain the significant increase in negative overjet that is desired. The aim of this study was to compare the quality of conventional and skeletal molar anchorage using mini-implants in association with pre-operative decompensation., Patients and Methods: Pre-operative decompensation involved the symmetric extraction of two lateral teeth from the maxilla as well as retraction of the front teeth in each of 20 patients with a marked skeletal Class III. The molar anchorage half of the patients received was conventional (transpalatal arch) while the other half were treated with skeletal anchorage via mini-implants inserted in the anterior palate. Study models were prepared and analyzed using a 3D scanner before and after space closure (OrthoProof). We analyzed the bilateral degree of mesial molar migration and change in the transversal dimension (DigiModel software)., Results: All patients demonstrated mesial migration of the upper molars as a response to the load on the anchorage unit. The 4.21 mm (+/- 1.17 mm) anchorage loss associated with conventional anchorage was greater than that of skeletal anchorage in the anterior palate (2.05 mm [+/- 1.39 mm]). We observed a tendency towards transversal expansion in the molar region according to the design and thickness of the transpalatal arch., Conclusions: Skeletal molar anchorage proved to be more effective than the conventional anchorage. Hence, skeletal anchorage is preferable, especially when patients are in serious need of preoperative decompensation. The anterior palate has proven to be an advantageous region for insertion in conjunction with the correct mechanics.

Aim: Treatment options in orthodontics have been expanded by skeletal anchorage via mini-implants over the last few years. Sufficient primary stability is imperative to minimize implant loss rate. The aim of this study was to quantitatively analyze the factors influencing primary stability: bone quality, implant-design, diameter, and depth of pilot drilling., Material and Methods: Thirty-six pelvic bone segments (ilium) of country pigs were dissected and embedded in resin. To determine the primary stability, we measured the insertion torque of five different mini-implant types (tomas-pin [Dentaurum, Ispringen, Germany] 08 and 10 mm, and Dual Top [Jeil Medical Corporation, Seoul, Korea] 1.6 x 8 and 10 mm plus 2 x 10 mm). Twenty-five or 30 implants were inserted into each pelvic bone segment following preparation of the implant sites using pilot drill diameters of 1.0, 1.1, 1.2 and 1.3 mm and pilot drill depths of 1, 2, 3, 6 and 10 mm. Five implants were inserted for reference purposes to establish comparability. Thicknesses of bone compacta were measured via micro-computer tomography., Results: Insertion torques of orthodontic mini-implants and therefore primary stability varied greatly, depending on bone quality, implant-design, and preparation of implant site. Compared with the tomas-pin, the Dual Top screw showed significantly greater primary stability. Torque moments beyond 230 Nmm caused fractures of 9 Dual Top screws., Conclusion: Compacta thickness, implant design and implant site preparation have a strong impact on the primary stability of mini-implants for orthodontic anchorage. Depending on the insertion site and local bone quality, the clinician should choose an optimum combination of implant and pilot-drilling diameter and depth.

Objectives: We hypothesized that an active follow-up program to assess for reinfection after gonorrhea treatment could be a useful disease control strategy., Goal: We evaluated an active follow-up and repeat testing program for all Baltimore sexually transmitted disease clinic patients diagnosed with gonorrhea., Study Design: From September 2003 to May 2004, all clients with a treated gonorrhea infection were advised to return 3 months later for repeat testing. If clients did not return as scheduled, field outreach was attempted. At follow-up visits, urine was tested for gonorrhea and consenting participants completed a behavioral survey. In addition, we reviewed morbidity records for any intercurrent gonorrhea infections reported during the project period., Results: Of the 667 participants diagnosed with gonorrhea at baseline, 54 had a gonorrhea reinfection diagnosed for an incidence of 13.8 per 100 person-years. One hundred seventy-eight (27%) either presented for a follow-up visit or were located through field efforts, and of these, 5 (2.8%) had gonorrhea detected on follow-up urine testing. No measured factors had predictive value in identifying gonorrhea reinfection., Conclusions: Although reinfection rates were high, we found that field staff intervention to increase follow-up testing rates did not identify a significant amount of repeat infections compared with passive surveillance.

Background: Retesting 3 to 4 months after treatment for those infected with chlamydia and/or gonorrhea has been recommended., Goal: We compared various methods of encouraging return for retesting 3 months after treatment for chlamydia or gonorrhea., Study: In study 1, participants were randomly assigned to: 1) brief recommendation to return, 2) intervention 1 plus $20 incentive paid at return visit, or 3) intervention 1 plus motivational counseling at the first visit and a phone reminder at 3 months. In study 2, participants at 1 clinic were randomly assigned to 4) intervention 1, 5) intervention 1 plus phone reminder, or 6) intervention 1 plus motivational counseling but no telephone reminder., Results: Using multiple logistic regression, the odds ratios for interventions 2 and 3, respectively, compared with intervention 1 were 1.2 (95% confidence interval [CI], 0.6-2.5) and 2.6 (95% CI, 1.3-5.0). The odds ratios for interventions 5 and 6 compared with intervention 4 were 18.1 (95% CI, 1.7-193.5) and 4.6 (95% CI, 0.4-58.0)., Conclusions: A monetary incentive did not increase return rates compared with a brief recommendation. A reminder phone call seemed to be the most effective method to increase return.

Spatial distribution of repeat and singly occurring gonorrhea in Baltimore, Maryland, was examined to identify clusters of core transmitters. Gonorrhea reported between 2001 and 2002 was geocoded and mapped. Those with more than one gonorrhea infection separated by more than 14 days during the 2-year period were considered repeaters. Other cases were treated as isolated events. Six definitions of core transmission were examined by census tracts with the highest quintiles of 1) cases for 2001, 2) cases for 2002, 3) rates for 2001, 4) rates for 2002, 5) counts of repeaters over the 2-year period, and 6) proportion of total cases that were repeaters. Of the 6,108 gonorrhea cases analyzed, 9% were repeaters. Repeaters were more likely to be female and younger. Core areas identified by definitions based on overall disease burden agreed well with each other but had poor agreement with definitions based upon repeat infections. Repeaters clustered to a greater extent at smaller distances than did singly occurring gonorrhea cases. Repeat gonorrhea infections are prevalent in Baltimore and likely represent behavior consistent with core transmission. Census tracts of core transmission defined by geographic distribution of repeat infections may indicate foci of highest risk sexual behaviors and high transmission., (Copyright 2004 Johns Hopkins Bloomberg School of Public Health)

The behavior of red cell shape of circulating blood was investigated following operations using the heart-lung machine by different quantitative methods. It was discovered, that the schistocytes (light microscopy) and echinocytes (scanning electron microscopy) significantly increases. The changes in erythrocyte shape are comprehended as summary effect of erythrocyte damage by the extracorporeal system and discussed in relation to the loss of phospholipid components of the erythrocyte membrane.

The phospholipid changes of erythrocytes were investigated following the storage under blood bank conditions up to 42 days (ACD-AG-stabilisator) without and with addition of procaine (25 mM). The phospholipid loss of erythrocytes is described as two-step occurrence. The rapid decrease in the first three days is followed by a further loss up to the end of the storage period. The concomitant increase of phospholipid fractions of supernatant shows that the erythrocyte membranes lose their phospholipids mainly by a passive way. For the further loss of phospholipids the action of a phospholipase A is adopted. The addition of procaine hinders strongly the phospholipid loss of erythrocytes. The clinical importance of these procaine action is shortly discussed.

Aneurysm of the extracranial internal carotid artery is a rarely observed condition. Intra-aneurysmatic thrombosis, cerebral embolism with possible neurological consequences, and rupture are the most common complications. Operations were performed on 20 patients for aneurysm of the internal carotid artery. The cases included 14 "genuine" arteriosclerotic aneurysms and seven "false" aneurysms in the wake of shell splinter injuries, tonsillectomy, thrombo-arteriectomy, and blunt traumata. Pulsating tumour was the most important clinical symptom in all aneurysm cases. Arterial continuity was restored by resection of aneurysm in all cases. Sixteen patients were dehospitalised without any complaint. Two patients with preoperative cerebral infarction were left with residual paresis. One patient died of pulmonary embolism, and one patient operated on for rupture died in shock.
