RESUMEN Objetivo: Proponer estrategias agile para este abordaje integral de la analgesia, sedación, delirium, implementación de movilidad temprana e inclusión familiar del paciente con síndrome de dificultad respiratoria aguda por COVID-19, considerando el alto riesgo de infección que existe entre los trabajadores de salud, el tratamiento humanitario que debemos brindar al paciente y su familia, en un contexto de falta estrategias terapéuticas específicas contra el virus globalmente disponibles a la fecha y una potencial falta de recursos sanitarios. Metodos: Se llevó a cabo una revision no sistemática de la evidencia científica en las principales bases de datos bibliográficos, sumada a la experiencia y juicio clínico nacional e internacional. Finalmente, se realizó un consenso de recomendaciones entre los integrantes del Comité de Analgesia, Sedación y Delirium de la Sociedad Argentina de Terapia Intensiva. Resultados: Se acordaron recomendaciones y se desarrollaron herramientas para asegurar un abordaje integral de analgesia, sedación, delirium, implementación de movilidad temprana e inclusión familiar del paciente adulto con síndrome de dificultad respiratoria aguda por COVID-19. Discusión: Ante el nuevo orden generado en las terapias intensivas por la progresión de la pandemia de COVID-19, proponemos no dejar atrás las buenas prácticas habituales, sino adaptarlas al contexto particular generado. Nuestro consenso está respaldado en la evidencia científica, la experiencia nacional e internacional, y será una herramienta de consulta atractiva en las terapias intensivas. Results: Recommendations were agreed upon, and tools were developed to ensure a comprehensive approach to analgesia, sedation, delirium, early mobility and family engagement for adult patients with acute respiratory distress syndrome due to COVID-19. Discussion: Given the new order generated in intensive therapies due to the advancing COVID-19 pandemic, we propose to not leave aside the usual good practices but to adapt them to the particular context generated. Our consensus is supported by scientific evidence and national and international experience and will be an attractive consultation tool in intensive therapies.

77 p. El concepto conciliación trabajo-familia se ha entendido de diversas maneras en el trayecto de la disciplina. En este sentido, conciliación refiere a la compatibilidad de los tiempos dedicados a la familia y al trabajo que combina el desarrollo pleno de las personas en el ámbito del trabajo y estudio con el afectivo, familiar, de ocio entre otros particulares. Por ello, conciliar significa mantener el equilibrio en las diferentes dimensiones de la vida con el fin de mejorar el bienestar, la salud y la capacidad de trabajo personal, que redundan en la calidad de vida a la que aspiramos positivamente las personas. Las medidas de conciliación se han dirigido fundamentalmente a las mujeres, contribuyendo a que la conciliación se considere como un "problema exclusivo de las mujeres". El presente proyecto pretende solucionar una problemática predominante en la empresa Narcobollo, la cual se centra en la baja precisión en la predicción de la demanda, debido a que la empresa actualmente utiliza métodos fundamentados en la experiencia e intuición de sus empleados, generando así un alto nivel de error en la misma. Es por esto que el propósito principal del proyecto en cuestión es el diseño de una plataforma para el cálculo de pronóstico de la demanda basada en modelos cuantitativos. Con la propuesta realizada se busca brindarle información confiable de demanda futura a la compañía, de forma automática, además de proporcionarles una forma de digitalizar el registro de sus ventas diarias y de esta manera conformar su propia base de datos ágilmente. Para desarrollar la plataforma, primero se realizó la recolección y digitalización de los datos en las instalaciones de la compañía. Luego, se efectuó un análisis de varios modelos de predicción, decidiéndose incluir en la plataforma cinco (5) de tipo cuantitativo para la ejecución de los cálculos internos, los cuales corresponden a comparaciones entre los errores arrojados por los modelos escogidos contra el error generado por el método actual de la empresa. Posteriormente, la plataforma define el método de pronóstico que mejor describa el comportamiento de los datos de los productos en estudio, lo optimiza y procede a pronosticar con él, además, con el fin de que la plataforma sea útil para varias áreas de la misma se dispuso el funcionamiento de las predicciones en 3 diferentes periodos de tiempo como lo son diario, semanal y mensual. Con la realización de este proyecto se logró reducir el error generado en un 24% para el periodo diario, un 41% para el semanal y un 46% el mensual con respecto al método de pronóstico actual de la empresa, logrando así obtener mediante técnicas de pronóstico datos más cercanos a la realidad. This project aims to solve a predominant problem in the company Narcobollo, which focuses on the low precision in the prediction of demand, because the company currently uses methods based on the experience and intuition of its employees, thus generating a high level of error in it. This is why the main purpose of the project in question is the design of a platform for the calculation of demand forecast based on quantitative models. With the proposal made it seeks to provide reliable information of future demand to the company, automatically, in addition to providing them with a way to digitize the record of their daily sales and in this way shape your own database agilely. To develop the platform, first it was done the collection and digitalization of the data in the company's facilities. Then, an analysis of several prediction models was made, deciding to include in the platform five (5) of quantitative type for the execution of the internal calculations, which correspond to comparisons between the errors thrown by the chosen models versus the error generated by the current method of the company. Subsequently, the platform defines the forecasting method that best describes the behavior of the data of the products under study, optimizes it and proceeds to forecast with it, also, in order that the platform is useful for several areas of it the functioning of predictions was arranged in 3 different periods of time as they are daily, weekly and monthly. With the realization of this project it was possible to reduce the error generated by 24% for the daily period, 41% for the weekly and 46% the monthly compared to the current forecast method of the company, achieving thus obtain by means of forecasting techniques data closer to reality.

- Abstract
Importance: Abnormal peripheral perfusion after septic shock resuscitation has been associated with organ dysfunction and mortality. The potential role of the clinical assessment of peripheral perfusion as a target during resuscitation in early septic shock has not been established., Objective: To determine if a peripheral perfusion-targeted resuscitation during early septic shock in adults is more effective than a lactate level-targeted resuscitation for reducing mortality., Design, Setting, and Participants: Multicenter, randomized trial conducted at 28 intensive care units in 5 countries. Four-hundred twenty-four patients with septic shock were included between March 2017 and March 2018. The last date of follow-up was June 12, 2018., Interventions: Patients were randomized to a step-by-step resuscitation protocol aimed at either normalizing capillary refill time (n = 212) or normalizing or decreasing lactate levels at rates greater than 20% per 2 hours (n = 212), during an 8-hour intervention period., Main Outcomes and Measures: The primary outcome was all-cause mortality at 28 days. Secondary outcomes were organ dysfunction at 72 hours after randomization, as assessed by Sequential Organ Failure Assessment (SOFA) score (range, 0 [best] to 24 [worst]); death within 90 days; mechanical ventilation-, renal replacement therapy-, and vasopressor-free days within 28 days; intensive care unit and hospital length of stay., Results: Among 424 patients randomized (mean age, 63 years; 226 [53%] women), 416 (98%) completed the trial. By day 28, 74 patients (34.9%) in the peripheral perfusion group and 92 patients (43.4%) in the lactate group had died (hazard ratio, 0.75 [95% CI, 0.55 to 1.02]; P = .06; risk difference, -8.5% [95% CI, -18.2% to 1.2%]). Peripheral perfusion-targeted resuscitation was associated with less organ dysfunction at 72 hours (mean SOFA score, 5.6 [SD, 4.3] vs 6.6 [SD, 4.7]; mean difference, -1.00 [95% CI, -1.97 to -0.02]; P = .045). There were no significant differences in the other 6 secondary outcomes. No protocol-related serious adverse reactions were confirmed., Conclusions and Relevance: Among patients with septic shock, a resuscitation strategy targeting normalization of capillary refill time, compared with a strategy targeting serum lactate levels, did not reduce all-cause 28-day mortality., Trial Registration: ClinicalTrials.gov Identifier: NCT03078712.
