Abstract

We report about a 230 GHz very long baseline interferometry (VLBI) fringe finder observation of blazar 3C 279 with the APEX telescope in Chile, the phased submillimeter array (SMA), and the SMT of the Arizona Radio Observatory (ARO). We installed VLBI equipment and measured the APEX station position to 1 cm accuracy (1 sigma). We then observed 3C 279 on 2012 May 7 in a 5 hour 230 GHz VLBI track with baseline lengths of 2800 M$\lambda$ to 7200 M$\lambda$ and a finest fringe spacing of 28.6 micro-arcseconds. Fringes were detected on all baselines with SNRs of 12 to 55 in 420 s. The correlated flux density on the longest baseline was ~0.3 Jy/beam, out of a total flux density of 19.8 Jy. Visibility data suggest an emission region <38 uas in size, and at least two components, possibly polarized. We find a lower limit of the brightness temperature of the inner jet region of about 10^10 K. Lastly, we find an upper limit of 20% on the linear polarization fraction at a fringe spacing of ~38 uas. With APEX the angular resolution of 230 GHz VLBI improves to 28.6 uas. This allows one to resolve the last-photon ring around the Galactic Center black hole event horizon, expected to be 40 uas in diameter, and probe radio jet launching at unprecedented resolution, down to a few gravitational radii in galaxies like M 87. To probe the structure in the inner parsecs of 3C 279 in detail, follow-up observations with APEX and five other mm-VLBI stations have been conducted (March 2013) and are being analyzed., Comment: accepted for publication in A&A

Abstract

The SARS-CoV-2 Omicron variant emerged in South Africa in November 2021, and has later been identified worldwide, raising serious concerns. A real-time RT-PCR assay was designed for the rapid screening of the Omicron variant, targeting characteristic mutations of the spike gene. The assay was used to test 737 sewage samples collected throughout Italy (19/21 Regions) between 11 November and 25 December 2021, with the aim of assessing the spread of the Omicron variant in the country. Positive samples were also tested with a real-time RT-PCR developed by the European Commission, Joint Research Centre (JRC), and through nested RT-PCR followed by Sanger sequencing. Overall, 115 samples tested positive for Omicron SARS-CoV-2 variant. The first occurrence was detected on 7 December, in Veneto, North Italy. Later on, the variant spread extremely fast in three weeks, with prevalence of positive wastewater samples rising from 1.0% (1/104 samples) in the week 5–11 December, to 17.5% (25/143 samples) in the week 12–18, to 65.9% (89/135 samples) in the week 19–25, in line with the increase in cases of infection with the Omicron variant observed during December in Italy. Similarly, the number of Regions/Autonomous Provinces in which the variant was detected increased from one in the first week, to 11 in the second, and to 17 in the last one. The presence of the Omicron variant was confirmed by the JRC real-time RT-PCR in 79.1% (91/115) of the positive samples, and by Sanger sequencing in 66% (64/97) of PCR amplicons. In conclusion, we designed an RT-qPCR assay capable to detect the Omicron variant, which can be successfully used for the purpose of wastewater-based epidemiology. We also described the history of the introduction and diffusion of the Omicron variant in the Italian population and territory, confirming the effectiveness of sewage monitoring as a powerful surveillance tool.

Abstract

Gallstone ileus is a surgical emergency that commonly goes unnoticed due to its low incidence, a high diagnostic suspicion is necessary to carry out a timely approach and treatment. A bibliographic review was carried out on the diagnosis and treatment of this entity, to exemplify it, through the clinical case of a female patient who presented with an acute abdomen, undergoes exploratory laparotomy showing gallstone ileus, for which enterolithotomy is performed. There is a discrepancy in the literature regarding the initial surgical management of this pathology, especially if the enterolithotomy must be associated with resection of a biloenteric fistula., El íleo biliar es una emergencia quirúrgica que comúnmente pasa desapercibida debido a su baja incidencia, es necesaria una alta sospecha diagnóstica para realizar un abordaje y tratamiento oportuno. Se realizó una revisión bibliográfica sobre diagnóstico y tratamiento de esta entidad, para ejemplificarlo mediante caso clínico de un paciente femenino que se presenta con un cuadro de abdomen agudo, se somete a laparotomía exploradora evidenciando íleo biliar, por lo que se realiza enterolitotomía. Existe discrepancia en la literatura en cuanto al manejo quirúrgico inicial de esta patología, especialmente si la enterolitotomía se debe de asociar a resección de fístula biloenterica.

Abstract

The dichotomy between mechanical performance and repairability is a well-known fact in the self-healing field. One alternative to overcome this trade-off is the inclusion of reinforcing fillers. In this research, hybrid reinforced self-healing styrene-butadiene rubber (SBR) composites are developed by combining mechano-chemically modified ground tire rubber (mGTR) with carbon black (CB). The SBR matrix is systematically analyzed by means of dynamic mechanical analysis and broadband dielectric spectroscopy. The SBR composite reinforced with 20 phr mGTR and 20 phr CB maintains the healing efficiency of the unfilled SBR (80%) and improves its tensile strength by 300%, balancing perfectly well both properties. The healing efficiency is determined at different damage levels. The damage at the microscopic scale is easier to recover and does not depend on the presence of fillers, while the recovery at the macroscopic level clearly relies on the addition of filler, with more demanding healing conditions.

Abstract

This paper presents an effective approach to simultaneous solution of capacitor placement and feeder reconfiguration for power-energy loss cost minimization and voltage profile enhancement in radial distribution systems. Considering a time variation load, an optimization method using a genetic algorithm is proposed to determine the optimal location and size of fixed and switchable capacitors, as well as the open/closed states of the sectionalizing and tie switches. Reduction of overall power and energy losses and capacitor costs are involved in problem formulation. The proposed approach is demonstrated using example systems from the literature and results show that taken into account both capacitor placement and feeder reconfiguration simultaneously can generate around 10% more energy and peak loss reduction than considering them separately. Simulations results are presented for a 16 bus IEEE system and for a 733 bus practical distribution network.Este trabajo presenta una metodología para la solución simultánea de los problemas de ubicación de condensadores y reconfiguración de alimentadores en sistemas eléctricos de distribución radiales, con el fin de mejorar el perfil de voltaje de la red y, al mismo tiempo, reducir los costos asociados a las pérdidas de energía y potencia. Considerando un perfil de carga dado, se propone un método de optimización usando un algoritmo genético para determinar el tamaño óptimo, ubicación, tipo y número de condensadores fijos o conmutables, así como los estados abierto/cerrado de las llaves (switches) seccionadoras y de interconexión. La formulación del problema considera la minimización de los costos totales, incluyendo costos fijos y variables de los bancos de condensadores. El método propuesto se ha probado en diversas redes y los resultados muestran que considerando ambos problemas simultáneamente los costos debidos a las pérdidas se pueden minimizar hasta un 10% más que tomando dichos problemas por separado. Se presentan los resultados obtenidos en un sistema IEEE de 16 barras y una red de distribución real de 733 barras.

Abstract

Recent randomized controlled trials confirmed the beneficial outcomes with coronary artery bypass grafting (CABG) compared with percutaneous coronary intervention (PCI) in patients with severe three-vessel coronary artery and left main disease. An increased long-term survival after CABG is associated with a reduction in spontaneous myocardial infarction and repeat revascularization rates. While PCI treats only flow-limiting lesions, CABG treats the whole coronary artery, preventing events in the future. Due to different clinical and anatomic factors affecting the outcomes, the heart team should formulate treatment assignment recommendations.

Abstract

Background: PCSK9 inhibitors are a treatment option for patients with familial hypercholesterolemia not on low-density lipoprotein cholesterol goals despite the use of maximally tolerated high intensity-statins dose. Objective: To evaluate the efficacy of alirocumab and evolocumab in LDL-C reduction and targets attainment in patients with heterozygous familial hypercholesterolemia in clinical practice setting. Methods: SAFEHEART is an open, long-term prospective study of a cohort of subjects with molecular diagnosis of familial hypercholesterolemia. This study analyze subjects >= 20 years of age on stable lipid-lowering therapy, who received PCSK9 inhibitors during the period 2016 to January 2020. Results: 433 patients (mean age 55 years, 53% male, 39% with cardiovascular disease) were included and followed-up for a median of 2.5 years (IQR 1.6-3.0). 36. Análisis de alteraciones del discurso en estados mentales de alto riesgo de psicosis (EMAR): una revisión sistemática.

Abstract

Resumen Introducción: La necesidad de intervenir precozmente en la psicosis ha llevado a la búsqueda de biomarcadores útiles en su predicción, donde el análisis del lenguaje hablado destaca por su fácil obtención y bajo coste económico. En esta revisión sistemática se analizan las principales alteraciones del lenguaje en pacientes con riesgo ultra alto de psicosis (UHR), evaluando su prevalencia y su relación con la transición a la psicosis. Métodos: Se realizó una búsqueda en las bases de datos PubMed y Embase de estudios en inglés o español, así como en las listas de referencias de los artículos encontrados. Resultados: De 140 artículos identificados, se incluyeron 15. Las variables del lenguaje analizadas fueron Cohesión Lingüística, Coherencia Semántica, Complejidad Sintáctica, Producción Metafórica, Prosodia Emocional y Trastorno del Pensamiento Formal (TFP). La prevalencia encontrada osciló entre el 21% y el 85% para las alteraciones incluidas en el TFP, sin que se hayan encontrado medidas de prevalencia para el resto de las variables. La precisión global para la transición de RH a psicosis osciló entre el 70% y el 100% en todos los estudios. Conclusiones: Existe un amplio abanico de alteraciones del habla en los pacientes con RHU, donde la PTF es la más estudiada. La prevalencia de estas alteraciones parece ser alta, especialmente en lo que respecta a la PTF. El análisis de esas alteraciones del habla en pacientes con RHU aparece como una excelente herramienta para predecir la transición a la psicosis, particularmente a través de la transcripción de entrevistas y el uso de la Inteligencia Artificial.

Abstract

AIMS Our aim was to report on a survey initiated by the European Association of Percutaneous Cardiovascular Interventions (EAPCI) collecting the opinion of the cardiology community on the invasive management of acute coronary syndrome (ACS), before and after the MATRIX trial presentation at the American College of Cardiology (ACC) 2015 Scientific Sessions. METHODS AND RESULTS A web-based survey was distributed to all individuals registered on the EuroIntervention mailing list (n=15,200). A total of 572 and 763 physicians responded to the pre- and post-ACC survey, respectively. The radial approach emerged as the preferable access site for ACS patients undergoing invasive management with roughly every other responder interpreting the evidence for mortality benefit as definitive and calling for a guidelines upgrade to class I. The most frequently preferred anticoagulant in ACS patients remains unfractionated heparin (UFH), due to higher costs and greater perceived thrombotic risks associated with bivalirudin. However, more than a quarter of participants declared the use of bivalirudin would increase after MATRIX. CONCLUSIONS The MATRIX trial reinforced the evidence for a causal association between bleeding and mortality and triggered consensus on the superiority of the radial versus femoral approach. The belief that bivalirudin mitigates bleeding risk is common, but UFH still remains the preferred anticoagulant based on lower costs and thrombotic risks.

45. KAT3-dependent acetylation of cell type-specific genes maintains neuronal identity in the adult mouse brain

Abstract

The lysine acetyltransferases type 3 (KAT3) family members CBP and p300 are important transcriptional co-activators, but their specific functions in adult post-mitotic neurons remain unclear. Here, we show that the combined elimination of both proteins in forebrain excitatory neurons of adult mice resulted in a rapidly progressing neurological phenotype associated with severe ataxia, dendritic retraction and reduced electrical activity. At the molecular level, we observed the downregulation of neuronal genes, as well as decreased H3K27 acetylation and pro-neural transcription factor binding at the promoters and enhancers of canonical neuronal genes. The combined deletion of CBP and p300 in hippocampal neurons resulted in the rapid loss of neuronal molecular identity without de- or transdifferentiation. Restoring CBP expression or lysine acetylation rescued neuronal-specific transcription in cultured neurons. Together, these experiments show that KAT3 proteins maintain the excitatory neuron identity through the regulation of histone acetylation at cell type-specific promoter and enhancer regions. Neuronal identity maintenance is highly regulated. Here, the authors showed that CBP and p300 safeguard neuronal identity through histone acetylation at promoters and enhancers of neuronal specific genes. The loss of both CBP and p300 impairs gene expression, circuit activity, and behavior in mice.
