- Abstract
Collection consists of glass plate negatives. There are corresponding modern contact prints made for a portion of the negatives. Identifying information, such as the people(s) pictured, the location or the date of the photographs is recorded on some negatives. Many of the locations recorded on negatives are small towns in West Virginia. A few negatives were signed by the photographer. Subjects include infants and young children, elderly people, families, school and religious groups, animals and rural scenes. While many photographs are posed portraits, there are also examples of more casual photographs that document daily life, including people working. Many people are pictured at their homes or businesses. Personal property, such as automobiles, buggies, children's toys, horses, musical instruments and wagons, can be seen in some photographs. Examples of posthumous and memorial photographs, such as portraits of deceased infants and families visiting cemeteries or graves are also found. There are some town views. The photographer Albert J. Ewing, his brother, Frank and the Ewing home are also pictured in the collection.

A total of 700 community-dwelling older persons enrolled in this study. Sarcopenia is defined according to the Asian Working Group for Sarcopenia 2019 (AWGS 2019). The bioelectrical impedance analysis assessed muscle mass while a digital dynamometer quantified muscle strength. Furthermore, a 6-meter gait speed test measured physical performance. The Geriatric Depression Scale (GDS-30) screened for depressive symptoms. Data collected include those of socio-demographic, lifestyle, and comorbidity.Results: The sample comprised 700 participants (455 women) with a mean age of 70.4± 6.8 years. 21.5% had sarcopenia, among them, 5.6% at severe stage, while depressive symptoms were present in 8.1% (57/700). After adjusting for the potential confounders of age, sex, marital status, living status, hypertension, body mass index, and physical activity, the depressive symptoms that were found were not associated with sarcopenia but, instead, low gait speed. However, severe sarcopenia significantly impacted the risk of depressive symptoms, particularly in females.Conclusion: Depressive symptoms are not associated with sarcopenia, but severe sarcopenia increases the risk of depressive symptoms in females, according to updated AWGS 2019 criteria. Maintaining a functional natural dentition plays an important role in keeping a satisfactory nutritional status. The aim of this study was to evaluate the relationship between oral health conditions determined by the presence of edentulism and the number of missing teeth, nutritional status and consumption of nutrients by the elderly. This cross-sectional study comprised 494 independent elderly of both genders, over 60 years of age, registered at the Brazilian public health service in Londrina, southern Brazil. The data collection included: oral examinations; anthropometric measurements by calculating the Body Mass Index (BMI); analysis of food consumption based on a multiple pass 24-hour dietary recall and a food intake frequency questionnaire; and structured interviews to obtain sociodemographic information. Multiple linear regression, the Fisher’s Exact test, chi-square and Mann‑Whitney tests, were applied at a 5% significance level. The prevalence of edentulism was 47.3%; this predominated in females, age group from 65 to 74 years, low education level and low/medium economic classification. A larger number of underweight and fewer number of overweight elderly were recorded among the edentulous versus dentate participants (< 0.0001). Significantly lower consumption of several nutrients, as well as fruit, was recorded among the edentulous group. In particular, carbohydrates, vitamins (B1, B9 and C), and the majority of the minerals studied were associated with a larger number of missing teeth. In conclusion, tooth loss was associated with the food consumption pattern of some macro- and micronutrients and the nutritional status of the Brazilian elderly.

Age-Related Macular Degeneration (AMD) is an eye disease associated with age with progressive loss of central vision, but the peripheral vision remains. The disease is considered the third most common cause of impaired vision and legal blindness in the world among people 60 years and older and as the elderly population increases, the prevalence of AMD is also expected to increase. Because of the progress of the disease, vision-related issues are accumulated and consequently also on the quality of life. The aim of the study was to explore the experience of living with AMD. The participants were ≥ 65 years living in their own homes and having a diagnosis of dry AMD in one or both eyes caused visual acuity of no more than 0.3 in the best eye. The method chosen was the constructivist grounded theory where reality is seen as fundamentally social and processual and a way of getting access to the participants’ experiences, thoughts, and feelings. Objective research on the role of positive affect, such as happiness, on health outcomes is burgeoning. Within this context, evidence for an inverse effect of happiness on mortality is inconclusive. Furthermore, few studies link happiness with mortality among older people, and in Asian populations. We examine the association between happiness and all-cause mortality among older people in Singapore. Methods data for 4,478 Singaporeans aged ≥60 years enrolled in a nationally-representative longitudinal survey (three waves: 2009; 2011; 2015) were utilised. Happiness, at baseline, in 2009, was measured using three positively-worded items from the Centre for Epidemiological Studies Depression Scale, and considered in two distinct ways in the analyses—continuous ('happiness score' [0–6]) and binary (happy [score = 6]/unhappy). All-cause mortality, until 31 December 2015, was assessed primarily using administrative databases, supplemented by data from survey waves 2 and 3. Multivariable Cox regression models assessed the association of 'happiness score' and the 'binary happiness variable' (separate models for each) with all-cause mortality. Results the likelihood of all-cause mortality was lower by 9% (multivariable hazard ratio (HR) [95% confidence interval]: 0.91 [0.87–0.95]) for each unit increase in 'happiness score', and was 19% lower for happy, versus unhappy, older people (HR: 0.81 [0.68–0.97]). Conclusions happiness is associated with reduced likelihood of all-cause mortality among older people in an Asian population, with the benefit observed even for incremental increases in happiness. Activities, policies and programs that maintain or improve happiness may be beneficial for a longer life among older people. [ABSTRACT FROM AUTHOR]

From social networks to health and fitness, everyday a lot of mobile devices applications (apps) are being developed. The variety and availability is such that people start to think that indeed “there's an app for everything”. Many of these apps address either problems or characteristics that affect older people and that are related with the ageing process (e.g. memory and visual aids apps). They can effectively help people and are under constant evolution. However, the lack of knowledge about these available technological aids can undermine its dissemination and consequently the help that people really receive, especially those who need it the most: older people. As such, a methodological search for available aid apps was made both in Google Play and in iTunes: 536 were selected, their classification analysed and the kind of help that they provide identified. It was noted that either in Google Play or in iTunes the apps’ categories are similar. Furthermore, it is not easily perceived what the type of help that each app can provide is and how is it provided. Hence, based on the results from the aforementioned methodological search, this paper proposes a new scalable tree-based classification methodology for aid apps, which is considered more suited to perceive what the available aid apps for mobile devices are. It includes holistic approach to patients examination. In this systematic review we analyzed results of tests used in CGA and correlation of results. Moreover, we underlined some issues concerning homogeneity of tests used in CGA as well as scientific validity of some of its tools. Results of prospective studies show that some of results of examination used in CGA are good predictors of functioning in another health domain in next years. Systolic blood pressure as well as BMI could be good predictors for neuropsychological functioning, however results are equivocal. Low scores in dual task (walking and cognitive engaging task simultaneously) are good predictor for falls occurrence. Education measured by socioeconomic status is very often mentioned as one of the best predictors of cognitive functioning. Undergoing Mini Nutritional Assessment by patients could be an effective tool to improve their nutrition status, possibly by indirectly influencing on their diet habits. Moreover, genotype examination could be worth to include into CGA due to, inter alia, examination of possible causes of dietary deficits. Aerobic examination could be done by cost-free tests, which sensitivity and specify are on tolerable level. It could help lowering CGA costs, which will help make it widely available for patients. Homicide‐suicide is a rare event, but it has a significant impact on the family and community of the perpetrator and victim(s). The phenomenon of late‐life homicide‐suicide has not been previously studied in New Zealand, and there is only limited data in the international literature. The aim of this study is to systematically review coroners' records of late‐life homicide‐suicides in New Zealand. After ethics approval was granted, the Coronial Services of New Zealand was approached to provide records of all closed cases with a suicide verdict (age 65+) over a five‐year period (July 2007–December 2012). Of the 225 suicides, 4 cases of homicide‐suicide were identified (an estimated incidence of 0.12 per 100 000 per persons year). All four perpetrators were men; three had been farmers. Their ages ranged from 65 to 82. One case occurred in the context of an underlying psychiatric illness (psychotic depression in bipolar disorder). Firearms were used in three cases. Two cases were categorized as spousal/consortial subtype, one case as filicide‐suicide, and one case as siblicide‐suicide. The prospect of major social upheaval in the form of losing their homes was present in all four cases. The findings of this case series were consistent with the limited existing literature on homicide‐suicide. Age‐related biopsychosocial issues were highlighted in this case series of late‐life homicide‐suicide. "Background and Significance. It is necessary to improve patient outcomes with older adults who present to the emergency department (ED). Patient-centered care strives to improve older adult outcomes from the ED. However, appropriate disposition decisions with older adults are becoming increasingly complex and challenging to achieve. Purpose. The purpose of this study was to explore the perceptions of older adults as to their disposition from the emergency department, the decision making process, and their engagement in that process. It also explored factors that older adults identify as important when making a disposition decision from the emergency department. This study utilized a conceptual Three-Talk Shared Decision Making model to guide the study. Methods. The research design utilized in the study was a descriptive qualitative approach. Recently discharged older adults treated in the emergency department agreed to a telephonic interview. Audiotapes were transcribed and thematically analyzed to identify codes, patterns, and themes. Findings. The study’s analysis revealed that the process of the Three-Talk Shared Decision Making model was not evident for this sample of older adults seeking care in the ED. Participants identified only one option regarding their disposition from the ED. Consequently, the participants did not identify the process of option talk. The emergent themes, emotional reactions, helplessness, and provider empathy provided insight into how older adults perceive their disposition decision. The three factors participants perceived as vital to them before making a disposition decision were safety, pain relief, and a definitive diagnosis. Discussion and Recommendations. The study revealed that older adults want to be heard regarding their treatment and disposition decisions. The study identified the importance of provider education when deciding treatment or disposition decisions with the older adult. It revealed a need to promote the utilization of the Three-Talk Shared Decision Making Model to improve patient-centered care and ED disposition outcomes. Recommendations include future qualitative studies to be conducted with both the older adult and provider’s perception of the ED visit. Additional strategies and skills are warranted to enhance shared decision making in the ED with the growing aging population. With the recent SARS-CoV-2 pandemic, older adults are at high risk for increased ED visits, and the decision making process may become increasingly complicated as this population continues to age. Background: Research has suggested that meals-on-wheels recipients can be at risk for poor nutritional status. Despite this, few countries have statutory minimum requirements for the nutrient content of meals-on-wheels. This study examined both the nutritional status of a sample of Irish recipients and the nutrient content of a sample of meals provided to determine whether Irish recipients would benefit from statutory minimum nutritional standards. Methods: The study had two phases. First, a nutritional assessment was carried out to analyse the nutritional status of a sample of Irish meals-on-wheels recipients (Mini Nutritional Assessment and 24-h dietary recall with 63 self-selected respondents). Second, an assessment of the nutrient content of a sample of 46 meals from eight meals-on-wheels services was undertaken to characterise the nutritional content of the meals. Results: Over one-third of recipients (38.5%) were malnourished or at-risk of malnutrition and over half (52.3%) were overweight or obese. The mean (SD) energy [kJ (kcal)] content of the meals assessed was 3008 (498) kJ [719 (119.1) kcal], contributing 35–40% of the recommended dietary allowance (RDA) for males aged 65 years and over and 42–45% of the RDA for females aged 65 years and over. In the meals assessed, the levels of vitamin C (25.3%), vitamin D (11.6%), folate (24.8%) and calcium (20.9%) were below one-third of the Irish RDA for these nutrients. Conclusions: Irish recipients may not be receiving adequate micronutrients from meals-on-wheels. Legislation that sets out minimum standards for the nutrient content of meals-on-wheels and greater variation in the portion sizes offered may benefit recipients. [ABSTRACT FROM AUTHOR]

Persistent lower limb oedema is an underestimated problem in the elderly population. Oedematous tissue predisposes the individual to the development of bacterial infections. Accurate assessment of the underlying aetiology and application of compression hosiery could prevent cellulitis and reduce the risk of recurrent episodes. Awareness of the complications of leg oedema needs to be raised in the general population. Healthcare professionals require education to enable the provision of sale, effective treatment to reduce morbidity relating to chronic oedema and cellulitis. The National Service Framework for Older People and the British Lymphology Society's Framework for Education could facilitate the development of national service provision to meet the needs of the population with chronic oedema. "Over the last 40 years, a new paradigm has been posited where the variability observed in physiological systems is a consequence of the interactions occurring between the various components that affect the system. While quantifying the magnitude of variability can be useful, analyses that measure how the structure of the variability (dynamics) changes over time have been posited to reflect the health of the system. Many researchers interpret the results of these analyses to be indicative of the system's adaptive capacity. While there is ample indirect evidence to support this notion, a lack of direct findings has left the literature lacking a definitive foundation to move forward with this interpretation. While many physiological systems are too invasive to safely perturb, the movement-based systems are routinely perturbed in real-world environments without dire consequences. Of particular interest is the locomotor system, which is constantly challenged in real-world environments via slips and trips. Furthermore, the locomotor system can be safely and validly perturbed in the laboratory. A range of locomotor dynamics-based measures have been used to describe differences between various clinical populations, but none have been directly associated with a person's ability to remain upright when perturbed. The objectives of this study are to (1) examine the relationship between locomotor dynamics/stability to overall fall-risk prior, (2) examine how locomotor dynamics relate to the ability to recover from a trip via global stability, and (3) determine the extent to which an acute trip-training session alters locomotor dynamics and global stability. Forty healthy, older adults (75.2±4.9 yrs) were recruited by convenience from the local community. The participants completed a variety of clinical assessments in order to determine overall fall-risk. Afterwards, they participated in three walking trials consisting of: 1) a 15-minute unperturbed walking session, 2) a 10-minute unperturbed walking session (control) or a 10-minute trip-training session (intervention), and 3) a 15-minute unperturbed walking session. Various measurements of locomotor dynamics and adaptability were calculated from full-body 3-D kinematics collected at 100Hz. Multiple regression and repeated measure analysis of variance models were calculated to determine to what extent locomotor dynamics and adaptability relate to one another and how an acute trip-training session affects their relationship. The results from our first experiment suggested that locomotor dynamics and stability during steady state do not significantly relate to overall fall-risk. However, the second experiment showed that locomotor dynamics are predictive of an individual's ability to recover from a trip. Our last experiment showed the feasibility of using an acute trip-training session to alter locomotor dynamics and stability. These data represent the first direct evidence of physiological variability being indicative of adaptive capacity in the locomotor system. "The purpose of this retrospective cohort research study was to explore the relationship between frailty and recurrent Clostridium difficile (CDI) in adults 55 years and older hospitalized between December 31, 2013 through December 31, 2015, with data extracted from 2012 to 2016 to identify initial and recurrent admission for CDI. A researcher-derived frailty index, based on the Accumulation of Deficits framework by Mitnitski, Mogilner, and Rockwood (2001), was created after careful review of the components contributing to frailty and following the guidance by Searle, Mitnitski, Gahbauer, Gill, and Rockwood (2008). As per Searle et al. (2008), the FI-CDI was constructed by following the method of selecting variables that are associated with health, increase with age, do not present early in the aging process, cover a wide representation of organ systems and not just one system, and items for the index remain unchanged when performing serial measurements in the sample. Variables for the FI-CDI included laboratory abnormalities, chronic diseases, functional status, and psychosocial indicators. The deficits were coded as "1" for present and "0" for absent, calculating the FI-CDI by dividing the number of deficits in an individual by the total number of deficits measured (36 in this study) as per the standard procedure for calculating the frailty index (Searle et al., 2008). Based on the derived FI-CDI, frailty was defined as ≥ 0.25. The initial sample for the inclusion criteria consisted of 871 patients with CDI. Only 450 patients had complete data on admission to calculate the FI-CDI for the 36 deficits. The overall sample (n=871) had a recurrence rate of 23.9% (n=208) for the study iii period. The average age for the overall sample was 73.6 years (SD=10.7), with 9.1% of the sample expiring during first hospitalization over the study period. Caucasian females comprised over half of the sample. Almost two-thirds (n=576, 66.1%) resided in a private residence prior to initial admission, followed by skilled nursing facility (n=125, 14.4%). About one-third of the patients were discharged to either home/self-care (n=279, 32.0%) or a skilled nursing facility (n=261, 30.0%) after initial admission. CDI recurrence was more prevalent for those discharged to a skilled nursing facility (37.5% vs. 27.6% with no recurrence) and with home health care services (24.5% vs. 15.2% no recurrence) (p<0.001). The average frailty score for the FI-CDI sample (n=450) was 0.37 (SD=0.10) on admission. The age group of 55 to 64 was significantly associated with recurrent CDI admission for both bivariate and logistic regression analyses. The chronic diseases found to be significantly more prevalent in recurrent CDI included hypertension (88.0% vs. 78.4%; p=0.003), heart failure (36.1% vs. 25.6%; p=0.005), and chronic kidney disease (34.1% vs 24.9%: p=0.011). Frailty prevalence, as measured by the FI-CDI (n=450) was 89.1% on admission as indicated by a FI-CDI score of ≥ 0.25. The FI-CDI scores on admission were significantly related to CDI recurrence, adjusting for sociodemographics. Proton pump inhibitor (PPI) use prior to initial admission was significantly associated with frailty in bivariate analysis, but PPI use was not associated with recurrent CDI. Evidence from this study bridges a knowledge gap that exists regarding frailty and recurrent CDI. Limited research has been explored with frailty and recurrent CDI, and this study provides a foundation for prospective studies. The FI-CDI could be used with existing medical record data at time of hospitalization, assessing frailty and allowing iv opportunities for intervention. The hospitalized frail older adult is vulnerable, with CDI as a stressor that can result in prolonged recovery time and possible recurrence. Recognition of frailty in this population through already existing medical record data can guide interventions to address the underpinnings contributing to frailty and decrease readmissions, recurrence, morbidity, and mortality."--Abstract from author supplied metadata.
