Following the success of the previous edition, the second edition of Geriatrics Models of Care is the definitive resource for systems-based practice improvement for the care of older adults. Several new models of care have been published in the last eight years, new outcomes have emerged to better understand the impact of existing models, and with the rise of the Age-Friendly Health Systems movement, promoting organized efforts to prepare our health care settings for older individuals is of more importance than ever. As the Baby Boomers age, concerns over healthcare systems'abilities to accommodate geriatric patients grow increasingly challenging. This is especially true with the population deemed to be “the oldest of the old,” specifically those over the age of 85. Unlike any other time in history, this demographic is the fastest growing segment of most developed countries. In the United States the oldest old is projected to double from 4.3 million to 9.6 million by 2030. The increased life expectancy of the population since the early 1900s has been built on the improvement of living conditions, diet, public health and advancement in medical care. With this we have seen a steady decline in the age-specific prevalence of vascular and heart diseases, stroke and even dementia. Older persons are healthier today than their counterparts decades ago. More importantly than in any other age group, the care of the oldest old must be individualized; management decisions should be made taking into consideration the older persons'expressed wishes, quality of life, function and mental capacity. The inevitable consequence is that there will be an increase in the prevalence of older persons with chronic diseases, multiple co-existing pathologies and neuro-degenerative diseases. The oldest of the aging population are often excluded from drug trials and their treatments are largely based on findings extrapolated from that of the younger old. Furthermore, among the oldest old, physiologically they are more diverse than other segments of the population. Their demographic characteristics are unparalleled and different compared to that of the younger old. Several studies have drawn attention to the differing attitudes among health professionals towards elderly people and many show prejudice because they are old. As a result, the use of age as a criteria in determining the appropriateness of treatment is of very limited validity, yet there are limited resources that guide physicians through these challenges. This book creates a greater awareness of these challenges and offers practical guidelines for working within the infrastructures vital to this demographic. This book is designed for geriatricians, primary care physicians, junior medical officers, specialty geriatrics nurses, and gerontologists. It is divided into 3 sections: General Considerations, Chronic diseases and Geriatric Syndromes. Current Challenges and Management of Disease in the Elderly Population is a comprehensive insight into the diseases and their management in elderly population during ageing. The book provides information on the development in understanding, diagnosis and management of major diseases through biological and technological interventions. Non-communicable diseases currently infesting old age population will have a huge epidemiological burden on the health care system in near future. Middle- and high-income countries warrant sincere efforts to provide information on the current challenges and management of disease in elderly population in health care professionals and social organizations related to the health of society. The book is a holistic approach by international authors from different fields on major diseases, which are predominantly increasing at an alarming rate in old age population around the globe. The first chapter provides information on stem cells involved in the regulation of cell homeostasis by process of DNA-based methylation/demethylation and regulating gene expression involved in cell fate to undergo apoptosis. The second chapter emphasizes the immunological decline in innate and adaptive immunity in ageing cells, which leads to decreased activity of T-cell and T-regulatory cells. The third, fourth and fifth chapters deal with chronic diseases like Type 2 Diabetes mellitus based neuropathy, different bone diseases in old age and involvement of Vitamin D in women's bone health in elderly population. The problem of Type 2 Diabetes mellitus based neuropathy is the result of unattended or poor medical interventions, therefore emphasis on this problem and its prevention is dealt with in detail. Further disease like osteoporosis, Paget disease, and Arthritis, which are very common in old age population and deficiency of Vitamin D, calcium, phosphorous in elderly women after menopause, where estrogen based hormonal deabsorbtion of Vitamin D lead to poor bone health, is detailed from mechanism, diagnosis, current challenges and future perspective of preventions. Ageing eye problems and associated challenges are incorporated with in depth information on dry eyes and vitreous detachment, glaucoma, age-related macular degeneration, and diabetic retinopathy, which leads to irreversible visual impairment or blindness, the details information on detection, treatment and prevention are discussed in Chapter 6. The task of detecting specific diseases is paramount for the treatment of neurodegenerative disease. Chapter 7 & 8 on biomarkers using omics technology and metalobomics provides complete information on the recent development in identification and analysis of metabolites and biomolecules using different technology platforms to detect the disease condition at an early stage. They also highlight challenges in the field that need to be addressed to overcome the problem of disease diagnosis for timely intervention. The majority of neurodegenerative disorders influencing the population above the age of 65 is mainly related to neurodegeneration and oxidative stress. Tthe last two chapters, 9 &10, of this book have tried to provide information related to mechanism of neurodegeneration in various neuronal disorders and the role of various plant flavanoids in prevention of disease onset, the effect on the choice of nutraceuticals and their role in brain heath and prevention from old age neuronal problems.

Weitere Einschränkungen sind durch den Verlust der Sehkraft und des Hörvermögens sowie der zunehmenden Gebrechlichkeit mit Fallneigung und der Inkontinenz zu erwarten. Eine altersgerechte Ernährung unter Berücksichtigung der veränderten Funktionen des Gastrointestinaltraktes ist notwendig. Ältere Menschen sind vermehrt auf Medikamente angewiesen. Die medikamentöse Therapie mehrerer Krankheitsentitäten führt dabei häufig zu einer Polypharmazie mit entsprechenden Arzneimittelnebenwirkungen. Arzneimittelnebenwirkungen bei älteren Patienten manifestieren sich häufig als gastroenterologische Symptome. Hier sind z. B. Obstipation, Durchfall, Magengeschwüre mit gastrointestinaler Blutung und Refluxösophagitis zu nennen. Bei älteren Patienten erfordern vielfältige interagierende medizinische Probleme also ein multidimensionales Management, das Gastroenterologen, Geriater, Ernährungsmediziner aber auch Pharmakologen einschließt. Das physiologische Alter und der „Funktionszustand“ des Patienten müssen mehr als das chronologische Alter berücksichtigt aber auch richtig eingeschätzt werden. Interdisziplinär müssen diagnostische und therapeutische Abläufe unter Berücksichtigung der Physiologie und Pathologie älterer gastroenterologischer Patienten angepasst und optimiert werden. Dazu zählen z. B. auch endoskopische Untersuchungen. Die abgestimmte Zusammenarbeit von Krankenhausärzten und niedergelassenen Kollegen ist zur reibungslosen Überleitung in die häusliche Betreuung der Patienten erforderlich.

Public health and antiseptic measures, vaccination and antibiotics, have all contributed to the reduction in the incidence and impact of diseases due to infections in younger age groups. Unfortunately, however, infections remain a very important cause of both morbidity and mortality in the elderly. The reasons for the continued effect of infection on the older person are multifactorial. Firstly, the immune response alters with age and may result in opportunistic infections. Secondly, while the diagnosis and management of some infections in the elderly can present little problem, altered signs and symptoms in other older patients, especially the old elderly, can cause considerable diagnostic difficulties with resulting delays in definitive treat ment. Thirdly, a degree of complacency can develop because some infec tions are seldom seen and, consequently, may not be considered in the differential diagnosis. The presence of other, more common, diseases serve only to distract attention and confuse the diagnosis. Since the number of elderly people is increasing, the need for continued high standard of skill in diagnosis and management is emphasized. These factors are considered in this book by contributors who are experi enced in their fields. The altered immune response with age, the appropriate use of antibiotics in older people and the latest developments of therapy are reviewed. Infections in various body systems are considered, with emphasis on differing presentations and diagnostic difficulties, ways of improving diagnostic skills as well as management and treatment.

Features that characterize the aging process include the gradual accumulation of cell damage after prolonged exposure to oxidative and inflammatory events over a lifetime. In addition to the accretion of lesions, the intrinsic levels of pro-oxidant and aberrant immune responses are elevated with age. These adverse events are often further enhanced by the chronic and slow progressing diseases that characterize the senescent brain and cardiovascular system. The incidence of some disorders such as Alzheimer's disease and vascular diseases are sufficiently prevalent in the extreme elderly that these disorders can arguably be considered'normal'. Aging and Aging-Related Disorders examines the interface between normal and pathological aging, and illustrates how this border can sometimes be diffuse. It explores and illustrates the processes underlying the means by which aging becomes increasingly associated with inappropriate levels of free radical activity and how this can serve as a platform for the progression of age-related diseases. The book provides chapters that examine the interactive relationship between systems in the body that can enhance or sometimes even limit cellular longevity. In addition, specific redox mechanisms in cells are discussed. Another important aspect for aging discussed here is the close relationship between the systems of the body and exposure to environmental influences of oxidative stress that can affect both cellular senescence and a cell's nuclear DNA. What may be even more interesting to note is that these external stressors are not simply confined to illnesses usually associated with aging, but can be evident in maturing and young individuals. A broad range of internationally recognized experts have contributed to this book. Their aim is to successfully highlight emerging knowledge and therapy for the understanding of the basis and development of aging–related disorders.
