16. Effect of LC9018 combined with radiation therapy on carcinoma of the uterine cervix: a Phase III, multicenter, randomized, controlled study

Abstract

Background. The failure rate with radiation therapy alone for Stage Ill cervical cancer is quite high, and therefore other modalities are being pursued as adjuvants to radiation therapy in hopes of improving the results. Methods. A randomized, controlled, comparative study on the efficacy and safety of radiation therapy combined with LC9018 (a biologic response modifier prepared from heat-killed Lactobacillus casei YIT9018) was conducted using 228 patients with Stage IIIB cervical cancer. Results. LC9018 enhanced tumor regression (P < 0.1) by radiation after both 30 Gy of external radiation and at the completion of radiation therapy. The combination therapy also prolonged survival and the relapse-free interval (P < 0.05) compared to radiation alone. Analysis of survival using the Cox proportional hazard model indicated that use of LC9018 was a significant factor related to survival duration. Major side effects of combined LC9018 included fever and skin lesions at the injection site, but no severe symptoms were noted. Radiation-induced leukopenia was significantly less severe (P < 0.05) in the LC9018-combined group than in the radiationalone group, suggesting that this agent might help to prevent leukopenia during radiation therapy. Conclusions. LC9018 was shown to be an effective agent for adjuvant immunotherapy when combined with radiation therapy. Cancer 1993; 72:1949-54.

Abstract

Biological response modifiers (BRMs) are a class of compounds which stimulate the immune system in complex ways that are not entirely understood. Some BRMs are synthetic, and some are derived from bacteria. One particular BRM, called LC9018, is derived from the bacteria Lactobacillus casei, strain YIT 9018. This BRM has antitumor effects in mice; it presumably stimulates a successful immune response against transplanted tumors. Similar effects may help patients undergoing radiotherapy for cancer of the cervix. Since suppression of the immune system is an unavoidable side effect of radiotherapy, BRMs may be of value in the recovery process. To determine if this is indeed the case, 49 patients with cervical cancer were randomly assigned to receive either radiotherapy alone, or radiotherapy with LC9018. The administration of the BRM began two weeks before radiotherapy and continued, one or twice a week, for up to four weeks after radiotherapy. Each patient's status was assessed by survival, biopsy of the tumor, laboratory values of immunological function, and BRM side effects. Side effects observed with LC9018 included fever, tenderness, and, in 20 percent of the cases, formation of an abscess at the site of injection. Tests of immune function did not reveal any significant differences between the groups, but blood counts in the treated patients showed less leukocytopenia, or reduction in number of white blood cells. Histological examination of the biopsies revealed a greater effect of the combination treatment than of radiotherapy alone. This effect was mirrored in the survival data of the patients; the rate of survival was higher in the group receiving radiotherapy and BRM. Unfortunately more testing on greater numbers of patients will be needed to determine if the effect of LC9018 is significant, and whether it is warranted as a useful adjunct to radiotherapy. Abstract

A case of stage IVB adenoacanthoma of the uterine corpus is described. The patient was admitted with a large amount of atypical genital bleeding. Computed tomography and magnetic resonance imaging revealed a large tumor accompanied by lymph node involvement in the left inguinal, multiple pelvic, and paraaortic regions. She was diagnosed as having stage IVB endometrial adenoacanthoma. Neoadjuvant chemotherapy with carboplatin (CBDCA) and 5-fluorouracil (5-FU) was performed, followed by radiotherapy. The tumor responded very well, but still remained in Douglas' pouch after treatment. The patient therefore underwent a simple hysterectomy, pelvic and paraaortic lymphadenectomy, and partial rectal resection. Histopathologically, viable cancer cells were observed only in the fundus of the uterus. The patient is alive with no evidence of recurrences 4 years the initiation of chemoradiotheraphy.
