Abstract

BackgroundThe natural history of coronary artery aneurysms (CAA) after intravenous immunoglobulin (IVIG) treatment in the United States is not well described. We describe the natural history of CAA in US Kawasaki disease (KD) patients and identify factors associated with major adverse cardiac events (MACE) and CAA regression.Methods and ResultsWe evaluated all KD patients with CAA at 2 centers from 1979 to 2014. Factors associated with CAA regression, maximum CA z‐score over time (zMax), and MACE were analyzed. We performed a matched analysis of treatment effect on likelihood of CAA regression. Of 2860 KD patients, 500 (17%) had CAA, including 90 with CAA z‐score >10. Most (91%) received IVIG within 10 days of illness, 32% received >1 IVIG, and 27% received adjunctive anti‐inflammatory medications. CAA regression occurred in 75%. Lack of CAA regression and higher CAA zMax were associated with earlier era, larger CAA z‐score at diagnosis, and bilateral CAA in univariate and multivariable analyses. MACE occurred in 24 (5%) patients and was associated with higher CAA z‐score at diagnosis and lack of IVIG treatment. In a subset of patients (n=132) matched by age at KD and baseline CAA z‐score, those receiving IVIG plus adjunctive medication had a CAA regression rate of 91% compared with 68% for the 3 other groups (IVIG alone, IVIG ≥2 doses, or IVIG ≥2 doses plus adjunctive medication).ConclusionsCAA regression occurred in 75% of patients. CAA z‐score at diagnosis was highly predictive of outcomes, which may be improved by early IVIG treatment and adjunctive therapies.

Abstract

Outcome of TGFβ1 signaling is context dependent and differs between individuals due to germ-line genetic variation. To explore innate genetic variants that determine differential outcome of reduced TGFβ1 signaling, we dissected the modifier locus Tgfbm3, on mouse chromosome 12. On a NIH/OlaHsd genetic background, the Tgfbm3b(C57) haplotype suppresses prenatal lethality of Tgfb1(-/-) embryos and enhances nuclear accumulation of mothers against decapentaplegic homolog 2 (Smad2) in embryonic cells. Amino acid polymorphisms within a disintegrin and metalloprotease 17 (Adam17) can account, at least in part, for this Tgfbm3b effect. ADAM17 is known to down-regulate Smad2 signaling by shedding the extracellular domain of TGFβRI, and we show that the C57 variant is hypomorphic for down-regulation of Smad2/3-driven transcription. Genetic variation at Tgfbm3 or pharmacological inhibition of ADAM17, modulates postnatal circulating endothelial progenitor cell (CEPC) numbers via effects on TGFβRI activity. Because CEPC numbers correlate with angiogenic potential, this suggests that variant Adam17 is an innate modifier of adult angiogenesis, acting through TGFβR1. To determine whether human ADAM17 is also polymorphic and interacts with TGFβ signaling in human vascular disease, we investigated hereditary hemorrhagic telangiectasia (HHT), which is caused by mutations in TGFβ/bone morphogenetic protein receptor genes, ENG, encoding endoglin (HHT1), or ACVRL1 encoding ALK1 (HHT2), and considered a disease of excessive abnormal angiogenesis. HHT manifests highly variable incidence and severity of clinical features, ranging from small mucocutaneous telangiectases to life-threatening visceral and cerebral arteriovenous malformations (AVMs). We show that ADAM17 SNPs associate with the presence of pulmonary AVM in HHT1 but not HHT2, indicating genetic variation in ADAM17 can potentiate a TGFβ-regulated vascular disease.

Abstract

パリ協定における目標を達成するために脱炭素社会を実現し，今後も変化する気候に社会が適応するためには，多くの人々が気候変動のリスクに関して理解を深めることが重要な課題である。このため我々の研究グループは，これまでに気候変動リスク連鎖を包括的に分かりやすく可視化する手法の開発を行った。本論文では，我々が開発した手法によって得られた，水資源・食料・エネルギー・産業とインフラ・自然生態系・災害と安全保障・健康の7つの分野に関連するリスク連鎖の可視化結果（ネットワーク図・フローチャート）について議論することにより，気候リスク連鎖の全体像を明らかにする。また，可視化結果を利用して行った市民対話イベントの実例を紹介することにより，我々の開発したネットワーク図・フローチャートの有用性や，気候リスクに関する市民対話の重要性について論じる。さらに，日本における気候変動リスク評価の概要について紹介し，気候変動リスク連鎖を評価するための今後の課題について議論する。

17. Examinations of performance for utilization of radiation portal monitors in contamination inspection for motor vehicles in nuclear emergency (Contract research)

Abstract

原子力災害時の避難退域時検査では、多数の車両及び避難住民等の検査を行うことが想定される。現在は、車両の指定箇所をタイヤとワイパー部とし、要員によるGM管式サーベイメータ等の表面汚染測定器で検査することを基本としているが、効率的な避難退域時検査を実施するため、車両検査では可搬型の車両用ゲート型モニタの活用が検討されている。本研究では、そのための基礎データを取得することを目的に、車両用ゲート型モニタの性能調査試験を実施した。本試験では、避難退域時検査における簡易除染の判断基準であるOIL4相当の汚染を密封線源の$^{133}$Baで模擬した。これを実車両のタイヤとワイパー部に取り付け、車両用ゲート型モニタにより模擬汚染からの$\gamma$線の計数率を測定することとした。試験方法は、車両を静止させて測定する静止試験、実際の検査を模擬した走行試験、高BG環境下での性能を調べるための高BG環境試験の3試験を採用した。試験車両は、普通自動車として広く使用されているセダンを主とし、加えて車体のフロント部分がバスに似た造りであるバンも使用した。また、車両用ゲート型モニタは、(株)千代田テクノル製のガンマ・ポールを使用した。本試験の結果、OIL4初期設定値である40,000cpm相当の汚染が車両のタイヤ接地面に付着していたとき、車両が10km/h以下で車両用ゲート型モニタを通過すれば、99\%以上の確率で、同じくワイパー部であれば、車速5km/h以下のとき84\%以上の確率で汚染検知が可能であると推定された。タイヤ接地面の汚染がOIL4以上か未満かを判定するには、メーカーで設定する機器内の換算定数の調整を要することが示された。しかし、その場合であっても、ワイパー部はタイヤ接地面よりも汚染の検知が難しいため、タイヤ接地面とワイパー部の汚染を同時に判定・評価することは困難であることが示された。車両用ゲート型モニタを活用し、避難退域時検査の車両検査の効率化(迅速化)を図るには、タイヤ接地面とワイパー部の汚染の同時測定・評価の実現が課題であることが明らかとなった。, In contamination inspections for public in nuclear emergency, it is assumed that a large number of motor vehicles and evacuees will be inspected. At present, first measurement points of vehicles are tires and around a wiper, and they are basically inspected by persons using portable radiation surface contamination meters such as GM survey meters. However, in order to efficiently inspect contamination of vehicles, utilization of portable radiation portal monitors is being considered for the inspection. In this study, examination of performance of the portal monitor was conducted in order to obtain basic data. In this examinations, sealed radiation sources, $^{133}$Ba, were substituted for the contamination of Operational Intervention Level 4 (OIL4), the evaluation criteria to conduct simple decontamination in the contamination inspection. The radiation source was attached practically to a tire and around the wipers of a vehicle, and the counting rate of $\gamma$-rays from the radiation sources was measured using the portal monitor. Three examinations were conducted: static examination with a vehicle stationary, moving examination to mock the actual inspection, and high back ground examination to investigate performance of the equipment in high back ground environment. The vehicle mainly used in the experiments was a sedan, which is generally used as standard vehicles. And, a van whose front body is like that of a bus was used at this experiment. In addition, the "Gamma Pole" manufactured by the Chiyoda Technol Corporation was used as the portal monitor. As the result, it was estimated that the case of contamination equivalent to 40,000 cpm, the default value of the OIL4, deposited on the tread on a vehicle tire, when the vehicle passes through the portal monitor at 10 km/h or less, the contamination would be detected with a probability of 99\% or more. Similarly, when the contamination deposited on around the wipers and the vehicle speed is 5 km/h or less

20. Transfer and operation of WSPEEDI-II automatic calculation system for responses to nuclear tests by North Korea

北朝鮮が地下核実験を実施した際には、原子力緊急時支援・研修センター(支援・研修センター)は、原子力規制庁からの要請に基づき、国による対応への支援活動として、原子力基礎工学研究センター(基礎工センター)の協力を得て、WSPEEDI-IIを用いて放射性物質の大気拡散予測計算を実施し、予測結果を原子力規制庁に提出する。本報告書は、北朝鮮地下核実験対応に特化するために基礎工センターで開発され、平成25年(2013年)2月から平成29年(2017年)9月までに実施された3回の地下核実験対応に使用されたWSPEEDI-II自動計算システムの支援・研修センターへの移管と整備について記述する。また、移管・整備した自動計算システムに関するその後の保守と運用について説明するとともに、北朝鮮地下核実験対応における今後の課題について記述する。, When North Korea has carried out nuclear tests, Nuclear Emergency Assistance and Training Center (NEAT) predicts atmospheric dispersion of radionuclides by using the WSPEEDI-II upon requests from Nuclear Regulation Authority (NRA) and submits the predicted results to NRA in cooperation with Nuclear Science and Engineering Center (NSEC). This is a part of the activity of NEAT supporting the Japanese Government in emergency responses. The WSPEEDI-II automatic calculation system specialized for responses to nuclear tests by North Korea was developed by NSEC and was used for responses to three nuclear tests from February 2013 to September 2017. This report describes the transfer and installation of the calculation system to NEAT, and the subsequent maintenance and operation. Future issues for responses to nuclear tests are also described in this report.

原子力機構は、災害対策基本法及び武力攻撃事態対処法に基づく指定公共機関として、原子力災害や放射線緊急事態が発生した場合には、災害対応に当たる国や地方公共団体の要請に応じて人的・技術的支援を行う。防災基本計画及び原子力災害対策マニュアルでは、原子力機構は原子力緊急時において公衆の被ばく線量の推計・把握を支援することが要求されている。しかし、その支援について、基本方策,調査対象,調査方法,実施体制等について具体的かつ詳細には検討されていない。本報告では、公衆の緊急時被ばく線量の推計・把握に関する技術的支援について、原子力緊急時支援・研修センター内に設置された「緊急時の線量評価検討WG」において調査・考察した結果を報告することにより、国や地方公共団体、及び原子力機構内における今後の具体的かつ詳細な検討及び活動に貢献することを目的とする。, In the case of a nuclear accident or a radiological emergency, the Japan Atomic Energy Agency (JAEA), as a designated public corporation assigned in the Disaster Countermeasures Basic Act and the Armed Attack Situation Response Law, undertakes technical supports to the national government and local governments. Abstract

Therapeutic plasma exchange (PLEX) is an adjunctive treatment for patients with anti-neutrophil cytoplasmic antibody (ANCA)-associated vasculitis and kidney involvement. Little is known about the effect of PLEX on early changes in kidney function. This post-hocanalysis of the PEXIVAS trial investigated the effects of PLEX on changes in kidney function within 12 months. PEXIVAS was a randomized controlled trial recruiting 691 patients with ANCA-associated glomerulonephritis, of whom 349 underwent PLEX and 342 received no-PLEX. The primary outcomes of this post hocstudy of PEXIVAS were change in estimated glomerular filtration rate (eGFR) from baseline and recovery of kidney function (defined as eGFR increase of 15ml/min/1.73m2or more). Baseline eGFR was 21.7 ± 20.3 and 20.6 ± 18.7 ml/min/1.73m2in the PLEX and no-PLEX groups, respectively. Mean improvements in eGFR at weeks two, four, and eight after initiation of therapy were greater for the PLEX vs. the no-PLEX groups. The greatest significant difference in recovery of kidney function in the PLEX compared to the no-PLEX groups was at week four (relative risk (RR): 1.41; 95% confidence interval:1.09-1.82). Increased eGFR or recovery of kidney function at week four were significantly associated with lower risk for end-stage kidney disease at week 52 (RR: 0.96: 0.95-0.97, and RR: 0.29: 0.16-0.52; respectively). Neither changes in eGFR nor recovery of kidney function differed by reduced- compared to standard-dose glucocorticoid group. Overall, our study indicates that PLEX improves early kidney function in patients with ANCA-associated glomerulonephritis.

Abstract

北朝鮮が地下核実験を実施した際には、原子力緊急時支援・研修センター(支援・研修センター)は、原子力規制庁からの要請に基づき、国による対応への支援活動として、原子力基礎工学研究センター(基礎工センター)の協力を得て、WSPEEDI-IIを用いて放射性物質の大気拡散予測計算を実施し、予測結果を原子力規制庁に提出する。本報告書は、北朝鮮地下核実験対応に特化するために基礎工センターで開発され、平成25年(2013年)2月から平成29年(2017年)9月までに実施された3回の地下核実験対応に使用されたWSPEEDI-II自動計算システムの支援・研修センターへの移管と整備について記述する。また、移管・整備した自動計算システムに関するその後の保守と運用について説明するとともに、北朝鮮地下核実験対応における今後の課題について記述する。, When North Korea has carried out nuclear tests, Nuclear Emergency Assistance and Training Center (NEAT) predicts atmospheric dispersion of radionuclides by using the WSPEEDI-II upon requests from Nuclear Regulation Authority (NRA) and submits the predicted results to NRA in cooperation with Nuclear Science and Engineering Center (NSEC). This is a part of the activity of NEAT supporting the Japanese Government in emergency responses. The WSPEEDI-II automatic calculation system specialized for responses to nuclear tests by North Korea was developed by NSEC and was used for responses to three nuclear tests from February 2013 to September 2017. This report describes the transfer and installation of the calculation system to NEAT, and the subsequent maintenance and operation.
