Kök kanal tedavisi sonrası restorasyonların uzun dönem başarısı için kalan diş dokusunun korunması, kök çevresinde ferrule etkinin korunması ve uygulanan kök kanal postunun adezyonu en etkili faktörler olarak kabul edilmiştir. Adeziv sistem kullanılarak yapıştırılan fiber postların uzun dönem başarısı, postların kök kırığını azaltan doğal davranışına bağlanmaktadır. Metal postlarda karşılaşılan en yaygın başarısızlık kök kırığı olmasına rağmen fiber postlarda karşılaşılan başarısızlık ise post desimantasyonudur. Fiber postların adezyonunda kök kanalının şekli, post boşluğunun preparasyonu, post translusentliği ve kullanılan adeziv siman tipi endodontik tedaviyle ilgili bir takım faktörlerdir. Fiber post yapıştırılmasında en güvenilir sonuçlar total-etch adezivlerin dual-cure rezin simanlar ile kombinasyonlarından elde edilmektedir. Çalışmalarda fiber postların metal postlar ve tercihen kanal tedavili dişlerde kullanılan zirkon postlar gibi diğer diş renkli postlar için alternatif olarak kullanılabileceğini göstermiştir. Bu derlememizde güncel post sistemlerinden fiberle güçlendirilmiş kompozit postlar, zirkon postlar ve polietilen fiber postların içeriği, kök kanalına ve dentine bağlantısı ve bu postlar ile yapılmış klinik çalışmalar değerlendirilmiştir. Sonuçta bu postların metal postlara alternatif olarak klinikte rutinde kullanılabileceği tavsiye edilmektedir. Uzun süreli klinik çalışmalar bu güncel post sistemlerinin dayanımları hakkında daha iyi bir fikir verecektir, Protection of the remaining tooth structure for long term success after root canal treatment, preserving the ferrule effect around the root and adhesion of root canal post are considered as the most important factors. Long term success of fiber posts bonded with adhesive systems is associated with the bio-mimetic nature of posts reducing vertical fractures. While the most encountered reason of failure in metal posts is root fracture, post decementation is also in fiber posts. Root canal morphology, preparation of post space, translucency of post and the adhesive cement type are some of the many factors affecting adhesion of fiber posts. the most reliable results in fiber post cementation are obtained with dual cure resin cements combined with acid-etched, rinsed adhesives. Silanization following silica coating takes a great place in post cementation. It was shown in studies that fiber posts could be used as an alternative to metal posts and zirconia posts used in root canal treated teeth like other tooth coloured posts. Composite posts among contemporary fiber-reinforced post systems, zirconia posts and content of polyethylene fiber posts, their adhesion to root canal and dentine and studies performed about these posts were evaluated in this review

The aim of this study was to evaluate the validity of the Nolla method for dental age estimation in western Turkish children. This study consisted of 688 orthopantomograms of patients of western Turkish children aged between 7 and 17 years. Dental maturity was evaluated according to the stages proposed by Nolla. A paired t-test was used for statistical analysis. The mean difference between the chronological and dental ages ranged from -1 to 0.13 years for males and from -1.15 to 0.25 years for females. Aim: The aim of this in vitro study was to assess the bond strength of three different self-adhesive resin cements on the post and dentin surface. Material and Methods: In the present study, 15 extracted human maxillary canine teeth with single and straight roots were used. After the clinical crowns were removed from cemento-enamal junction, root canals were prepared with Reciproc R25 (VDW GmbH, Munich, Germany) files. The smear layer of roots were removed using 17% EDTA followed by 5.25% NaOCl and distillate water. Then all the root canals were obturated with lateral compaction technique using epoxy resin based sealer and gutta-percha. Then post space was prepared. The roots were then randomly divided into three groups according to resin cements. Fiber posts was cemented, using three different resin cement : Bifix SE (Voco GmbH,Cuxhaven, Germany), Breeze (Pentron,Wallingford,USA) , RelyX U 200 (3M ESPE,Neuss,Germany). The roots were sliced to 1mm-thick horizontal sections from the coronal and mid thirds (n: 5 root 4 slice = 20 specimen), and the push-out bond strength test applied. The data was calculated as MPa and statistically analyzed using one-way ANOVA and post-hoc Tukey test.Results: There was no statistically significant difference between the Bifix SE, and the Breeze. There was a statistically significant difference between RelyX U 200 and the other resin cements.Conclusion: The toughest of three tested resin cement was RelyX U 200 in comparison to push out bond strength. There was no difference between the bond strengths of Bifix SE and Breeze., Amaç: Bu in vitro çalışmanın amacı, üç farklı self-adeziv rezin simanın post ve dentin yüzeyine karşı bağlanma dayanımlarını push out test yöntemi ile değerlendirmektir. Gereç ve Yöntem: Bu in vitro çalışmada tek köklü ve tek kanallı 15 adet üst çene kesici dişleri kullanıldı. Kronlar mine sement sınırından uzaklaştırıldıktan sonra kök kanalları Reciproc R25 eğelerle (VDW GmbH, Munich, Germany) genişletildi. Kanal dolumu öncesinde smear tabakası % 17 EDTA akabinde % 5,25 NaOCl ve distile su kullanılarak uzaklaştırıldı. Kök kanalları lateral kompaksiyon tekniği ile epoksi rezin içerikli kanal patı ve güta-perka kullanılarak dolduruldu. Daha sonra post boşlukları hazırlandı. Kökler, kullanılan rezin simana göre rastgele 3 gruba ayrıldı (n=5). Fiber postlar, üç farklı rezin siman kullanılarak simante edildi: Bifix SE (Voco GmbH, Cuxhaven, Germany), Breeze (Pentron, Wallingford, USA) ve RelyX U 200 (3M ESPE, Neuss, Germany). Bütün köklerin koronal ve orta üçlü kısmından 1 mm kalınlığında horizontal kesitler alındı (n: 5 kök * 4 kesit = 20 örnek) ve push out bağlanma dayanımı testi uygulandı. Bütün veriler MPa'a çevrildi ve istatistiksel analizler tek yönlü ANOVA ve post hoc Tukey testleri kullanılarak yapıldı. Bulgular: Bifix SE ve Breeze arasında istatiksel olarak anlamlı bir fark bulunmazken, Rely X U 200 ve diğer rezin simanlar arasında istatiksel olarak anlamlı fark bulundu.Sonuç: Test edilen üç rezin siman arasında bağlanma dayanımı açısından en dayanıklı olanı RelyX U 200 idi. Bifix SE ve Breeze arasında bağlanma dayanımları arasında fark yoktur.

Endodontik tedavi uygulaması enfekte kök kanallarından, pulpa, nekrotik doku ve bakterilerin temizlenmesini içerir. Bu işlemler doku çözücü ve antibakteriyel özelliklere sahip irrigasyon solüsyonları ile desteklenmiş kanal enstrümanları ile yapılır. Bunun için sodyum hipoklorit, salin, klorheksidin glukonat gibi farklı irrigasyon solüsyonları kullanılmaktadır ve yeni solüsyon arayışı da devam etmektedir.Kekik içerdiği timol ve karvakrol sayesinde antiseptik, güçlendirici ve uyarıcı etkisi vardır. Timol ya da kekik kâfuru virüs ve bakterileri uzaklaştırır. Ulaşılabilir kaynaklarda, süt ve daimi insan dişi veya herhangi bir deney hayvanı dişleri üzerinde, kekik suyunun kök kanal irrigasyon solüsyonu olarak kök kanallarında smear tabakası ve mikroorganizmalara karşı etkisini inceleyen herhangi bir in vivo veya in vitro çalışmaya rastlanılmamıştır.Bu in vitro çalışmanın amacı; %1, %2 ve %5'lik kekik bitkisi (O. minutiflorum) suyunun kök kanal irrigasyon ve dezenfeksiyonunda kullanılarak, kök kanalında oluşan smear tabakasına etkisini SEM (Tarayıcı Elektron Mikroskobu) ile değerlendirmek, sitotoksisitesinin ve kök kanalında persiste kalan E. faecalis'e etkisinin incelenmesi amaçlanmaktadır.Bu çalışma için 15 grupta toplam 180 insan dişi kullanıldı. İlk 7 grupta antimikrobiyal inceleme yapıldı. Diğer 8 grupta irrigasyon solüsyonlarının smear tabakasına etkisi incelendi. 15 mm kök uzunluğu kalacak şekilde kuronlar uzaklaştırıldı. ProTaper marka döner aletler ile kök kanalları genişletildi. Dişlerin otoklavda sterilizasyonu sonrası 4 hafta inkübe edildi. Kök kanallarından üçer tane kâğıt kon ile irrigasyon öncesi ve sonrası örnek alındı. Daha sonsa ProTaper F4 ve F5 serisi döner aletler ile dentinden örnek alındı. Alınan örnekler 48 saat kanlı agarda inkübe edilerek CFU sayımı yapıldı ve verilerin istatistiksel değerlendirmesi yapıldı. (Kruskall-Wallis, Mann-Whitney U testi, P=0,05). Kullanılan solüsyonların sitotoksisite testi ise WST-1 analizi kullanılarak yapıldı. Sitotoksisite testi sonuçlarını değerlendirmek için ANOVA ve Duncan's Multiple Range testi kullanıldı (P=0,05).İrrigasyon sonrası kanal içinden alınan sonuçlara göre CHX, %5 KS ve %2 KS'nin E. faecalis'e karşı antibakteriyel etkinlikleri arasında istatistiksel olarak anlamlı bir fark bulunmamıştır. %1 KS NaOCl ile benzer antimikrobiyal etki gösterirken, EDTA ve salinden iyi olduğu görülmüş ancak CHX kadar başarılı bulunmamıştır. Dentinden alınan sonuçlara göre ise en etkili solüsyon CHX' dir.1 saatlik sitotoksisite sonuçlarına göre %1 KS ve CHX arasında istatistiksel olarak anlamlı bir fark bulunmazken, %1 KS, %0,5 KS'den kötü, KS'nin diğer konsantrasyonu ve NaOCl'den daha iyi bulunmuştur. 24 ve 72 saatlik sitotoksisite değerlendirmesinde ise tüm solüsyonlar sitotoksik bulunmuştur.Bu çalışmanın sınırları içerisinde %1 KS antimikrobiyal olarak %5,25 NaOCl ile benzer sonuçlar verirken sitotoksisite çalışmasında %5,25 NaOCl'den daha iyi bulunmuştur. Endodontic therapy includes cleaning pulp, necrotic tissue and bacteria from infected root canals. This process is done by root canal instruments which are supported by irrigation solutions which have the tissue solvent capacity and antibacterial properties. Therefore, irrigation solutions such as NaOCl, Saline, CHX have been used up to date, and the search for new solutions still continue.Oregano which contains thymol and carvacrol has an antiseptic, booster and stimulant effect. Thymol, or thyme camphor, eradicates virus and bacteria. In literature, any in vivo and in vitro study has not been encountered in primary teeth and permanent teeth or any experimental animal study of thyme solution as root canal irrigation solution affecting smear layer and microorganisms.The aim of this in vitro study is to use 1%, 2% and 5% of oregano plant (O. minutiflorum) solution in root canal irrigation and disinfection, to evaluate the effect of the smear layer by SEM (Scanning Electron Microscope), to assess its effect on the cytotoxicity and E. faecalis which has persisted in root canal.In this study, a total of 180 human teeth were used in 15 groups and antimicrobial evaluation was performed in the first 7 groups. In the other 8 groups, effect of the solutions on the smear layer was studied. The crowns were removed, and the roots were adjusted to 15 mm long. Root canals were prepared with ProTaper rotary instruments. After the teeth were sterilized in autoclave, incubated for 4 weeks. Before and after irrigation samples were taken from the root canals with three paper point. Then, dentin samples were taken with ProTaper F4 and F5 series rotary instruments. Samples were incubated for 48 hours in blood agar, and CFU were counted, and the statistical evaluation of the data was performed. (Kruskall-Wallis and Mann-Whitney U test, P = 0,05). Solutions cytotoxicity test which was used in this study was performed using WST-1 analysis. ANOVA and Duncan's Multiple Range test was used to evaluate cytotoxicity test results (P = 0,05).According to the results obtained in the root canal after irrigation, CHX, 5% and 2% Oregano extract (OE) weren?t found to be statistically significant regarding their antibacterial activities against E. faecalis. 1% OE and NaOCl showed similar antimicrobial effect, and 1% OE and NaOCl were better than EDTA and saline but not as successful as CHX. According to the results obtained from dentin, CHX is the most effective solution.According to the results of 1-hour cytotoxicity test, no statistically significant difference was found between 1% OE and CHX. 1% OE was found worse than 0,5% KS but better than other concentrations of OE and NaOCl. In 24 and 72-hour cytotoxicity evaluations, all solutions were found cytotoxic.Within the limitations of this study, while 1% KS and 5,25% NaOCl showed similar results in terms of antimicrobial effect, 1% KS was found better than 5,25% NaOCl in cytotoxicity study. 1% KS is a highly effective irrigation solution in eradicating E. faecalis from root canal.Key Words: Origanum solution, CHX, NaOCl, EDTA, Salin, Cyctotoxicity, Smear layer, Antimicrobial effect. 149
