Here, we explored the potential impact of HCC etiology on SBRT efficacy. Tumor control probability (TCP) models were established to estimate the likelihood of local control (LC).Methods: Data from 415 patients who were treated with SBRT for HCC were reviewed. Cox proportional hazards models were used to identify key predictors of LC. TCP models accounting for biologic effective dose (BED) and tumor diameter were generated to quantify associations between etiology and LC.Results: Cox models demonstrated that hepatitis C virus (HCV) infection was associated with favorable LC following SBRT (HR=0.52, 95% CI 0.04– 0.96, p=0.036). The 2-year LC rate for patients with HCV etiology was 88%, compared to 78% for other patients. Small tumor and high BED were also associated with favorable LC. TCP models demonstrated a 10– 20% absolute increase in predicted LC across the range of SBRT doses and tumor sizes.Conclusion: We found a novel association between HCV status and LC after SBRT for HCC that warrants further exploration. Real-world data on efficacy and safety of FCM is limited in India.Methods: A retrospective, observational and real-world study was conducted to assess the efficacy and safety of FCM in adolescents and adults with IDA across 269 centers in India. Data was retrieved from medical records of patients who received FCM for management of IDA. Physicians’ clinical assessment of efficacy and safety of FCM was also assessed. Data were analyzed for hematological parameters at baseline and at 4 ± 1 week for study population, and for severity of anemia.Results: In 1800 patients with IDA, intravenous FCM resulted in a significant increase in hemoglobin (Hb) of 2.76 g/dL, serum ferritin of 35.85 μg/L, red blood cell (RBC) count, hematocrit, mean corpuscular volume (MCV), and mean corpuscular hemoglobin (MCH) (P < 0.001 for all) at 4 ± 1 week as compared to baseline. In subjects with severe IDA, an increase in Hb was of 3.31 g/dL, serum ferritin increased of 35.84 μg/L, RBC count, hematocrit and MCH improved significantly (P < 0.001 for all). In subjects with moderate IDA, Hb (increase of 2.63 g/dL), serum ferritin (increase of 35.92 μg/L), RBC count, hematocrit, MCV, and MCH improved significantly (P < 0.001 for all). In subjects with mild IDA, only the mean Hb values at 4 weeks were significantly higher (P < 0.001; increased by 1.89 g/dL). Physicians rated efficacy of FCM as very good to good in 97.5% of patients. Similarly, safety of FCM was rated very good to good in 97.2% subjects.Conclusion: FCM efficiently, safely and quickly corrects moderate-to-severe anemia in Indian patients in a short span of 4 weeks. Assessments by clinicians (eg, performance status, toxicity grade) and patients (eg, quality of life) play a critical role in current practice but have significant limitations. Technological advances now provide an opportunity to track a new class of objective measures of patient activity, such as daily step counts. Here, we describe recent efforts to incorporate this technology into the field of oncology. Design: We conducted a structured literature search using MEDLINE electronic database to identify published observational studies of tracking steps in cancer patients and trials of exercise programs for cancer survivors incorporating pedometers until February 2016. Results: Data indicate that physical activity information may supplant existing scales for the assessment of cancer patients’ functional capacity. Conclusion: Objective activity monitoring is poised to revolutionize the way health care providers assess cancer patients at the time of diagnosis, during treatment, and in the survivorship setting. Initial reports from the S elective P ersonalized R adioimmunotherapy for Locally Advanced N SCLC T rial (SPRINT) indicated that patients with high PD-L1 expression can be treated safely and effectively with immunotherapy and radiotherapy (RT). Here, we compare patient-reported outcomes (PROs) from SPRINT to PROs from patients who were treated with standard concurrent chemoradiotherapy on contemporary prospective trials to test the hypothesis that the SPRINT approach yields favorable patient-reported outcomes. SPRINT participants with PD-L1 tumor proportion score ≥ 50% were treated with "PembroRT": three 21-day cycles of induction pembrolizumab, a four-week course of risk-adapted and de-intensified RT (55 Gy to tumors or lymph nodes measuring 20 cc or larger and 48 Gy to smaller lesions, all in 20 fractions and without concurrent chemotherapy), and up to 13 cycles of consolidation pembrolizumab. SPRINT participants with lower PD-L1 expression and participants on the control arm of NCT03481114 were treated with "ChemoRT": a standard RT dose of 60 Gy in 30 fractions with concurrent weekly carboplatin and paclitaxel and standard adjuvant therapy (e.g., durvalumab). Participants in both trials completed a 12-item PRO-CTCAE survey at baseline, every two weeks during RT, and at follow-up visits. Study participants who completed the planned RT course and completed at least two PRO-CTCAE surveys were included in this analysis. Peak PRO-CTCAE grades recorded up to 90 days after RT completion were compared across treatment cohorts using Kruskal-Wallis testing. Multivariable logistic regression models were utilized to evaluate predictors of grade 2+ patient-reported toxicity. Kaplan-Meier curves and log rank testing were utilized to compare progression-free survival (PFS) and overall survival (OS) across treatment cohorts. The PembroRT cohort included 19 patients, and the ChemoRT cohort included 26 patients. Mean esophagus and lung doses were significantly lower in the PembroRT cohort, with median values approximately 5 Gy lower than in the ChemoRT group. Average PRO-CTCAE grades were numerically higher in the ChemoRT group for all 12 symptoms, with statistically significant differences observed for cough, wheezing, dermatitis, dizziness, anxiety, and depression. Multivariable models demonstrated that patient-reported toxicity was generally not associated with chemotherapy receipt, while several patient-reported adverse events were associated with organ at risk doses. For example, grade 2+ dysphagia incidence was associated with mean esophageal dose. The PembroRT cohort had superior 3-year rates of PFS (61% v. 23%, P = 0.033) and OS (88% v. 49%, P = 0.013). Compared to standard chemoradiotherapy, the SPRINT treatment approach, which utilizes induction immunotherapy to reduce target volumes and risk-adapted RT, can improve patient-reported outcomes without compromising clinical outcomes.
