Objective: The purpose of this study was to photoelastically evaluate the effects of cervical root lesions and their restoration on stress distribution in periodontally compromised teeth. Method and materials: Three-dimensional composite photoelastic models of a maxillary first premolar with buccal cervical root lesions were fabricated. Two different lesion configurations, wedge- and shallow saucer-shaped, at 20% alveolar bone height reduction were tested. A 35% reduction model was given a wedge-shaped lesion. The lesions were restored with microfine resin composite. Vertical loads of 7.5 lbs were applied to the unrestored and restored models at the tip of the buccal cusp and the tip of the lingual cusp. The resulting stresses within the tooth models were monitored and recorded photographically in the field of a circular polariscope arrangement. Results: For buccal cusp loading of the unrestored models, stress concentrated at the apex of the lesion regardless of the lesion shape or periodontal support conditions. The highest stress concentration was observed around the apex of the wedge-shaped lesion on the 35% reduction model. Restoring the lesions changed stress distribution. Restoration of the lesions resulted in a marked stress reduction at the lesion apex. Stress along the gingival restoration-model interface was characteristic for the restored situation. The interfacial stress followed the contour of the restoration most closely for the wedge-shaped lesion on the 20% support reduction model. Conclusion: The shape and dimension of the lesion as well as periodontal support status has considerable influence on stress distribution, especially around the lesion, restored or not restored. PET Scans are most often used for detecting cancer and other malignant tumors because for most of them, glucose uptake is higher than in normal tissues. However, in mesothelioma, our study showed excessive deposits of Asbestos of 0.5mg BDORT units, and markedly reduced Glucose uptake of less than 1/30 of normal tissue. As a consequence, the authors found that, in the location where there is a mesothelioma, distinctive dark black areas much darker than any normal tissue appear in the PET Scan. Therefore, a PET Scan taken parallel to the front & back of the chest wall often shows pitch-black areas on the chest wall of the patient, located on the ribs in the case of large black areas and between the ribs in the case of small black areas. If the amount of Asbestos in these areas is found to be very high in the same location where the glucose uptake is very low, one can suspect the presence of mesothelioma, which is often found at the inner wall of the chest cavity or the peritoneum in the abdomen with significantly increased Osteopontine (about 350-400 times that of normal tissue) and very significant increase in Gastrin Releasing Peptides (more than 1200 times that of normal tissue). In some patients, the only abnormal blood chemistry detected was abnormally increased Pro-Gastrin Releasing Peptide. This dark black area on the PET Scan image taken parallel to front and back of chest wall (with marked increase in asbestos, Gastrin-Releasing Peptide, & Osteopontine and marked decrease in glucose uptake in the pathological tissue) can be considered a characteristic finding for the diagnosis of mesothelioma. Study Design: This case report includes the results of long-term follow-up after extended total sacrectomy in a 13-year-old boy with a sarcoma originating in the sacrum with an extraskeletal extension and infiltration into the left ilium.Objective: To report and discuss a case of sacral tumor treated by extended sacrectomy.Summary Of Background Data: Sacral tumors are often at an advanced stage with a large volume at diagnosis. Although total or extended sacrectomy is the only radical means to treat the massive sacral tumor, unavoidable complications in total sacrectomy are serious in the treatment selection.Methods: Initial histologic findings indicated a synovial sarcoma. Additional genetic analysis redesignated the tumor as an unclassified sarcoma. Preoperative neoadjuvant chemotherapy and radiotherapy were completed. The response to the preoperative treatment appeared as a reduction in tumor size (approximately 50%) on radiographs. After extended sacrectomy, the L5 vertebral body was fixed between the ilia, and the pelvic ring was compressed by the Zielke system. The ISOLA instrumentation system connected the lumbar spine and both ilia. All sacral nerve roots and the L5 root on the left side were cut.Results: At the 5-year follow-up examination, the patient was disease-free, could walk with crutches, and could climb stairs using the handrail and one crutch.Conclusions: The patient's excellent response to preoperative antitumor treatment was considered crucial to the long-term outcome. But the decision between a radical resection with reconstruction and a less extensive procedure with combined therapy remains controversial. [ABSTRACT FROM AUTHOR]

Aims: The increasing prevalence of AmpC β-lactamase (AmpC)- and extended-spectrum β-lactamase (ESBL)- producing food pathogens is a serious public health concern. AmpC- and ESBL-producing Salmonella species pose a high risk of food contamination. This study aimed to investigate changes in the prevalence of Salmonella among food handlers in Japan from 2006 to 2021 using 100 randomly selected isolates from 2006, 2012, 2018, and 2021 with different serotypes and antimicrobial resistance patterns., Methods and Results: The average Salmonella isolation rate was 0.070% (19 602/27 848 713). Serotyping revealed that the most common serotypes were Enteritidis in 2006, Infantis in 2012, Agoueve/Cubana in 2018, and Schwarzengrund in 2021. Antimicrobial susceptibility testing showed that Salmonella isolates exhibited the highest resistance to streptomycin (<40%), followed by tetracycline (<20%-40%). Moreover, 6% of the Salmonella isolates produced cephalosporinases with the blaCMY-2, blaCTX-M-14, and blaTEM genes. The annual incidence of cephalosporin resistance has increased. Plasmid conjugation assays revealed that cephalosporin-resistant Salmonella spp. transmitted their resistance to Escherichia coli. Additionally, plasmid genome analysis showed that the insertion sequence IS26 was encoded in the upstream and downstream regions of blaCTX-M-14 and qnrS1 in the IncHI1 plasmid, which could be transmitted to other bacteria., Conclusions: The tested Salmonella isolates showed high resistance to specific antibiotics, with differences in resistance depending on the serotype. Plasmid-mediated antibiotic-resistant bacteria's transmission is fatal and a major threat to public health. This study aimed to clarify the presence of plasmid-mediated quinolone resistance（PMQR）genes in extended-spectrum β-lactamase（ESBL）-producing or/and mcr-harbouring colistin（COL）-resistant Escherichia coli（ESBL-COL-EC）isolates from Vietnamese and Japanese chicken meat. Resistance towards ciprofloxacin（CIP）was examined in 308 ESBL-COL-EC isolates; CIP-resistant ESBL-COL-EC isolates were examined for the PMQR gene. Approximately, 71.1% and 38.1% of ESBL-COL-EC and ESBLproducing E. coli isolates from Vietnamese and Japanese chicken meat were CIP-resistant, respectively. Multiplex PCR led PMQR detection showed that 35.2% of CIP-resistant ESBL-COL-EC isolates from Vietnamese food contained PMQR gene, whereas CIP-resistant ESBL-COL-EC isolates from Japanese chicken meat did not. Conjugation assays showed that the transmission of qnrS gene carried by E. coli to Salmonella. In conclusion, ESBL-COL-EC isolates from Vietnamese food are associated with a high frequency of fluoroquinolone resistance and a high distribution of the qnrS gene.

Salmonella enterica SE20-C72-2 and Escherichia coli EC20-C72-1 were isolated from the edible fish Anabas testudineus in Vietnam. The chromosomes and plasmids from both strains were sequenced using Oxford Nanopore and Illumina sequencing. We demonstrate a master oscillator power amplifier (MOPA) system that emits single-frequency high-energy optical pulses at 1540 nm using an Er,Yb:glass planar waveguide amplifier and a large mode area Er-doped fiber amplifier. A double under-cladding and a 50-µm-thick core structure are employed for the planar waveguide amplifier to increase the output energy without degrading the beam quality. A pulse energy of 45.2 mJ with a peak power of 27 kW is generated at a pulse repetition rate of 150 Hz with a pulse duration of 1.7 µs. Moreover, the beam quality factor M 2 of the output beam is 1.84 at the highest pulse energy thanks to its waveguide structure.

Purpose: Self-reported measures have been widely used to indicate the presence of possible and probable sleep bruxism (SB) in both research and clinical situations. However, few studies have attempted to assess the diagnostic validity of this approach. The aim of this study was to estimate the diagnostic validity of self-reported measures of SB using an ambulatory single-channel electromyographic (EMG) device., Methods: A total of 115 participants were enrolled and examined by standardized Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD) including two questions related to SB: self-reported SB and morning-jaw symptoms. An ambulatory single-channel EMG device (GrindCare3™, Medotech A/S) was used for measuring jaw-muscle EMG activity during sleep for seven consecutive nights. Cut-off values for different measures of EMG activity (average, maximum and minimum) and the coefficient of variation (CV) were selected to divide participants into two groups, with higher or lower EMG activity or CV values. The sensitivity and specificity for each question and combination of them were calculated., Results: Self-reported SB had the highest sensitivity (compared with morning-jaw symptoms) for all measures of EMG activity and CV, although the values were low to modest (average: 76.0%, maximum: 76.9%, minimum: 77.3%, CV: 61.0%). The specificity was low for both the questions related to the different measures of EMG activity and CV (35.1-52.4%)., Conclusions: This study indicated that the diagnostic validity of self-reported measures of SB was low to modest using an ambulatory EMG device assessment as a reference. We adopted this technique for a rib fracture by bending the plate into a U-shape and fixing it with suture through the holes in the mesh of the plate and holes that are drilled in the edge of the fractured rib. Sclerostin, a secreted protein encoded by the Sost gene, is produced by osteocytes and is inhibited by osteoblast differentiation and bone formation. Recently, a functional association between bone and fat tissue has been suggested, and a correlation between circulating sclerostin levels and lipid metabolism has been reported in humans. However, the effects of sclerostin on adipogenesis remain unexplored. In the present study, we examined the role of sclerostin in regulating adipocyte differentiation using 3T3-L1 preadipocytes. In these cells, sclerostin enhanced adipocyte-specific gene expression and the accumulation of lipid deposits. Sclerostin also upregulated CCAAT/enhancer binding protein β expression but not cell proliferation and caspase-3/7 activities. Sclerostin also attenuated canonical Wnt3a-inhibited adipocyte differentiation. Recently, the transcriptional modulator TAZ has been involved in the canonical Wnt signaling pathway. Sclerostin reduced TAZ-responsive transcriptional activity and TAZ-responsive gene expression. Transfection of 3T3-L1 cells with TAZ siRNA increased the lipid deposits and adipogenic gene expression. These results show that sclerostin upregulates adipocyte differentiation in 3T3-L1 cells, suggesting a possible role for the osteocyte-derived sclerostin as a regulator of fat metabolism and as a reciprocal regulator of bone and adipose tissues metabolism., (© 2015 Wiley Periodicals, Inc.)

Background: The traditional view contends bruxism, such as tooth grinding/clenching, is part of the etiology of temporomandibular disorders (TMD) including some subtypes of headaches. The purpose of this study is to investigate if a low-level but long-lasting tooth-clenching task initiates TMD symptoms/signs., Methods: Eighteen healthy participants (mean age ± SD, 24.0 ± 4.3 years) performed and repeated an experimental 2-hour tooth-clenching task at 10% maximal voluntary occlusal bite force at incisors (11.1 ± 4.6 N) for three consecutive days (Days 1-3). Pain and cardiovascular parameters were estimated during the experiment., Results: The task evoked pain in the masseter/temporalis muscles and temporomandibular joint after 40.0 ± 18.0 minutes with a peak intensity of 1.6 ± 0.4 on 0-10 numerical rating scale (NRS) after 105.0 ± 5.0 minutes (Day 1). On Day 2 and Day 3, pain had disappeared but the tasks, again, evoked pain with similar intensities. The onset and peak levels of pain were not different between the experimental days (P = .977). However, the area under the curve of pain NRS in the masseter on Day 2 and Day 3 were smaller than that on Day 1 (P = .006). Cardiovascular parameters changed during the task but not during the days., Conclusions: Prolonged, low-level tooth clenching evoked short-lived pain like TMD. This intervention study proposes that tooth clenching alone is insufficient to initiate longer lasting and self-perpetuating symptoms of TMD, which may require other risk factors., (© 2015 American Headache Society.)

The aim of this study was to investigate the effect of tunnel structured β-tricalcium phosphate (β-TCP) on the regenerative potential of basic fibroblast growth factor-2 (bFGF-2) in class III furcation defects in dogs. The furcations of 30 mandibular premolar teeth received: 1) 0.3% bFGF-2 solution in conjunction with β-TCP; 2) 0.3% bFGF-2 solution; and 3) no implant material (Control group). The dogs were sacrificed 8 weeks post-surgery, and healing was evaluated histologically. New bone formation was significantly greater in the bFGF-2/β-TCP group compared to the bFGF-2 solution and Control groups (p<0.01). New cementum formation in the bFGF-2/β-TCP and bFGF-2 solution groups was significantly greater than that in the Control group (p<0.01). These findings suggested that bFGF-2 alone enhances connective tissue attachment in a manner similar to the combination of bFGF-2 and β-TCP. Furthermore, this combination enhances bone formation up to the fornix in class III furcation defects.
