Objective: The canal wall up bony obliteration technique lowers the incidence of recurrent cholesteatoma, but carries the potential risk of obliterating residual cholesteatoma. The objective of this study was to report long-term follow-up radiological findings after performing a canal wall up bony obliteration technique procedure, in order to detect residual and/or recurrent cholesteatoma. Patients: Fifty-one patients presenting with a cholesteatoma or a troublesome cavity were operated upon using the canal wall up bony obliteration technique, and were evaluated by follow-up imaging a mean of 76.4 months post-operatively (range, 53.8-113.6 months). Intervention: All patients were evaluated with high resolution computed tomography and magnetic resonance imaging (including delayed contrast, T1-weighted imaging and non-echo-planar, diffusion-weighted imaging). Results: Imaging revealed the presence of one residual, one recurrent and one congenital petrosal apex cholesteatoma. On high resolution computed tomography, completely obliterated mastoid filled with bone was observed in 74.5 per cent (38/51) of patients, and an aerated middle-ear cavity in 64.7 per cent (33/51). High resolution computed tomography clearly detected any associated soft tissue present in the middle-ear cavity (18/51) and in the obliterated mastoids (13/51), but could not characterise this tissue. Non-echo-planar, diffusion-weighted magnetic resonance imaging clearly identified all three cholesteatomas, and differentiated them from other associated soft tissues. No cholesteatoma was found within the obliterated mastoids. Conclusion: Long-term follow up indicated that the canal wall up bony obliteration technique is a safe method with which to treat primary and recurrent cholesteatoma and to reconstruct unstable cavities. Soft tissue was found quite often in the middle ear and obliterated mastoids. High resolution computed tomography identified its presence but could not further characterise it. However, non-echo-planar, diffusion-weighted magnetic resonance imaging succeeded in differentiating soft tissues, enabling detection of residual or recurrent cholesteatoma after a canal wall up bony obliteration technique procedure. Purpose: The objective of this study was to assess the value of high-resolution computed tomography (HRCT) and that of magnetic resonance imaging (MRI), including postcontrast T(1)-weighted images and echo-planar diffusion-weighted (EP-DW) images, in the detection of residual cholesteatomas after primary bony obliteration of the mastoid. Patients and Methods: Twenty-three patients underwent a second-look surgery 8 to 18 months after they underwent a primary bony obliteration technique. All patients were evaluated by HRCT and MRI before their second-look surgery. A retrospective analysis was performed. Results: A residual cholesteatoma was found in 2 of the 23 patients; both cases of cholesteatoma had a diameter less than 4 mm. In these 2 patients, residual cholesteatoma was found in the middle ear cavity and not in the obliterated mastoid. In all cases, HRCT showed a homogeneous obliteration of the mastoid cavity. On MRI, only one cholesteatoma pearl was detected using contrast-enhanced T(1)-weighted imaging. Findings from the EP-DW imaging were negative for all cases. Conclusion: This study demonstrates that HRCT is still the imaging technique of choice for the evaluation of bony obliterated mastoids. It shows the low sensitivity and specificity of HRCT for the characterization of an associated opacified middle ear and those of contrast-enhanced T(1)-weighted imaging and EP-DW imaging for the detection of small residual cholesteatomas after primary bony obliteration. Objectives: There is increasing evidence in the literature that cochlear implantation (CI) in patients with a vestibular schwannoma is a reasonable extended indication of C.I and should be given the preference over an ABI. It can be performed simultaneouslu with the tumor removal (during translabyrinthine surgery) or in a staged procedure (after retro-sigmoïd or middle fossa surgery) and when the auditory could be anatomically preserved but not the hearing function. Material and methods: We report our first experience in 10 vestibular schwannoma patients: 7 solitary schwannomas and three NF2 cases. Results and conclusions: Three patients had an intralabyrinthine schwannoma removed with simultaneous CI implantation, one patient had a CI one year after VS removal and space holder implantation. Four patients had a space holder implantation after tumor removal. One patient had a CI implantation while leaving the tumor in place. Two patients had a combined ABI and CI implantation (called ABCI). The auditory results after cochlear implantation vary widely from excellent open set speech perception to no auditory benefit at all (non-user). The individual results will be presented and compared with data from the literature. This expanded indication for CI could profoundly influence our management of vestibular schwannomas in the future. The objective of this research was to test whether efficient tinnitus suppression could be achieved by electrical stimulation of the single most basal electrode contact of a cochlear implant. This approach simulates the effects of electrical stimulation using a round-window electrode. The study was performed in 10 adult cochlear implant patients showing complete or almost complete tinnitus suppression during electrical stimulation with their standard fitting-MAP. In all patients, tinnitus appeared again when the implant was switched off. Five Nucleus implant (1 CI532, 4 CI24RE CA) users and 5 Mi12xx series with FLEX28 electrodes with at least 6 months of CI experience were included. Two types of stimulation were presented at the most basal CI contact: a constant pulse train and a modulated pulse train. The variation in pulse rates was low rate (100-300 pps) and high (≥900 pps), and the current level ranged from the C-level to less than the T-level for both stimulation types. The effect of acute electrical stimulation at the most basal electrode contact was compared to the effect obtained with multichannel stimulation with the patient's current fitting MAP. Electrical stimulation was paused between tests with different stimulation types until tinnitus returned to baseline intensity. Patients reported Visual Analog Scale (VAS) scores for tinnitus loudness and intrusiveness during normal CI use and for each single contact stimulation type. Eight participants perceived complete suppression with one or more stimulation patterns. In 2 patients, suppression was less efficient than full-band CI stimulation. Louder stimuli are generally perceived as annoying and less effective in reducing tinnitus. In FLEX28 patients, it was also possible to obtain full tinnitus suppression with current amplitudes under the thresholds for auditory perception (this was not tested in patients with the Nucleus device). In 8 of the 10 included patients, we were able to obtain complete or almost complete tinnitus suppression with electrical stimulation at only 1 most basal electrode contact. Therefore, round-window stimulation with a single electrode may be a potential treatment for tinnitus in patients with significant residual hearing. The long-term effects of this therapy should be confirmed in future studies.

This paper provides a step-by-step guide for organizing the scientific program (OSP) of international conferences. Through informal discussions, a panel of experts organizing international conferences came up with this guide, which includes a flowchart, checklist, and detailed discussions of each step. Subsequently, additional specialists were invited to evaluate this synopsis and provide their input. All of the participants approved the final version after the outline was improved. This guide proposes the following six steps: 1) preparation, 2) recruitment, 3) building the agenda, 4) cross-checking the program, 5) reviewing and finalizing, and 6) in-conference refining. Thirteen items are specified across the six main steps in a detailed checklist. This OSP guide includes a flowchart and a checklist for providing a comprehensive manual for establishing, conducting, and organizing international scientific conferences. Understanding the procedures that are expected to be followed when holding a scientific conference enables the involved parties to organize and assign tasks to one another as well as create a schedule that allows them to finish their work on time.
