Abstract

We investigated mu+ decays at rest produced at the ISIS beam stop target. Lepton flavor (LF) conservation has been tested by searching for \nueb via the detection reaction p(\nueb,e+)n. No \nueb signal from LF violating mu+ decays was identified. We extract upper limits of the branching ratio for the LF violating decay mu+ -> e+ \nueb \nu compared to the Standard Model (SM) mu+ -> e+ nue numub decay: BR < 0.9(1.7)x10^{-3} (90%CL) depending on the spectral distribution of \nueb characterized by the Michel parameter rho=0.75 (0.0). These results improve earlier limits by one order of magnitude and restrict extensions of the SM in which \nueb emission from mu+ decay is allowed with considerable strength. Abstract

The KARMEN experiment at the spallation neutron source ISIS used \numub from \mup--decay at rest in the search for neutrino oscillations \numubnueb in the appearance mode, with p(\nueb,e+)n as detection reaction of \nueb. In total, 15 candidates fulfill all conditions for the \nueb signature, in agreement with the background expectation of 15.8+-0.5 events, yielding no indication for oscillations. A single event based likelihood analysis leads to upper limits on the oscillation parameters: sin^2(2theta)<1.7x10e-3 for Dm^2>100 eV^2 and Dm^2<0.055 eV^2 for sin^2(2theta)=1 at 90% confidence. Abstract

The KARMEN experiment uses the reaction 12C(\nu_e,e-)12N to measure the energy distribution of \nu_e emitted in muon decay at rest. The \nu_e analog \omega_l of the famous Michel parameter \rho has been derived from a maximum-likelihood analysis of events near the kinematic end point, E_max. The result, \omega_l = (2.7^{+3.8}_{-3.3}\pm 3.1) * 10^{-2}, is in good agreement with the standard model prediction \omega_l= 0. We deduce a 90% confidence upper limit of \omega_l <= 0.113, which corresponds to a limit of |g^S_RL + 2 g^T_RL| <= 0.78 on the interference term between scalar and tensor coupling constants., Comment: 11 pages including 3 figures, submitted to Phys. Abstract

The 56 tonne high resolution liquid scintillation calorimeter KARMEN at the beam stop neutrino source ISIS has been used to search for neutrino oscillations in the disappearance channel nu_e->x. The nu_e emitted in mu+ decay at rest are detected with spectroscopic quality via the exclusive charged current reaction 12-C(nu_e,e-)12-N_g.s. almost free of background. Analysis of the spectral shape of e- from the nu_e induced reaction as well as a measurement of the absolute nu_e flux allow to investigate oscillations of the type nu_e->nu_tau and nu_e->nu_mu . The flux independent ratio R(CC/NC) of charged current events 12-C(nu_e,e-)12-N_g.s. to neutral current events 12-C(nu,nu')12-C* provides additional information in the oscillation channel nu_e->x . All three analysis methods show no evidence for oscillations. For the nu_e->nu_tau channel 90%CL limits of sin^2(2t)<0.338 for dm^2>100eV^2 and dm^2<0.77eV^2 for maximal mixing in a simple 2 flavor oscillation formalism are derived. A complete 3 flavor analysis of the experimental data from five years of measurement with respect to nu_e<->nu_tau and nu_e<->nu_mu mixing is presented., Comment: 20 pages including 13 figures, submitted to Phys.Rev.C

Abstract

Background: Internet-based Cognitive Behavioural Therapy (iCBT) is found effective in treating common mental disorders. However, the use of these interventions in routine care is limited. The international ImpleMentAll study is funded by the European Union’s Horizon 2020 programme. It is concerned with studying and improving methods for implementing evidence-based iCBT services for common mental disorders in routine mental health care. A digitally accessible implementation toolkit (ItFits-toolkit) will be introduced to mental health care organizations with the aim to facilitate the ongoing implementation of iCBT services within local contexts. This study investigates the effectiveness of the ItFits-toolkit by comparing it to implementation-as-usual activities. Methods: A stepped wedge cluster randomized controlled trial (SWT) design will be applied. Over a trial period of 30 months, the ItFits-toolkit will be introduced sequentially in twelve routine mental health care organizations in primary and specialist care across nine countries in Europe and Australia. Repeated measures are applied to assess change over time in the outcome variables. The effectiveness of the ItFits-toolkit will be assessed in terms of the degree of normalization of the use of the iCBT services. Several exploratory outcomes including uptake of the iCBT services will be measured to feed the interpretation of the primary outcome. Data will be collected via a centralized data collection system and analysed using generalized linear mixed modelling. A qualitative process evaluation of routine implementation activities and the use of the ItFits-toolkit will be conducted within this study. Abstract

The KARMEN experiment at the ISIS spallation source has enhanced its sensitivity for νμ → νe oscillations in 1996 by the installation of an additional large area veto counter system. Consisting of 136 highly transparent plastic scintillator modules, the new veto is completely embedded inside the experiment's 7000 tonne steel blockhouse. It allows to eliminate the main background component in the search for νμ → νe appearance originating from high energy neutrons induced by deep inelastic scattering of cosmic ray muons in the massive shielding. First measurements after the detector upgrade show a substantial reduction of this background meeting the expected factor of 40. If no oscillation signal is seen after 3 years of measuring (1997–1999) KARMEN will be able to exclude mixing amplitudes of sin22Θ≃1x10-3. The ongoing KARMEN2 measurements will thus allow a decisive and reliable test of the entire parameter space favoured by the positive result of LSND. An even higher oscillation sensitivity can be achieved by experiments at future high intensity short spill proton accelerators such as the planned European Spallation Source ESS.

23. Patupilone (Epothilone B) for recurrent glioblastoma: Clinical outcome and translational analysis of a single-institution phase I/II trial

Abstract

Background: Patients with glioblastoma (GBM) inevitably develop recurrent or progressive disease after initial multimodal treatment and have a median survival of 6-9 months from time of progression. To date, there is no accepted standard treatment for GBM relapse or progression. Patupilone (EPO906) is a novel natural microtubule-stabilizing cytotoxic agent that crosses the blood-brain barrier and has been found to have preclinical activity in glioma models. Methods: This is a single-institution, early-phase I/II trial of GBM patients with tumor progression who qualified for second surgery with the goal of evaluating efficacy and safety of the single-agent patupilone (10 mg/m(2), every 3 weeks). Patients received patupilone 1 week prior to second surgery and every 3 weeks thereafter until tumor progression or toxicity. Primary end points were progression-free survival (PFS) and overall survival (OS) at 6 months as well as patupilone concentration in tumor tissue. Secondary end points were toxicity, patupilone concentration in plasma and translational analyses for predictive biomarkers. Results: Nine patients with a mean age of 54.6 ± 8.6 years were recruited between June 2008 and April 2010. Median survival and 1-year OS after second surgery were 11 months (95% CI, 5-17 months) and 45% (95% CI, 14-76), respectively. Median PFS was 1.5 months (95% CI, 1.3-1.7 months) and PFS6 was 22% (95% CI, 0-46), with 2 patients remaining recurrence-free at 9.75 and 22 months. At the time of surgery, the concentration of patupilone in tumor tissue was 30 times higher than in the plasma. Tumor response was not predictable by the tested biomarkers. Treatment was generally well tolerated with no hematological, but cumulative, though reversible sensory neuropathy grade ≤3 was seen in 2 patients (22%) at 8 months and grade 4 diarrhea in the 2nd patient (11%). Abstract

BACKGROUND: To study whether and how c-MYC expression determines response to radio- and chemotherapy in childhood medulloblastoma (MB). METHODS: We used DAOY and UW228 human MB cells engineered to stably express different levels of c-MYC, and tested whether c-MYC expression has an effect on radio- and chemosensitivity using the colorimetric 3-(4,5-dimethylthiazol-2-yl)-5-(3-carboxymethoxyphenyl)-2-(4-sulfophenyl)-2H-tetrazolium inner salt (MTS) assay, clonogenic survival, apoptosis assays, cell cycle analysis, and western blot assessment. In an effort to validate our results, we analyzed c-MYC mRNA expression in formalin-fixed paraffin-embedded tumor samples from well-documented patients with postoperative residual tumor and compared c-MYC mRNA expression with response to radio- and chemotherapy as examined by neuroradiological imaging. RESULTS: In DAOY - and to a lesser extent in UW228 - cells expressing high levels of c-MYC, the cytotoxicity of cisplatin, and etoposide was significantly higher when compared with DAOY/UW228 cells expressing low levels of c-MYC. Irradiation- and chemotherapy-induced apoptotic cell death was enhanced in DAOY cells expressing high levels of c-MYC. The response of 62 of 66 residual tumors was evaluable and response to postoperative radio- (14 responders (CR, PR) vs. 5 non-responders (SD, PD)) or chemotherapy (23 CR/PR vs. 20 SD/PD) was assessed. c-MYC mRNA expression was similar in primary MB samples of responders and non-responders (Mann-Whitney U test, p = 0.50, ratio 0.49, 95% CI 0.008-30.0 and p = 0.67, ratio 1.8, 95% CI 0.14-23.5, respectively). CONCLUSIONS: c-MYC sensitizes MB cells to some anti-cancer treatments in vitro. As we failed to show evidence for such an effect on postoperative residual tumors when analyzed by imaging, additional investigations in xenografts and larger MB cohorts may help to define the exact function of c-MYC in modulating response to treatment.

Abstract

BackgroundAn increasing number of studies suggest that web-based interventions for patients with depression can reduce their symptoms and are expected to fill currently existing treatment gaps. However, evidence for their efficacy has mainly been derived from comparisons with wait-list or treatment as usual controls. In particular, designs using wait-list controls are unlikely to induce hope and may even have nocebo effects, making it difficult to draw conclusions about the intervention’s efficacy. Studies using active controls are rare and have not yielded conclusive results. ObjectiveThe main objective of this study is to assess the acute and long-term antidepressant efficacy of a 6-week, guided, web-based self-management intervention building on the principles of cognitive behavioral therapy (iFightDepression tool) for patients with depression compared with web-based progressive muscle relaxation as an active control condition. MethodsA total of 348 patients with mild-to-moderate depressive symptoms or dysthymia (according to the Mini International Neuropsychiatric Interview) were recruited online and randomly assigned to 1 of the 2 intervention arms. Acute antidepressant effects after 6 weeks and long-term effects at 3-, 6-, and 12-month follow-up were studied using the Inventory of Depressive Symptomatology–self-rating as a primary outcome parameter and change in quality of life (Short Form 12) and user satisfaction (client satisfaction questionnaire) as secondary outcome parameters.
