Abstract

Background Coproduction is defined as patients and clinicians collaborating equally and reciprocally in healthcare and is a crucial concept for quality improvement (QI) of health services. Learning Health Networks (LHNs) provide insights to integrate coproduction with QI efforts from programmes from various health systems.Objective We describe interventions to develop and maintain patient and family partner (PFP) coproduction, measured by PFP-reported and programme-reported scales. We aim to increase percentage of programmes with PFPs reporting active QI work within their programme, while maintaining satisfaction in PFP-clinician relationships.Methods Conducted in the Cystic Fibrosis Learning Network (CFLN), an LHN comprising over 30 cystic fibrosis (CF) programmes, people with CF, caregivers and clinicians cocreated interventions in readiness awareness, inclusive PFP recruitment, onboarding process, partnership development and leadership opportunities. Interventions were adapted by CFLN programmes and summarised in a change package for existing programmes and the orientation of new ones. We collected monthly assessments for PFP and programme perceptions of coproduction and PFP self-rated competency of QI skills and satisfaction with programme QI efforts. We used control charts to analyse coproduction scales and run charts for PFP self-ratings.Results Between 2018 and 2022, the CFLN expanded to 34 programmes with 52% having ≥1 PFP reporting active QI participation. Clinicians from 76% of programmes reported PFPs were actively participating or leading QI efforts. PFPs reported increased QI skills competency (17%–32%) and consistently high satisfaction and feeling valued in their work.Conclusions Implementing system-level programmatic strategies to engage and sustain partnerships between clinicians and patients and families with CF improved perceptions of coproduction to conduct QI work. Key adaptable strategies for programmes included onboarding and QI training, supporting multiple PFPs simultaneously and developing financial recognition processes. Interventions may be applicable in other health conditions beyond CF seeking to foster the practice of coproduction.

Eps15 (epidermal growth factor receptor pathway substrate 15) homology domain-containing proteins (EHDs) comprise a family of eukaryotic dynamin-related ATPases that participate in various endocytic membrane trafficking pathways. Dysregulation of EHDs function has been implicated in various diseases, including cancer. The lack of small molecule inhibitors which acutely target individual EHD members has hampered progress in dissecting their detailed cellular membrane trafficking pathways and their function during disease. Here, we established a Malachite green-based assay compatible with high throughput screening to monitor the liposome-stimulated ATPase of EHD4. In this way, we identified a drug-like molecule that inhibited EHD4's liposome-stimulated ATPase activity. Structure activity relationship (SAR) studies indicated sites of preferred substitutions for more potent inhibitor synthesis. Moreover, the assay optimization in this work can be applied to other dynamin family members showing a weak and liposome-dependent nucleotide hydrolysis activity.

8. Aflatoxin contamination in groundnut and maize food products in Eastern and Northern Uganda

Abstract

AbstractGroundnut and maize are among the economically important crops grown widely in Uganda for household food and income security. However, those crops and their products are vulnerable to aflatoxigenic fungi and aflatoxin contamination. The present study sought to establish the levels of aflatoxin in the different products of groundnut and maize in local markets and roadside retail shops. A cross-sectional survey was conducted in ten districts in the eastern and northern regions of Uganda. Groundnut products: paste, unshelled, shelled/grain, and flour/powder were collected; and maize products: cobs (covered and uncovered), grain, flour, bran, mixed feed, flour reject, and roasted/boiled maize were also collected. A total of 241 samples (133 groundnut and 108 maize samples) were collected from the community and analysed for aflatoxin contamination using ELISA test. Regardless of the source, 41.8% of groundnuts had aflatoxin levels higher than 20 parts per billion (ppb), with groundnut paste (196.52 ± 437.24 ppb) and flour (187.90 ± 289.95 ppb) being the most contaminated. Aflatoxin levels in 62.8% of maize products were higher than 20 ppb, with maize cobs having the highest levels (126.4 ppb). Groundnut and maize products from the eastern region were the most contaminated. These results indicate that most of the maize and groundnut products in the markets away from major urban centres are also highly contaminated and unsafe for food and feed. This calls for immediate action to develop mitigation measures to curb the impact of aflatoxin contamination on the health and income of households, particularly through sensitization and training on appropriate pre-and post-harvest handling practices of the products.

Abstract

There is limited information on the phytochemical profile and antioxidant activity of ginger and garlic consumed in Uganda. This could have an impact on its widespread use and industrial application. Thus, this study was done to determine the phytochemical profile and antioxidant activity of two varieties of ginger and garlic commonly consumed in Uganda. Fresh ginger rhizomes and garlic cloves of ''local'' and ''hybrid'' varieties were acquired from a local food market, washed, grated, and extracted using acetone, ethanol, methanol, and water. Standard techniques were used to determine the phytochemical composition. Total phenolic and flavonoid content were measured using Folin-Ciocalteu and aluminium chloride assays, respectively. Antioxidant activity was determined using the 2, 2-Diphenyl-1-picryl hydrazyl (DPPH) assays. Ginger extracts exhibited significantly higher total phenolic and flavonoid content compared to garlic (p˂0.05). The highest total phenolic and flavonoid content was in ethanol and methanol extracts of local ginger: 1968.49 and 2172.65 mg GAE/100 g; 254.24 and 184.62 mg QE/100 g, respectively. Tannins, alkaloids, saponins, and terpenoids were in varying concentrations in the extracts. Levels of Vitamin C were significantly high in aqueous extracts (p˂0.05), 38.34 and 40.80 AAE/100 g in local and hybrid ginger; 33.65 and 35.24 mg AAE/100 g in local and hybrid garlic, respectively. The free radical scavenging activity of extracts varied depending on concentration, with a strong positive correlation between antioxidant activity and total phenolic and flavonoid content. The half maximal inhibitory concentration (IC50) ranged from 0.16 to 8.93 mg/ml in local ginger, 4.43–6.44 mg/ml in hybrid ginger, 3.93–5.64 mg/ml in local garlic, and 4.44–5.27 mg/ml in hybrid garlic. The best antioxidant activity was exhibited by ethanol extracts of the local ginger. According to the findings, the two varieties of ginger and garlic have strong antioxidant activity due to their different phytochemical compositions, which could make them useful as natural antioxidants in food and medicine applications.

Six parks, nearly 500 hours of observation, more than 60 interviews, just one question: what do young people aged 16 to 25 do at night in Montreal? While often placed in opposition to the civilized and legitimate life of day, the night is defined from two angles: on the one hand, as a moment of accentuated insecurity and vulnerability for some, on the other hand, as a moment of freedom and festive explorations. Abstract

Chronic lymphocytic leukemia (CLL) is a clinically and biologically heterogeneous disease with varying outcomes. In the last decade, the application of next-generation sequencing technologies has allowed extensive mapping of disease-specific genomic, epigenomic, immunogenetic, and transcriptomic signatures linked to CLL pathogenesis. These technologies have improved our understanding of the impact of tumor heterogeneity and evolution on disease outcome, although they have mostly been performed on bulk preparations of nucleic acids. As a further development, new technologies have emerged in recent years that allow high-resolution mapping at the single-cell level. These include single-cell RNA sequencing for assessment of the transcriptome, both of leukemic and non-malignant cells in the tumor microenvironment; immunogenetic profiling of B and T cell receptor rearrangements; single-cell sequencing methods for investigation of methylation and chromatin accessibility across the genome; and targeted single-cell DNA sequencing for analysis of copy-number alterations and single nucleotide variants. In addition, concomitant profiling of cellular subpopulations, based on protein expression, can also be obtained by various antibody-based approaches. In this review, we discuss different single-cell sequencing technologies and how they have been applied so far to study CLL onset and progression, also in response to treatment. This latter aspect is particularly relevant considering that we are moving away from chemoimmunotherapy to targeted therapies, with a potentially distinct impact on clonal dynamics. We also discuss new possibilities, such as integrative multi-omics analysis, as well as inherent limitations of the different single-cell technologies, from sample preparation to data interpretation using available bioinformatic pipelines. Finally, we discuss future directions in this rapidly evolving field.

Abstract

Introduction: Osteogenesis imperfecta (OI) is a hereditary disorder primarily caused by mutations in type I collagen genes, resulting in bone fragility, deformities, and functional limitations. Studies on upper extremity deformities and associated functional impairments in OI are limited. This cross-sectional study aimed to evaluate upper extremity deformities and functional outcomes in OI. Methods: We included patients regardless of their OI subtypes with a minimum age of 7 years. Radiographic analysis of radial head dislocation, ossification of the interosseous membrane, and/or radioulnar synostosis of the forearm were performed, and deformity was categorized as mild, moderate, or severe. Clinical evaluation was performed using the Quick Disabilities of Arm, Shoulder, and Hand (qDASH) questionnaire and shoulder-elbow-wrist range of motion (ROM). Three-dimensional motion analysis of the upper limb was conducted using the Southampton Hand Assessment Procedure (SHAP). The SHAP quantifies execution time through the Linear Index of Function (LIF) and assesses the underlying joint kinematics using the Arm Profile Score (APS). Additionally, the maximum active Range of Motion (aRoM) was measured. Results: Fourteen patients aged 8 to 73 were included. Radiographic findings revealed diverse deformities, including radial head dislocation, interosseous membrane ossification, and radioulnar synostosis. Six patients had mild, six moderate, and two severe deformities of the upper extremity. Severe deformities and radial head dislocation correlated with compromised ROM and worse qDASH scores. The qDASH score ranged from 0 to 37.5 (mean 11.7). APS was increased, and LIF was reduced in OI-affected persons compared with non-affected peers. APS and LIF also varied depending on the severity of bony deformities. aRoM was remarkably reduced for pro-supination. Conclusion: Patients with OI showed variable functional impairment from almost none to severe during daily life activities, mainly depending on the magnitude of deformity in the upper extremity. Abstract

At universities, we observe a great range of variation in previous knowledge and decreasing mathematical competences of students -- partly due to less limited qualification rules at universities in Germany. To address the challenges which arise by teaching very heterogeneous student groups we use e-tutorials as an addition to traditional classroom teaching. We show how virtual 3D rooms should be constructed for the purpose of e-tutorials. Based on our experience we discuss advantages and limitations using virtual 3D rooms in university teaching.

Abstract

Companies purchase computer systems to make their processes more efficient through automation. Some academic medical centers (AMC) have purchased computer systems for their institutional review boards (IRB) to increase efficiency and compliance with regulations. IRB computer systems are expensive to purchase, deploy, and maintain. An AMC should expect a positive return on investment from the purchase of an IRB system. An IRB system should decrease the processing time for IRB applications and affect the number of IRB staff members necessary to adequately handle IRB processes. This study examined these and other factors at AMCs to determine the value of IRB computer systems.

Abstract

The Turkish Constitutional Court demonstrates the resistance-deference paradox as a pattern in its judicial behavior under autocratic pressure. The docket management strategies including prioritization and late responsiveness are also employed in politically sensitive cases. The deferring stances of the Court legitimize autocratization when core issues of the regime are at stake. In these cases, the Court develops an autocratic partnership that makes itself an unreliable actor without any commitment to judicial ethos. The resistant stances of the Court trigger the political backlash and clashes with the judiciary, leading to further contestation of political autocratization.

Abstract

The current state of antimicrobial resistance to synthetic antimicrobial drugs has led to renewed interest in natural antimicrobial compounds. Antimicrobial activity of extracts of (local and hybrid) ginger and garlic was investigated using the agar well diffusion method against Staphylococcus aureus, Escherichia coli and Candida albicans. Aqueous and organic solvent extracts of both varieties of ginger and garlic exhibited varied and concentration-dependant antimicrobial activity. Inhibition zones at 25 mg/mL varied significantly against the microorganisms, being highest on C. albicans; 18.00 ± 2.00 to 30.67 ± 1.16 mm for acetone extracts and raw juice of hybrid ginger and 19.67 ± 1.16 to 30.33 ± 1.53 mm for methanol and raw extracts of local garlic respectively. Minimum Inhibitory Concentration ranged from 2.5 to 10 mg/mL in garlic extracts. The study concluded that both varieties of ginger and garlic possess antimicrobial substances, though ginger is more potent as antifungal agent.

Abstract

We describe the formation, characterization and theoretical understanding of microlenses comprised of alternating polystyrene and polymethylmethacrylate layers produced by multilayer coextrusion. These lenses are fabricated by photolithography, using a grayscale mask followed by plasma etching, so that the refractive index alternation of the bilayer stack appears across the radius of the microlens. The alternating quarter-wave thick layers form a one-dimensional photonic crystal whose dispersion augments the material dispersion, allowing one to sculpt the chromatic dispersion of the lens by adjusting the layered structure. Using Huygen's principle, we model our experimental measurements of the focal length of these lenses across the reflection band of the multilayer polymer film from which the microlens is fashioned. Abstract

The present study proposes a method for designing small bioactive nanoparticles using silk fibroin as a carrier to deliver hydrophobic polyphenols. Quercetin and trans-resveratrol, widely distributed in vegetables and plants, are used here as model compounds with hydrophobic properties. Silk fibroin nanoparticles were prepared by desolvation method and using various concentrations of ethanol solutions. The optimization of the nanoparticle formation was achieved by applying Central Composite Design (CCD) and the response surface methodology (RSM). The effects of silk fibroin and ethanol solution concentrations together with the pH on the selective encapsulation of phenolic compounds from a mixture were reported. The obtained results showed that nanoparticles with an average particle size of 40 to 105 nm can be prepared. The optimized system for the selective encapsulation of the polyphenols on the silk fibroin substrate was determined to be 60% ethanol solution and 1 mg/mL silk fibroin concentration at neutral pH. The selective encapsulation of the polyphenols was achieved, with the best results being obtained in the case of resveratrol and quercetin and encapsulation of gallic and vanillic acids being rather poor. Thin-layer chromatography confirmed the selective encapsulation and the loaded silk fibroin nanoparticles exhibited antioxidant activity.

Abstract

Eps15 (epidermal growth factor receptor pathway substrate 15) homology domain-containing proteins (EHDs) comprise a family of eukaryotic dynamin-related ATPases that participate in various endocytic membrane trafficking pathways. Dysregulation of EHDs function has been implicated in various diseases, including cancer. The lack of small molecule inhibitors which acutely target individual EHD members has hampered progress in dissecting their detailed cellular membrane trafficking pathways and their function during disease. Here, we established a Malachite green-based assay compatible with high throughput screening to monitor the liposome-stimulated ATPase of EHD4. In this way, we identified a drug-like molecule that inhibited EHD4's liposome-stimulated ATPase activity. Structure activity relationship (SAR) studies indicated sites of preferred substitutions for more potent inhibitor synthesis. Moreover, the assay optimization in this work can be applied to other dynamin family members showing a weak and liposome-dependent nucleotide hydrolysis activity. [ABSTRACT FROM AUTHOR]
