Background: Characterization of seafood allergens is important to understand the immune response to these allergens. Moreover, a detailed comparison between atopic and nonatopic patients with adverse reactions to shrimp has never been reported. Methods: Raw and boiled shrimp extracts were analyzed by immunoblotting using sera from 9 atopic and 7 nonatopic patients with a history of adverse reactions to shrimp, and 13 control subjects. Total IgE, specific IgE and skin prick tests (SPT) to shrimp were also investigated. Results: The level of specific IgE to shrimp was higher in atopic patients than nonatopic patients (p < 0.05). Symptoms, SPT results and major allergens involved were similar in atopic and nonatopic patients. The 16.5-kD protein had the highest frequency of IgE binding followed by the 40-kD protein in these patients. Other minor IgE-binding proteins were observed at the 20-, 22-, 54-, 72-, 129- and 140-kD regions. Patients who had binding to the 16.5-kD protein had either positive (25% raw/31% cooked) or negative (13% raw/cooked) CAP-FEIA-RAST, while patients who recognized the 40-kD protein all had positive (31% raw/19% cooked) CAP-FEIA-RAST. All control subjects had negative immunoblots for these two proteins. Conclusion: The 16.5-kD protein was the most frequent protein identified regardless of CAP-FEIA-RAST results, while the 40-kD protein was only present in patients with positive CAP-FEIA-RAST. Therefore, 16.5-kD protein may be an important allergen that is clinically relevant in both atopic and nonatopic patients with adverse reactions to shrimp even if it is not detected by the CAP-FEIA-RAST system. Objective: To examine the causes of sequelae or death of patients who presented them subsequently to seizures that developed during theophylline administration., Methods: Among 334 patients who had been examined in Study (I), we extracted 56 who had sequelae or died subsequently to the onset of seizures and examined their causes., Results: Many patients had fever at the onset of seizures, many patients had neurologic predispositions prior to seizure onset, and only one patient was found to be free from fever and neurologic findings. At the onset of seizures, most of these patients had blood theophylline concentrations that were within or inferior to the target range (5-15 microg/mL)., Conclusions: We presume it necessary to pay heed to fever and neurologic predispositions in order to prevent sequelae or death that may occur subsequently to seizures that developed during theophylline administration.

Objectives: To discover features of patients with theophylline toxicosis among patients who developed convulsions during theophylline administration., Methods: Fifteen patients, whose measured or estimated blood theophylline concentration at the time of convulsion development was >/=40 microg/mL, were extracted out of 334 patients who were reported between October 1987 and April 2004 and who developed convulsions during theophylline administration (oral theophylline: 255 patients; intravenous theophylline: 79 patients). Patient background, state of drug administration, blood theophylline concentration, presence or absence of status epilepticus, and outcome were examined., Results: Young children and children with fever were predominant, and eight of 15 patients had both features. Furthermore, the dose exceeded the currently recommended target dose in most of patients whose given dose is known. Patients with misdispensing and patients having problems with administration and ingestion methods were included in them. Outcomes included one case of death, two cases of sequelae, 11 cases of recovery or alleviation, and 1 case of unknown. The patient who died took large doses of theophylline, and both of two patients who had sequelae had fever associated with an infection., Conclusions: Our study revealed 1) heed should be given to the age of the affected child and to theophylline administration when fever is present in order to avoid theophylline toxicosis and 2) many patients with the toxicosis (11/15) present a relatively favorable prognosis.

Objective: In recent years, attempts have been made to broadly deem seizures developing during theophylline administration as theophylline-related seizures. Consequently, a tendency to consider all the seizures attributable to theophylline has occurred. We investigated background factors in pediatric patients with seizures to examine the involvement of theophylline in the event., Methods: Of adverse events developing during theophylline administration which were reported by two theophylline preparation manufacturers between October 1987 and April 2004, we investigated underlying disease, regimen, outcome, and other items in 255 patients with seizures provoked by oral preparations and in 79 patients with seizures provoked by intravenous preparations., Results: The most predominant factors were represented by neurological predispositions, e.g., epilepsy, and by a history of fits prior to the onset of seizures in not less than two-thirds of pediatric patients with seizures who were receiving theophylline. Furthermore, not less than 90% of pediatric patients who remained with sequelae and who died had the factors. Our investigation revealed that the factors developed predominantly when the relevant patients presented fever, aged under 3 years and had history of seizures., Conclusions: Therefore, we consider that advertence to this point is most important in administering theophylline safely.

Purpose: Sick house syndrome (SHS; sick house syndrome) presents healthy damage owing to the indoor environment of a building. Although SHS is recognized socially, there is no medical definition and its illness concept is also ambiguous. This time, large-scale epidemiology investigation about SHS of our country was conducted., Method: The investigation paper in the "epidemiological research on SHS" group of the Ministry of Health, Labour and Welfare was used. The reply of 8737 adults and 9387 children was obtained., Result: By the definition of a disease, prevalence of SHS was 8.5 to 22.1% at the adult and 5.6-19.8% in the child. Cause environmental substances were "a shampoo, makeup and a perfume", "the smell of a wall or the building materials of a floor", and "paint". There was no significant difference in SHS development of symptoms in construction years and the situation of extension and alteration of a dwelling. As a feature of a life style, symptoms developed in the strong group of stress and sensitive to a smell developed mostly. CONSIDERATION: Prevalence differed greatly by how SHS is defined. In order to diagnose truer SHS, we think that the prudent definition based also on the international standard is required.

A retrospective study was conducted on 28 patients with bacterial meningitis who were admitted to our department between April 1988 and March 2002. The most commonly detected pathogen was group B Streptococcus in those under 1 month of age and Haemophilus influenzae (72.2%) among those over 1 month. The most commonly administered antibiotic combination (67.9%) at the initial treatment was that of cefotaxime (CTX) and ampicillin (ABPC). We encountered one case that was resistant to both CTX and ABPC. Through this experience, it became apparent that for the initial treatment of bacterial meningitis in infants, it is necessary to apply a combination of two antibiotics, instead of a single agent, and new antibiotics should be considered for such combinations rather than persisting on conventional CTX and ABPC. The aforementioned 28 patients were divided into 2 groups--7 patients (25.0%) with sequelae and 21 (75.0%) without--and various factors noted during the diagnosis were evaluated retrospectively. It was found that the number of days leading to admission at the hospital and the development of convulsions were unrelated to the prognosis. Those who succumbed or suffered sequelae were all infants under 1 year of age. All cases were caused by genus Haemophilus.

Human basophils may play a role in bronchial asthma. The effect of pranlukast on histamine release and LTC4 generation from human peripheral basophils was examined. Histamine release induced by FMLP and C5a was inhibited by pranlukast in concentration-dependent manner, whereas anti-IgE Ab-induced histamine release was not affected. Both anti-IgE Ab-and FMLP-induced LTC4 were inhibited by pranlukast. These results suggest that pranlukast may improve symptoms of bronchial asthma by inhibiting basophil function in addition to antagonizing Cys-LT receptor.
