Fluorescence guidance is routinely used in surgery to enhance perfusion contrast in multiple types of diseases. Pressure-enhanced sensing of tissue oxygenation (PRESTO) via fluorescence is a technique extensively analyzed here, that uses an FDA-approved human precursor molecule, 5-aminolevulinic acid (ALA), to stimulate a unique delayed fluorescence signal that is representative of tissue hypoxia. The ALA precontrast agent is metabolized in most tissues into a red fluorescent molecule, protoporphyrin IX (PpIX), which has both prompt fluorescence, indicative of the concentration, and a delayed fluorescence, that is amplified in low tissue oxygen situations. Applied pressure from palpation induces transient capillary stasis and a resulting transient PRESTO contrast, dominant when there is near hypoxia. This study examined the kinetics and behavior of this effect in both normal and tumor tissues, with a prolonged high PRESTO contrast (contrast to background of 7.3) across 5 tumor models, due to sluggish capillaries and inhibited vasodynamics. This tissue function imaging approach is a fundamentally unique tool for real-time palpation-induced tissue response in vivo, relevant for chronic hypoxia, such as vascular diseases or oncologic surgery. Diffuse optical imaging probes deep living tissue enabling structural, functional, metabolic, and molecular imaging. Recently, due to the availability of spatial light modulators, wide-field quantitative diffuse optical techniques have been implemented, which benefit greatly from structured light methodologies. Such implementations facilitate the quantification and characterization of depth-resolved optical and physiological properties of thick and deep tissue at fast acquisition speeds. We summarize the current state of work and applications in the three main techniques leveraging structured light: spatial frequency-domain imaging, optical tomography, and single-pixel imaging. The theory, measurement, and analysis of spatial frequency-domain imaging are described. Then, advanced theories, processing, and imaging systems are summarized. Preclinical and clinical applications on physiological measurements for guidance and diagnosis are summarized. General theory and method development of tomographic approaches as well as applications including fluorescence molecular tomography are introduced. Lastly, recent developments of single-pixel imaging methodologies and applications are reviewed. [ABSTRACT FROM AUTHOR]

Significance: Fluorescence sensing within tissue is an effective tool for tissue characterization; however, the modality and geometry of the image acquisition can alter the observed signal., Aim: We introduce a novel optical fiber-based system capable of measuring two fluorescent contrast agents through 2 cm of tissue with simple passive electronic switching between the excitation light, simultaneously acquiring fluorescence and excitation data. The goal was to quantify indocyanine green (ICG) and protoporphyrin IX (PpIX) within tissue, and the sampling method was compared with wide-field surface imaging to contrast the value of deep sensing versus surface imaging., Approach: This was achieved by choosing filters for specific wavelengths that were mutually exclusive between ICG and PpIX and coupling these filters to two separate detectors, which allows for direct swapping of the excitation and emission channels by switching the on-time of each excitation laser between 780- and 633-nm wavelengths., Results: This system was compared with two non-contact surface imaging systems for both ICG and PpIX, which revealed that the fluorescence depth sensing system was superior in its ability to resolve kinetics differences in deeper tissues that would normally be dominated by strong signals from skin and other surface tissues. Specifically, the system was tested using pancreatic adenocarcinoma tumors injected into murine models, which were imaged at several time points throughout tumor growth to its ∼ 6 - mm diameter. This demonstrated the system's capability to track longitudinal changes in ICG and PpIX kinetics that result from tumor growth and development, with larger tumors showing sluggish uptake and clearance of ICG, which was not observable with surface imaging. Similarly, PpIX was quantified, which showed slower kinetics over different time points, and was further compared with the wide-filed imager. These results were further validated through depth measurements in tissue phantoms and model-based interpretation., Conclusion: This fluorescence depth sensing system can be used to sample the interior blood flow characteristics by ICG sensing of tissue as deep as 20 mm into the tissue with sensitivity to kinetics that are superior to surface imaging and may be combined with other imaging modalities such as ultrasound to provide guided deep fluorescence measurements., (© 2024 The Authors.)

Widefield illumination and detection strategies leveraging structured light have enabled fast and robust probing of tissue properties over large surface areas and volumes. However, when applied to diffuse optical tomography (DOT) applications, they still require a time-consuming and expert-centric solving of an ill-posed inverse problem. Deep learning (DL) models have been recently proposed to facilitate this challenging step. Herein, we expand on a previously reported deep neural network (DNN) -based architecture (modified AUTOMAP - ModAM) for accurate and fast reconstructions of the absorption coefficient in 3D DOT based on a structured light illumination and detection scheme. Furthermore, we evaluate the improved performances when incorporating a micro-CT structural prior in the DNN-based workflow, named Z-AUTOMAP. This Z-AUTOMAP significantly improves the widefield imaging process's spatial resolution, especially in the transverse direction. The reported DL-based strategies are validated both in silico and in experimental phantom studies using spectral micro-CT priors. Precision medicine promises to improve therapeutic efficacy while reducing adverse effects, especially in oncology. However, despite great progresses in recent years, precision medicine for cancer treatment is not always part of routine care. Indeed, the ability to specifically tailor therapies to distinct patient profiles requires still significant improvements in targeted therapy development as well as decreases in drug treatment failures. In this regard, preclinical animal research is fundamental to advance our understanding of tumor biology, and diagnostic and therapeutic response. Most importantly, the ability to measure drug-target engagement accurately in live and intact animals is critical in guiding the development and optimization of targeted therapy. However, a major limitation of preclinical molecular imaging modalities is their lack of capability to directly and quantitatively discriminate between drug accumulation and drug-target engagement at the pathological site. Recently, we have developed Macroscopic Fluorescence Lifetime Imaging (MFLI) as a unique feature of optical imaging to quantitate in vivo drug-target engagement. MFLI quantitatively reports on nanoscale interactions via lifetime-sensing of Förster Resonance Energy Transfer (FRET) in live, intact animals. Hence, MFLI FRET acts as a direct reporter of receptor dimerization and target engagement via the measurement of the fraction of labeled-donor entity undergoing binding to its respective receptor. MFLI is expected to greatly impact preclinical imaging and also adjacent fields such as image-guided surgery and drug development., (© 2022.
