В роботі подано результати проведення бактеріологічного моніторингу 217 дітей раннього віку з м. Запоріжжя та Запорізької області, хворих на гострий обструктивний бронхіт. Встановлено, що в 53 % випадків збудником гострого обструктивного бронхіту була бактеріальна мікрофлора, серед якої провідне місце посідали Streptococcus pneumoniaе (43,5 %) та Haemophilus influenzae (33,0 %). Проведений аналіз антибіотикограм показав, що найбільшу чутливість дані мікроорганізми виявляли щодо цефалоспоринів ІІІ покоління (100 %). За результатами отриманих даних обґрунтовано доцільність застосування цефподоксиму проксетилу як стартового антибактеріального препарату в терапії гострого обструктивного бронхіту у дітей раннього віку.

BACKGROUND: Clinical studies and meta-analyzes have shown that reduced levels of 25-hydroxyvitamin D in the blood contribute to impaired immunity and stimulate excessive inflammation that adversely affects the health of children: The risk of developing bronchial asthma, obstructive bronchitis, and allergic rhinitis increases. AIM: The main purpose of this review is to summarize the present literature data on the relationship between Vitamin D, VRI, and other infectious diseases with a clinical example. MATERIALS AND METHODS: A comprehensive analysis was carried out under Vitamin D in respiratory viral infection and other diseases. RESULTS: The results of fundamental research allow us to assert that Vitamin D is fundamentally necessary for maintaining the normal physiological functioning of the immune system. Актуальність. За даними Всесвітньої організації охорони здоров’я, у структурі причин звернення до педіатра чи лікаря загальної практики — сімейної медицини близько 70 % випадків припадає на гострі респіраторні захворювання (ГРЗ). Серед дитячого населення їх частота на одну дитину становить у середньому 6–10 випадків на рік, що робить ГРЗ однією з найбільш частих інфекційних хвороб у дитячій популяції усього світу. Серед ГРЗ у дітей гострий бронхіт та гострий обструктивний бронхіт є одними з найбільш поширених захворювань. Метою нашої роботи стало вивчення ефективності застосування екстракту листя плюща в комплексній терапії гострих обструктивних бронхітів у дітей. Матеріали досліджень. Під нашим спостереженням перебували 33 дитини віком від 4 до 8 років, які проходили лікування з діагнозом: ГРВІ: гострий обструктивний бронхіт, дихальна недостатність І або II ступеня. Усі пацієнти отримували в комплексі терапії Пектолван Плющ. Результати дослідження. Комплексне лікування гострого обструктивного бронхіту в дітей із включенням препарату Пектолван Плющ сприяє розрідженню бронхіального секрету та полегшенню його відходження в середньому вже на 3-й день від початку терапії, що проявляється зменшенням інтенсивності та числа нападів кашлю, збільшенням його продуктивності та переходом сухих свистячих хрипів у різнокаліберні вологі при аускультації легень. При цьому препарат Пектолван Плющ має задовільний профіль безпеки застосування в дітей. Висновки. Проведені дослідження із застосування препарату Пектолван Плющ у комплексній терапії гострих обструктивних бронхітів показали його ефективність. Препарат може бути рекомендований до широкого застосування в педіатричній практиці.

Background. Acute obstructive bronchitis (AOB) ranks second among the respiratory diseases in children. In recent years, the results of clinical studies demonstrate a correlation between the low level of vitamin D (VD) and the risk of recurrent respiratory infections in adults and children. At the same time, the impact of VD insufficiency/deficiency and timely compensation of these changes on the frequency and severity of broncho-obstructive syndrome (BOS) in young children remains poorly understood. Thus, the purpose of the research was to study the features of VD level in young children with recurrent BOS and the efficacy of vitamin D3 supplementation on incidence and severity of the disease. Materials and methods. We examined 120 children with acute obstructive bronchitis. Group I included 60 patients with episodic BOS (up to 3 episodes per year), group II consisted of 60 children with recurrent BOS (3 or more episodes per year). The control group represented 30 healthy children of the corresponding age. We evaluated the severity of BOS and determined the serum level of 25-hydroxyvitamin D (25(OH)D) using an electrochemiluminescence method. Subsequently, the group of children with recurrent BOS was divided into two subgroups: IIa subgroup included 30 children who received vitamin D3 1000 IU daily throughout the year, IIb included 30 patients who were not supplemented with VD. The effectiveness of vitamin D3 supplementation was determined by the changes in the blood level of 25(OH)D, frequency and severity of BOS episodes. We used non-parametric methods for statistical analysis of data. For all types of analysis, the critical value of the significance level (p) was taken to be < 0.05. Results. The analysis of the clinical features of BOS episode showed that in the group of children with recurrent BOS compared to patients with episodic BOS the disease was characterized by a more severe course (23.3 % vs. 10.0 %, respectively; p = 0.034). We found that the mean VD level in children with recurrent BOS was 13.68 (7.96; 19.51) ng/ml and in children with episodic BOS 33.0 (28.19, 41.97) ng/ml (p < 0.001). Objective – to assess blood pressure indicators in children with acute obstructivebronchitis.Materials and methods. The paper presents the results of a survey of 68 childrenhospitalized for acute obstructive bronchitis. Patients were examined for clinical symptomsand severity of the clinical course according to the BSS-ped scale. BP parameters andechocardiography data were determined and compared with the results of a control groupof healthy individuals (20 children).Results. In patients, the severity of bronchitis clinical course on the fi rst day was average –according to the BSS scale – 7.79±0.19 points. BP indicators in patients were probablylower than in healthy children and had an inverse correlative dependence on the severityof the disease. Echocardiography showed a decrease in the end systolic and diastolicvolume of the left ventricle, a decrease of the pressure gradient in the mitral valve and itsincrease in the pulmonary valve.Conclusions. In acute obstructive bronchitis in children, changes in the activity of thecardiovascular system are observed due to a decrease in systemic blood pressure and anincrease of pressure in the pulmonary circulation system., Мета роботи – oцінити показники артеріального тиску в дітей із гостримобструктивним бронхітом.Матеріали і методи. У роботі представлені результати обстеження 68 дітей,госпіталізованих з приводу гострого обструктивного бронхіту. У пацієнтіввивчали клінічну симптоматику, тяжкість перебігу за шкалою BSS-ped. Визначалипоказники артеріального тиску та дані ехокардіографії, які порівнювализ результатами контрольної групи здорових осіб (20 дітей).Результати. У пацієнтів тяжкість перебігу бронхіту в першу добу буласередньою – за шкалою BSS – 7,79±0,19 балів. Показники АТ у хворих були вірогіднонижчими, ніж у здорових дітей, і мали зворотну кореляційну залежність відтяжкості хвороби. За ехокардіографією відзначено зниження показників кінцевогосистолічного та діастолічного об’єму лівого шлуночка, зниження градієнта тискуна мітральному клапані та його підвищення – на пульмональному.Висновки. При гострому обструктивному бронхіті у дітей спостерігаютьсязміни діяльності серцево- судинної системи у вигляді зниження системногоартеріального тиску та підвищення тиску у системі малого кола кровообігу.

The article presents data on the state of collagen metabolism in terms of oxyproline in the blood, its clinical significance for assessing the severity and outcome of the disease, the criterion for the effectiveness of therapy in children of early age withacute obstructive bronchitis and bronchiolitis. The author recommends to determine the level of free and protein-bound oxyproline in blood plasma, an increase in which reflects the degree of destruction in the bronchopulmonary apparatus. The Article considers the argumentative issues of the diagnosis of the virus-induced bronchial asthma and recurrent obstructive bronchitis in the children of infant and preschool age, the commonality of their pathogenesis, the feasibility and efficiency of their therapy using the monteleukast – the antileukotriene receptor inhibitor. Along with the anti-inflammatory properties of the monteleukast, the potential antiviral activity discovered in it was considered, in particular, against the SARS-CoV-2 virus, which causes COVID-19.

The great interest of researchers in respiratory pathology in children is due to the high incidence rate. Respiratory diseases are most common in children attending organized preschool groups. Goal. To study the effectiveness and safety of using Derinat in the treatment of children with obstructive bronchitis. Material and methods. There were 48 children under supervision. Determination of CD-cells was carried out using CD-typing of leukocytes (using monoclonal antibodies). Statistical analysis was performed using the Wilcoxon test (Statistica 6.0 software). All children underwent dynamic laboratory examination and immunological parameters. The primary examination was carried out when the children were admitted to the hospital. Re-examination was carried out after the end of therapy. Group I (n = 19) included patients who received traditional treatment. Group II children (n = 16) underwent complex therapy with an immunomodulator. Derinat was selected as the study drug. Results. The data obtained reflect the effectiveness of the use of the drug polyoxidonium in the treatment of obstructive bronchitis in children. Conclusion. Thus, the results obtained have confirmed the effectiveness of Derinat in the treatment of children aged 3–6 years with obstructive bronchitis.

Purpose. To assess the efficacy and safety of the use of the immunomodulator polyoxidonium in preschool children with obstructive bronchitis. Material and methods. The study included 35 children, who, based on anamnesis, clinical, laboratory and instrumental examination methods, were diagnosed with obstructive bronchitis. The age of the examined children ranged from 3 to 6 years. The control group consisted of 11 healthy children. The material for obtaining immunological data in children was peripheral venous blood. In order to analyze immunological changes, the content of B-lymphocytes and immunoglobulins in the blood was studied. The investigated immunological data included 6 indicators. The content of immunoglobulins was determined by enzyme immunoassay according to the attached instructions. Statistical analysis of the revealed data was carried out using the Wilcoxon test using the Statistica 6.0 software. All children underwent dynamic laboratory examination and immunological parameters. The primary examination was carried out when the children were admitted to the hospital. Re-examination was carried out after the end of therapy. Group I (n = 19) included patients who received treatment according to the classical scheme. Group II children (n = 16) underwent complex therapy with an immunomodulator. The drug polyoxidonium was selected as the study drug. Results. The data obtained reflect the effectiveness of the use of the immunomodulator polyoxidonium in the complex therapy of obstructive bronchitis in children. Conclusion. Immunological data obtained during the initial examination reflect the development of dysfunction of the immune system in children. It was revealed that polyoxidonium has an immunotropic effect, this is verified by the optimization of the parameters of the humoral link of the immune system.

Verification of acute infectious inflammatory processes of the lower respiratory tract and indication for antibiotic therapy are often based on the evaluation of the activity of inflammatory blood markers, but the data on their informative value in the diagnosis of acute pneumonia and bronchitis in children are controversial and conflicting. Purpose - to study the diagnostic value of some inflammatory blood markers in the verification of uncomplicated community-acquired pneumonia and acute obstructive bronchitis with the purpose of management’s optimization of infectious inflammatory diseases of the lower respiratory tract in children. Materials and methods. At the pulmonological department of the Regional children’s clinical hospital in Chernivtsi city 75 children have been examined. In the I clinical group 51 patients with diagnosis of «community-acquired pneumonia» were enrolled, and the second (II) group consisted of 24 children with acute obstructive bronchitis. According to the main clinical characteristics, the comparison groups have been comparable. The results of the study have been analyzed by parametric («P», Student’s criterion) and non-parametric («Рφ», Fisher’s angular transform method) calculation methods, and methods of clinical epidemiology with an evaluation of the diagnostic value of the tests has been performed taking into account their sensitivity (Se) and specificity (Sp), as well as the odd ratio (OR) of the event, taking into account their 95% confidence intervals (95% CI). Results. The analysis of the obtained dada has showed that in the patients with community-acquired pneumonia such common inflammatory blood markers (leukocytosis, relative neutrophilosis, shift of leukocyte formula to the left, elevation of erythrocyte sedimentation rate (ESR) or high level of С-reactive protein - CRP) are characterized by low sensitivity (Se in range between 11% and 63%) indicating that they are inadvisable for use as the screening tests for the verification of pneumonia. At the same time, it has been shown that these inflammatory blood markers are characterized by sufficient specificity (in the range from 75% to 93%) in the verification of pneumonia only under their significant increase (total leukocyte count >15.0x10⁹, ESR >10 mm/h and СRP level in blood >6 mg/ml), indicating that they are relevant enough, but only for confirming inflammation of the lung parenchyma. It has been found that the normal level of leukocytes (15.0 x10⁹, ESR ≥10 mm/h and CRP level >6mg/ml), which indicates the possibility of their use to confirm inflammation of the lung parenchyma. Herewith the number of rod neutrophils in the hemogram ≤5% is characterized by a high specificity (Sp=84%), which allows to use this marker in confirming the diagnosis of acute obstructive bronchitis and, as well as, an indication for antibiotic therapy cancellation. The research was carried out in accordance with the principles of the Helsinki declaration. The study protocol was approved by the Local ethics committee of all participating institutions. The informed consent of the patient was obtained for conducting the studies. No conflict of interest was declared by the authors., Верификация острых инфекционно-воспалительных процессов нижних дыхательных путей и обоснование антибиотикотерапии нередко базируется на оценке активности маркеров воспаления крови, хотя данные об их информативности в дифдиагностике острой пневмонии и бронхита у детей характеризуются противоречивостью. Цель - изучить диагностическую ценность отдельных показателей воспалительного ответа крови в верификации внебольничной неосложненной пневмонии и острого обструктивного бронхита для оптимизации менеджмента инфекционно-воспалительных заболеваний нижних дыхательных путей у детей. Материалы и методы. На базе пульмонологического отделения Черновицкой областной детской клинической больницы обследованы 75 детей, среди которых первую (І) клиническую группу сформировал 51 пациент с диагнозом «Внебольничная пневмония», а вторую (II) группу - 24 ребенка с острым обструктивным бронхитом. По основным клиническим характеристикам группы сравнения были сопоставимы. Полученные результаты исследования проанализированы с помощью параметрических («Р», по критерию Стьюдента), и непараметрических («Рφ», методом углового преобразования Фишера) методов вычисления, а оценка диагностической ценности тестов проведена с позиции клинической эпидемиологии с учетом их чувствительности (ЧТ) и специфичности (СТ), а также отношения шансов (ОШ) реализации события с учетом их 95% доверительных интервалов (95% ДИ). Результаты. Анализ полученных данных показал, что у детей, больных внебольничной пневмонией, такие общепринятые показатели воспалительного ответа крови (лейкоцитоз, относительный нейтрофилез, сдвиг лейкоцитарной формулы влево, ускорение скорости оседания эритроцитов (СОЭ) или высокий уровень С-реактивного протеина (СРП) в крови) характеризуются низкой чувствительностью тестов (чувствительность варьирует от 11% до 63%), что указывает на нецелесообразность их использования в качестве скрининговых тестов для верификации пневмонии. В то же время показано, что указанные воспалительные маркеры крови характеризуются достаточной специфичностью (в пределах от 75% до 93%) в верификации пневмонии только при их значительном повышении (количество лейкоцитов крови >15,0x10⁹, СОЭ≥10мм/час и уровень СРП в крови >6 мг/мл), что свидетельствует о целесообразности их использования только для подтверждения воспаления паренхимы легких. Установлено, что нормальный уровень лейкоцитов (15,0x10⁹, СОЭ ≥10 мм/ч и уровень СРП >6 мг/мл), что указывает на возможность их использования для подтверждения воспаления паренхимы легких. При этом количество палочкоядерных нейтрофилов ≤5% в гемограмме отличается высоким уровнем специфичности (СТ=84%), что позволяет использовать данный маркер для подтверждении диагноза острого обструктивного бронхита и для отмены антибиотикотерапии. На базі пульмонологічного відділення Чернівецької обласної дитячої клінічної лікарні обстежено 75 дітей, серед яких першу (І) клінічну групу сформував 51 пацієнт із діагнозом «Позалікарняна пневмонія», а другу (ІІ) групу - 24 дитини із гострим обструктивним бронхітом. За основними клінічними характеристиками групи порівняння були зіставленими. Одержані результати дослідження проаналізовано за допомогою параметричних («Р», за критерієм Стьюдента) і непараметричних («Рφ», методом кутового перетворення Фішера) методів обчислення, а оцінку діагностичної цінності тестів проведено з позиції клінічної епідеміології з урахуванням їх чутливості (ЧТ) та специфічності (СТ), а також відношення шансів (ВШ) реалізації події з урахуванням їх 95% довірчих інтервалів (95% ДІ). Результати. Аналіз отриманих даних показав, що в дітей, хворих на позалікарняну пневмонію, такі загальноприйняті показники запальної відповіді крові (лейкоцитоз, відносний нейтрофільоз, зсув лейкоцитарної формули ліворуч, прискорення швидкості осідання еритроцитів (ШОЕ) або високий рівень С-реактивного протеїну (СРП) у крові характеризуються низькою чутливістю тестів (у межах від 11% до 63%), що вказує на недоцільність їх використання як скринінгових тестів для верифікації пневмонії. Водночас показано, що зазначені запальні маркери крові характеризуються достатньою специфічністю (у межах від 75% до 93%) у верифікації пневмонії лише за їх значущого підвищення (кількість лейкоцитів крові >15,0x10⁹, ШОЕ ≥10 мм/год та рівень СРП у крові >6 мг/мл), що свідчить про доцільність їхнього застосування лише для підтвердження запалення паренхіми легень. Установлено, що нормальний рівень лейкоцитів (15,0x10⁹, ШОЕ≥10 мм/год та рівень СРП у крові >6 мг/мл), що вказує на можливість їх використання для підтвердження запалення паренхіми легень. При цьому кількість у гемограмі паличкоядерних нейтрофілів ≤5% вирізняється високим рівнем специфічності (СТ=84%), що дає змогу використовувати цей маркер у підтвердженні діагнозу гострого обструктивного бронхіту та для скасування антибіотикотерапії. Дослідження виконано відповідно до принципів Гельсінської декларації. Протокол дослідження ухвалено Локальним етичним комітетом усіх зазначених у роботі установ. На проведення досліджень отримано інформовану згоду батьків дітей. Автори заявляють про відсутність конфлікту інтересів.

ANAMNESTIC RISK FACTORS FOR THE FORMATION OF ASTHMA IN INFANTS Malakhova V. Introduction. Allergic diseases are one of the major problems of the present. Almost 700 million people in the world are suffering from this disorder. Respiratory allergies rank as some of the most common chronic health conditions among them. About 330 million people are suffering from bronchial asthma today. The aim of the study. To identify the risk factors for the development of asthma in children with wheezing. Objective. The purpose of this study was to determine the risk factors for asthma formation. The study involved assessment of clinical data of 94 children aged 1 to 7 years with recurrent wheezing. Children were observed for 5 years. The first group included 62 patients with diagnosed acute obstructive bronchitis, and the second group included 32 patients diagnosed with asthma. Results. Assessment of anamnestic data revealed significant risk factors for the formation of asthma. The study showed a relationship between the risk factor and the development of the disease in children with recurrent wheezing. Conclusions. The obtained data can be used in the algorithm of examination of patients with wheezing. The study implied the assessment of risk factors for the formation of asthma. This is important for pediatric practice. Thorough history taking and determination of risk factors are important when examining patients with recurrent wheezing. Алергійні захворювання являють собою одну з головних проблем сьогодення. Майже 700 мільйонів людей у світі страждають на дану патологію. Серед них респіраторні алергози займають значну частку, на даний час близько 330 мільйонів людей страждають на бронхіальну астму. Мета дослідження. Виявити анамнестичні фактори ризику розвитку бронхіальної астми у дітей з повторними епізодами бронхообструктивного синдрому. Об’єкт і методи дослідження. Проведено аналіз клінічних даних 94 дітей у віці від 1 до 7 років з повторними епізодами бронхообструктивного синдрому. Діти знаходились під постійним динамічним спостереженням протягом 5 років з метою верифікації формування бронхіальної астми. Усім пацієнтам проведено повне дослідження згідно протокольних методик. До першої групи увійшло 62 пацієнта із встановленим діагнозом гострий обструктивний бронхіт, до другої групи увійшло 32 дитини, котрим діагностовано бронхіальну астму. Результати досліджень та їх обговорення. Серед вивчених показників анамнестичних даних виявлено статистично значимі несприятливі чинники формування бронхіальної астми та встановлено характер та силу зв’язку між факторами ризику та формуванням захворювання у дітей з повторними епізодами бронхообструктивного синдрому. Висновки. Отримані дані можуть бути включені в алгоритм обстеження пацієнтів з бронхообструктивним синдромом для об’єктивної оцінки факторів ризику та можливості формування бронхіальної астми, що надзвичайно важливо для педіатричної практики. Тому ретельний збір анамнезу та виявлення цих факторів має велике значення при ведені пацієнтів із повторними епізодами бронхообструктивного синдрому. Дети находились под постоянным динамическим наблюдением в течение 5 лет с целью верификации формирования бронхиальной астмы. В первую группу вошло 62 пациента с установленным диагнозом острого обструктивного бронхита, ко второй группе отнесены 32 пациента, которым установлен диагноз бронхиальной астмы. Результаты и обсуждение. Среди изученных показателей анамнестических данных выявлены статистически значимые неблагоприятные факторы формирования бронхиальной астмы, установлены характер и сила связи между факторами риска и формированием заболевания у детей с повторными эпизодами бронхообструктивного синдрома. The specific features of the course of chronic occupational bronchitis (OCB) and its relationships with occupational chronic obstructive pulmonary disease (OCOPD) have not been adequately investigated in the clinic of occupational pathology. The aim of the study was to study risk factors, clinical features and prognosis of chronic non-obstructive bronchitis. 222 patients (metal workers) with OCB and OCOPD were randomly selected for the study. The medical histories of diseases were studied from the initial diagnosis to the present status (in average the period of 10 years).Patients were divided into three groups: with the initial diagnosis of OCOPD (1), with the initial diagnosis of OCB, but transformed to OCOPD (2) and the patients with the initial diagnosis of OCB (3). Patients were divided into three groups: those initially diagnosed with COPD PE made up group No. 1, those initially diagnosed with CKD PE who did not show signs of COPD PE during the annual examination made up group No. 2, and those initially diagnosed with CKD PE who showed signs of COPD PE during the dynamic examination made up group No. 3. All groups showed a decrease in spirometry parameters. FEV1 was statistically significantly decreased by 12% in group 1, by 13% in the second group and by 27% in the third group. When analyzing modified Typhno index (MTI) values in group 3, there was a statistically significant decrease in MTI from baseline by 21% (p=0.002, Z = 2.9, Wilcoxon test). In 52 people out of 156 (or 34.6%) with the initial diagnosis of OCB, a manifestation of OCOPD occurred during the observation period. Two phenotypes of chronic bronchitis were distinguished: with favorable and unfavorable prognoses. Patients with OCB, after establishing a professional diagnosis, should be recommended to rational employment out of exposure to industrial aerosols, due to the high risk of disease progression.Funding. Objective The aim of the work is to improve the prognosis of acute bronchitis on the basis of the study of etiology, clinical anamnestic and laboratory-instrumental features of acute bronchitis in children infected with Mycoplasma pneumoniae. Patients and methods Materials and methods: Medical records of inpatients, data from laboratory and clinical examinations, orders from the Ministry of Health of Ukraine. Statistical processing of the obtained results was carried out with the calculation of parametric and non-parametric criteria. The study included 72 patients with acute obstructive bronchitis, infected with Mycoplasma pneumoniae and patients not infected with intracellular pathogens who were hospitalized under the conditions of pediatric ward of children of younger and older children of the National Children's Clinical Hospital №24. Results Results: Found that infected with mycoplasma compared with uninfected intracellular infection, more characteristic course of acute obstructive bronchitis on the background of febrile temperature with its duration of more than 4 days, the presence of midbubble and single dry or wet rales, as well as. In general, in patients infected with mycoplasma, compared with patients infected with chlamydia, there is a higher functional stress of immunity with the phenomena of exhaustion. Conclusion Conclusions: Attention is drawn to the fact that clinical, laboratory data and immunological examinations with the use of systematic analysis make it possible to predict the consequences in the history of children with intracellular infections of various somatic pathologies. In doing so, multivariate and correlation analyzes make it possible to develop new diagnostic criteria.

Objective: The objective is to assess the clinical efficacy of the inclusion of release-active medication of affinity purified antibodies to protein S100 in complex rehabilitation of infants with old acute obstructive bronchitis affected by perinatal pathology of central nervous system of hypoxic genesis.Methods: 210 infants aged from 2–36 months, who suffered events of acute obstructive bronchitis, were examined. Along with clinical methods of examination, a computer bronchophonography and assessment of heart rate variability were performed.Results: Additional inclusion of release-active medication ‘Tenoten for children’ in the complex of rehabilitation of these patients had a positive effect on autonomic balance and autonomic component of bronchial obstruction. At the same time, the short course of Tenoten (1 month) did not prevent recurrent events of bronchial obstruction.Conclusions: Probably, patients with recurrent events of acute bronchial obstruction in combination with pronounced functional disorders of vegetative state require longer courses of release-active medication which normalized the activity of the central nervous system.

According to WHO, acute respiratory diseases are at the forefront of the incidence rate of infants and are among the most common diseases due to which children are hospitalized. The aim of the research was to analyze the structure of morbidity and features of the course in acute respiratory diseases in infants with the study of the frequency of leading nosological forms that required inpatient treatment. A retrospective analysis of the medical records of inpatients (3845 children), treated during 2016-2018 was conducted. The study found that there is a tendency for an increase in the number of infants who require inpatient treatment, as well as an increased number of nosologies in the treated children. As the underlying disease, the first place in the structure of the morbidity belonged to acute respiratory viral infections. In the dynamics of the studied years, an increase in the frequency of acute obstructive bronchitis was noted. This pathology has the second place in the structure of morbidity among hospitalized children. The permanent third place in the structure of nosologies belongs to acute pneumonia. The premorbid background of most children with bronchial obstruction syndrome and acute community-acquired pneumonia was aggravated. The most commonly determined disorders were I-II degree anemia, overweight, impaired calcium metabolism, aggravated allergic history, transient immune deficiency. Conclusions. Our analysis of the structure of acute respiratory pathology in infants who required inpatient treatment showed that in the dynamics of 2016-2018, there was an increase in cases of acute obstructive bronchitis and acute community-acquired pneumonia. The study of general structure of nosologies in children treated at the inpatient department revealed an increase in their number and a tendency for the formation of comorbid pathology in infants.

CLINICAL AND ANAMNESTIC FEATURES OF THE COURSE OF ACUTE BRONCHITIS IN CHILDRENStrelkova M., Senatorova G. The purpose of the study wasto detect clinical and anamnestic features of the course of acute obstructive bronchitis in children with a background of undifferentiated connective tissue dysplasia. Dynamic examination of 42 children with acute obstructive bronchitis was performed. The registration card for all children was developed and filled, it consisted of several sections: general information, family history, allergic history, life and past medical history, phenotypic assessment of undifferentiated connective tissue dysplasia. The results indicate that compromised allergic and genealogical history, as well as burdened obstetric history of the mother (threatened preterm delivery and gestosis) is significant risk factors. Розроблено і заповнено реєстраційну картку для всіх дітей, вона складалася з кількох розділів: загальна інформація, сімейний анамнез, алергологічний анамнез, анамнез життя та хвороби дитини, фенотипічні оцінки недиференційованої дисплазії сполучної тканини. Результати свідчать про наявність факторів ризику - наявність обтяженого алергологічного та генеалогічного анамнезу, а також обтяжений акушерський анамнез матері (наявність загрози передчасних пологів та гестозів). При аналізі клінічних проявів ГОБ у дітей було встановлено, що ступінь тяжкості та тривалості симптомів захворювання була пов'язана з кількістю ознак недиференційованої дисплазії сполучної тканини.Ключові слова: гострий обструктивний бронхіт, діти, недиференційована дисплазія сполучної тканини, фенотипічні ознаки. Разработаны и заполнены регистрационную карточку для всех детей, она состояла из нескольких разделов: общая информация, семейный анамнез, аллергологический анамнез, анамнез жизни и болезни ребенка, фенотипическая оценка признаков недифференцированнойдисплазиисоединительной ткани. Результаты свидетельствуют о наличии факторов риска - наличие отягощенного аллергологического и генеалогического анамнеза, а также отягощенный акушерский анамнез матери (наличие угрозы преждевременных родов и гестозов). Diagnosis of acute infectious-inflammatory processes of the lower respiratory tract with a respect to justify etiotropic therapy is often based on evaluation of the activity of blood inflammatory markers and data of lungs' X-ray examination, but scientific evidence of their informativity in the differential diagnosis of community-acquired pneumonia and acute bronchitis is conflicting.Purpose — to study the predictor role of some paraclinical indices in the verification of infectious and inflammatory diseases of the lower respiratory tract (community-acquired pneumonia and acute obstructive bronchitis) in children of different ages in order to optimize the treatment.Materials and methods. To achieve the goal of the study, a cohort of patients with acute infectious-inflammatory pathology of children with different ages (75 patients) who received inpatient treatment at the pulmonology department of the Regional Children's Clinical Hospital in Chernivtsi has been formed by the method of simple random sampling. The first (I) clinical group was formed by 51 patients with a verified diagnosis of community-acquired pneumonia (CAP), acute course, and the second (II) clinical group included 24 children, in which the infiltrative acute process in the lungs was excluded, but who had broncho-obstructive syndrome. According to the main clinical characteristics, the comparison groups have been comparable. The results of the study have been analyzed by parametric («P», Student's criterion) and non-parametric («Рϕ», Fisher's angular transform method) calculation methods, and methods of clinical epidemiology with an evaluation of the diagnostic value of the tests has been performed taking into account their sensitivity (Se) and specificity (Sp), as well as attributive (AR) and relative (RR) risks, and the odd ratio (OR) of the event, taking into account their 95% confidence intervals (95% CI).Results. The analysis of the obtained dada has showed that in the patients with CAP such common inflammatory blood markers (leukocytosis, relative neutrophilosis, shift of leukocyte formula to the left, elevation of erythrocyte sedimentation rate (ESR) or high level of CRP — С-reactive protein) are characterized by low sensitivity (Se in range between 11% and 63%) indicating that they are inadvisable for use as the screening tests for the verification of pneumonia. At the same time, it has been shown that these inflammatory blood markers are characterized by sufficient specificity (in the range from 75% to 93%) in the verification of pneumonia only under their significant increase (total leukocyte count >15.0x109, ESR>10 mm/h and CRP level in blood >6 mg/ml), indicating that they are enough, but only for confirming inflammation of the lung parenchyma. From the standpoint of clinical epidemiology, it has been proved that the asymmetry of findings at lung radiographs (asymmetry of pulmonary enhancement, asymmetric changes of lung roots and, especially, the presence of infiltrative changes at lung parenchyma) are the most informative diagnostic tests in pneumonia verification (ST=90–95%) and have a statistically significant predictor role in the final diagnosis (OR=11.6–150). Методом простої випадкової вибірки сформовано когорту дітей різного віку (75 пацієнтів), хворих на гостру інфекційно-запальну патологію нижніх дихальних шляхів, які отримували стаціонарне лікування в пульмонологічному відділенні обласної дитячої клінічної лікарні м. Чернівці. Першу (І) клінічну групу сформовано з 51 пацієнта із верифікованим діагнозом «Позалікарняна пневмонія, гострий перебіг», а другу (ІІ) — з 24 дітей без інфільтративного процесу в легеневій тканині, але з проявами бронхообструктивного синдрому. За основними клінічними характеристиками групи порівняння були зіставними. Отримані результати дослідження проаналізовано за допомогою параметричних («Р», за критерієм Стьюдента) і непараметричних («Рϕ», методом кутового перетворення Фішера) методів обчислення, а оцінку діагностичної цінності тестів проведено з позиції клінічної епідеміології з урахуванням їх чутливості (ЧТ) і специфічності (СТ), атрибутивного (АР) і відносного (ВР) ризиків, співвідношення шансів (СШ) реалізації події з їх 95% довірчими інтервалами (95% ДІ).Результати. Аналіз отриманих даних показав, що в дітей, хворих на позалікарняну пневмонію, загальноприйняті показники запальної відповіді крові (зокрема, лейкоцитоз, відносний нейтрофільоз, зсув лейкоцитарної формули ліворуч, прискорення швидкості зсідання еритроцитів або високий рівень С-реактивного протеїну в крові) характеризуються низькою чутливістю тестів (у межах від 11% до 63%), що вказує на недоцільність їх застосування як скринінгових тестів для верифікації пневмонії. Водночас показано, що зазначені запальні маркери крові характеризуються достатньою специфічністю (у межах від 75% до 93%) у верифікації пневмонії лише за їх значущого підвищення (кількість лейкоцитів крові >15,0x109, швидкість зсідання еритроцитів >10 мм/год і рівень С-реактивного протеїну в крові >6 мг/мл), що свідчить про доцільність їх використання лише для підтвердження запалення паренхіми легень. З позиції клінічної епідеміології доведено, що асиметричність знахідок на рентгенограмах легень (асиметричність посилення легеневого малюнку, асиметричні зміни коренів легень та, особливо, інфільтративні зміни паренхіми легень) є найбільш інформативними діагностичними тестами у верифікації пневмонії (СТ=90–95%), які виконують статистично значущу предикторну роллю у встановленні остаточного діагнозу (СШ=11,6–150). В то же время выявлено, что указанные воспалительные маркеры крови характеризуются достаточной специфичностью (в пределах от 75% до 93%) в верификации пневмонии только при их значительном повышении (количество лейкоцитов крови >15,0x109, скорость оседания эритроцитов >10 мм/ч и уровень С-реактивного протеина в крови >6 мг/мл), что свидетельствует о целесообразности их использования только для подтверждения воспаления паренхимы легких. Метою дослідження є вивчення стану клітинної ланки імунітету у дітей раннього віку з гострим обструктивним бронхітом на тлі тимомегалії. Встановлено, що зміна параметрів клітинного компонента імунітету при наявності супутньої тимомегалії характеризується значним зменшенням вмісту CD3 +, CD4 +, CD8 + та збільшенням субпопуляцій В-лімфоцитів. Aim of study is to study the state of the cellular link of immunity in young children with acute obstructive bronchitis against the background of thymomegaly.It was found that the change in the parameters of the cellular component of immunity in the presence of concomitant thymomegaly was characterized by a significant decrease of content of CD3 +, CD4 +, CD8 + and an increase in B-lymphocyte subpopulations.

One of the leading positions among the infant diseases belongs to the pathology of the respiratory tract. The causes of the obstructive bronchitis in children are very diverse including those, which can be associated with the disorder of the normal immune system functioning or immunodeficiency state. The purpose of the study is to determine the nature of the immune disorders in the children with the obstructive bronchitis, to assess the efficiency of the immunocorrecting therapy. For 2 groups of the children with the obstructive bronchitis, the comparative study with determination of the cytokine and immunoglobulin levels in the blood serum against the background of standard therapy (1st group) and against the background with immune stimulator ismigen included into the therapy was carried out; the ismigen is a multivalent antigenic complex of the bacterial lysates. The efficiency of the immune-enhancing therapy carried out towards the immune system condition of children during the comprehensive treatment of the obstructive bronchitis was shown. The practical importance of the work is to optimize diagnosis and immunocorrection in obstructive bronchitis in children, accompanied by the disorder of the body’s immunoreactivity.

Objective: to establish the efficiency of Laferobion administration in children with recurrent episodes of acute obstructive bronchitis (AOB) in the dynamics of treatment based on the cytokine profile evaluation. Material and methods: In total 59 inpatiently treated children of the main group with repeated episodes of AOB (I-AOB) and 30 children of the comparative group suffered from the acute bronchitis (II-AB) in their 2 to 8 years old were examined. The results of laboratory tests were compared with the data of 30 healthy children of the control group (III-HC). Results. In the children with repeated episodes of AOB, the serum level of interferon-γ and interleukin-4 was significantly lower in comparison with the healthy children. After the treatment with Laferobion, these cytokines significantly increased, reaching the values of healthy children. The interleukin-1β level in the children with repeated episodes of AOB was significantly higher than in the healthy children, and it normalized to the level of healthy children after immunomodulatory therapy with Lauferobion. Conclusions. In the children with repeated episodes of AOB, the imbalance of cytokines is established; the efficacy of immunomodulatory therapy with Laferobion has been proved, as a result of which the levels of the studied serum cytokines were normalized.

Saienko Diseases of the respiratory system are one of the most frequent causes of the treatment of children in hospital facilities. The younger the child, the more often these diseases occur and the more severe it can be. Cardiovascular disorders with bronchopulmonary pathology develop slowly, potentially reversible in childhood, which requires a correct assessment of the risk of their development for a particular patient and timely correction of therapy. The presence of dyspnea, prolonged exhalation, pale skin, cyanosis of the nasolabial triangle and acrocyanosis, swelling of the nose (symptoms common to cardiac and respiratory failure) indicate the reaction of the cardiovascular system in diseases of the respiratory system. We examined 14 children: among them 7 boys and 7 girls. Ten children were diagnosed with acute obstructive bronchitis, and 4 with pneumonia. In the course of the study, the parameters were assessed, reflecting the linear dimensions of the heart cavity and the main vessels, followed by the calculation of central hemodynamics, diastolic transmittal blood flow by the method of L.K. Чим молодша дитина, тим частіше зустрічаються ці захворювання і тим важче можуть протікати. Кардіоваскулярні порушення при бронхолегеневої патології розвиваються повільно і є потенційно зворотними в дитячому віці, що вимагає правильної оцінки ризику їх розвитку для конкретного хворого і своєчасної корекції тактики терапії.Про реакцію серцево-судинної системи при захворюваннях органів дихання свідчить наявність задишки, протяжного видоху, блідості шкірних покривів, ціанозу носогубного трикутника і акроцианозу, роздування крил носа (симптоми загальні для серцевої і дихальної недостатності). Нами було обстежено 14 дітей: серед них було 7 хлопчика та 7 дівчаток. У 10 дітей діагностовано гострий обструктивний бронхіт, у 4 дітей – пневмонію. В ході дослідження оцінювали показники, відображаючи лінійні розміри порожнин серця та магістральних судин з послідуючим розрахунком центральної гемодинаміки, діастолічного трансмітрального кровотоку за методом L.K. Кардиоваскулярные нарушения при бронхолегочной патологии развиваются медленно, потенциально обратимы в детском возрасте, что требует правильной оценки риска их развития для конкретного больного и своевременной коррекции терапии. О реакции сердечно-сосудистой системы при заболеваниях дыхательной системы свидетельствует наличие одышки, удлененного выдоха, бледности кожных покровов, цианоза носогубного треугольника и акроцианоза, раздувание крыльев носа (симптомы общие для сердечной и дыхательной недостаточности). Нами было обследовано 14 детей: среди них было 7 мальчика и 7 девочек. У 10 детей диагностирован острый обструктивный бронхит, у 4 - пневмонию. В ходе исследования оценивали показатели, отражая линейные размеры полостей сердца и магистральных сосудов с последующим расчетом центральной гемодинамики, диастолического трансмитрального кровотока по методу L.K. Цель: определение эффективности и безопасности использования растворов натрия хлорида в комплексной терапии детей раннего возраста, больных обструктивным бронхитом (ОБ).Пациенты и методы. Под наблюдением находились 45 детей с ОБ в возрасте от 1,5 мес. до 3 лет. У всех детей развитие ОБ спровоцировала острая респираторная вирусная инфекция. Все больные получали комплексное лечение, включавшее бронхолитическую терапию через небулайзер, противовоспалительную терапию, системные кортикостероиды; по показаниям проводилась инфузионная и симптоматическая терапия. Все дети были распределены на три группы: I группа — 15 (33,3%) больных с дыхательной недостаточностью II степени, которые получали ингаляции монопрепарата хлорида натрия (3% LORDE); II группа — 16 (35,6%) детей с дыхательной недостаточностью III степени, которые ингалировали комбинированный препарат — 3% гипертонический раствор хлорида натрия + гиалуроновая кислота (3% LORDE hyal hyper); III группа (группа сравнения) 14 (31,1%) пациентов с дыхательной недостаточностью IІ–III степеней, не получавших препаратов гипертонического раствора натрия хлорида.Результаты. На фоне комплексной терапии у пациентов І группы отмечалось улучшение общего состояния, уменьшение дыхательных расстройств и появление продуктивного кашля в среднем на третьи сутки (3,08±0,95). У детей ІІ группы к концу третьих суток (3,18±1,01) регистрировалось аналогичное улучшение, у а больных ІІІ группы — на пятые сутки (5,46±1,05). Длительность стационарного лечения уменьшилась у детей, получавших один из препаратов гипертонических солевых растворов, в среднем до семи дней. Больные ІІІ группы находились в больнице 9,46±2,4 дня. Несмотря на то, что дети ІІ группы имели более тяжелые дыхательные расстройства, у них получены лучшие результаты лечения благодаря свойствам гиалуроната натрия, что способствует снижению интенсивности воспалительного процесса, увлажнению и восстановлению слизистой оболочки бронхов. Побочных явлений лечения не отмечено.Выводы. Доказана эффективность гипертонических растворов натрия хлорида, как монопрепарата (3% LORDE), так и в комбинации с гиалуроновой кислотой (3% LORDE hyal hyper), в комплексном лечении ОБ у детей раннего возраста. Использование данных препаратов сокращает пребывание детей в стационаре и улучшает переносимость базисной терапии. Небулайзерная терапия LORDE hyal hyper, благодаря увлажняющему действию гиалуроната натрия, демонстрирует выраженную эффективность и отличную переносимость препарата, предотвращает развитие негативного эффекта в виде бронхоспазма и может быть рекомендована для применения при выраженной бронхообструкции. Благодаря увлажняющему действию гиалуроновой кислоты, добавление ее к гипертоническому раствору натрия хлорида позволяет избежать бронхоспазма как возможного его побочного эффекта., Objective: efficiency determination and safety of the saline solution use in the comprehensive treatment of young children with obstructive bronchitis (OB).Material and methods. In total 45 children were followed-up with OB aged from 1.5 months to 3 years. In all children an acute respiratory viral infection triggered the OB development. У дітей ІІ групи до кінця третьої доби (3,18±1,01) реєструвалося аналогічне покращання, у а хворих ІІІ групи — на п`яту добу (5,46±1,05). Тривалість стаціонарного лікування зменшилася у дітей, що отримували один з препаратів гіпертонічних сольових розчинів, у середньому до сімох днів. Хворі ІІІ групи знаходилися у лікарні 9,46±2,4 дня. Незважаючи на те, що діти ІІ групи мали важчі дихальні розлади, у них отримані кращі результати лікування завдяки властивостям гіалуронату натрію, що сприяє зниженню інтенсивності запального процесу, зволоженню та відновленню слизової оболонки бронхів. Побічних явищ лікування не спостерігалося.Висновки. Доведена ефективність гіпертонічних розчинів натрію хлориду, як монопрепарату (3% LORDE), так і в комбінації з гіалуроновою кислотою (3% LORDE hyal hyper), у комплексному лікуванні ОБ у дітей раннього віку. Використання цих препаратів скорочує перебування дітей у стаціонарі та покращує переносимість базової терапії. Небулайзерна терапія LORDE hyal hyper, завдяки зволожувальній дії гіалуронату натрію, демонструє виразну ефективність та відмінну переносимість препарату, запобігає розвитку небажаного ефекту у вигляді бронхоспазму і може бути рекомендована для застосування при виразній бронхообструкції. Завдяки зволожувальній дії гіалуронової кислоти, додавання її до гіпертонічного розчину хлориду натрію дозволяє уникнути бронхоспазму як можливого його побічного ефекту.

A molecular genetic study of the polymorphic C-159T locus of the CD14 gene has been carried out in 31 children with recurrent episodes of acute obstructive bronchitis as well as in a cohort of 50 randomly sampled subjects forming the control group. As a result of the study, no higher statistically significant frequency of the CC, CT, and TT genotypes has been recorded at the polymorphic C-159T locus of the CD14 gene among children of the main group compared with the control. However, there is a trend towards a more frequent observation of the TT genotype among children with recurrent episodes of acute obstructive bronchitis compared with the control group (32.25 vs. 22.0%).

Diagnosis of bronchial asthma (BA) for babies in the first years of life is of particular interest in the practice of paediatric pulmonologist, as there is currently no reliable diagnostics, including functional criteria, to verify the disease at this age. The purpose of the study was to find additional clinical and functional criteria to forecast the formation of BA in early childhood.The study found that children who had undergone acute obstructive bronchitis (AOB) against the background of the perinatal CNS involvement (PCNSI), and consequently having pathoreceptor disorders and variability of the heart rate, had a longer and heavier period of AOB. It has been shown that the presence of PCNSI is an unfavourable factor in the prediction of repeatepisodes of AOB and development of BA. As a result of the follow-up study and the ROC analysis, additional criteria for the projection of BA in early childhood were identified, taking into account the functional state of the respiratory and vegetative nervous system. The additional criteria developed for the projection of BA in young children, together with the clinical and disease history data can contribute to early diagnosis of the BA, which has an important clinical significance for the timely delivery of curative and preventive measures

Actuality in the structure of general morbidity of children, potential allergic markers of risk of developing bronchitis. The main group consisted of 62 children age from 1 to 6 years with obstructive and nonobstructive bronchitis. As a result of research it is established that allergies the first year of life increases the probability of developing obstructive bronchitis.

One of the highest risk factors for bronchial asthma (BA) in children is atopic dermatitis (AD). In this regard, current research is aimed at identifying additional predictors of asthma in the population of patients with AD. Aim: to develop a mathematical model for predicting the risk of BA in children with AD. Material and methods: 315 children aged 0 to 18 years were examined, of which 138 with AD and no BA and 177 with AD and BA. Logistic regression method was used for the development of the mathematical model. Results: based on the obtained data, logistic regression equation with 7 predictors was chosen (age; intrauterine infection; pneumonia, acute obstructive bronchitis and chronic adenoids over the age of two; allergic diseases of the father and mother’s relatives) the statistical significance of which was within 5% Wald test.

Aim: to determine the value of the parameters of microcirculation (MC) in children with bronchial asthma (BA) and acute obstructive bronchitis (AOB). Methods: 78 children aged from 2 to 17 years with the mild BA in exacerbation or incomplete remission (group (Gr) 1) and 20 children of the same age with AOB (Gr2) were examined. Catamnesis lasted from 6 to 18 months. The control group (C) included 60 healthy children. All patients underwent examination and computerized nailfold capillaroscopy (NCS). Results: It was found that patients of Gr1 in contrast to Gr2 and C, were characterized by deformation of all parts of the capillaries: a decrease in the diameter of the arterial part of the capillaries and the unevenness of the caliber of the capillaries (Nkc) (0.89 [0.88-0.90] conv.units, 0.92 [0.89-0.93] conv.units and 0.94 [0.917-0.967] conv.units, respectively; р Thus, findings of the NCS can be used to improve the effectiveness of early diagnosis of BA in children, as an additional functional criteria, along with clinical parameters.
