- Abstract
The objective was to describe nursing care in the first hour of life of the newborn (NB). The method of choice was the integrative literature review carried out in the Regional Virtual Health Library (VHL) and Google Scholar, using the Health Sciences Descriptors (DECs): Newborn, Postnatal Care and the Boolean operator AND. In the VHL, 5,446 publications were evaluated by availability and publication in the last 5 years, resulting in 856 analyzed by title and abstract, resulting in 38 publications read in full and the selection of 1 for theoretical synthesis. Google Scholar found 10,400 publications filtered by publication in the last 5 years, resulting in 3,620 analyzed by title and theme, with 22 publications read in full and 6 selected. The results show that nursing care in the first hour of a newborn's life is essential to ensure a smoother adaptation to extrauterine life and, among the results of this practice, the reduction of neonatal mortality, the prevention of complications and the promotion of the bond between mother and child stand out. It is concluded that it is of utmost importance for health institutions to offer training and qualification to nursing professionals to qualify care in this critical period. [ABSTRACT FROM AUTHOR]

We aimed to investigate the impact of COVID-19 infection on maternal characteristics and obstetric and neonatal outcomes in a cohort of women in labor previously vaccinated who tested positive for SARS-CoV-2 infection, compared to aged-matched healthy controls. A retrospective case-control study was conducted among 66 women in labor. Clinical data were obtained from medical records. The attendance rates at childbirth and parenting classes, as well as the implementation of a birth plan, were significantly lower in the COVID-19 infection group (6.1% vs. 48.5%, <0.001; 6.1% vs. 33.3%, p =.005, respectively). Women with COVID-19 had a higher prevalence of prolonged postpartum hospital stay (33.3% vs. 9.1%, p =.016), and significantly higher prevalence of spontaneous preterm birth (27.3% vs. 1.09%, p =.006). Breastfeeding within the first 24 hr was also lower in women with COVID-19 (72.7% vs. 97.0%, p =.006). Maternal characteristics and neonatal outcomes are influenced by COVID-19 infection in vaccinated women. Complications include spontaneous preterm birth, prolonged postpartum hospital stay, and lack of breastfeeding within the first 24 hr. Childbirth education, parenting classes and implementing a birth plan may be associated with a decreased risk of COVID-19 infection. [ABSTRACT FROM AUTHOR]

BACKGROUND: Evidence is limited on nurse staffing in maternity units. PURPOSE: To estimate the relationship between hospital characteristics and adherence with Association of Womens Health, Obstetric and Neonatal Nurses nurse staffing guidelines. METHODS: We enrolled 3,471 registered nurses in a cross-sectional survey and obtained hospital characteristics from the 2018 American Hospital Association Annual Survey. We used mixed-effects linear regression models to estimate associations between hospital characteristics and staffing guideline adherence. FINDINGS: Overall, nurses reported strong adherence to AWHONN staffing guidelines (rated frequently or always met by ≥80% of respondents) in their hospitals. Higher birth volume, having a neonatal intensive care unit, teaching status, and higher percentage of births paid by Medicaid were all associated with lower mean guideline adherence scores. DISCUSSION AND CONCLUSIONS: Important gaps in staffing were reported more frequently at hospitals serving patients more likely to have medical or obstetric complications, leaving the most vulnerable patients at risk.

The People's self-concept contributes to their sense of identity, and if associated with methods that promote the involvement of participants, we develop knowledge that promotes causes and social change. Introduction: In recent decades, the scientific community has identified a high prevalence of work-related musculoskeletal disorders in nurses, as a result of work processes, high physical demands, workspaces, and the emotional and sometimes stressful association of clinical contexts. Obstetric nurses, due to their specific work, have been a group that has been little studied and, above all, little involved in research processes. Research Objective: This study aims to identify the self-perception of Obstetric nurses regarding daily activities in the labour context, which are selfperceived as high risk for work-related musculoskeletal disorders. Methodology: A qualitative approach was used, utilising an empowering research method - Photovoice. Twenty who work full time in the delivery room took part. Photographic records were presented by the Obstetric nurses and their self-perceptions were shared through interviews. The interviews and photographs were then transcribed, analysed and categorised. Results: The average number of years as a Obstetric nurses was 14.4 years, but with 24.8 years working as a nurse. Twenty-two work activities with a potential risk to health were photographically recorded. [ABSTRACT FROM AUTHOR]

Dysmenorrhea, characterized by pain and related symptoms, significantly impacts women's quality of life in work and education, prompting a comprehensive evaluation of associated factors. The objective of this study was to utilize structural equation modeling (SEM) to analyze and assess the biopsychosocial factors influencing dysmenorrhea among university students. Three hundred and thirty-nine university students were included in this cross-sectional descriptive study. Data were collected using the Participant Information Form, the Beck Anxiety Scale (BAS), the Beck Depression Scale (BDS), the Multidimensional Scale of Perceived Social Support (MSPSS), the Adverse Childhood Experiences Scale (ACES), the Pain Catastrophizing Scale (PCS) and the Visual Analogue Scale (VAS). It was determined that the variables of pain onset (t = 3.24, p <.05) and age at menarche (t = -2.16, p <.05) showed a significant relationship with the model. The variables of the PCS (t = 16.87, p <.001), BDS (t = 3.06, p <.05), and BAS (t = 5.13, p <.001) showed a significant relationship with the model. Social factors in the model were examined, and a family history of dysmenorrhea and the ACES variables did not contribute significantly to the model (p >.05). The study indicates primary dysmenorrhea influenced by biological and psychological factors. Nurses should conduct holistic assessments and provide comprehensive care for affected women. Objective: To evaluate the association between sociodemographic and obstetric factors and the health‐related quality of life of pregnant women in high‐risk prenatal care. Methods: A cross‐sectional study of women in high‐risk prenatal care in Ceara, Brazil. The investigated outcomes were health‐related quality of life, using the Medical Outcomes Study 36‐item short‐form health survey; the investigated covariates were sociodemographic and obstetric data. Associative analyses were performed using the Jamovi® software version 0.9. Results: Of the 276 women included in the study, women with the following characteristics presented a better quality of life in some domain of the scale: age equal to or greater than 35 years, higher income per dependent, religious, living with three or fewer persons, with their own home, in primigestation, nulliparous, with no history of previous abortion, and with up to two living children. The regression model showed an association between the total scale score, which means a higher quality of life in women with age equal to or greater than 35 years and a higher income per dependent. Conclusion: The study identified sociodemographic and obstetric factors that may affect the quality of life of high‐risk pregnant women, providing subsidies to health providers so that they can promote better prenatal care. - Abstract
Purpose: To evaluate maternal dietary intake during pregnancy compared with the 2015-2020 and 2020-2025 Dietary Guidelines for Americans (DGA). Study Design and Methods: A retrospective observational study design was used. The cohort included women who gave birth to preterm infants between 25 1/7 weeks and 33 6/7 weeks of gestation. Within 2 weeks of birth, participants were asked to recall their diet in the last month of pregnancy using the Dietary Screener Questionnaire. Participants' dietary intakes were compared to current 2020-2025 DGA that include specific recommendations for pregnant women and prior 2015-2020 DGA that were in place during the study period but were not pregnancy-specific. Results: Forty-five women participated in the study. None met the 2015-2020 or 2020-2025 DGA recommended intake of all seven dietary components. When compared to both DGAs, 2.2% of participants met the recommended dairy intake, 26.7% met the calcium intake, 15.6% were below the threshold for added sugar, and none met the vegetable or whole grain intake. When compared to the 2015-2020 DGA, 28.9% met the fruit intake and 2.2% met the fiber intake. However, when compared to the 2020-2025 DGA, only 2.2% met the fruit intake and none met the fiber intake. Clinical Implication: Women who gave birth preterm did not adhere to the 2015-2020 or 2020-2025 DGA recommendations in the last month of pregnancy. Nurses are uniquely positioned to provide pregnant women with nutritional screening and counseling as part of routine prenatal care. Diet is an important part of a healthy pregnancy, however all women do not have access to or can afford foods that promote good health. In this study, the diets of women who gave birth preterm were not consistent with the 2015–2020 or 2020–2025 recommendations in the Dietary Guidelines for Americans during the last month of their pregnancy. Nurses are uniquely positioned to provide pregnant women with nutritional screening and counseling as part of routine prenatal care to enhance pregnancy and birth outcomes. [ABSTRACT FROM AUTHOR]

- Abstract
Objective: To compare maternal and neonatal outcomes of women who used and did not use neuraxial analgesia during labor. Methods: A cross-sectional, comparative, documentary study, with retrospective data collection, carried out at a tertiary reference maternity hospital in Ceará. Sample size was calculated by the difference between two proportions: vaginal births with and without analgesia, with 130 women for each group, totaling 260 medical records. Data collection took place between July 2019 and February 2020. Student's t test, Mann-Whitney U test, Pearson's chi-square test, Fisher's exact test and the IBM SPSS program were used. Results: The group with analgesia had a higher mean number of prenatal consultations (8.24; p<0.001), greater exposure to induction (74; 56.9%; p<0.001), with use of oxytocin (57; 43.8%; p<0.001), longer duration of active labor (mean: 392 min; p<0.001) and expulsive period (mean: 85.3 min; p<0.001), higher frequency of episiotomy (7; 7.9%; p=0.03), by cesarean section (41; 31.5%; p<0.001), and heavier babies were born (mean: 3.28 kg; p=0.007). Conclusion: The use of analgesia is associated with a greater frequency of obstetric interventions as well as an increase in the duration of labor. Regarding neonatal outcomes, the group with analgesia gave birth to heavier newborns; Furthermore, no association was observed with the Apgar score, nor referrals to high-risk units. Abstract Background The American College of Obstetricians and Gynecologists, in its opinion of the Committee on Midwifery Practice, points out that planned home birth is a woman's and family's right to experience, but also to choose and be informed about, their baby's place of birth. The aim of this study was to understand obstetric nurses' perceptions of planned home childbirth care within the framework of the Brazilian obstetric model. Method A qualitative study, with Snowball Sampling recruitment, totaling 20 obstetric nurses through semi-structured interviews between September 2022 and January 2023, remotely, using the Google Meet application and the recording feature. After the data had been collected, the material was transcribed in full and subjected to content analysis in the thematic modality with the support of ATLAS.ti 8.0 software. Results Obstetric care at home emerged as a counterpoint to hospital care and the biomedical model, providing care at home based on scientific evidence and humanization, bringing qualified information as a facilitator of access and financial costs as an obstacle to effective home birth. Conclusion Understanding obstetric nurses' perceptions of planned home birth care in the context of the Brazilian obstetric model shows the need for progress as a public policy and for strategies to ensure quality and regulation.

INDUCED labor (Obstetrics) , *DELIVERY (Obstetrics) , *URINARY catheters , *CATHETERS , *CASE-control method - Abstract
The use of a Foley catheter is one of the oldest known methods of labor induction. Therefore, protocols using different volumes of Foley catheter balloons have been developed and tested to accurately determine their effectiveness. In this study, it was decided to retrospectively evaluate two induction of labor (IOL) protocols. The last 300 eligible patients who met the criteria and underwent the low-volume balloon protocol (40–60 mL) IOL were selected. Then next, 300 patients who met the criteria and underwent high-volume balloon (80–100 mL) IOL were selected. Outcomes included time to delivery and parturition type, oxytocin augmentation, operative deliveries and application of intrapartum anesthesia. Overall, the majority of patients delivered within 24 h. Patients who received a high-volume Foley catheter had statistically significantly more vaginal deliveries. The mean-time to delivery in the high-volume catheter group was statistically significantly shorter than in the low-volume catheter group. Patients who received a high-volume Foley catheter required statistically significantly less oxytocin augmentation during induction of labor compared to patients with a low-volume Foley catheter. Regardless of the balloon volume used, the percentage of operative deliveries remained at a similar, low level (8.36% and 2.14%). Regardless of the catheter volume used, the majority of patients chose epidural over intravenous anesthesia. In conclusion, a high-volume balloon Foley catheter IOL is characterized by an increased percentage of vaginal deliveries, shortened time to delivery regardless of the type of delivery, and lower need for oxytocin augmentation. Objective: to apprehend the social representations of obstetrical nurses regarding the care of parturient women in the maternity ward. Method: qualitative study, based on the Theory of Social Representations, according to Moscovici’s principles, carried out with nine obstetric nurses working in a public maternity ward in the Northeast Region of Brazil, from March to July 2020. Data were obtained through interviews, and then were organized and processed in the IRAMUTEQ software. Results: nurses represent parturient care from a functional dimension, expressed through technical and scientific knowledge and their attributions in terms of promotion, protection and rehabilitation of the individual, as well as from the perspective of the lived reality, the difficulties related to the structure and the work processes permeating the daily lives of obstetric nurses. Final considerations: nurses represent the nursing care of parturient women from a technical perspective, adding, however, elements of a scientific, humanistic and unique type of care, such as welcoming, presence, listening, respect and empathy. BACKGROUND:To develop effective and sustainable simulation training programs in low-resource settings, it is critical that facilitators are thoroughly trained in debriefing, a critical component of simulation learning. However, large knowledge gaps exist regarding the best way to train and evaluate debrief facilitators in low-resource settings. METHODS:Using a mixed methods approach, this study explored the feasibility of evaluating the debriefing skills of nurse mentors in Bihar, India. Videos of obstetric and neonatal post-simulation debriefs were assessed using two known tools: the Center for Advanced Pediatric and Perinatal Education (CAPE) tool and Debriefing Assessment for Simulation in Healthcare (DASH). Video data was used to evaluate interrater reliability and changes in debriefing performance over time. Additionally, twenty semi-structured interviews with nurse mentors explored perceived barriers and enablers of debriefing in Bihar. RESULTS:A total of 73 debriefing videos, averaging 18 min each, were analyzed by two raters. The CAPE tool demonstrated higher interrater reliability than the DASH; 13 of 16 CAPE indicators and two of six DASH indicators were judged reliable (ICC > 0.6 or kappa > 0.40). All indicators remained stable or improved over time. The number of 'instructors questions,' the amount of 'trainee responses,' and the ability to 'organize the debrief' improved significantly over time (p

Postpartum Anxiety is a frequent phenomenon, since the puerperium is the period of greatest risk for the onset or worsening of anxiety. It is usually manifested by irritability, fatigue, concentration difficulties, tension and sleep disturbances. It can have consequences both for the woman and for her relationship with the baby and her partner, interfering with the quality of life and the future of the family. Objective: to identify the factors that relate to the anxiety levels of postpartum women during hospitalization in an obstetrics service. Methodology: this is a descriptive, correlational and analytical quantitative study, with the application of the Zung Self-Rating Anxiety Scale, the Supportive Hospital Environment Design Scale and the Maternal Self-Perceived Scale of Neonatal Care Skills. Participants were 85 postpartum women who were hospitalized in the obstetrics service, in a hospital in the northern region of Portugal. Results: Most mothers (51.8%) had no anxiety, 11.8% had non-pathological anxiety and 36.5% had pathological anxiety. Of the anxiety factors studied, a global relationship was established between the level of postpartum anxiety and the perception of neonatal care skills (p=0.00). Conclusion: The postpartum women showed greater anxiety in the Central Nervous System (CNS) dimension. A better perception of social support in the hospital environment reduces levels of maternal anxiety in different aspects. The higher the maternal self-perception of skills in caring for the baby, the lower the level of postpartum anxiety.

ObjectiveTo evaluate the degree to which registered nurses perceive their labor and delivery units to be adhering to Association of Women's Health, Obstetric and Neonatal Nurses (AWHONN) staffing guidelines.DesignProspective, cross-sectional study via an online survey of labor nurses recruited from hospitals in three states.Setting/local problemIn late 2016 and early 2017, labor nurses in selected hospitals in California, Michigan, and New Jersey were contacted via e-mail invitation to participate in a study about nursing care during labor and birth. Nurse leaders in each hospital facilitated the invitations.ParticipantsA total of 615 labor nurses from 67 hospitals.Intervention/measurementsDescriptive statistics and linear regression models were used for data analysis.ResultsMost nurses reported that the AWHONN nurse staffing guidelines were frequently or always followed in all aspects of care surveyed. Hospitals with annual birth volumes of 500 to 999 range were significantly more likely than hospitals with 2,500 or more annual births to be perceived as compliant with AWHONN staffing guidelines.ConclusionWhen the AWHONN staffing guidelines were first published in 2010, there was concern among some nurse leaders that they would not be adopted into clinical practice, yet nurses in our sample overwhelmingly perceived their hospitals to be guideline compliant. There remains much more work to be done to determine nurse-sensitive outcomes for maternity care and to ensure that all women in labor in the United States are cared for by nurses who are not overburdened or distracted by being assigned more women than can be safely handled.

Summary: Purpose: The aim of this paper is to develop a scale for measuring the perinatal bereavement care competence of midwives and assess its psychometric properties. Methods: The Perinatal Bereavement Care Competence Scale was developed in four phases. (1) Item generation: 75 items were formulated based on a literature review and interviews with midwives. (2) Delphi expert consultation: 15 experts evaluated whether the items were clear/appropriate/relevant to the questionnaire dimensions, and the items were optimized. (3) Pilot test: The comprehensibility, acceptability, and time required to complete the questionnaire by midwives were assessed. (4) Evaluation of reliability and validity: The scale was validated by initial item analysis, exploratory and confirmatory factor analyses, and internal consistency reliability and test–retest reliability. Results: The final scale consisted of six dimensions and 25 items: maintaining belief (three items), knowing (four items), being with (six items), preserving dignity (four items), enabling (five items), and self-adjustment (three items). Exploratory factor analysis yielded a six-factor structure that was consistent with the theoretical framework and explained 70.8% of the total variance. Confirmatory factor analysis indicated a good fit for the six-factor model. Cronbach's α for the scale was 0.931, and the test–retest reliability coefficient was 0.968. Conclusion: The Perinatal Bereavement Care Competence Scale is a valid and reliable instrument for measuring the competence of midwives in caring for bereaved parents who have experienced perinatal loss.

Introduction During labour, pain can affect the health of the mother and the foetus, and its relief is a right of the mother and a duty of the professionals. In the promotion of a positive experience of labour, it is crucial that nurses specialised in maternal and obstetric health give priority to pain assessment and the use of non-pharmacological methods for pain relief, providing the necessary resources and empowering parturients to use them. Among said methods, virtual reality stands out due to its ease of use and for allowing the reduction of pain levels by diverting attention from the real world, using computers and other devices. Since it is a new approach that is not yet implemented in delivery rooms in Portugal, it is important to map the facilitating factors and barriers associated with its use, so that the dissemination of existing knowledge and its transfer to skilled nursing care during the first stage of labour can be planned. Objective To map the research evidence on the facilitating factors and barriers in the use of VR as a non-pharmacological strategy for pain relief during labour in hospital settings. Methods This protocol follows the guidelines published by the Joanna Briggs Institute. The databases MEDLINE, CINAHL, Cochrane Database of Systematic Reviews and MedicLatina through the EBSCOhost platform, the Joanna Briggs Institute EBP Database, through the Ovid platform are considered for the search and, the grey literature is also included. As inclusion criteria, qualitative, quantitative and mixed studies that address virtual reality as a pain relief strategy during labour in hospital settings (based on the mnemonic PCC - Population, Concept and Context), published in Portuguese, French, Spanish or English language between 2017 and 2022 are considered. The titles and abstracts of identified references will be independently reviewed and assessed for eligibility by two reviewers. In the event of a tie, a third reviewer will be used. Full text studies and data will be extracted using a form. The data extraction table will show the mapped data in a descriptive way answering the research questions. Discussion The results will allow summarising the barriers and facilitating factors in the use of Virtual Reality for pain relief during labour and thus contribute to decision making in planning the dissemination of this strategy to pregnant women and health professionals and its implementation in delivery rooms. Objective: to understand the motivations of women in choosing the planned home birth and the perceptions of this experience. Method: descriptive, exploratory and qualitative study, developed with 14 women who experienced a planned home birth between January 2019 and December 2020. The interviews were conducted between March and May 2021, transcribed in full and submitted to content analysis. Results: the allowed understanding that the motivations for choosing planned home birth were related to the feeling of fear of hospital obstetric practices. Safety in the home environment, the guarantee of autonomy and the possibility of the presence of children were both motivations and positive perceptions of this experience. Resistance on the part of society was often experienced. Conclusion: the current obstetric model needs to be rethought and readjusted, in order to provide safe and respectful birth, whether at home or in the hospital. [ABSTRACT FROM AUTHOR]

Objective:to identify the risk of depression during pregnancy among pregnant women receiving routine prenatal care and the associated factors. Method: a cross-sectional study, carried out with 201 pregnant women, in a routine prenatal clinic of a university maternity hospital. Data were collected using an electronic form containing a characterization instrument and the Escala de Risco de Depressão na Gravidez (Depression during Pregnancy Scale). The dependent variable was the risk of depression during pregnancy. Statistical analysis was performed by calculating the Odds Ratio and using the Chi-square and Fischer's Exact tests. Results: among the participants, 68.2% had a higher risk of depression during pregnancy. There was a statistically significant association between a higher risk of depression during pregnancy and occupation (p=0.04), that is, unemployment (OR=2.00) doubled the risk of depression. Conclusion: the high prevalence of the risk of depression during pregnancy indicates the necessity of planning, prioritizing, and integrating mental health into prenatal health services, especially in the primary healthcare environment, by health managers and policymakers. - Abstract
Objective: to analyze the health care provided to women in peripartum and parturition during the pandemic period of COVID-19. Method: analytical and cross-sectional study, conducted with 404 puerperal women from three maternity hospitals in Paraná - Brazil, between the months of September-December/2021. Data were analyzed by chi-square test (p<0.05) to verify the association. Results: care was taken to prevent COVID-19 in peripartum and parturition (physical distance 89.4%, use of mask 96.8%, respiratory etiquette 74.3%, hand hygiene 97.8%), presence of a companion (97.2%), respect for the choice of parturition route (71%) and skin-to-skin contact (70.2%). A high rate of early hospitalization (dilation between 0-3 cm), low offer of non-pharmacological methods for pain relief and low incentive to breastfeeding were observed. Conclusion: the study contributes to improve health actions about the natural physiology of parturition and to strengthen the rights in parturition, even in vulnerable pandemic periods.
