Die vorliegende Validierungs-Studie hat das Ziel zu überprüfen, ob die neuartige Ohrsensorik der Firma cosinuss° die Herzfrequenz genauso exakt misst wie ein Brustgurt und die Körperkerntemperatur so exakt bestimmt wie Nasen-, Blasen- oder Pulmonalis-Arterien-Sensoren. Hierzu wurden ca. 12000 Datenpaare von 44 Probanden ausgewertet und verglichen. Die Messungen zur Datenerhebung erfolgten im Labor des Lehrstuhls Präventive Pädiatrie der Technischen Universität in München und im Deutschen Herzzentrum in München., The aim of this validation study is to investigate whether the new ear sensor technology of the cosinuss system measures the heart rate just as exactly as a chest belt and the body's core temperature as accurately as nasal, bladder or pulmonary artery sensors. For this purpose, approximately 12,000 data pairs from 44 subjects were evaluated and compared. The measurements for data collection were carried out in the laboratory of the Chair of Preventive Pediatrics at the Technical University in Munich and at the German Heart Center in Munich.

Background: Although cardiovascular rehabilitation (CR) is well accepted in general, CR-attendance and delivery still considerably vary between the European countries. Moreover, clinical and prognostic effects of CR are not well established for a variety of cardiovascular diseases. Methods: The guidelines address all aspects of CR including indications, contents and delivery. By processing the guidelines, every step was externally supervised and moderated by independent members of the Association of the Scientific Medical Societies in Germany (AWMF). Four meta-analyses were performed to evaluate the prognostic effect of CR after acute coronary syndrome (ACS), after coronary bypass grafting (CABG), in patients with severe chronic systolic heart failure (HFrEF), and to define the effect of psychological interventions during CR. All other indications for CR-delivery were based on a predefined semi-structured literature search and recommendations were established by a formal consenting process including all medical societies involved in guideline generation. Results: Multidisciplinary CR is associated with a significant reduction in all-cause mortality in patients after ACS and after CABG, whereas HFrEF-patients (left ventricular ejection fraction <40%) especially benefit in terms of exercise capacity and health-related quality of life. Patients with other cardiovascular diseases also benefit from CR-participation, but the scientific evidence is less clear. There is increasing evidence that the beneficial effect of CR strongly depends on treatment intensity including medical supervision, treatment of cardiovascular risk factors, information and education, and a minimum of individually adapted exercise volume. Additional psychologic interventions should be performed on the basis of individual needs. Seit Einführung der Fontan-Operation im Jahre 1968 haben weltweit viele tausend Patienten mit morphologisch oder funktionell univentrikulärem Herzen (UVH), einen derartigen Eingriff erhalten. Gegenüber der präoperativen Situation profitieren Patienten mit UVH insbesondere hinsichtlich ihrer körperlichen Belastbarkeit und Belastungsdauer von einer Fontan-Operation. Trotz aller Fortschritte der modernen Medizin und sinkender Letalität sind die Patienten nach Fontan-Operation durch diesen Eingriff allerdings nicht geheilt, sondern ausnahmslos chronisch herzkrank, da vielfältige anatomische und funktionelle Rest- und Folgezustände bestehen. Aus diversen Studien ist bekannt, dass die körperliche Leistungsfähigkeit von Fontan-/TCPC (= total cavo-pulmonary connection) -Patienten nur bei 50-60 % Gleichaltriger liegt. Wegen Leistungslimitationen bei der körperlichen Belastung der bekannten gesundheitlichen und psychischen Probleme nach Fontan-Operation ist es wichtig, bei den Betroffenen frühzeitig protektiv einzugreifen und durch präventive und/oder rehabilitative Maßnahmen den Krankheitsverlauf günstig zu beeinflussen, da sich auf Grund der vorliegenden Daten eine Verbesserung der Leistungsfähigkeit und der kardiopulmonalen Funktionsfähigkeit positiv auf die Langzeitverläufe auswirken kann. Ob auch ein spezielles Atemtraining bei Patienten nach TCPC-Operation die Leistungsbreite und das Befinden der Patienten verbessern kann, ist bislang nicht untersucht worden. Die vorliegende Studie beschäftigt sich mit den Auswirkungen eines zu Hause durchgeführten Atemtrainings, welches das Ziel hat, über die Stärkung der Atemmuskulatur eine Verbesserung der Lungenfunktion sowie der peripheren Sauerstoffversorgung zu erzielen, um damit die körperliche Leistungsfähigkeit zu steigern. In der vorliegenden randomisierten Studie konnte erstmals in einem vergleichsweise großen Kollektiv von 40 konsekutiven Patienten im Alter zwischen 8 und ≤ 17 Jahren (Mädchen: n= 25%) mit Fontan-Zirkulation, Since the introduction of the Fontan operation in 1968, worldwide many thousands of patients with a morphologically or functionally univentricular heart have received a total cavopulmonary connection (TCPC). Because of exercise limitations as well as health and psychological problems, it is important to intervene early in preventive and / or rehabilitative care. Based on current data, exercise training can improve performance as well as cardiopulmonary function and an improvement can be expected for the long-term course. Whether or not a breathing training can also improve exercise performance after TCPC operation has not been tested yet. The present study deals with the impact of a home respiratory training, aiming to strengthen respiratory muscles, improve lung function and the peripheral oxygen supply, in order to increase physical performance. For the very first time, in a relatively large cohort of 40 consecutive patients with a Fontan circulation (TCPC), aging 8 to 17 years (female 25%), the effects of respiratory training on lung capacity and exercise performance were measured. In the training group, physical performance increased on average by 14%, from 123 to 140 watts after a respiratory training. In the control group exercise performance increased only 6.5% (from 111 to 119 watts). After respiratory training, in the training group there was an 10% increase in the forced vital capacity (FVC) from 2.58 ± 0.75 to 2.84 ± 0.75 liters, in the control group an 11% increase (from 2.3 ± 0.79 to 2.57 ± 0.87 liters). Diese Arbeit beschäftigt sich mit Parametern der subklinischen Arteriosklerose und wie sich diese über die Dauer einer sechsmonatigen Bewegungsintervention verändern. Ein besonderer Fokus wird hierbei auf die laborchemischen Parameter wie Lipoproteine, Entzündungsparameter und Zytokine und deren Assoziation mit den klassischen kardiovaskulären Risikofaktoren gelegt., This thesis work focuses on the parameters of subclinical arteriosclerosis and how these parameters change during a six month physical training programme. Particular attention was payed to laboratory-chemical parameters like lipoproteins, inflammation parameters and cytokines and their association with classic cardiovascular risk factors.

Das Ziel der Arbeit ist die Auswirkungen jugendlichen Leistungssports auf die Intima-Media-Dicke (IMT) und Distensibilität der A. carotis communis (ACC) zu untersuchen. In Zusammenschau mit Wall-to-Lumen-Ratio (W-to-L-Ratio), ein Parameter, der Gefäßdurchmesser und Wanddicke kombiniert, und kardialen Strukturparametern, die Septum- und linksventrikuläre Wand beurteilen, werden vaskuläre Adaptionsmechanismen der ACC analysiert. In dieser prospektiven Studie wurden 34 Leistungsfußballer im Alter von 14 bis 18 Jahren untersucht. Nach Erhebung der Anthropometrie erfolgte eine Ultraschall-gesteuerte Messung der IMT in vier Winkeln bei 90, 120, 210 und 270 Grad. Mittels eTracking wurde die Gefäßdistensibilität links und rechts jeweils zweimal bestimmt. Die Spiroergometrie lieferte anhand der prozentualen Sauerstoffaufnahme (VO2%) und der relativen Leistung (Watt_re) die Werte über die Leistungsfähigkeit der Sportler. Herzechographische Ultraschalluntersuchungen gaben Aufschluss über die kardialen Strukturbeschaffenheiten. Es resultierte eine im Vergleich zu einem entsprechenden Normkollektiv erhöhte cIMT. Zudem zeigte sich eine Korrelation der rechten cIMT mit relativen Leistung der jugendlichen Leistungsfußballer, die oberhalb der Normwerte gemessen wurde. Bezüglich Distensibilitätsparameter, W-to-L-Ratio oder kardialer Parameter konnten keine Auffälligkeiten und Korrelationen zur Leistungsfähigkeit konstatiert werden. Demzufolge könnte die strukturelle Verdickung der cIMT Ausdruck vaskulärer Adaption im jugendlichen Leistungssport sein. In Analogie zum Sportlerherzen könnte sie als Hypertrophie glatter Gefäßmuskelzellen, die die Arbeit der Arterien unter hoher Belastung ökonomisieren, interpretiert werden. Eine mögliche Erklärung der normwertigen Distensibilitätseigenschaften der ACC wäre eine Balance förderlicher und hinderlicher Einflussfaktoren durch Leistungssport. Zusammenfassend lassen sich nach Abschluss dieser Studie noch keine ursächlichen Erklärungen bezüglich, Objective: The aim of this study is to detect associations between elite performance in youths and their intima-media-thickness (IMT) and distensibility of the common carotid artery. Considering wall-to-lumen-ratio (W-to-L-ratio) and thickness of cardiac septum and left ventricle better understanding of vascular adaptions should be possible. Design: A prospective study with elite youth soccer players (n = 34) aged 14 to 18 was designed. Methods: After collecting anthropometric data carotid intima-media-thickness (cIMT) and distensibility were measured using ultrasound. IMT was measured accordingly to AEPC recommendations at two angles at each side of the neck. E-tracking for distensibility parameters was performed twice at the same angle at each side. The athletes´ performance was quantified by percentage oxygen consumption (VO2%) and relative performance parameter (Watt_re) conducting a spiroergometry. A following heart ultrasound measurement evaluated cardiac structure with regard to thickness of septum and wall of the left ventricle. Results: cIMT was significantly increased in the youths´ arteries in comparison of a reference collective. A correlation with their relative performance and the right-sided cIMT could be found. Respecting distensibility of the carotid artery, W-to-L-ratio and cardiac parameters no outstanding detections or correlations occurred. Conclusion: Therefore increased cIMT might be considered a vascular adaption in young elite athletes. Complementary to the athletes´ heart the thickening could be interpreted as hypertrophy of smooth muscle cells, which economizes the artery´s work under elevated shear stress. A possible explanation for the normal distensibility values is the balance of unfavorable and beneficial effects under extensive sportive exercise.
