Background: Because of the growth of nursing care in ambulatory clinics, nursing education and professional development needs have increased. Method: At one academic medical center, development of a combined centralized-decentralized nursing professional development model addressed the needs of this expansion. Results: Hiring dedicated nursing professional development staff for prioritized education programs improves throughputs and outputs associated with nursing professional development. Standardized blood pressure (BP) measurements for patients with chronic kidney disease (CKD) are paramount in the management of hypertension. Evidence shows nursing staff adherence to best practice guidelines for BP measurement are suboptimal. A pre-/posttest pilot study implementing a six-week hybrid educational intervention for nursing staff was conducted in an outpatient nephrology office. The Evidence-Based Practice (EBP) Beliefs Scale was administered to participants (n = 6) to assess individual beliefs about EBP and implementing EBP guidelines for BP measurement. One Likert-type question measured participant change in BP measurement. Improvement was noted in EBP beliefs and ability to implement EBP guidelines post-intervention. A favorable response was present for participant change in BP measurement per guidelines post-intervention. This article describes standardizing ambulatory oncology nursing orientation within an academic comprehensive cancer center to reduce turnover rates. Background and aims: On 1 May 2018, Scotland introduced a minimum unit price (MUP) of £0.50 for alcohol, with one UK unit of alcohol being 10 ml of pure ethanol. This study measured the association between MUP and changes in the volume of alcohol‐related ambulance call‐outs in the overall population and in call‐outs subsets (night‐time call‐outs and subpopulations with higher incidence of alcohol‐related harm). Design: An interrupted time−series (ITS) was used to measure variations in the daily volume of alcohol‐related call‐outs. We performed uncontrolled ITS on both the intervention and control group and a controlled ITS built on the difference between the two series. Data were from electronic patient clinical records from the Scottish Ambulance Service. Setting and cases: Alcohol‐related ambulance call‐outs (intervention group) and total ambulance call‐outs for people aged under 13 years (control group) in Scotland, from December 2017 to March 2020. Measurements Call‐outs were deemed alcohol‐related if ambulance clinicians indicated that alcohol was a 'contributing factor' in the call‐out and/or a validated Scottish Ambulance Service algorithm determined that the call‐out was alcohol‐related. Findings No statistically significant association in the volume of call‐outs was found in both the uncontrolled series [step change = 0.062, 95% confidence interval (CI) = −0.012, 0.0135 P = 0.091; slope change = −0.001, 95% CI = −0.001, 0.1 × 10−3P = 0.139] and controlled series (step change = −0.01, 95% CI = −0.317, 0.298 P = 0.951; slope change = −0.003, 95% CI = −0.008, 0.002 P = 0.257). Similarly, no significant changes were found for the night‐time series or for any population subgroups. Conclusions: There appears to be no statistically significant association between the introduction of minimum unit pricing for alcohol in Scotland and the volume of alcohol‐related ambulance call‐outs. Patients receiving oncology care are at risk for malnutrition, which is associated with decreased cancer treatment tolerance, lower quality of life, and increased mortality. Implementation of frequent nutritional screening is critical to mitigating malnutrition risk and improving patient outcomes. Guidelines recommend frequent nutritional screening using validated screening tools in the ambulatory oncology setting, yet screening is not part of standard supportive care. This quality improvement project implemented a nurse-led nutritional screening process and risk screening tool in a community oncology practice. Climate change, an urgent public health problem, will increase the morbidity and mortality of older adults. The 2020 report of The Lancet Countdown highlighted that older adults are susceptible to the effects of extreme weather events, including heatwaves. Through the lens of the socio-ecological model, we identified influences on persons at risk for heat-related illnesses. We applied the Heat-related Intolerance Screening Tool (HIST) to a case that demonstrates the impact of physical, social, environmental, and societal factors on older people. The hypothetical case provides a context for ambulatory care nurses to explore innovative ways to screen for heat intolerance to mitigate the risk of heat-related illnesses, such as heat stroke. Our innovation supports the broad utility of the simple HIST by ambulatory care nurses to identify and prevent heat-related illnesses, thereby reducing emergency room visits and hospital admissions. This scoping review of ambulatory care nurse-sensitive indicators (ACNSIs) was based on standards set forth by JBI Scoping Review Guidance and PRISMA-ScR guidelines. Workplace violence in ambulatory care is less controlled than in inpatient settings due to a variety of factors, including location, patient population, volume of visits, hours of operation, physical environment, and availability of resources. Personal knowledge, skills, and ability, as well as data and organizational resources, are needed to mitigate events. Continued research, publication of evidence-based practice, and advocacy for laws protecting health care workers in ambulatory care are necessary. The development of workplace violence risk assessments, violence screening questions, and national nursing quality indicators are needed to help move the prevention needle in ambulatory care settings. Patient care needs in ambulatory care (AC) settings continue to grow and evolve in the United States, with commensurate growth of nursing responsibilities in AC. Conducting research on the nursing workforce and nursing practice is essential to understanding and meeting the needs of nurses and patients in this setting. However, the structures and characteristics of AC settings pose challenges for conducting research on AC nursing practice. This article explains unique barriers to participation in research for nurses in AC, describes recruitment challenges for nurse researchers in AC, and provides strategies to increase recruitment of nurses for AC research. Researchers in AC must find ways to recruit representative participant samples, be clear and precise in defining terms, and report robust demographic information about participants and their practice settings. Background: In view of demographic change, there is a growing need for qualified nursing professionals. More and more people in need of care prefer to be cared for at home. A shortage of skilled workers and constantly changing and increasing professional requirements are some of the consequences. The aim of this study is to generate further insights into work-related psychosocial stress factors and the resources of outpatient caregivers in their subjective perceptions to derive approaches for possible health-promoting interventions for practice and research that promote healthier and more sustainable work. In addition, coping strategies and needs should be inquired about in order to determine support offers. Methods: Using a semi-structured interview guide, thirty outpatient nursing professionals in Germany were interviewed in 2022. The subject of this interview was the psychosocial working conditions and resources in outpatient care. The collected data were evaluated by means of the Kuckartz qualitative content analysis. Results: This qualitative study identified work-related job demands and resources in outpatient care. The way nurses are treated in the companies they work for and the appreciation of patients were explored as important resources. Physical demands, the time factor, and dealing with personal fates are primarily mentioned as stress factors. The learning effect plays an important role in dealing with these. Conclusion: This study contributes to a better understanding of current job strain and resources as well as job satisfaction in outpatient care. In any case, it became clear that the nursing staff love their profession, which they see more as a vocation. Future quantitative studies are necessary to build on these findings. Background: Although there is a substantial body of evidence regarding full practice authority's (FPA) effects on health care access and quality, very little research has examined how nurse practitioner (NP) licensure laws affect the status of NPs as clinicians, employees, and leaders in health care organizations. Purpose: This study examined whether states' implementation of FPA leads to higher pay, business ownership, assigned patient panel, and billing transparency for NPs' and whether NPs' gains from FPA increase over time in states where FPA has been in effect longer. Methodology: Data from a nationwide survey of licensed NPs (N = 5,770) were used to compare NPs' employment conditions between FPA and non-FPA states. After balancing the FPA and non-FPA groups on demographic characteristics (e.g., urbanicity, education), adjusted mean differences in outcomes between the groups were estimated using weighted multivariable regression. Results: Compared with NPs in non-FPA states, NPs in FPA states had higher mean earnings (p < .05), were more likely to be practice owners or shareholders (p < .01), and billed a greater percentage of their patient visits under their own National Provider Identifier (p < .001). Having FPA in place for ≥10 years was associated with greater improvements in conditions of employment compared with having FPA <10 years. Conclusions: States' adoption of FPA for NPs is associated with improved conditions of employment among NPs. Implications: Untethering NPs from physicians establishes a cascade of modest gains in income and practice ownership that may indicate changes over time. The American Academy of Ambulatory Care Nursing (AAACN) recognizes a paradigm shift represented by the volume of health care delivery flowing from episodic acute care encounters to long-term relationships. This shift presents opportunities to further recognize and prepare nurse graduates for the complex and expanded role of the ambulatory care registered nurse (RN). This position paper discusses the current state of the RN role in ambulatory care and provides a vision for the future. COVID-19 has further complicated this process by limiting access to professional development resources typically available to new leaders. The COVID-19 pandemic has been difficult for registered nurses. Media reports, most of them anecdotal, have reported upticks in nurse resignations, and plans to resign and/or leave nursing due to COVID-19. This article reports findings from an online anonymous 95-item survey completed by about 1,600 nurses from a New York City metropolitan area health system's (HS) four hospitals and ambulatory care centers about their COVID-19 experience in the spring of 2020, their intent to stay at the HS, and their intent to stay working as a nurse. Conducted early in the pandemic, this survey addresses a major gap in the literature, as there was no timely evaluation of nurses' intent to leave during the "Great Attrition" wave or to stay during the "Great Attraction" trend. Among those nurses completing the survey, 85.7% reported that they planned to work as a nurse one year later and 77.9% reported that they planned to work at the HS one year later. Those nurses who obtained a master's or doctoral degree as their first professional degree in nursing, or had a high level of mastery, were less likely to report an intent to stay at the HS. Those with no children, those who thought the HS was more supportive, and those who thought that registered nurse–medical doctor relations were higher were more likely to intend to stay at the HS. Those nurses who reported worse communication with their nurse manager were less likely to report an intent to stay in nursing. Those who reported lower stress, who were unmarried and had no children were more likely to intend to work as nurses. Our findings on nurses' intent to leave their organization and their intent to leave nursing are much lower than reports in the popular press. Our data were collected early in the pandemic and it may not reflect the accumulated stress nurses experienced from witnessing the death of so many patients. [ABSTRACT FROM AUTHOR]

BACKGROUND: For children, the post-concussion return to school process is a critical step towards achieving positive health outcomes. The process requires integration between healthcare professionals, parents, and school personnel. OBJECTIVE: This research team conducted focus groups with stakeholders including parents, education personnel, school nurses, external healthcare providers (nurses) and athletic trainers to identify communication patterns between healthcare providers outside of the school setting and school personnel. METHODS: Data from focus groups were analyzed using a Thematic Analysis approach. Researchers used an inductive (bottom-up) coding process to describe semantic themes and utilized a critical realist epistemology. RESULTS: We identified four key themes within focus group data: (1) lack of effective communication between hospital and outpatient healthcare providers to school personnel; (2) parents who were strong advocates had improved communication with healthcare professionals and garnered more accommodations for their children; (3) non-school professionals and families were often confused about who the point of contact was at a given school; and (4) differing experiences for athletes vs. non-athletes. CONCLUSION: This study suggests gaps in communication between healthcare and school professionals when children return to school following a concussion. Background: Falls are the leading cause of injury-related death and disability in community-dwelling older adults. Local Problem: Evidence-based fall prevention activities in primary care, including nurse-facilitated wellness visits, have been limited. Barriers including patient engagement and adherence exist. Method: A quality improvement project integrating components of the Centers for Disease Control and Prevention's Stopping Elder Accidents, Deaths & Injury (STEADI) was introduced by registered nurses during older adult annual wellness visits. Intervention: Nurses assessed risk and implemented patient-centered fall prevention plans including follow-up. Results: A total of 522 patients were screened, with 21% (n = 111) having increased fall risk. Of these, 78% (n = 87) engaged in home safety, gait, strength, and balance assessments and the majority (n = 83; 95%) participated in fall prevention plans of care. At 2-week follow-up, patients' self-reported adherence was 74% for gait/strength/balance and 67% for home safety. The ANCC Magnet Recognition Program's® mission and vision is to continually elevate patient care and transform healthcare globally through an environment where nurses and the interprofessional team flourish. Reflecting the larger shifts in the healthcare delivery system from inpatient to ambulatory care settings, the Commission on Magnet® continues to emphasize the contributions of nurses in ambulatory care settings in the 2023 Magnet Application Manual®. This Magnet® Perspectives column will discuss the valuable role of ambulatory care nurses across the continuum of care and review new requirements in the 2023 Magnet Application Manual for measurement of nurse-sensitive outcomes in the ambulatory care setting using nationally benchmarked measures. The article discusses the implementation and outcomes of nurse-led evidence-based educational sessions and emergency simulations at an outpatient cardiology imaging center in Alaska to help improve emergency skills of staff. The study evaluated the impact of Basic Life Support or Advanced Cardiovascular Life Support training courses on the knowledge, awareness and confidence of outpatient healthcare staff when responding to medical emergencies.

Ambulatory staffing to workload based on visit volume in an outpatient setting is an elusive formula, and the literature describing such processes is limited. One health system tasked a multidisciplinary team with developing an ambulatory staffing to workload tool to meet the needs of staff, management, and leadership. The resultant tool includes an automated dashboard for determining staffing needs on the basis of quantified workload, prospective modeling, and historical dashboards to demonstrate actual staffing (full-time equivalents) to workload (outpatient volumes) compared with budget. Baccalaureate nursing education is moving to adopt the new American Association of Colleges of Nursing Essentials for Professional Nursing Education. As identified in two of the six domains of the essentials, graduates need to be prepared to address population health and utilize informatics and healthcare technologies. Community/public health nursing also has eight domains for generalist nurses linked to population health which will help prepare a skilled nursing workforce for the 21st century. The Institute for Healthcare Improvement's Triple Aim which evolved into the Quadruple Aim is focused on improving health outcomes within healthcare delivery. Through a literature review, a need for a Quadruple Aim model for nursing education was identified. Mirroring the Institute for Healthcare Improvement's Triple Aim for healthcare delivery, a Quadruple Aim for Nursing Education Model was developed. The model dimensions include (1) Population‐focused Care, (2) Maximize Student Learning Experience, (3) Cost‐effective Pedagogy, and (4) Nurse Educator Well‐being. The Quadruple Aim for Nursing Education Model supports nursing education to prepare future nurses effectively and efficiently bridging population health concepts and issues with nursing informatics. The opioid crisis continues to impact the United States despite efforts to reduce or remove barriers to expanding access to opioid use disorder (OUD) care and treatment. There is significant work still to be done. Given the overall financial impact to health care, the criminal justice system, and general lost productivity in the United States that continues to grow each year, it is imperative that interventions supportive approaches be implemented. As licensed clinicians, ambulatory care registered nurses have the potential to make a significant impact to the expansion of OUD services. Ambulatory care RNs and health professions students have limited education on interprofessional care before entering clinical settings. This article describes a program evaluation of a simulation-enhanced interprofessional education (Sim-IPE) experience designed for ambulatory care RNs and health professions students. An 11-item electronic post-Sim-IPE survey was administered to collect perceptions of the Sim-IPE experience. Most responses indicated the Sim-IPE promoted learning about each other's roles, was designed for their level of knowledge and skills, and provided sufficient information. The participants indicated that they felt supported and would use their learning in a clinical setting. Open-ended survey responses identified positive aspects of the Sim-IPE, areas for improvement, and suggestions for future Sim-IPE. The National League for Nursing Jeffries Simulation Theory was used to guide a program evaluation of the Sim-IPE. Background: In Japan, the number of pediatric cancer cases was approximately 2,200 in 2019. Notably, 15 hospitals are designated for pediatric cancer in Japan. This study focused on the outpatient nurses of a pediatric specialty hospital supporting these designated hospitals. The study aimed to clarify the support that these nurses provide to pediatric cancer survivors as well as their thoughts on supporting the patients at pediatric specialty hospitals. Methods: This study used qualitative inductive research methods and conducted semi-structured freeresponse interviews with outpatient nurses working at a pediatric specialty hospital. Data analysis was performed using the Modified Grounded Theory Approach (M-GTA). Results: Overall, 15 outpatient nurses were included in the study. Moreover, the M-GTA analysis generated four core categories. The thought process related to a long-term follow-up outpatient clinic for pediatric cancer survivors included considering factors that posed difficulty in the establishment of such a clinic, reconsidering the importance of establishing such a clinic, and contemplating about establishing a desirable clinic. This was based on the belief of the outpatient nurses regarding building a trusting relationship with the patients. Conclusion: Owing to manpower issues, no guidance was provided for the pediatric cancer survivors at outpatient clinics, and follow-up care was provided only by physicians. Moreover, outpatient nurses recognized the problems faced by patients throughout their lives and understood the need to solve them, but they could not provide effective guidance. Because pediatric cancer survivors are at high risk of disease- and treatment-related outcomes, there is a need to develop a system that facilitates collaboration between different professional groups in order to improve their quality of life. Recent surveys of Magnet facilities and nurses found low rates of implementation of evidence-based practice in U.S. health care settings. Nursing Experts: Translating the Evidence (NExT) is a collaboration of nurses and librarians providing free online evidence-based practice nursing education benefiting nurses in all settings. The NExT online modules empowered participants to efficiently access valuable resources to inform and improve their practice in a convenient, accessible, self-paced format. The "Future of Nursing Report 2020-2030: Charting a Path to Achieve Health Equity" presents an overarching framework for advancing health equity over the next decade. Given their knowledge and experience, ambulatory care nurses informed the report development; however, they must embrace opportunities for further action. Examples of ambulatory care nurses' innovative work and practice are showcased as they lead efforts across diverse settings with multiple partners. Background/Purpose: Meeting recommendations that nurses should partner in leading health care change is hampered by the lack of ambulatory care nurse-sensitive indicators (ACNSIs). This scoping review was conducted to identify evidence regarding ACNSI identification, development, implementation, and benchmarking. Methods: Following the PRISMA-ScR reporting guide, we performed PubMed/MEDLINE, CINAHL, and Cochrane Library searches for the period January 2006 to March 2021. Results: Twelve of the 1984 articles from 6 countries met inclusion criteria. All focused on identifying, developing/pilot testing indicators, and included structure, process, and outcome indicators. Seven articles were level II and all were at least grade B quality. Leverage points involved leadership support, automated data extraction infrastructure, and validating links between nurses' roles/actions and patient outcomes. Conclusions: While high-quality work is ongoing to identify clinically meaningful and feasible ACNSIs, knowledge in this field remains underdeveloped. Purpose: The UMass Chan Medical School/New England AIDS Education and Training Center Community-Based HIV and Viral Hepatitis Fellowship was launched in 2014 to train physicians and nurse practitioners to become experts in outpatient management of HIV, hepatitis B and C, and latent tuberculosis. The purpose of this study was to identify areas of strength and improvement and understand fellows' perceptions of the program and its impact on their current positions and career trajectories. Methods: Qualitative study utilizing a semi-structured interview guide with (11) fellowship graduates (8 MDs; 3 NPs). 45 to 60 min interviews were conducted in April and May 2021, recorded and transcribed. Transcripts were analyzed for representative themes and general patterns in the data. Results: Results indicate high satisfaction with the fellowship, which left a positive and indelible impact on their careers and patient care. Fellows highlighted the program's commitment to health equity, its role in transforming them into leaders and advocates for HIV in primary care, and their ability to balance their work and training demands with their personal lives and needs. The fellowship motivated them to become more involved in public health initiatives, serve marginalized communities and reduce their health disparities. They expressed confidence in their ability to independently manage outpatient HIV, viral hepatitis B and C, and latent tuberculosis, and found areas of overlap with their work in primary care. Conclusion: As the care of people with HIV becomes more commonplace in primary care clinics, it is imperative that primary care providers receive the necessary training and education to meet this need. Our study of 11 former fellows shows that the Community-Based HIV and Viral Hepatitis Fellowship offers such training, spreads it to other institutions, and can be a model for other programs nationwide. OBJECTIVE: The aim of this study was to evaluate scientific acceptability, feasibility, utility, and value of ambulatory care nurse-sensitive quality indicators (ACNSIs) in military outpatient clinics. BACKGROUND: Optimizing healthcare quality and cost requires scientifically sound metrics. However, ACNSI development and measurement have lagged behind inpatient nurse-sensitive indicators. Validated and reliable ACNSIs are urgently needed to maximize nurses' efforts to improve healthcare quality. METHODS: Two staffing and 4 patient-centered ACNSIs were pilot tested in 5 military clinics over a 6-month period using a mixed-methods design. RESULTS: Reliability and validity were generally acceptable. Most ACNSIs demonstrated feasibility, utility, and value for performance measurement. Challenges to using ACNSIs include absence of standardized administrative and clinical processes, infrastructure, and leadership and technological support. CONCLUSIONS: These ACNSIs show promise as valid performance metrics. This article discusses the role of and need for nursing leadership in ambulatory care spaces. Challenges within health care and increasing complexity of patients exist, regardless of whether a nurse leader works in a traditional hospital setting or outside of that setting. Regardless of setting, nurse leaders want the same things for their patients and staff. The required skill set and characteristics for nurse leaders are the same, just used in a different venue. Traditional hospital nurse leaders must collaborate with nurse leaders in ambulatory to ensure each patient navigates through the continuum with as little disruption as possible. Nurse leaders in ambulatory can offer hospital leaders a fresh perspective on leadership, highlighting the value they bring to the table and how transferrable their skill set is to this setting, given the same support for success that is fundamental in the hospital setting. BACKGROUND: Studies conducted in hospital settings have associated negative clinical outcomes with 12-hour shifts. Despite this, 12-hour shifts are common in nursing and popular among nurses. Little is known about outcomes associated with 12-hour shifts in ambulatory care settings. OBJECTIVE: A mixed-methods, quality improvement project was conducted in a large, ambulatory cancer center to evaluate oncology nursing staff perspectives on 12-hour shift work. METHODS: One hundred ambulatory oncology nurses completed surveys and 11 participated in focus group interviews. FINDINGS: Nurses expressed predominately positive perspectives about 12-hour shift work in ambulatory oncology care. CONCLUSIONS: Ambulatory oncology nurses perceived benefits to quality, safety, and satisfaction for both nurses and patients related to 12-hour shifts. Further evaluation of patient, nurse, and organizational outcomes unique to ambulatory settings is essential for nurse executives in formulating data-driven staffing plans. The ambulatory setting presents unique challenges for leadership and staff transitioning from other care areas, such as the inpatient environment. Nurses and support staff newly entering the world of ambulatory care may experience challenges acclimating to their new roles as they shift from an environment that provides focused care to achieve a safe discharge to one that focuses on holistic prevention and treatment over long periods. Inpatient hospital care relies on many protocols, orders, and guidelines and a multidisciplinary team of physicians, pharmacists, dieticians, and rehabilitation support to progress the patient through their illness or injury during their hospital admission. Nurses in ambulatory care have increased responsibility and autonomy in patient care. New challenges are encountered, for example, transportation to appointments, socioeconomic challenges, immediate resource needs, and patient support for future success in their own environment. In these situations, the ambulatory nurse is an immediate caregiver and a case manager, providing direct patient care for a short period, but following up over a longer period, in some cases, years. As in many areas of healthcare delivery, ambulatory care practice is being transformed by technological advances such as artificial intelligence (AI). Registered nurses (RNs) are challenged to integrate these new technologies into practice. As RNs are integral to care delivery, they must be knowledgeable about AI. In addition to nursing practice, nursing education will need to identify the knowledge, skills, and competencies required to practice with AI. Although the importance of the nurse–patient relationship in oncology is established, a consolidated body of research describing nurse–patient rapport, especially in ambulatory care, is lacking. This scoping review aimed to explore knowledge about rapport between adult patients with cancer and their nurses in ambulatory oncology care, including nurse, patient, nurse–patient dyad, and system-level factors that influence rapport. A scoping review was conducted to explore sources of evidence and gaps in knowledge pertinent to future research. The Joanna Briggs Institute methodology for Systematic Reviews was used with searches in MEDLINE, CINAHL, and ProQuest Dissertations and Theses Global databases. Experimental, descriptive observational, and qualitative study designs that focused on patients with cancer and their nurses within an ambulatory care setting were included. Results were reported using the Preferred Reporting Items for Systematic Reviews and Meta-Analyses Extension for Scoping Reviews. The search strategy collected 4538 studies of which 10 met study criteria after screening. Forward and backward citation tracking of included articles resulted in 4 additional studies. In total, 14 studies were extracted and analyzed. Synthesis of results from the individual sources resulted in three key observations: (a) there is an absence of studies that focus directly on nurse–patient rapport; (b) the majority of extracted data on rapport-related factors described aspects of nurses' attitudes; and (c) there was minimal information on patient, nurse–patient dyad, and system-level factors reported in these studies.
